Introduction: This study is the first nationally representative survey of U.S. adults (18+) to examine perceptions of the relative harms of eight non-cigarette tobacco products.
Methods: Data are from Wave 1 of the Population Assessment of Tobacco and Health (PATH) Study Adult Questionnaire, a nationally representative study of 32,320 adults in the United States conducted from September 2013 to December 2014.
Results: 40.7% of adults believed that electronic cigarettes were less harmful than cigarettes, and 17.8% of adults believed that hookah was less harmful than cigarettes. Those less knowledgeable about the health risks of smoking were more likely to believe …

Every year, and in countries around the world, significant time and resources are devoted to the noble cause of developing drugs to treat and cure human disease. With rare exception, drug interventions cannot reach commercialization without safety and efficacy having first been demonstrated in animal models. The intention of regulations, which require the use of animal models in such contexts, is to ensure that only safe and effective drugs end up being used by patients. The main objective of this research was to develop an o/w nanoemulsion dosage form of insulin for intranasal delivery where insulin is loaded into the oil phase of the nanoemulsion for enhanced absorption. When loaded into the lipid droplets (oil phase), insulin can be protected from enzymatic degradation, can permeate through the mucus gel barrier in a comparatively efficient manner and can be absorbed through transcellular permeation along with paracellular route. Statins are one of the most heavily prescribed medications. The 2018 ACC/AHA guidelines support statin therapy for most older adults, but recommendations are less clear for those over 75. The literature was systematically reviewed for evidence of the efficacy of statin treatment in different patient populations. Significant evidence was found supporting a decreasing association between low-density lipoprotein cholesterol (LDL-C) and mortality as well as decreasing statin efficacy with increasing age. The consequences of unnecessary statin therapy can be severe for older adults. Improved methods for evaluating atherosclerotic cardiovascular disease (ASCVD) risk, such as using negative markers to help identify those …

Purpose/background: Dopamine receptor blocking agents (DRBAs), also known as antipsychotics, are medications widely used to treat a growing number of mental health diagnoses. However, their utility is limited by the potential to cause serious adverse movement reactions. Akathisia, dystonia, parkinsonism, and tardive dyskinesia (collectively known as extrapyramidal symptoms or EPSs) are associated with reduced social and occupational functioning, negative patient attitudes toward treatment, and nonadherence to pharmacotherapy. Neuroleptic malignant syndrome is a life-threatening reaction that can result from DRBA use and cause musculoskeletal dysfunction. Atropine Sulfate (AS) auto-injector (AtroPen®) is being used as an effective and safe antidote for the treatment of organophosphate (OP) pesticides or nerve gas poisoning. The use of AtroPen® is associated with several drawbacks including: bulky size, availability, affordability, invasiveness, and administration