Stress Reduction: A Neighborhood Introduction, Barbara D. Johnson

Master's Projects

Responding to reported health concerns a stress reduction program was developed for residents of an urban Latino neighborhood struggling with poverty, crime, and limited access to healthcare. The revised Health Promotion Model (HPM) was used as a framework (Pender, 1996). A qualitative study testing a 5-week Mindfulness-based Stress Reduction (MBSR) intervention was imbedded into the program 18 months after its implementation. Utilizing a pre/post test design participants were predicted to demonstrate significant reductions in post-intervention mean scores for 3 self-report measures, the Cohen Perceived Stress Scale, General Sleep Disturbance Scale, and a modified Memorial Symptom Assessment Scale. Due to mine cross cultural living, (I was born in the U.S. but grew in Southeast Asia), I became very interested in the Anthological, Social and Scientific difference between cultures. This led to a historical meta evaluation of humanity in general. Using the principals of Noetic Science, this evaluation reviled the ignorance and down right absence of any educational opportunities to learn the true functional abilities of mankind.

Impact Of An Adherence Program On The Health And Outlook Of Hiv-Infected Patients Failing Antiretroviral Therapy, Michael F. Parry, Pamela Wright, Julie G. Stewart, Gavin Mcleod, James Tucker, Amy R. Weinberg

Nursing Faculty Publications

Background: We prospectively studied the impact of an adherence counselor on the outcome of patients failing antiretroviral therapy because of nonadherence. Methods: Forty-six patients, identified as chronically nonadherent were enrolled. Individual attention was provided using the information, motivation and behavioral methodology. HIV RNA (viral load, in copies/mL), CD4 count (in cells/[mm.sup.3]), and body weight before and after the adherence counselor were measured. Qualitative outcome and patient satisfaction were assessed by deidentified third-party interviews. Results: Over half completed at least 1 year; only 8 patients were lost to follow-up. Mean CD4 counts increased significantly (P < .05) for completers at 6 and 12 months. Viral loads decreased between baseline and 6 months. Most clients reported subjective benefit from working with the adherence counselor. Conclusion: Although few clients showed complete virologic suppression, the value of an adherence counselor was validated. Longer term adherence programs should be evaluated.

Community needs assessment and client and community surveys were used to define the problems. Patient care flowcharting and the creation of an electronic patient database facilitated patient tracking across the entire community. Clinical guidelines and a consultation and referral immunology clinic standardized care practices. Measurable improvements in the quality of care were noted in multiple areas. Flowchart completion rates rose from 44% to 100%; medication adherence assessment rose from 82% …

The study was designed to answer four research questions: 1). What is the validity and reliability of the Personal Respiratory Protection Survey (PRPS)? 2). How frequently do workers in SCBs wear PRP? 3). How do demographics, occupational histories, and self-reported respiratory health histories influence the use of PRP in SCBs? and 4). What are the relationships of the constructs (benefits, barriers, susceptibility, severity, norms) to the use of PRP by workers in SCBs? …

Using Standardized Patients To Teach And Evaluate Nurse Practitioner Students On Cultural Competency, Carolyn M. Rutledge, Laurel Garzon, Micah Scott, Karen Karlowicz

Ellmer School of Nursing Faculty Publications

With the increasing diversity in the American population, it is imperative that nurse practitioners learn to manage patients with varying healthcare beliefs and needs. In order to develop culturally competent nurse practitioners, a number of methods have been developed. Many of the current methods focus on improving the awareness and knowledge of nurse practitioners regarding diverse populations. However, very few of the current programs focus on improving the skills and increasing the encounters the students have with diverse populations. This paper focuses on providing nurse practitioner students with diverse encounters using culturally enhanced standardized patient scenarios. The standardized patient programs …

Providing Education And Support For Rural Palliative Care Nurses In Western Australia: An Intervention Study, Ruth Mcconigley

Theses: Doctorates and Masters

Rural palliative care nurses (RPCNs) in Western Australia (WA) are a small population, who have embraced the specialist palliative care role since the first palliative care service in rural WA began in 1990. However, there are some challenges involved with being a specialist nurse in a generalist health care setting. fu particular, there is a suggestion in the literature that RPCNs may be professionally isolated, may be prone to burnout, and may therefore experience decreased job satisfaction and may consider leaving the palliative care field.

Functional Health Literacy In An Urban Primary Care Clinic, Nancy Trygar Artinian, M. Patricia Lange, Thomas Templin, Lynda G Stallwood, Christopher E. Hermann

Nursing Faculty Research Publications

The purpose of this study was to determine the level of functional health literacy in a sample of patients treated in 1of our 5 primary care clinics. A total of 92 randomly selected adults (mean age = 59 years) completed the TOFHLA while waiting for a scheduled appointment. Twenty eight percent of the sample had less than adequate levels of functional health literacy. Income, car ownership and education were significant and independent predictors of literacy level in this sample. Low levels of functional health literacy limit a patient's ability to read, understand and act on health information. It is essential …
