วัตถุประสงค์: เพื่อพัฒนารายการยาที่มีแนวโน้มไม่เหมาะสมสำหรับพระภิกษุอาพาธสูงอายุ (PIM-BHIKKHU) โดยใช้เทคนิคเดลฟาย และเปรียบเทียบความชุกของการใช้ยาที่มีแนวโน้มไม่เหมาะสมซึ่งประเมินโดยเกณฑ์ที่พัฒนาขึ้นกับ Beers ปี 2023 และ STOPP version 3 ปี 2023 วิธีการศึกษา: ระยะที่ 1 การศึกษาเชิงคุณภาพเพื่อพัฒนารายการ PIM-BHIKKHU โดยใช้เทคนิคเดลฟายกับผู้เชี่ยวชาญรวม 18 คน ประกอบด้วยแพทย์และเภสัชกร ระยะที่ 2 การศึกษาเชิงพรรณนาแบบภาคตัดขวางย้อนหลัง โดยทบทวนรายการยาของพระภิกษุอาพาธที่มีอายุ 65 ปีขึ้นไป และรับการรักษาแบบผู้ป่วยนอก โดยมีรายการยาที่ได้รับร่วมกันอย่างน้อย 5 รายการ จากโรงพยาบาลสงฆ์ ผลการศึกษา: จากกระบวนการเดลฟาย 2 รอบในระยะที่ 1 ได้ข้อยุติเกณฑ์ PIM-BHIKKHU จำนวน 89 รายการยา/กลุ่มยา ในระยะที่ 2 จากพระภิกษุอาพาธสูงอายุ 225 รูป พบความชุกของการใช้ยาที่มีแนวโน้มไม่เหมาะสมตามเกณฑ์ Beers ปี 2023, STOPP version 3 ปี 2023 และ PIM-BHIKKHU ปี 2024 เท่ากับร้อยละ 60.0, 45.33 และ 86.7 ตามลำดับ โดยมีความแตกต่างอย่างมีนัยสำคัญทางสถิติ (p < 0.05) ทั้งนี้ ความชุกของการใช้ PIMs สูงกว่าในกลุ่มอายุ ≥75 ปีเมื่อเทียบกับกลุ่มอายุ 65–74 ปีในทุกเกณฑ์ สรุป: รายการยาที่มีแนวโน้มไม่เหมาะสมที่พัฒนาขึ้น มีความจำเพาะกับบริบทของพระภิกษุอาพาธสูงอายุนิกายเถรวาทในประเทศไทย ทั้งนี้ ควรมีการศึกษาเพิ่มเติมเพื่อประเมินประสิทธิผลทางคลินิกเมื่อมีการใช้ในในเวชปฏิบัติจริง

คุณภาพชีวิตด้านสุขภาพ ภาวะซึมเศร้า และคุณภาพการนอนหลับในผู้ป่วยไทยโรคสมองขาดเลือด, รัตยาภรณ์ ใจกุศล

วัตถุประสงค์: เพื่อประเมินคุณภาพชีวิตด้านสุขภาพ (HRQOL)ในผู้ป่วยโรคสมองขาดเลือดด้วยเครื่องมือ EQ-5D-5L, เพื่อประเมินภาวะซึมเศร้า (PSD) ด้วยเครื่องมือ PSDS-T, เพื่อประเมินคุณภาพการนอนหลับด้วยเครื่องมือ T- PSQI เพื่อหาความสัมพันธ์ระหว่าง HRQOL, PSD, คุณภาพการนอนหลับกับปัจจัยด้านประชากรศาสตร์และทางคลินิก วิธีดำเนินการวิจัย: การศึกษาเชิงวิเคราะห์แบบภาคตัดขวางเก็บข้อมูลจากผู้ป่วยนอกโรคสมองขาดเลือดที่คลินิกโรคหลอดเลือดสมองและอายุรกรรมประสาท 2 โรงพยาบาลจุฬาลงกรณ์ เครื่องมือที่ใช้: 1) EQ-5D-5L ฉบับภาษาไทย 2) PSDS-T 3) T- PSQI ผลการวิจัย: ผู้ป่วยทั้งหมด 360 คน ส่วนใหญ่เป็นเพศชาย(ร้อยละ 54.2) ค่าเฉลี่ยอายุ 64 ± 13.8 ปี และโรคสมองขาดเลือดมีค่าเฉลี่ยระยะเวลา 4.8 ± 3.8 เดือน. การประเมิน HRQOL ด้วยเครื่องมือ EQ-5D-5L พบปัญหาส่วนใหญ่ด้านอาการเจ็บปวด (ร้อยละ 32.0) ค่าเฉลี่ยอรรถประโยชน์ EQ-5D-5L (0.67 ± 0.31) และ EQ-VAS (0.72 ± 0.18) และพบ 5 ปัจจัยที่ส่งผลต่อ HRQOL .การประเมิน PSD ด้วยเครื่องมือ PSDS-T พบความชุก PSD (ร้อยละ 8.1) คุณภาพชีวิตด้านสุขภาพผู้ป่วยที่มีภาวะ PSD แย่กว่าอย่างมีนัยสำคัญ (p<0.001) และพบ 2 ปัจจัยที่สัมพันธ์กับ PSD คือจำนวนยาโรคประจำตัวที่ได้รับ (p=0.035). และการเกิดโรค สมองขาดเลือดซ้ำภายใน 1 ปี (p<0.001). การประเมินคุณภาพการนอนหลับด้วยเครื่องมือ TPSQI พบผู้ป่วยร้อยละ 57.2 คะแนนเฉลี่ย (PSQI>5) 8.21 ± 2.33 คะแนน และพบ 3 ปัจจัยที่ ส่งผลได้แก่ ประวัติการได้รับยานอนหลับ (p<0.001) ระดับความ mRS ≥3 คะแนน (p<0.008) และ PSD (คะแนน PSDS-T>6) (p<0.001). สรุปผลการวิจัย:การเกิดโรคสมองขาดเลือดส่งผลต่อ คุณภาพชีวิตด้านสุขภาพ ภาวะซึมเศร้าหลังเกิดโรคสมองขาดเลือด และคุณภาพการนอนหลับของผู้ป่วย