errors. Previously, AS fast disintegrating sublingual tablets (FDSTs) were developed and the feasibility of AS sublingual permeability were demonstrated. However, AS permeability was delayed due to the negative impact of higher doses of AS on FDST’s physical characteristics. With contributions from the global natural product (NP) research community, and continuing the Raw Data Initiative, this review collects a comprehensive demonstration of the immense scientific value of disseminating raw nuclear magnetic resonance (NMR) data, independently of, and in parallel with, classical publishing outlets. A comprehensive compilation of historic to present-day cases as well as contemporary and future applications show that addressing the urgent need for a repository of publicly accessible raw NMR data has the potential to transform natural products (NPs) and associated fields of chemical and biomedical research. © 2019 Tremblay et al. Methylphenidate (MPH), commonly known as Ritalin, is the most widely prescribed drug worldwide to treat patients with attention deficit disorders. Although MPH is thought to modulate catecholamine neurotransmission in the brain, it remains unclear how these neurochemical effects influence neuronal activity and lead to attentional enhancements. Studies in rodents overwhelmingly point to the lateral prefrontal cortex (LPFC) as a main site of action of MPH. Limited information is available on how fetal growth retardation (FGR) affects the lung in the neonatal period in males and females. This led us to test the hypothesis that FGR alters lung mechanics and the surfactant system during the neonatal period. To test this hypothesis a model of FGR was utilized in which pregnant rat dams were fed a low protein diet during both the gestation and lactation period. Pharmaceutical companies contribute to the accessibility of medications in developing countries (Cottingham & Berer, 2011). Hot melt extrusion is a versatile continuous manufacturing process extensively investigated for its applications with production of oral solid dosage forms, the aim of this study was to venture the applicability of HME in continuous manufacturing of topical semi-solid ointment formulations. Prescription drugs have been a major topic in the news for much of the past year. There are two issues which appear often: first, the very high prices of new drugs, particularly the "specialty" drugs developed for serious diseases; and second, the time required for FDA approval in relation to the perceived need for earlier access to new therapies for critically ill patients. Direct-to-consumer (DTC) advertising of prescription pharmaceuticals has been the subject of much criticism and the issue has become even more pressing with the Trump administration’s proposal to require the disclosure of prices in DTC ads. In this article I argue that a more powerful approach to the problem of DTC ads would require the disclosure of the effectiveness of the advertised drugs, at least as found in the clinical trials submitted for FDA approval. APOBEC3G (A3G) is a single-stranded DNA (ssDNA) binding protein that restricts the HIV virus by deamination of dC to dU during reverse transcription of the viral genome. A3G has two zinc-binding domains: the N-terminal domain (NTD), which efficiently binds ssDNA, and the C-terminal