Background: Individuals with dementia are often burdened with behavioral problems, which augment health care costs and burden of disease. Treatment of behavioral symptoms with pet therapy is acknowledged to be emotionally beneficial. Robotic pets have demonstrated similar promising effects without the undesirable outcomes of traditional pet therapy. Robotic pet therapy offers an effective replacement to traditional pet therapy.

Aim: This project aimed to assess the effectiveness of the STOPP/START tool in identifying and reducing potentially inappropriate medications among multi-morbid, older, long-term care residents in a metropolitan area of a Southwestern state.

Design: In this quality improvement project, seven prescribers working within four long-term care facilities were trained to assess for potentially inappropriate medications using the STOPP/START tool. The medication lists for long-term care residents (n =45) over age 65 were collected at baseline and again at three months post-intervention and assessed for overall medication use, psychotropics, and proton pump inhibitors. Purpose/Background Delirium is an acute syndrome characterized by disturbances in cognition occurring in critically ill patients, particularly those over 65 in the intensive care unit (ICU). Numerous risk factors are associated with delirium development, with sleep being a modifiable factor. This review explores existing literature regarding the relationship between sleep deprivation and delirium development while evaluating the effectiveness of a sleep promotion protocol to decrease the incident of delirium in those ICU patients 65 years or older.

Abstract

Care management (CM) services offered in primary care settings are used to increase patient engagement in self-care and improve communication between patients and health care providers. The aim of this article is to describe the creation of an evidenced-based toolkit within an urban health center that addresses the structures, processes, and outcome measures necessary for the development of a sustainable CM program. Based on the findings from an organizational assessment and comprehensive literature synthesis, several policies, procedures, and workflow diagrams of processes within the health center were created to guide the implementation of a CM program.

Phenomenological studies have been critiqued when analytic activities and intersection with the underpinning philosophy lack clarity. This methodological discussion paper describes data analysis in hermeneutic interpretive phenomenology. The term “silent epidemic” is fitting for Alzheimer’s disease (AD), as its negative impact is widely felt but rarely discussed. Burnout among AD caregivers has become an epidemic of its own as caregivers experience an increase in health risks, stress, and financial burden. This literature review focuses on caregiver burnout and how imperative it is that caregivers are better supported in their role. Researchers have developed instruments to assess and intervene in caregiver burnout that have shown effectiveness among caregivers and their families.Nevertheless, further longitudinal research is warranted regarding more effective interventions, including stress management and social support mechanisms.

Person-centered care (PCC) is considered the standard to assure quality of care and quality of life in longterm care, benefiting both residents and staff. This study examines the associations between nursing home staff perceptions of person-centered care practices, the organizational system, and work-related attitudes in a sample of 340 nurses and direct care workers across 32 nursing homes in Oregon. Random-intercepts regression models were used to estimate within- and between-nursing home variation in staff perceptions of PCC practices as measured by the Staff Assessment of Person-Directed Care (SA-PDC), and identify characteristics associated with these perceptions. Purpose: Physical performance measures, like walking speed, identify and predict preclinical mobility disability but are rarely used in routine medical care. A preventive model of care called Mobility Checkup is being designed to reduce mobility disability in older adults. This study had two purposes: 1) determine feasibility and outcomes of the Mobility Checkup, and 2) identify preferences of older adults regarding this model of care using a discrete choice experiment.

