The aim of this research was to develop a nanogel formulation-based on modified natural polymer, hyaluronic acid (HA), as a biodegradable material for adjuvant delivery. Polyinosinic:polycytidylic acid (poly(I:C)) have been approved by FDA as a promising adjuvant candidate for the TLR3 (Toll-Like Receptor 3) activation. However, it suffers from being poor stability and is subjected to rapid enzymatic hydrolysis in serum, so that it requires high administered dose leading to adverse effects. Vancomycin pharmacokinetics has been described by 1- and 2-compartment models. One-compartment models built from routine monitoring data, which were mainly trough samples, are commonly used to predict area under the curves (AUC), the useful indicator for vancomycin efficacy. The question stands whether AUCs from 1-compartment models with sparse data can sufficiently represent the true AUC. This study aimed to compare AUCs from 1- and 2-compartment models using sparse data. A previously published model was used to simulate full individual profiles for 100 patients. Human epidermal growth factor (hEGF) is a short polypeptide that has gained clinical importance in the recent decade for wound healing. Currently, plant-based expression system is considered as an alternative viable platform for low-cost recombinant protein production. Hence, this study aims to produce hEGF in Nicotiana benthamiana plants via., transient expression using a geminiviral vector. The hEGF gene constructs were designed into six different constructs. The optimal expression was observed from the construct targeting the endoplasmic reticulum with C-terminal histidine tag at the yield level up to 15.695 µg/g LFW or 0.499% TSP. Anti-RANKL monoclonal antibody is a fully human monoclonal antibody (mAb) available for treatment of osteoporosis. In this study, anti-RANKL mAb was transiently expressed using the Geminiviral expression system in Nicotiana benthamiana. Moreover, the structure of plant-produced mAb was characterized and functional activity was also determined. The result indicated that anti-RANKL mAb was produced with the maximum expression level on 8 days post infiltration and estimated to be 0.5 mg/g leaf fresh weight. The incidence of metastasis stage crucially contributes to high recurrence and mortality rate in lung cancer patients. Unfortunately, no available treatment inhibits migration, a key metastasis process in lung cancer. In this study, the effect of 22-O-(N-Boc-L-glycine) ester of renieramycin M (22-Boc-Gly-RM), a semi-synthetic amino ester derivative of bistetrahydroisoquinolinequinone alkaloid isolated from Xestospongia sp., on migratory behavior of human lung cancer cells was investigated. This work was divided into three study phases: the two-phase randomized controlled clinical trials and proteomics study. First, the phase I clinical trial aimed