Exosomes, spherical vesicles secreted by cells containing diverse biomolecules, are gaining attention in veterinary medicine. Mesenchymal stem cell (MSC)-derived exosomes show promise for treating animal diseases. However, their safety profile, particularly the acute toxicity of canine MSC-derived exosomes, has yet to be adequately explored. This study evaluates the acute toxicity of canine MSC-derived exosomes on rat systemic health, adhering to ISO 10993-11 guidelines for systemic toxicity testing to support veterinary drug registration and development. Human interleukin-15 (hIL-15) is a cytokine that plays a crucial role in immune responses like T cell proliferation, activation, and prolongation of natural killer (NK) cells. This cytokine also impacts the cell proliferation of human adult low calcium temperature (HaCaT) keratinocytes which can lead to the wound-healing effect. This study aimed to optimize the expression and characterize the recombinant hIL-15 in Nicotiana benthamiana with the Agrobacterium-mediated transient expression system. Lusianthridin (LST), a bioactive compound isolated from Dendrobium venustum, exhibits notable vasoprotective properties. This study aimed to elucidate the underlying molecular mechanisms of LST by evaluating its effects on platelet function and endothelial cell activation in vitro. In human platelets, LST significantly inhibited collagen-induced aggregation, an effect associated with elevated phosphorylation of vasodilator-stimulated phosphoprotein (VASP) and suppression of Akt phosphorylation. In EA.hy926 endothelial cells, LST attenuated tumor necrosis factor-alpha (TNF-α)-induced endothelial dysfunction by reducing nitric oxide (NO) production and suppressing the expression of pro-inflammatory cytokines, including interleukin-6 (IL-6) and interleukin-1β (IL-1β). Curcumin is widely recognized for its various pharmacological properties, including antioxidant, anti-inflammatory, and anti-tumor activities. Nevertheless, the development of curcumin as a therapeutic agent is impeded by its limited oral bioavailability, which stems from its chemical instability, poor aqueous solubility, and rapid degradation. This study aimed to develop granule formulations incorporating hyaluronic acid graft poly(N-isopropylacrylamide) or HA-g-pNIPAM to enhance dissolution and protect curcumin from degradation. Three formulations were developed: F10 (HA-g-pNIPAM physically mixed with curcumin), F10 Encap (curcumin encapsulated