การวิจัยครั้งนี้เป็นการศึกษาความสัมพันธ์เชิงบรรยายเพื่อศึกษาปัจจัยคัดสรรที่สัมพันธ์กับพฤติกรรมการรับประทานอาหารของผู้ป่วยกลุ่มอาการภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลัน กลุ่มตัวอย่าง คือ ผู้ป่วยกลุ่มอาการภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลัน จำนวน 195 คน คัดเลือกกลุ่มตัวอย่างด้วยวิธีการสุ่มแบบหลายขั้นตอน จากผู้ป่วยที่มารับบริการ ณ แผนกผู้ป่วยนอกระบบหัวใจและหลอดเลือด โรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย โรงพยาบาลตำรวจ และโรงพยาบาลศิริราช เก็บข้อมูลโดยใช้แบบสอบถามทั้งหมด 7 ส่วน คือ 1) แบบบันทึกข้อมูลส่วนบุคคล 2) แบบสอบถามความรู้เรื่องโรคหัวใจขาดเลือด 3) แบบสอบถามการรับรู้การเจ็บป่วย 4) แบบสอบถามแรงจูงใจ 5) แบบสอบถามการสนับสนุนทางสังคม 6) แบบสอบถามการรับรู้สมรรถนะแห่งตน และ 7) แบบสอบถามพฤติกรรมการรับประทานอาหาร ตรวจสอบความตรงตามเนื้อหาโดยผู้ทรงคุณวุฒิ 5 คน มีค่าดัชนีความตรงตามเนื้อหาเท่ากับ 0.94, 1.0, 1.0, 0.97, 1.0 และ 0.95 ตามลำดับ และตรวจสอบความเที่ยงของเครื่องมือเท่ากับ .76, .80, .73, .91, .96 และ .87 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้วิธีการทดสอบค่าสัมประสิทธิ์สหสัมพันธ์เพียร์สัน ผลการวิจัยสรุปได้ดังนี้ 1. ผู้ป่วยกลุ่มอาการภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันมีคะแนนเฉลี่ยพฤติกรรมการรับประทานอาหารเฉพาะโรค ความรู้ แรงจูงใจ การสนับสนุนทางสังคม และการรับรู้สมรรถนะแห่งตนอยู่ในระดับสูง (Mean = 100.98 SD = 12.39, Mean = 16.48 SD = 2.16, Mean = 76.03 SD = 8.63, Mean = 67.43 SD = 9.09 และ Mean = 25.73 SD = 3.79 ตามลำดับ) และมีคะแนนการรับรู้การเจ็บป่วยระดับปานกลาง (Mean = 49.81 SD …

การวิจัยกึ่งทดลองครั้งนี้ มีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมการช่วยเลิกบุหรี่ต่อพฤติกรรมการเลิกบุหรี่ในผู้ที่เป็นวัณโรคปอด กลุ่มตัวอย่างคือ ผู้ป่วยวัณโรคปอดที่สูบบุหรี่อย่างน้อย 1 มวนในช่วง 1 สัปดาห์ที่ผ่านมา เข้ารับการรักษาที่คลินิกวัณโรค โรงพยาบาลระดับตติยภูมิแห่งหนึ่ง กรุงเทพมหานคร คัดเลือกกลุ่มตัวอย่างแบบเจาะจงตามคุณสมบัติที่กำหนด จำนวน 60 คน กำหนดให้ 30 คนแรกเป็นกลุ่มควบคุม และ 30 คนหลังเป็นกลุ่มทดลอง จับคู่กลุ่มตัวอย่างให้มีลักษณะคล้ายคลึงกันในเรื่องอายุและระดับการติดนิโคติน กลุ่มควบคุมได้รับการพยาบาลตามปกติ ในขณะที่กลุ่มทดลอง ได้รับการพยาบาลตามปกติและโปรแกรมการช่วยเลิกบุหรี่แบบสั้น เป็นเวลา 8 สัปดาห์ เก็บรวบรวมข้อมูลโดยใช้แบบประเมินพฤติกรรมการเลิกบุหรี่ และแบบสอบถามสถานะการสูบบุหรี่ ประเมินการเลิกบุหรี่จากการเลิกสูบบุหรี่ได้ติดต่อกันในช่วง 7 วันก่อนระยะเวลาประเมินผลที่ 2 เดือน วิเคราะห์ข้อมูลโดยใช้ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน สถิติทดสอบไคสแควร์ สถิติทีและสถิติซี ผลการทดลองพบว่า 1. คะแนนเฉลี่ยพฤติกรรมการเลิกบุหรี่ของผู้ป่วยวัณโรคปอดที่สูบบุหรี่ภายหลังการเข้าร่วมโปรแกรมการช่วยเลิกบุหรี่แบบสั้น สูงกว่าก่อนเข้าร่วมโปรแกรม อย่างมีนัยสําคัญทางสถิติที่ระดับ .05 2. คะแนนเฉลี่ยพฤติกรรมการเลิกบุหรี่ของผู้ป่วยวัณโรคปอดที่สูบบุหรี่ภายหลังการเข้าร่วมโปรแกรมการช่วยเลิกบุหรี่แบบสั้น สูงกว่ากลุ่มควบคุมอย่างมีนัยสําคัญทางสถิติ (p<.05) 3. เมื่อประเมินที่ระยะเวลา 2 เดือนหลังการทดลอง พบว่า ผู้ป่วยวัณโรคปอดที่สูบบุหรี่ในกลุ่มทดลอง เลิกบุหรี่ได้ 12 คน ส่วนกลุ่มควบคุม เลิกบุหรี่ได้ 3 คน อัตราการเลิกบุหรี่ในช่วง 7 วันก่อนประเมินผล ในกลุ่มทดลองสูงกว่ากลุ่มควบคุม อย่างมีนัยสำคัญทางสถิติ (p<.05)

