Abstract
A multi-prong falls awareness program to reduce the occurrence of falls in a skilled nursing unit
Keywords: Fall awareness program, skilled nursing unit, fall rate
The project goal was to reduce the patient fall rate by 20% by implementing a fall awareness program for clinical staff. The program had two prongs and began with a pre-survey to determine current fall prevention knowledge. The staff was encouraged to participate in the training by providing risk factors that contribute to falls and simple strategies to prevent falls based on their knowledge of the patients (Boushon et al, 2012). A post-survey was …

Interdisciplinary Team Approach In Fall Prevention: Physician Perspective Focus, Anna Gryn
Interdisciplinary Team Approach In Fall Prevention: Physician Perspective Focus, Anna Gryn
Master's Projects and Capstones
An in-hospital fall is a devastating event for patients and care providers resulting in injuries, physiological and psychological patient declines, and sometimes deaths. Furthermore, fall related costs greatly increase hospitals’ expenses and cause massive distress to caregivers and health providers. Many patient-specific, organizational, and seemingly erratic factors contribute to the occurrence of in-hospital falls. Despite this, hospitals must undertake consistent evidence-based measures to prevent fall occurrences as much as possible.
The traditional nursing approach to falls prevention is not sufficient to control fall rates because the issue is too complex and must be approached from multiple perspectives rather than just …

Individualized Glycemic Targets For The Elderly Residing In Long Term Care Facilities, Abegail Hubberd
Doctor of Nursing Practice
Of people age 65 and older residing in a long-term care facility, 1 in 4 has Type 2 diabetes (Smide & Nygren, 2013). Care and management of this population is a challenge for nurses and other providers. Results of a needs assessment in a local long-term care facility identified deficiencies in the provision of evidence-based Type 2 diabetes care for this target population. American Diabetes Association Guidelines (2017) recommend application of individualized HbA1c targets for the elderly residing in long-term care facilities, especially for those with compromises in clinical and functional status. This two prong quality improvement project strove to …

Heart failure is a complex, debilitating, chronic disease characterized by high mortality and frequent hospital readmissions. Heart failure affects 5.7 million people in the United States with 670,000 new patients diagnosed each year. Patients are most vulnerable during the transition from the acute care setting to home. Limited patient knowledge on symptom recognition leads to detrimental outcomes for the patients; therefore, discharge education is paramount for decreasing readmissions. The national benchmark for 30-day readmission is 27%. The purpose of this quality improvement project was to improve compliance with the 2013 American Heart Association guidelines through discharge education by Telemetry staff …

Grace Peterson Nursing Research Colloquium
Background: Of the huge influx of retirees expected in the next 10 years, the vast majority prefers aging in place; a concept commonly used to refer to the practice of remaining in a community-based setting as the changes of life unfold with age. Extant health systems currently fail to provide the structure to promote staying at home when increasing health care assistance or monitoring is needed. Clinical pilot studies using telehealth are providing evidence for improving patient healthcare provision and outcomes from the setting of the patient’s choice. As the landscape of healthcare changes with technological advances, nursing practice also …