within HA-g-pNIPAM), and F11 (curcumin granules without HA-g-pNIPAM). This study aimed to develop novel positron emission tomography (PET) tracers for C-X-C chemokine receptor type 4 (CXCR4) imaging. [68Ga]Ga-TD-01 and [18F]PICO-F were designed as PET tracers based on the molecular scaffolds of two potent CXCR4 antagonists, TIQ15 and PICO. TIQ15 was used to prepare a bifunctional (BFC) to facilitate 68Ga-labeling while structural modification of PICO was made to obtain a precursor, PICO-Br for 18F-labeling. This study aims to develop multiplex qPCR to detect mycoplasma contamination. Firstly, the specific primers and probes for aligning with specific mycoplasma. were designed from conserved region called 23S rRNA, then, the designed primers and probes were verified their alignment with specific nucleotide of mycoplasma in Bioedit and checked the hairpin, self-complementary, and primer-dimer through Multi Primer Analyzer. After verification, the primers and probes do not have hairpin, self-complementary, and primer-dimer. Besides, the suitable temperature for annealing is 58oC. วัตถุประสงค์: เพื่อหาปัจจัยนำ (Predisposing factors) ที่สัมพันธ์กับการเกิดภาวะกรดคั่งในเลือดจากสารคีโตน (Diabetic ketoacidosis; DKA) ในผู้ป่วยชาวไทยโรคเบาหวานชนิดที่ 2 ที่ใช้ยาในกลุ่ม Sodium glucose co-transporter 2 inhibitors (SGLT-2 inhibitors) วิธีการศึกษา: งานวิจัยนี้เป็นการศึกษาจากผลไปหาเหตุแบบเก็บข้อมูลย้อนหลังในหลายศูนย์ รวบรวมข้อมูลผู้ป่วยโรคเบาหวานชนิดที่ 2 ที่ได้รับยา SGLT-2 inhibitors ณ โรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทยและโรงพยาบาลราชวิถี ช่วง 1 ตุลาคม 2557 ถึง 30 เมษายน 2567 ผลการศึกษา: กลุ่มตัวอย่างจำนวน 310 ราย แบ่งเป็นกลุ่มศึกษาคือผู้ป่วยที่เกิดภาวะ DKA 62 ราย (คิดเป็นความชุกของการเกิดภาวะ DKA ณ ช่วงเวลาที่ศึกษาเท่ากับร้อยละ 1.37) และกลุ่มควบคุมคือผู้ป่วยที่ไม่เกิดภาวะ DKA 248 ราย ส่วนปัจจัยนำที่มีความสัมพันธ์อย่างมีนัยสำคัญทางสถิติกับการเกิดภาวะ DKA ได้แก่ การติดเชื้อ (Adjusted OR = 28.31, P < 0.001) การปรับลดขนาดยาอินซูลินภายใน 1 เดือน (Adjusted OR = 5.58, P = < 0.001) และการมีประวัติภาวะ DKA ภายใน 1 ปี (Adjusted OR = 8.76, P = 0.038) สรุปผล: ปัจจัยที่มีผลต่อการเกิดภาวะ DKA ในผู้ป่วยโรคเบาหวานชนิดที่ 2 ที่ใช้ยา SGLT-2 inhibitors ได้แก่ การติดเชื้อ การปรับลดขนาดอินซูลินภายใน 1 เดือน และการมีประวัติภาวะ DKA ภายใน 1 ปี ผู้ป่วยที่มีปัจจัยดังกล่าวจึงควรได้รับการดูแลติดตามอย่างใกล้ชิด เพื่อช่วยลดความเสี่ยงในการเกิดภาวะ DKA และเพิ่มความปลอดภัยจากการใช้ยา SGLT-2 inhibitors