catalytic domain (CTD), which supports deaminase activity of A3G. Until now, structural information on A3G has been lacking, preventing elucidation of the molecular mechanisms underlying its interaction with ssDNA and deaminase activity. The number of immigrants entering the United States has increased dramatically in the past 30 years, but minorities and ethnic groups are underrepresented in research studies. This study examined first- and second-generation Arab American immigrants in Dearborn, Michigan, using a survey from 65 participants to determine the impact of generation on clinical research participation. Fifty-one (78%) of the respondents were first-generation immigrants, and 14 (22%) were second generation. There was no statistical difference between first- and second-generation immigrants in their willingness to participate in clinical research. The blood brain barrier (BBB) is a barrier in the brain that separates the peripheral blood circulation system from the central nerve system (CNS). The barrier effectively protects the brain from xenobiotics. The BBB serves as a physical barrier through the tight junction of endothelial cells that were found to be 50-100 times tighter than that of normal endothelial cells. Different drug efflux pumps such as P-glycoproteins and multidrug-resistance proteins are found to be overdistributed on the BBB. Helicobacter pylori is a gram-negative bacteria that is responsible for causing chronic gastritis, ulcers in the stomach and intestine, and eventually even gastric lymphoma or cancer. Multiple treatment options have been recommended for treating an H. pylori infection based on kidney function, previous antibiotic exposure, and whether or not the infection is recurrent. The most common regimens used at St. Vincent's primary care centers closely mimic the American College of Gastroenterology's clinical guidelines. สปอร์เห็ดหลินจือมีสารสำคัญออกฤทธิ์กระตุ้นการทำงานของเซลล์ในระบบภูมิคุ้มกัน เซลล์เหล่านี้ใช้เวลาในการฟื้นตัวมากกว่า 9 เดือนหลังจากผู้ป่วยมะเร็งหยุดยาเคมีบำบัด จึงนำมาสู่การศึกษาครั้งนี้โดยมีวัตถุประสงค์เพื่อศึกษาผลของสปอร์เห็ดหลินจือที่มีต่อระบบภูมิคุ้มกันในผู้ป่วยมะเร็งเต้านม ผู้ป่วยมะเร็งลำไส้ใหญ่หรือทวารหนักหลังครบการรักษาด้วยยาเคมีบำบัดในช่วงเดือนพฤศจิกายน พ.ศ. 2561 ถึงธันวาคม พ.ศ. 2562 การศึกษานี้เป็นการศึกษาทางคลินิกแบบสุ่ม ปกปิด 2 ทาง มีการเปรียบเทียบกับกลุ่มควบคุม โดยกลุ่มตัวอย่างที่เข้าร่วมการศึกษาจนครบการศึกษาทั้งสิ้น 44 ราย แบ่งโดยการสุ่มเป็น 2 กลุ่มเท่า ๆ กัน ได้แก่ กลุ่มที่ได้สปอร์เห็ดหลินจือที่ผ่านการกะเทาะขนาด 250 มิลลิกรัมและกลุ่มที่ได้ยาหลอก รับประทานผลิตภัณฑ์ครั้งละ 1 แคปซูล วันละ 3 ครั้ง ก่อนอาหาร เป็นระยะเวลา 8 สัปดาห์ วัดระดับอิมมูโนโกลบูลิน (IgG, IgM, IgA และ IgE) ความสมบูรณ์เม็ดเลือด ค่าการทำงานของตับและไต อาการไม่พึงประสงค์ ณ สัปดาห์ที่ 0, 4 และ 8 รวมไปถึงประเมินคุณภาพชีวิตด้วยแบบประเมิน FACT-G ก่อนและหลังเข้าร่วมการศึกษา พบว่า สัปดาห์ที่ 8 กลุ่มที่ได้สปอร์เห็ดหลินจือมีการเปลี่ยนแปลงของระดับอิมมูโนโกลบูลิน เม็ดเลือดแดง เม็ดเลือดขาวชนิด neutrophil basophil eosinophil monocyte และเกล็ดเลือดไม่แตกต่างอย่างมีนัยสำคัญเมื่อเปรียบเทียบกับกลุ่มที่ได้ยาหลอก ในขณะที่จำนวนเม็ดเลือดขาวทั้งหมดและ absolute neutrophil count ในกลุ่มที่ได้สปอร์เห็ดหลินจือมีระดับเพิ่มขึ้นอย่างมีนัยสำคัญเมื่อเปรียบเทียบกับกลุ่มที่ได้ยาหลอก (p-value= 0.031 และ 0.021 ตามลำดับ) ผู้เข้าร่วมการศึกษาทั้ง 2 กลุ่มมีการเปลี่ยนแปลงของค่าการทำงานของตับและไตอย่างไม่มีนัยสำคัญตลอดระยะเวลาการศึกษา อาการไม่พึงประสงค์ที่พบ เช่น ปากแห้ง คอแห้ง คลื่นไส้ เป็นต้น มีความรุนแรงระดับน้อย นอกจากนี้กลุ่มที่ได้สปอร์เห็ดหลินจือมีคุณภาพชีวิตไม่แตกต่างอย่างมีนัยสำคัญเมื่อเปรียบเทียบกับกลุ่มที่ได้ยาหลอก กล่าวโดยสรุป การรับประทานสปอร์เห็ดหลินจือในขนาดที่ศึกษามีความปลอดภัยสามารถเพิ่มจำนวนเม็ดเลือดขาวทั้งหมดและ ANC ของผู้ป่วยมะเร็งหลังได้รับยาเคมีบำบัดได้ แต่ไม่สามารถเพิ่มระดับอิมมูโนโกลบูลินได้อย่างมีนัยสำคัญ

