Obesity and inactivity are prevalent among central Appalachian adolescents. Appalachian residents have been labeled "fatalistic," a time perspective unsupportive of health-promoting behaviors such as regular participation in physical activity. The theory of planned behavior has been used extensively to explain the physical activity behaviors of adolescents. Constructs within the theory of planned behavior include attitudes toward the behavior, subjective norms, perceived behavioral control, and intention to perform the behavior. New graduate registered nurses (RN) are facing a difficult time finding jobs since the economy of the United States has declined. Since then the California Institute of Nursing and Health Care, along with funding from the Betty Moore Foundation, have developed RN transition programs. These programs were developed to increase skills, confidence, and experience among new RN graduates to promote their transition into the nursing workforce. The programs also were designed to retain newly licensed nurses in the nursing profession while engaging competencies that could be transferred to both acute and outpatient care settings. Objective: The objective of this study was to test the feasibility, acceptability, and preliminary clinical outcomes of a method to leverage existing home healthcare telemonitoring technology to deliver depression care management (DCM) to both Spanish- and English-speaking elderly homebound recipients of homecare services.
Materials and Methods: Three stand-alone, nonprofit community homecare agencies located in New York, Vermont, and Miami participated in this study. Evidence-based DCM was adapted to the telemonitor platform by programming questions and educational information on depression symptoms, antidepressant adherence, and side effects. Poor Women With Sexually Transmitted Infections: Providers’ Perspectives On Diagnoses, Genevieve R. Cox

This article presents results from a study of health care providers, mainly nurses and nurse practitioners, who routinely diagnose sexually transmitted diseases (STDs) in rural low-income populations in West Virginia (WV). A qualitative analysis of eighteen semi-structured interviews reveals that providers who consistently work with low-income populations believe patients undergo a negative change in self-image in response to a chronic STD diagnosis. Providers express concerns about a number of issues related to low-income, rural women’s access to sexual health care and see the need for more sexuality education, more funding for free and reduced cost clinics, and more available health …

Objetivou-se neste estudo realizar uma discussão de caráter introdutório sobre a análise de algumas características demográficas e ocupacionais de estudantes-trabalhadores de enfermagem e suas relações com o riscode acidentes de trabalho. Bullying is a form of violence characterized as an aggressive behavior that is unprovoked and intended to cause harm. Prior studies have found that lesbian, gay, bisexual, and transgender (LGBT) youth experience high levels of bullying related to their sexuality and this harassment can lead to engagement in risk behaviors, depression, and suicide. Ethnic/racial minority young men who have sex with men (YMSM) may experience dual levels of stigma and maltreatment due to both their sexuality and their race. The average reported age of sexual debut for youth in the United States is 14.4 years, with approximately 7% reporting their sexual debut prior to age 13. While the research literature on sexual debut for youth addresses gender and ethnic differences (with males and African-American youth experiencing earlier sexual debut), there is limited data regarding factors associated with sexual debut for young men who have sex with men (YMSM). Early sexual debut poses potential health risks, such as contracting HIV with an increased risk of unprotected intercourse. Sexual Behaviors Of Racial/Ethnic Minority Young Men Who Have Sex With Men., Sheldon Fields

We assessed changes in sexual behaviors from baseline to 12-month follow-up among a multisite cohort of HIV-positive racial/ethnic minority young men who have sex with men enrolled in an outreach, linkage, and retention study. In the 3 months prior to their baseline interview, more than three-quarters of participants (78.5%) reported sex with at least one man (mean: 2.3 partners). Among sexually active participants, 44.2% had one partner; 50.5% had 2-9 partners; and 5.3% had 10 or more partners. Over three-quarters (77.5%) reported engaging in sex with at least one steady partner, 43.5% with at least one casual partner, and 29.5% …

The federal government has established rapid identification, linkage, and engagement in medical care of HIV-positive individuals as a high priority. Outreach workers and other linkage coordinators are identified as key personnel in implementing this policy. Young racial/ethnic minority men who have sex with men (MSM) have relatively high and growing rates of HIV infection and would benefit from the services of outreach workers. This study compared workplace protection factors (WPFs) for five different contaminants (endotoxin, fungal spores, (1→3)-β-D-glucan, total particle mass, and total particle number) provided by an N95 elastomeric respirator (ER) and an N95 filtering facepiece respirator (FFR). We previously reported size-selective WPFs for total particle numbers for the ER and FFR, whereas the current article is focused on WPFs for bioaerosols and total particle mass. Farm workers (n = 25) wore the ER and FFR while performing activities at eight locations representing horse farms, pig barns, and grain handling facilities. Background: In Australia approximately 25% of Emergency Department (ED) attendances are via ambulance. ED overcrowding in Australia, as in many countries, is common. Measures to reduce overcrowding include the provision of enhanced timely primary care in the community for appropriate low risk injury and illness. Therefore paramedic assessment and referral to a community home hospital service, in preference to transfer to ED, may confer clinical and cost benefit.