to determine efficacy and safety of mulberry leaves on postprandial glucose following the 50-g sucrose ingestion in healthy nondiabetic adults and to explore the optimal administered dose of 1-deoxynojirimycin (DNJ), the major antihyperglycemic compound of mulberry leaves. The results showed the alleviation of postprandial hyperglycemia by mulberry leaves in a dose-dependent fashion. Adverse effects of mulberry leaves included bloating and flatulence, loose stool, and nausea. In addition, 12 mg of DNJ was considered the optimal dose …

The genus Derris belonging to Fabaceae family are distributed along the equatorial zone in Africa and Asia. In Thailand, 16 species of Derris species can be found throughout the country. Derris elliptica (Hanglai-Dang), is well-known among fishermen and gardeners as fish poison and insecticide, especially the stem and root which contain isoflavonoid compound named rotenone. สปอร์เห็ดหลินจือมีสารสำคัญออกฤทธิ์กระตุ้นการทำงานของเซลล์ในระบบภูมิคุ้มกัน เซลล์เหล่านี้ใช้เวลาในการฟื้นตัวมากกว่า 9 เดือนหลังจากผู้ป่วยมะเร็งหยุดยาเคมีบำบัด จึงนำมาสู่การศึกษาครั้งนี้โดยมีวัตถุประสงค์เพื่อศึกษาผลของสปอร์เห็ดหลินจือที่มีต่อระบบภูมิคุ้มกันในผู้ป่วยมะเร็งเต้านม ผู้ป่วยมะเร็งลำไส้ใหญ่หรือทวารหนักหลังครบการรักษาด้วยยาเคมีบำบัดในช่วงเดือนพฤศจิกายน พ.ศ. 2561 ถึงธันวาคม พ.ศ. 2562 การศึกษานี้เป็นการศึกษาทางคลินิกแบบสุ่ม ปกปิด 2 ทาง มีการเปรียบเทียบกับกลุ่มควบคุม โดยกลุ่มตัวอย่างที่เข้าร่วมการศึกษาจนครบการศึกษาทั้งสิ้น 44 ราย แบ่งโดยการสุ่มเป็น 2 กลุ่มเท่า ๆ กัน ได้แก่ กลุ่มที่ได้สปอร์เห็ดหลินจือที่ผ่านการกะเทาะขนาด 250 มิลลิกรัมและกลุ่มที่ได้ยาหลอก รับประทานผลิตภัณฑ์ครั้งละ 1 แคปซูล วันละ 3 ครั้ง ก่อนอาหาร เป็นระยะเวลา 8 สัปดาห์ วัดระดับอิมมูโนโกลบูลิน (IgG, IgM, IgA และ IgE) ความสมบูรณ์เม็ดเลือด ค่าการทำงานของตับและไต อาการไม่พึงประสงค์ ณ สัปดาห์ที่ 0, 4 และ 8 รวมไปถึงประเมินคุณภาพชีวิตด้วยแบบประเมิน FACT-G ก่อนและหลังเข้าร่วมการศึกษา พบว่า สัปดาห์ที่ 8 กลุ่มที่ได้สปอร์เห็ดหลินจือมีการเปลี่ยนแปลงของระดับอิมมูโนโกลบูลิน เม็ดเลือดแดง เม็ดเลือดขาวชนิด neutrophil basophil eosinophil monocyte และเกล็ดเลือดไม่แตกต่างอย่างมีนัยสำคัญเมื่อเปรียบเทียบกับกลุ่มที่ได้ยาหลอก ในขณะที่จำนวนเม็ดเลือดขาวทั้งหมดและ absolute neutrophil count ในกลุ่มที่ได้สปอร์เห็ดหลินจือมีระดับเพิ่มขึ้นอย่างมีนัยสำคัญเมื่อเปรียบเทียบกับกลุ่มที่ได้ยาหลอก (p-value= 0.031 และ 0.021 ตามลำดับ) ผู้เข้าร่วมการศึกษาทั้ง 2 กลุ่มมีการเปลี่ยนแปลงของค่าการทำงานของตับและไตอย่างไม่มีนัยสำคัญตลอดระยะเวลาการศึกษา อาการไม่พึงประสงค์ที่พบ เช่น ปากแห้ง คอแห้ง คลื่นไส้ เป็นต้น มีความรุนแรงระดับน้อย นอกจากนี้กลุ่มที่ได้สปอร์เห็ดหลินจือมีคุณภาพชีวิตไม่แตกต่างอย่างมีนัยสำคัญเมื่อเปรียบเทียบกับกลุ่มที่ได้ยาหลอก กล่าวโดยสรุป การรับประทานสปอร์เห็ดหลินจือในขนาดที่ศึกษามีความปลอดภัยสามารถเพิ่มจำนวนเม็ดเลือดขาวทั้งหมดและ ANC ของผู้ป่วยมะเร็งหลังได้รับยาเคมีบำบัดได้ แต่ไม่สามารถเพิ่มระดับอิมมูโนโกลบูลินได้อย่างมีนัยสำคัญ