การศึกษาครั้งนี้เป็นการศึกษาบรรยายเชิงเปรียบเทียบเพื่อศึกษาภาพลักษณ์ของผู้ป่วยก่อนและหลังผ่าตัดทำทางเบี่ยงหลอดเลือดหัวใจ กลุ่มตัวอย่าง คือ ผู้ป่วยที่ได้รับการผ่าตัดทำทางเบี่ยงหลอดเลือดหัวใจ จำนวน 320 คน คัดเลือกกลุ่มตัวอย่างด้วยวิธีการสุ่มแบบหลายขั้นตอนจากผู้ป่วยที่เข้ารับบริการ ณ แผนกผู้ป่วยนอก โรงพยาบาลตติยภูมิ เขตพื้นที่กรุงเทพมหานคร จำนวน 3 โรงพยาบาล เก็บรวบรวมข้อมูลโดยใช้แบบสอบถาม 2 ส่วน คือ 1) แบบบันทึกข้อมูลส่วนบุคคล 2) แบบสอบถามภาพลักษณ์ ผ่านการตรวจสอบความตรงเชิงเนื้อหาโดยผู้ทรงคุณวุฒิ 5 คน ค่าสัมประสิทธิ์แอลฟาครอนบาค ของแบบสอบถามภาพลักษณ์ได้เท่ากับ 0.88 วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา สถิติทดสอบทีและสถิติการวิเคราะห์ความแปรปรวนแบบวัดซ้ำ ผลการวิจัยสรุปได้ดังนี้ 1. ภาพลักษณ์ของผู้ป่วยก่อนการผ่าตัดทำทางเบี่ยงหลอดเลือดหัวใจ จากการศึกษาครั้งนี้พบว่า กลุ่มตัวอย่างมีภาพลักษณ์ก่อนการผ่าตัดทำทางบี่ยงหลอดเลือดหัวใจอยู่ในระดับดีมาก โดยค่าเฉลี่ยคะแนนภาพลักษณ์เท่ากับ 13.10 ± 4.14 คะแนน 2. ภาพลักษณ์ของผู้ป่วยหลังการผ่าตัดทำทางเบี่ยงหลอดเลือดหัวใจ จากการศึกษาครั้งนี้พบว่า กลุ่มตัวอย่างมีภาพลักษณ์หลังการผ่าตัดทำทางบี่ยงหลอดเลือดหัวใจอยู่ในระดับดี โดยค่าเฉลี่ยคะแนนภาพลักษณ์เท่ากับ 19.49 ± 6.61 คะแนน 3. เมื่อเปรียบเทียบภาพลักษณ์ก่อนและหลังผ่าตัดทำทางเบี่ยงหลอดเลือดหัวใจ พบว่า ภาพลักษณ์ก่อนและหลังผ่าตัดทำทางเบี่ยงหลอดเลือดหัวใจมีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05 (t = -24.16, df = 319, p = 0.00) 4. เมื่อพิจารณา 7 ตัวแปร ได้แก่ ตัวแปรเพศ อายุ อาชีพ โรคร่วม ดัชนีมวลกาย สถานภาพสมรสและระดับการศึกษา พบว่าภาพลักษณ์ก่อนและหลังผ่าตัดทำทางเบี่ยงหลอดเลือดหัวใจมีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05 (F = 544.484, F = 136.410, F = 199.973, F = 183.928, F = 165.042, F = 391.982, F = 361.773 ตามลำดับ)