Despite recent advances in healthcare, Streptococcus pneumonia-associated infections contribute to significant morbidity and mortality in older adults. Because of the pneumococcal vaccine’s efficacy for preventing invasive disease, guidelines for vaccinating older adults have been developed; yet, the nation has not met immunization goals. The purpose of this EBP project was to determine if a telephone reminder, as compared to provider recommendation, would increase pneumococcal vaccination rates. The Iowa model and Kotter’s eight stages of change guided this project within a multi-provider office in Northwest Indiana. This project measured the effect of a clinical algorithm on the provision of care to medically uninsured adult patients with type 2 diabetes and a low socioeconomic status (SES). Primary providers often fail to implement established standards for diabetes care to their maximal benefit and do not achieve American Diabetes Association (ADA) treatment standards. Saydah, et al. (2004) reported that only 48% of patients with diabetes achieved the recommended HbA1c goal, and 33% reached blood pressure and LDL targets. Goals for all three clinical parameters were obtained by only 7% of patients. The Stetler Model of Evidence-based Practice (EBP) provided …

Hospital-acquired infections cost hospitals approximately $30.5 billion per year and also result in longer hospital stays, chronic conditions, and even death with associated malpractice costs. According to the Centers for Disease Control and Prevention, hand hygiene is a simple, effective way to prevent illness and infection. The purpose of this research was to determine if visitors to a neonatal intensive care unit (NICU) in a regional medical center comply with hand-washing policies and procedures. O estudo teve como objeto analisar algumas características da produção bibliográfica sobre a Qualidade de Vida (QV) de portadores de Feridas Crônicas (FC) no Brasil entre 2000 e 2009. Para tanto, realizou-se uma revisão sistemática da literatura através da busca de estudos indexados nas bases de dados da BIREME. Os materiais encontrados e utilizados foram sete artigos, uma dissertação e uma tese. Destes, a maioria dos artigos se concentrou no ano de 2001 e 2006, correspondendo a 50% das publicações levantadas. People who are experiencing homelessness have the highest rates of morbidity, and the lowest age of mortality in Canada. The importance of health care relationships in addressing the needs of people who are experiencing homelessness has been demonstrated. However, two gaps still exist in the literature. Firstly, the dialectical relationship between space/place and social relations has been neglected in the context of health care and homelessness. This was a mixed design study that assessed the caregivers' perception of their role while caring for patients in the household model. A sample of 12 caregivers currently employed in a long-term care facility operating in the household model was asked to complete a survey and answer 11 questions during a one on one interview. This interview tool allowed the caregivers the opportunity to rate their personal experiences, environment and express their feelings associated with their role as a certified nursing assistant working in a household model. Through a culturally responsive partnership process, a doctoral project to advance nursing education on the Pine Ridge Indian Reservation in Pine Ridge, South Dakota is presented. The collaborative partnership is explored across a dominant Western culture and an Indigenous tribal community. A lens of awareness for White dominant culture is explained in relation to building a partnership with a Lakota tribal community. Traditional Indigenous values of mutuality, reciprocity, honor, and respect are discussed as significant in building a partnership. Objetivou-se analisar o estilo de vida de estudantes de enfermagem por meio de um estudo exploratório, descritivo e com abordagem quantitativa. Verificou-se que os estudantes ingeriam apenas dois grupos de alimentos (“verduras e frutas” ou “verduras e grãos”); 54,2% não praticava atividade física; 57,8% deles dormiam menos de sete horas/ dia; havia grande proporção de estudantes-trabalhadores; 9,9% de fumantes e a frequência do uso de tabaco foi de 52,6% fumando de 2 a 5 cigarros/ dia, e 26,3% fumando de 1 a 3 maços de cigarros/ semana; 27,1% de estudantes etilistas e destes, 42,3% desses fazem o uso de álcool …

The purpose of the study was to explore the difference in problem areas perceived and reported by patients with type 2 diabetes and registered nurses caring for patients with type 2 diabetes. Diabetes affects a large number of patients both physically and fiscally. Decreasing perception of problems by nurse case management intervention has the potential to adversely affect patients. A convenience sample of 20 patients and 10 nurses was obtained and completed the Problem Areas in Diabetes (PAID) questionnaire. Statistical analysis found a significant difference in perception of problems among patients and nurses.

The purpose of the study was to identify the effects of the emergency department length of stay on clinical outcomes for critically ill or injured patients in the regional referral community hospital. Delays in transfer have been associated with increased mortality rates, prolonged length of stays, and poor outcomes. Emergency departments are designed to respond to immediate life or limb threatening emergencies.