การวิจัยนี้เป็นการวิจัยเชิงวิเคราะห์แบบเก็บข้อมูลย้อนหลัง เพื่อประเมินการสั่งใช้ยากลุ่มสแตตินในทางปฏิบัติเพื่อป้องกันโรคหัวใจและหลอดเลือด และศึกษาประสิทธิผลการลดระดับไขมันในเลือดของยากลุ่มสแตตินในผู้ติดเชื้อเอชไอวีที่ได้รับยาต้านไวรัส ณ โรงพยาบาลกระทุ่มแบน มีผู้ป่วยเข้าร่วมการวิจัย 551 ราย โดยผู้ป่วยส่วนใหญ่เป็นเพศชาย ร้อยละ 52.5 เมื่อประเมินการสั่งใช้ยากลุ่มสแตตินตามแนวทางเวชปฏิบัติการใช้ยารักษาภาวะไขมันผิดปกติเพื่อป้องกันโรคหัวใจและหลอดเลือด พ.ศ. 2559 มีผู้ป่วยที่ควรได้รับยากลุ่มสแตตินตามคำแนะนำเพื่อป้องกันโรคหัวใจและหลอดเลือด 198 ราย (ร้อยละ 35.9) แต่ได้รับยากลุ่มสแตตินเพียง 37 ราย (ร้อยละ 6.7) และไม่ได้รับยากลุ่มสแตตินสำหรับป้องกันปฐมภูมิและทุติยภูมิ 155 ราย (ร้อยละ 28.1) และ 6 ราย (ร้อยละ 1.1) ตามลำดับ เมื่อจำแนกตามระดับความเสี่ยงต่อการเกิดโรคหัวใจและหลอดเลือดในการป้องกันโรคหัวใจและหลอดเลือดชนิดปฐมภูมิ พบว่า ผู้ป่วยที่ระดับความเสี่ยงปานกลางและสูงถึงสูงมากได้รับการสั่งใช้ยากลุ่มสแตตินไม่ตรงตามคำแนะนำมากกว่าตรงตามคำแนะนำในแนวทางเวชปฏิบัติฯ อย่างมีนัยสำคัญทางสถิติ (P<0.01 และ P<0.01 ตามลำดับ) สำหรับข้อมูลประสิทธิผลของการลดระดับไขมันในเลือดของยากลุ่มสแตตินในผู้ติดเชื้อเอชไอวีที่ระยะเวลา 3, 6 และ 12 เดือน พบว่า กลุ่มผู้ป่วยที่ไม่ได้รับยากลุ่มสแตตินและกลุ่มผู้ป่วยที่ได้รับยากลุ่มสแตตินมีผลต่างของระดับแอลดีแอลในเลือด ซึ่งมีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (P<0.01) ผลจากการศึกษานี้แสดงให้เห็นว่า อัตราการสั่งใช้ยากลุ่มสแตตินเพื่อป้องกันโรคหัวใจและหลอดเลือดในผู้ติดเชื้อเอชไอวีที่ไม่เหมาะสมตามเกณฑ์คิดเป็นร้อยละ 37.7 แม้ว่ายากลุ่มสแตตินจะมีประสิทธิผลในการลดระดับไขมันในเลือดและมีความปลอดภัยในการใช้ร่วมกับยาต้านไวรัส ดังนั้นควรมีการพิจารณาเพิ่มการสั่งใช้ยากลุ่มสแตตินอย่างสมเหตุผลตามเกณฑ์ เพื่อป้องกันความเสี่ยงต่อการเกิดโรคหัวใจและหลอดเลือดในผู้ติดเชื้อเอชไอวี

ยาไมโครฟีโนลิกชนิดมีสารเคลือบเป็นยากดภูมิคุ้มกันใช้ป้องกันการปฏิเสธไตในผู้ป่วยที่ได้รับการปลูกถ่ายไต ยามีความผันแปรภายในตัวบุคคลและระหว่างบุคคลค่อนข้างสูง (high intra-and inter-individual variation) จึงควรตรวจติดตามระดับยาโดยหาจุดเวลาที่เหมาะสมที่สัมพันธ์ดีกับพื้นที่ใต้กราฟ (AUC คือ MPA exposure) รวมทั้งยังไม่มีรายงานการหาจุดตัด (cut-off point) ความเข้มข้นที่เหมาะสมที่บ่งถึงค่า MPA exposure ≥ 30 ไมโครกรัม•ชั่วโมง/มิลลิลิตรในผู้ป่วยปลูกถ่ายไต การศึกษานี้มีวัตถุประสงค์คือหาความสัมพันธ์ของพื้นที่ใต้กราฟกับความเข้มข้นของ MPA และเมทาบอไลท์ทั้ง total และ free form แต่ละจุดเวลา และหาค่าจุดตัด (cut-off point) ความเข้มข้นที่บ่งถึงค่า MPA exposure ≥ 30 ไมโครกรัม•ชั่วโมง/มิลลิลิตร จึงทำการศึกษาในผู้ป่วยปลูกถ่ายไตจำนวน 20 ราย ที่ได้รับยา EC-MPS ขนาด 1,080 มกต่อวัน ร่วมกับทาโคลิมุสและเพรดนิโซโลน เก็บเลือดผู้ป่วยที่ 0 ก่อนรับประทานยาและ 0.5, 1, 1.5, 2, 2.5, 3, 3.5, 4, 6, 8, 10 และ 12 ชั่วโมงภายหลังรับประทานยา EC-MPS ในวันที่ 3 วัดความเข้มข้นของ MPA และเมทาบอไลท์ทั้ง total และ free form ด้วยวิธี LC-MS/MS จากการศึกษาพบว่าจุดเวลาที่ความเข้มข้นสัมพันธ์ดีที่สุดกับพื้นที่ใต้กราฟของ MPA total form C4 (r2 = 0. 50) MPA free form C8 (r2 = 0. 41) MPAG total form C6 (r2 = 0. 97) …