การวิจัยกึ่งทดลองนี้ ศึกษาผลของโปรแกรมการสอนแนะที่เน้นการออกกำลังกายแบบนิวโรบิกส์ต่อความจำและการทำหน้าที่ด้านการบริหารจัดการของผู้ป่วยโรคหลอดเลือดสมอง กลุ่มตัวอย่าง คือ ผู้ป่วยโรคหลอดเลือดสมองชนิดขาดเลือดระยะหลังเฉียบพลัน ทั้งเพศชายและหญิง อายุ 40 ปี ขึ้นไป ที่เข้ารับการรักษา ณ Siriraj Acute Stroke Unit โรงพยาบาลศิริราช จำนวน 34 คน แบ่งเป็นกลุ่มทดลอง และกลุ่มควบคุม กลุ่มละ 17 คน กลุ่มทดลองได้รับโปรแกรมการสอนแนะที่เน้นการออกกำลังกายแบบนิวโรบิกส์ ที่พัฒนาจากแนวคิดการสอนแนะของ Girvin (1999) ร่วมกับแนวคิดการออกกำลังกายแบบนิวโรบิกส์ของ Katz and Rubin (1999) 3 ครั้งต่อสัปดาห์ เป็นเวลา 4 สัปดาห์ และกลุ่มควบคุมได้รับการพยาบาลตามปกติ ระหว่างการวิจัยมีกลุ่มควบคุม 1 คน ถูกคัดออกจากการศึกษา จึงเหลือกลุ่มตัวอย่างทั้งหมด 33 คน ประเมินความจำ ด้วยแบบประเมินพุทธิปัญญา และประเมินการทำหน้าที่ด้านการบริหารจัดการ ด้วยแบบทดสอบการทำหน้าที่ของสมองกลีบฟรอนทรัล ค่าความเที่ยงของทั้งสองเครื่องมือ จากการหาความสอดคล้องภายใน ได้ค่าสัมประสิทธิ์แอลฟาของครอนบาค เท่ากับ .82 เท่ากัน วิเคราะห์ข้อมูลด้วย Descriptive Statistics, Pearson Chi-square, Fisher's Exact test และ t-test ผลการวิจัยสรุปได้ ดังนี้ ค่าเฉลี่ยคะแนนความจำและการทำหน้าที่ด้านการบริหารจัดการหลังทดลองของทั้งสองกลุ่มสูงกว่าก่อนทดลองอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 และค่าเฉลี่ยคะแนนความจำและการทำหน้าที่ด้านการบริหารจัดการหลังทดลองของกลุ่มที่ได้รับโปรแกรมสูงกว่ากลุ่มที่ได้รับการพยาบาลตามปกติอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 ผลการวิจัยนี้แสดงให้เห็นว่า โปรแกรมการสอนแนะที่เน้นการออกกำลังกายแบบนิวโรบิกส์ สามารถช่วยให้ความจำและการทำหน้าที่ด้านการบริหารจัดการหลังโรคหลอดเลือดสมองมีการฟื้นตัวเพิ่มมากขึ้น พยาบาลและบุคลากรทางสุขภาพสามารถนำไปประยุกต์ใช้เพื่อส่งเสริมการฟื้นตัวด้านความจำและการทำหน้าที่ด้านการบริหารจัดการในผู้ที่เป็นโรคหลอดเลือดสมองได้