N-glycosylation is a crucial post-translational modification that significantly affects immune recognition. The plant-based platform, especially Nicotiana benthamiana, offers scalable and cost-effective production of therapeutic proteins with glycosylation patterns similar to mammalian cells. However, plant-specific N-glycans, such as α(1-3)-fucose and β(1-2)-xylose (XF), have the potential to trigger allergic reactions. This study investigates how various N-glycan structures on the RBD-Fc fusion protein influence the immune response in mice. Cobra (Naja kaouthia) venom contains many toxins including α-neurotoxin (α-NTX) and phospholipase A2 (PLA2), which can cause neurodegeneration, respiratory failure, and even death. The conventional antivenom produced by animal immunization faces many challenges and limitations. Heavy-chain-only antibodies (HCAb), fusing VHH with human IgG Fc region, offer advantages in tissue penetration, antigen binding, and extended half-life. Botulism is an uncommon but serious neurological condition caused by the botulinum toxin produced by the gram-positive bacterium named Clostridium botulinum. Common symptoms of botulism include muscle cramps, nausea, vomiting, diarrhea, difficulty swallowing, respiratory failure, muscle weakness, and potentially death. Currently, two types of antitoxin are used: equine-derived heptavalent antitoxin and human-derived immunoglobulin (BabyBIG®). Carpaine, a major alkaloid present in Carica papaya leaves, has been shown to increase platelet counts in patients suffering from thrombocytopenia. Numerous commercial papaya leaf products are available, but few provide proper bioactive ingredient information. We present herein a technique for rapid screening of carpaine in these products using DART-MS. The results indicate that carpaine was detected in various forms (powder, solution) of papaya leaves. กรดแกลลิกเป็นสารกลุ่มกรดฟีนอลิก มีฤทธิ์ทางเภสัชวิทยาที่ดีแต่มีข้อจำกัดเนื่องจากจะถูกดูดซึมและถูกกำจัดออกจากร่างกายอย่างรวดเร็วหลังจากเข้าสู่กระแสเลือดรวมถึงมีความคงตัวต่ำ ปัจจุบันจึงมีการศึกษาระบบนำส่งยารูปแบบต่างๆ รวมถึงไฮโดรเจล เพื่อนำมาประยุกต์ใช้ในการนำส่งกรดแกลลิก งานวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาการควบคุมการปลดปล่อยกรดแกลลิกจากไฮโดรเจลผสมระหว่างคาร์บอกซีเมทิลเซลลูโลสและเซริซิน โครงสร้างสัณฐานของไฮโดรเจลที่ถูกเตรียมจากคาร์บอกซีเมทิลเซลลูโลสและเซริซิน ที่อัตราส่วน 1:1 และ 1:2 โดยปริมาตร และเชื่อมขวางด้วยสารละลายกลูตารัลดีไฮด์ 4 เปอร์เซ็นต์โดยปริมาตร และขึ้นรูปด้วยวิธีแช่แข็ง-ละลาย โดยมีรูพรุนสม่ำเสมอและเชื่อมโยงกันเป็นโครงร่างสามมิติ จากผลค่าโมดูลัสกักเก็บ พบว่าค่าโมดูลัสกักเก็บของไฮโดรเจลมีค่าสูงขึ้นเมื่อสัดส่วนของเซริซินเพิ่มขึ้น ผลการย่อยสลายได้ทางชีวภาพพบว่าอัตราการย่อยสลายลดลงเมื่อเพิ่มสัดส่วนของเซริซิน ชี้ให้เห็นว่าสัดส่วนของเซริซินส่งผลต่อความแข็งแรง ความยืดหยุ่น และความคงรูปของไฮโดรเจล และเมื่อนำไฮโดรเจลไปดูดซับกรดแกลลิกเพื่อศึกษาการควบคุมการปลดปล่อย