Using Community Pharmacy Immunization Screening Forms To Identify Potential Immunization Opportunities, Albert T. Bach, Jeffery A. Goad

Immunization screening forms are completed for each patient that is to be vaccinated in the pharmacy. Screening forms contain demographic and health questions, which are used to determine if a patient is contraindicated to receive a vaccine. The objective is to determine if patient responses to questions on these forms can be used to identify potential vaccine indications. De-identified data was retrospectively collected from 11 community pharmacies in California and Michigan that included basic demographics, answers to immunization screening questions, and vaccine(s) administered during that visit. PPCN pharmacies have been able to successfully collaborate, bi-directionally, with health plan care managers to improve patient health and enable appropriate utilization of healthcare resources

To build off of our early success, additional systems and tracking capabilities are being explored related to the following:

  • Bi-directional tracking of referrals
  • Care managers to pharmacists
  • Pharmacists to care managers
  • Delivery drivers as community health workers
  • Drug class specific interventions (ex. a focus on opioids)
  • Disease state specific interventions (ex. a focus on asthma or type 2 diabetes)
  • Formal interventions focused on dialing patients back into the services/resources offered by health plans

In the past 20 years, pharmacists have seen expansions to legislation involving contraceptive authority, medical marijuana, vaccines and provider status. With the advancement of the profession of pharmacy, the demand for pharmacist leadership continues to rise. In order for pharmacists to acquire the necessary leadership skills for the profession, there is a call for colleges of pharmacy to incorporate leadership development into their academic programs. The Accreditation Council for Pharmacy Education (ACPE) has released the new 2016 Standards and Guidance Documents for institutions to follow in order for their pharmacy students to graduate with leadership skills.

The Spring Into Health community health fair was an interprofessional education (IPE) event

hosted by the University of Louisiana Monroe (ULM) College of Pharmacy's Christian

Pharmacists Fellowship International (CPFI) organization. The event included the participation

of several student pharmacy groups, as well as the ULM nursing, dental hygiene, medical

laboratory science, and health studies departments. Every organization involved in the health fair

provided either a health screening or a health education topic to the community of Ouachita

Background and purpose

The goal of this prospective, observational cohort study was to determine if simulated interdisciplinary teaching rounds improved student perceptions of confidence and attitudes towards working as part of a team. The secondary objective of this study was to investigate changes in student knowledge of the management of sepsis.

Educational activity and setting

Students participated in a traditional sepsis lecture followed by a simulated interdisciplinary rounding experience. Confidence and collaborative attitudes were assessed using a 5-point Likert scale (1 = strongly disagree, 5 = strongly agree). Changes in knowledge were measured using multiple choice questions. Practitioners in the field of pharmacy are often confronted with ill-structured problems. Specifically, pharmacists are tasked with making patient-specific recommendations that are both safe and effective, which requires combining knowledge from the biomedical, behavioral, and pharmaceutical sciences. Given the dynamic nature of pharmacy as a profession, the field has begun to explore learning strategies that go beyond mere content coverage to strategies that better support higher-order learning outcomes. One of these approaches is problem-based learning (PBL). While studies have focused on how to support PBL to improve learning outcomes, the role of assessment is often overlooked. Purpose: Teaching certificate programs have been developed by US colleges and schools of pharmacy to provide organized instruction for pharmacy residents seeking competence in classroom teaching or clinical precepting. The objective of this study is to appraise current status of teaching certificate programs offered by US colleges and schools of pharmacy. Method: A survey instrument was developed and sent to pharmacy practice department chairs/vice chairs at 132 US colleges/schools of Pharmacy. A Certified Nurse Practitioner (CNP) expanded her clinical services to include a project called Senior Transitions (ST). The CNP visited assisted living facilities in the area and conducted rounds on each of the facility’s patients participating in the ST program. The CNP had collaborated with pharmacists in her prior practice setting and wanted to continue doing so in her new practice setting. The issue of opioid overdose has become a national crisis and is receiving a lot of attention in the media. In the state of Oklahoma, 388 people died from opioid overdose in 2017. Nationally, there were over 47,600 opioid-related drug overdose deaths in 2017.

Assessment And Analysis Of H.Pylori Infection Treatment Strategies Of St. Vincent Hospital's Family And Internal Medicine Clinics, Rebecca Orr

Helicobacter pylori is a gram-negative bacteria that is responsible for causing chronic gastritis, ulcers in the stomach and intestine, and eventually even gastric lymphoma or cancer. Multiple treatment options have been recommended for treating an H. pylori infection based on kidney function, previous antibiotic exposure, and whether or not the infection is recurrent. The most common regimens used at St. Vincent's primary care centers closely mimic the American College of Gastroenterology's clinical guidelines. However, these medications (clarithromycin and levofloxacin) have been associated with high rates of resistance in other countries. The United States has very limited data on H. pylori's …