ปัจจัยคัดสรรที่สัมพันธ์กับกลุ่มอาการขาอยู่ไม่สุขขณะพักของผู้ป่วยไตวายเรื้อรังระยะสุดท้ายที่ได้รับการฟอกเลือดด้วยเครื่องไตเทียม, เบญจมาศ แสงสว่าง

การวิจัยครั้งนี้เป็นการวิจัยแบบหาความสัมพันธ์ มีวัตถุประสงค์เพื่อศึกษาความสัมพันธ์ระหว่างระยะเวลาในการฟอกเลือด โรคร่วม ดัชนีมวลกาย ระดับแคลเซียม ระดับฮีโมโกลบิน อาการนอนไม่หลับ ภาวะซึมเศร้า และภาวะหยุดหายใจขณะนอนหลับจากการอุดกั้นทางเดินหายใจ และกลุ่มอาการขาอยู่ไม่สุขขณะพักของผู้ป่วยไตวายเรื้อรังระยะสุดท้ายที่ได้รับการฟอกเลือดด้วยเครื่องไตเทียม กลุ่มตัวอย่างคือ ผู้ป่วยไตวายเรื้อรังระยะสุดท้ายที่ได้รับการฟอกเลือดด้วยเครื่องไตเทียมที่เข้ารับการรักษาที่โรงพยาบาลพระนครศรีอยุธยา โรงพยาบาลสระบุรี จำนวน 101 คน เลือกกลุ่มตัวอย่างโดยวิธีการสุ่มอย่างง่าย เครื่องมือที่ใช้ในการวิจัยคือ แบบสอบถามข้อมูลส่วนบุคคล แบบสอบถามภาวะสุขภาพ แบบบันทึกข้อมูลผลการตรวจทางห้องปฏิบัติการที่เกี่ยวข้อง แบบสอบถามความรุนแรงของกลุ่มอาการขาอยู่ไม่สุขขณะพัก แบบสอบถามอาการนอนไม่หลับ แบบสอบถามภาวะซึมเศร้า และแบบสอบถามภาวะหยุดหายใจขณะนอนหลับจากการอุดกั้นทางเดินหายใจ แบบสอบถามได้ผ่านการตรวจสอบความตรงตามเนื้อหาโดยผู้ทรงคุณวุฒิ 5 ท่าน ตรวจสอบความเที่ยงเท่ากับ 0.84, 0.89, 0.89 และ 0.73 ตามลำดับ วิเคราะห์ข้อมูลโดยการหาค่าความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน ค่าสัมประสิทธิ์สหสัมพันธ์ของเพียร์สัน สเปียร์แมน และอีต้า ผลการวิจัย สรุปได้ดังนี้ 1. ความรุนแรงของกลุ่มอาการขาอยู่ไม่สุขขณะพักของผู้ป่วยไตวายเรื้อรังระยะสุดท้ายที่ได้รับการฟอกเลือดด้วยเครื่องไตเทียมอยู่ในระดับรุนแรงปานกลาง (x=13.91, SD=9.74) 2. โรคเบาหวาน ดัชนีมวลกาย อาการนอนไม่หลับ ภาวะซึมเศร้า และภาวะหยุดหายใจขณะนอนหลับจากการอุดกั้นทางเดินหายใจมีความสัมพันธ์ทางบวกกับกลุ่มอาการขาอยู่ไม่สุขขณะพักผู้ป่วยไตวายเรื้อรังระยะสุดท้ายที่ได้รับการฟอกเลือดด้วยเครื่องไตเทียมอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05 (r=0.249, 0.213, 0.521, 0.416 และ 0.222 ตามลำดับ) โรคความดันโลหิตสูง โรคไขมันในเลือดสูง ระยะเวลาในการฟอกเลือด ระดับแคลเซียม ระดับฮีโมโกลบินไม่มีความสัมพันธ์กับทางบวกกับกลุ่มอาการขาอยู่ไม่สุขขณะพักผู้ป่วยไตวายเรื้อรังระยะสุดท้ายที่ได้รับการฟอกเลือดด้วยเครื่องไตเทียมอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05