พบว่ากรดแกลลิกถูกดูดซับบนไฮโดรเจลในอัตราการดูดซับที่สูงกว่าร้อยละ 80 และพบว่ากรดแกลลิกถูกปลดปล่อยออกมาอย่างรวดเร็วในระยะเวลา 15 นาที ถึง 2 ชั่วโมงแรก จากนั้นจะค่อย ๆ ปลดปล่อยออกมาลดลงเรื่อย ๆ โดยมีการปลดปล่อยอยู่ในช่วงร้อยละ 65 รวมทั้งพบว่ากรดแกลลิกถูกปลดปล่อยในสภาวะที่เป็นเบส ได้มากกว่าในสภาวะที่เป็นกลางและกรด โดยกลไกการปลดปล่อยกรดแกลลิกสามารถอธิบายได้ด้วยสมการเภสัชจลนศาสตร์แบบ Kopcha ชี้ให้เห็นว่าการปลดปล่อยอาศัยการแพร่และการย่อยสลายของไฮโดรเจล และเมื่อศึกษาการบวมน้ำของไฮโดรเจลพบว่าสอดคล้องกับอัตราการปลดปล่อยของกรดแกลลิก เมื่อทดสอบความเข้ากันได้ทางชีวภาพต่อเซลล์ในระดับห้องปฏิบัติการพบว่าไฮโดรเจลคาร์บอกซีเมทิลเซลลูโลสและเซริซินที่บรรจุกรดแกลลิกไม่มีความเป็นพิษต่อเซลล์ไฟโบบลาสต์และเคราติโนไซท์ของมนุษย์ และไม่มีผลกระตุ้นกระบวนการอักเสบภายในเซลล์แมคโครฟาจของหนูเมาส์ และเมื่อนำไฮโดรเจลที่เตรียมขึ้นไปทดสอบการยับยั้งการสร้างอนุมูลอิสระในเซลล์ไฟโบบลาสต์และเคราติโนไซท์ของมนุษย์ พบว่ากรดแกลลิกที่ถูกปลดปล่อยจากไฮโดรเจลสามารถยับยั้งการสร้างอนุมูลอิสระได้ จากผลการวิจัยสามารถสรุปได้ว่า ไฮโดรเจลคาร์บอกซีเมทิลเซลลูโลสและเซริซินที่เตรียมขึ้นสามารถนำไปประยุกต์ใช้ในการดูดซับและควบคุมการปลดปล่อยกรดแกลลิกที่มีผลต่อการยับยั้งการสร้างอนุมูลอิสระในเซลล์ไฟโบบลาสต์และเคราติโนไซท์ของมนุษย์ในระดับห้องปฏิบัติการได้อย่างมีประสิทธิภาพโดยที่ไม่ทำลายฤทธิ์ทางเภสัชวิทยาของกรดแกลลิก

Given the high prevalence of self-medication among children, drug regulatory agencies should ensure the accessibility of non-prescription drugs for children's safety, as many commonly used drugs are not entirely non-prescription. This study explores the drug classification status of commonly used self-medication drugs for children across 10 countries: the US, the UK, Canada, Japan, Australia, Singapore, Malaysia, Thailand, Indonesia, and the Philippines. Information was sourced from official drug regulatory agency websites and drug index databases. ผิวกระจ่างใสนั้นเป็นหนึ่งในเป้าหมายหลักของผู้หญิงในอุตสาหกรรมเครื่องสำอางในระดับโลก โดยมีผู้บริโภคจำนวนมากที่เริ่มหันมานิยมใช้ผลิตภัณฑ์จากสารสกัดธรรมชาติมากขึ้น โดยมีรายงานว่ามีพืชบางชนิดเช่น ชะเอมเทศและกลุ่มของเบอร์รี่มีประสิทธิภาพทำให้ผิวขาวขึ้น จุดมุ่งหมายของการศึกษานี้เพื่อศึกษาประสิทธิภาพของสารสกัดจากหม่อน สารสกัดจากต้นปอสาและสารสกัดจากชะเอมเทศ ที่มีการเพาะปลูกในประเทศไทยเพื่อนำไปใช้เป็นสารให้ความขาวในเครื่องสำอาง โดยในการศึกษาครั้งนี้เลือกสารสกัดทั้ง 3 ชนิดที่สามารถเพาะปลูกได้ในจังหวัดเชียงใหม่ ประเทศไทยและยังเป็นพืชในอุตสาหกรรมต่างๆแล้วได้นำมาสกัดด้วยการหมักเอทานอล ซึ่งในงานวิจัยนี้ได้ทำการศึกษาองค์ประกอบของพฤกษเคมีรวม ปริมาณฟีนอลิกทั้งหมด ปริมาณแทนนินทั้งหมด และปริมาณฟลาโวนอยด์ทั้งหมด รวมไปถึงฤทธิ์การต้านเอนไซม์ไทโรซิเนสในหลอดทดลองของสารสกัดเดี่ยวและสารสกัดผสม จากผลการศึกษาพบว่าปริมาณฟีนอลิกรวมและปริมาณแทนนินรวมสูงสุดพบได้ในสารสกัดหม่อนความเข้มข้น 