การวิจัยนี้เป็นการวิจัยเชิงวิเคราะห์แบบเก็บข้อมูลย้อนหลัง เพื่อประเมินการสั่งใช้ยากลุ่มสแตตินในทางปฏิบัติเพื่อป้องกันโรคหัวใจและหลอดเลือด และศึกษาประสิทธิผลการลดระดับไขมันในเลือดของยากลุ่มสแตตินในผู้ติดเชื้อเอชไอวีที่ได้รับยาต้านไวรัส ณ โรงพยาบาลกระทุ่มแบน มีผู้ป่วยเข้าร่วมการวิจัย 551 ราย โดยผู้ป่วยส่วนใหญ่เป็นเพศชาย ร้อยละ 52.5 เมื่อประเมินการสั่งใช้ยากลุ่มสแตตินตามแนวทางเวชปฏิบัติการใช้ยารักษาภาวะไขมันผิดปกติเพื่อป้องกันโรคหัวใจและหลอดเลือด พ.ศ. 2559 มีผู้ป่วยที่ควรได้รับยากลุ่มสแตตินตามคำแนะนำเพื่อป้องกันโรคหัวใจและหลอดเลือด 198 ราย (ร้อยละ 35.9) แต่ได้รับยากลุ่มสแตตินเพียง 37 ราย (ร้อยละ 6.7) และไม่ได้รับยากลุ่มสแตตินสำหรับป้องกันปฐมภูมิและทุติยภูมิ 155 ราย (ร้อยละ 28.1) และ 6 ราย (ร้อยละ 1.1) ตามลำดับ เมื่อจำแนกตามระดับความเสี่ยงต่อการเกิดโรคหัวใจและหลอดเลือดในการป้องกันโรคหัวใจและหลอดเลือดชนิดปฐมภูมิ พบว่า ผู้ป่วยที่ระดับความเสี่ยงปานกลางและสูงถึงสูงมากได้รับการสั่งใช้ยากลุ่มสแตตินไม่ตรงตามคำแนะนำมากกว่าตรงตามคำแนะนำในแนวทางเวชปฏิบัติฯ อย่างมีนัยสำคัญทางสถิติ (P<0.01 และ P<0.01 ตามลำดับ) สำหรับข้อมูลประสิทธิผลของการลดระดับไขมันในเลือดของยากลุ่มสแตตินในผู้ติดเชื้อเอชไอวีที่ระยะเวลา 3, 6 และ 12 เดือน พบว่า กลุ่มผู้ป่วยที่ไม่ได้รับยากลุ่มสแตตินและกลุ่มผู้ป่วยที่ได้รับยากลุ่มสแตตินมีผลต่างของระดับแอลดีแอลในเลือด ซึ่งมีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (P<0.01) ผลจากการศึกษานี้แสดงให้เห็นว่า อัตราการสั่งใช้ยากลุ่มสแตตินเพื่อป้องกันโรคหัวใจและหลอดเลือดในผู้ติดเชื้อเอชไอวีที่ไม่เหมาะสมตามเกณฑ์คิดเป็นร้อยละ 37.7 แม้ว่ายากลุ่มสแตตินจะมีประสิทธิผลในการลดระดับไขมันในเลือดและมีความปลอดภัยในการใช้ร่วมกับยาต้านไวรัส ดังนั้นควรมีการพิจารณาเพิ่มการสั่งใช้ยากลุ่มสแตตินอย่างสมเหตุผลตามเกณฑ์ เพื่อป้องกันความเสี่ยงต่อการเกิดโรคหัวใจและหลอดเลือดในผู้ติดเชื้อเอชไอวี

Paclitaxel is recommended first-line therapy for patients with advanced non-small cell lung cancer. Genetic polymorphisms of Paclitaxel’s metabolic pathway may affect treatment outcomes and adverse events. The purpose of this study was to determine the association of genetic polymorphisms and clinical outcomes. A total of 58 Thai NSCLC patients with Paclitaxel-based chemotherapy were enrolled for this cohort study. The prevalence of variant allele of SLCO1B3 (rs7311358), ABCB1 (rs2032582) and CYP3A5 (rs776746) were 72.4%, 40.5% and 67.5%, respectively. A total of 831 moderately halophilic bacteria isolated from various samples in Thailand were screened for their lipolytic activity. Three hundred and forty-nine isolates showed activity on agar plate while 322 isolates exhibited in broth. The grouping and identification of 322 isolates based on DNA-fingerprinting using (GTG)5+ERIC2 primer and 16S rRNA gene sequence, 180 DNA patterns could be generated. The result indicated that the representative isolates belonged to genera Bacillus, Halobacillus, Lentibacillus, Marinobacter, Thallossobacillus, and Virgibacillus. Lusianthridin is a phenanthrene derivative isolated from Dendrobium venustum. Some phenanthrene compounds