5% โดยมีค่าเท่ากับ 0.590 mg GAE/g และ 0.606 mg TAE/g ตามลำดับ และปริมาณฟลาโวนอยด์รวมสูงสุดพบได้ในสารสกัดต้นปอสาความเข้มข้น 5% โดยมีค่าเท่ากับ 0.682 mg QE/g ในบรรดาพืชทั้ง 3 ชนิดพบว่าสารสกัดจากชะเอมเทศที่ความเข้มข้น 5% มีฤทธิ์ยับยั้งเอนไซม์ไทโรซิเนสสูงที่สุดทั้งสารเดี่ยวและสารผสม ตามด้วยใบปอสา และใบหม่อนซึ่งมีเปอร์เซ็นต์ยับยั้งเอนไซม์ไทโรซิเนสเท่ากับ 88.84, 87.05 และ 24.41 % ตามลำดับและจากการทดสอบกับสารผสมพบว่าเมื่อนำมาผสมกันในอัตราส่วน 5:0,4:1,3:2,2:3,1:4 และ0:5 พบว่าไม่มีสารใดที่มีประสิทธิภาพเหนือกรดโคจิก(96.83%) และยังพบว่าที่สารเดี่ยวความเข้มข้น 5% มีประสิทธิภาพใกล้เคียงกับกรดโคจิกยกเว้นสารสกัดใบหม่อน จากการทดลองมีแนวโน้มเป็นไปได้ว่ามีการออกฤทธิ์แบบต้านกันอาจเกิดมาจากความคงตัวของสารที่มาจากธรรมชาติส่งผลให้การทดลองอาจจะมีการคลาดเคลื่อน อย่างไรก็ตามงานวิจัยนี้แสดงให้เห็นว่าสารสกัดเหล่านี้มีความสามารถยับยั้งเอนไซม์ไทโรซิเนส

Cymensifin A, a compound isolated from the orchid Cymbidium ensifolium, has shown promise as a novel chemotherapeutic agent in treating lung cancer. This study investigated its anticancer activity against human lung cancer cells, focusing on its ability to target p53-related pathways. The dose-dependent induction of apoptosis, as assessed by nuclear staining and Annexin V/PI flow cytometry, was shown in human lung cancer cells cultured with 5–50 μM cymensifin A for 24 h. Self-medication practice was found among children either self-practice or with caregiver assistance. This study aimed to assess the prevalence and patterns of self-medication practice in children. This study utilized systematic review and meta-analysis by searching papers published during 2013 to 2022 in PubMed, ScienceDirect, Scopus, and Embase. Mathurameha (MT) is a Thai traditional herbal formulation consisting of 26 herbs used for diabetes over 30 years. The study was to identify inhibitory compounds in Mathurameha against pancreatin amylase using the HPTLC-bioautographic technique. The results showed that MT inhibited pancreatin amylase activities, and the most active compounds were identified as andrographolide and gallic acid. The hypoglycemic effect of MT was determined using cell-based assays in L6 myotubes, 3T3-L1 adipocytes, and Caco2 cells. Objective: This systematic review aimed to describe how people store and dispose of unused and expired medicines, explore the extent of proper storage and disposal and examine factors influencing these behaviors. A comprehensive search of five databases, PubMed, Scopus, Springer Link, Science Direct, and EBSCO. The QualSyst assessment tool was applied to evaluate the quality of the included studies. The review encompassed 55 eligible studies from 27 countries, involving 31,246 participants.