have antiplatelet aggregation activities via undefined pathways. The objective of this study is to determine the inhibitory effects and potential mechanisms of lusianthridin on platelet aggregation. Turbidometric aggregometry method was used to measure the extent of platelet aggregation, then cyclic adenosine monophosphate (cAMP) levels were measured by ELISA kit. Inhibitory effect of lusianthridin on cyclooxygenase enzymes was analyzed by COX fluorescent inhibitor screening assay kit. Chemical investigation of two orchids native to Thailand led to the isolation of eighteen compounds. From the whole plants of Cymbidium finlaysonianum Wall. ex Lindl, a new dihydrophenanthrene, 1-(4-hydroxybenzyl)-2,7-dihydroxy-4,6-dimethoxy-9,10-dihydrophenan-threne, and nine known stilbenoids, including a phenanthrenequinone, cymbinodin-A, a dihydrophenanthrenequinone, ephemeranthoquinone B, a bibenzyl, batatasin III, two phenanthrenes, 2,4-dimethoxy-3,7-dihydroxyphenanthrene and 3,7-dihydroxy-2,4,6-trimethoxyphenanthrene, and four dihydrophenanthrenes i.e. coelonin, 6-methoxycoelonin, flavanthridin and lusianthridin, were obtained. Indomethacin (INDO) and diclofenac (DIC) cause intestinal cell death through reactive oxygen species (ROS), endoplasmic reticulum stress (ER stress) and mitochondrial dysfunction. This study investigated the cytoprotective effects of certain natural polyphenolic antioxidants against INDO- and DIC-mediated Caco-2 cell death as well as underlying mechanisms. วัตถุประสงค์: ประเมินผลของการใช้แบบสอบถาม PROMPT ในการให้บริบาลทางเภสัชกรรม ในด้านการสื่อสารเรื่องคุณภาพชีวิตด้านการใช้ยาระหว่างเภสัชกรและผู้ป่วย การค้นหาปัญหาจากการใช้ยา การติดตามผลการแก้ไขปัญหาจากการใช้ยา ความร่วมมือในการใช้ยา การเปลี่ยนแปลงแนวทางการรักษาผู้ป่วยของแพทย์ และคุณภาพชีวิตด้านการใช้ยา วิธีการวิจัย: การทดลองแบบสุ่มที่มีกลุ่มควบคุม ในผู้ป่วยนอกจำนวน 315 คน ที่เข้ารับการรักษาที่โรงพยาบาลสมุทรปราการระหว่างเดือนสิงหาคม 2561 ถึง กุมภาพันธ์ 2562 แบ่งผู้ป่วยเป็น 2 กลุ่มคือ กลุ่มมาตรฐาน (กลุ่มที่ไม่ใช้แบบสอบถาม PROMPT ในการบริบาลทางเภสัชกรรม) และกลุ่มศึกษา (กลุ่มที่ใช้แบบสอบถาม PROMPT ในการบริบาลทางเภสัชกรรม) โดยมีการเก็บข้อมูลจำนวน 2 ครั้ง คือ ก่อนการแทรกแซกและหลังการแทรกแซงเมื่อผู้ป่วยมาพบแพทย์ในครั้งถัดไป ผลการวิจัย: คะแนนการสื่อสารของผู้ป่วยทั้ง 2 กลุ่ม มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p < 0.001) กลุ่มศึกษาพบปัญหาจากการใช้ยามากกว่ากลุ่มมาตรฐาน (p<0.05) และมีคะแนน คุณภาพชีวิตด้านการใช้ยาดีขึ้นอย่างมีนัยส าคัญทางสถิติเมื่อเทียบกับกลุ่มมาตรฐาน (p<0.01) สรุปผลการวิจัย: การให้บริบาลทางเภสัชกรรมโดยใช้แบบสอบถาม PROMPT ช่วยเพิ่มการ สื่อสารเรื่องคุณภาพชีวิตการใช้ยาระหว่างผู้ป่วยกับเภสัชกร และช่วยให้คุณภาพชิวิตด้านการใช้ยาของ ผู้ป่วยดีขึ้นได้

Near-Infrared Spectroscopy (NIRs) technique was used to quantitatively analyzed 5 mg amlodipine tablets. The aim of this study was to develop calibration model by NIRs technique to accurately determine content uniformity of amlodipine tablets during commercial scale production. Amlodipine tablets were initially manufactured in laboratory scale to generate calibration model with concentrations ranging from 80% to 120% labeled amount using NIRs analysis with spectral range of 4000 –10000 cm-1. Mixture Of Medium Chain Fatty Acids For Protein Precipitation In Purification Process Of Equine Anti-Rabies Immunoglobulin As F(Ab′)2, Sirisak Sathorn

Besides the widely used caprylic acid (C8) as protein precipitating agent, capric acid (C10) and lauric acid (C12) were investigated as alternative medium chain fatty acids (MCFAs) for their capabilities on protein purification. The optimization process conducted with normal bovine plasma as a model antibody demonstrated the essential effect of temperature during filtration to significantly reduce the content of albumin, a contaminant marker, compared with filtration at room temperature (p<0.05). From SDS-PAGE analysis, the optimal concentrations of 3% C8 and 5% C10 increased IgG purity from 30.2% to 94.6% and 80.4%, respectively. Interestingly, precipitation by C10 provided the lowest turbidity of filtrates. Purification by C12 showed unsatisfied results that albumin content was minutely decreased when increasing C12 concentration. C8 and C10 were then variously combined into 2 levels of 3% and 5% binary mixture for plasma purification. The synergist effect of C8 and C10 mixtures was clearly demonstrated in albumin elimination that the purity of fractionates were increased to 95%. Moreover, binary mixtures with higher concentration of C10 improved the filtration velocity in plasma model. Selected C8:C10 mixtures were then applied for both ERIG-IgG and ERIG-F(ab′)2 purification. SDS-PAGE analysis confirmed the absence of albumin protein in all purified samples. For ERIG-IgG purification, C8:C10 mixture at ratio 1.5:3.5 demonstrated the best in homogeneity with high recovery rate of bioactive antibody, while the mixtures at 2.25:0.75 showed the best purification capabilities to obtain highly purified ERIG-F(ab′)2. Furthermore, the purification by C8:C10 mixtures displayed superior process characteristic than the commercially non-chromatographic technique for ERIGF(ab′)2 purification in terms of antibody recovery, purity and lower in aggregate formation. The novelty on beneficial use of MCFA mixture as protein precipitating agent for antibody purification has been demonstrated.

At present, many compounds targeting on Mcl-1 are currently under different clinical stages of development for cancer treatment. Herein, we report the Mcl-1 targeting activity of renieramycin T (RT), an alkaloid isolated from the blue sponge Xestospongia sp. RT was shown to target on Mcl-1 through ubiquitin-proteasomal degradation in lung cancer cells including chemotherapeutic resistant primary lung cancer cells.
