วัตถุประสงค์: เพื่อเปรียบเทียบอาการซึมเศร้าของพระสงฆ์ที่อาพาธด้วยโรคจิตเภทก่อนและหลังได้รับโปรแกรมกลุ่มจิตบำบัดแบบประคับประคอง

รูปแบบการวิจัย: การวิจัยกึ่งทดลอง แบบศึกษากลุ่มเดียววัดแบบอนุกรมเวลา

วิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ พระสงฆ์อาพาธที่ได้รับการวินิจฉัยว่าเป็นโรคจิตเภทตามคู่มือการวินิจฉัยตามเกณฑ์การวินิจฉัยโรคทางจิตเวช (ICD-10) ที่เข้ารับการบริการรักษาที่กลุ่มงานพยาบาลจิตเวชผู้ป่วยนอกของโรงพยาบาลสงฆ์ ซึ่งมีคุณสมบัติตามเกณฑ์จำนวน 15 รูป ได้รับการดูแลตามโปรแกรมกลุ่มจิตบำบัดแบบประคับประคองที่ผู้วิจัยสร้างขึ้นเป็นระยะเวลา 6 สัปดาห์ เครื่องมือที่ใช้ในการวิจัย คือ 1) โปรแกรมกลุ่มจิตบำบัดแบบประคับประคอง 2) แบบประเมินอาการซึมเศร้าในผู้ป่วยจิตเภท และ 3) แบบประเมินสัมพันธภาพระหว่างบุคคล  เครื่องมือทุกฉบับได้รับการตรวจสอบความตรงตามเนื้อหาโดยผู้ทรงคุณวุฒิ มีค่าดัชนีความตรงตามเนื้อหาเท่ากับ 0.84 และ 0.90 ตามลำดับ ค่าความเที่ยงสัมประสิทธิ์อัลฟาของครอนบาคเท่ากับ 0.92 และ 0.94 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติการวิเคราะห์ความแปรปรวนเมื่อมีการวัดซ้ำ (Repeated Measures ANOVA)

ผลการวิจัย: อาการซึมเศร้าของพระสงฆ์อาพาธด้วยโรคจิตเภทที่ได้รับโปรแกรมกลุ่มจิตบำบัดแบบประคับประคองน้อยกว่าก่อนได้รับโปรแกรม อย่างมีนัยสำคัญทางสถิติที่ ระดับ .05

วัตถุประสงค์: เพื่อศึกษาประสบการณ์การดูแลผู้ป่วยมะเร็งระยะลุกลามที่บ้านของผู้ดูแลในครอบครัวรูปแบบการวิจัย: วิจัยเชิงคุณภาพแบบปรากฏการณ์วิทยาของ Heidegger

วิธีดำเนินการวิจัย: คัดเลือกผู้ให้ข้อมูลแบบเฉพาะเจาะจง ผู้ให้ข้อมูล คือ ผู้ดูแลในครอบครัวที่ทำหน้าที่เป็นผู้ดูแลหลักให้การดูแลผู้ป่วยที่ได้รับการวินิจฉัยจากแพทย์ว่าเป็นผู้ป่วยมะเร็งระยะลุกลาม เก็บรวบรวมข้อมูลโดยการสัมภาษณ์เชิงลึก บันทึกเทป วิเคราะห์ข้อมูลตามวิธีของ van Manen (1990)  ข้อมูลอิ่มตัวจากผู้ให้ข้อมูล จำนวน 13 ราย

ผลการวิจัย: ผลการวิจัยแบ่งออกได้เป็น 2 ส่วน ได้แก่ ส่วนที่ 1 การให้ความหมายของความเจ็บป่วยด้วยมะเร็งระยะลุกลาม แบ่งออกเป็น 2 ประเด็นหลัก ดังนี้ 1) ผู้ป่วยมีชีวิตอยู่ได้อีกไม่นาน และ 2) ยังหวังว่าการรักษาอาจจะช่วยให้หายได้ และส่วนที่ 2 ประสบการณ์การดูแลผู้ป่วยมะเร็งระยะลุกลามที่บ้านของผู้ดูแลในครอบครัว แบ่งออกเป็น 3 ประเด็นหลัก ดังนี้ 1) การดูแลเพื่อเพิ่มความสุขสบายไม่ทุกข์ทรมาน  2) การได้รับความช่วยเหลือสนับสนุน  และ 3) การเรียนรู้และการปรับตัวจากการดูแล

สรุป: ผลการวิจัยนี้นำมาเป็นข้อมูลพื้นฐานสำหรับทีมสุขภาพในการส่งเสริมศักยภาพของผู้ดูแลในครอบครัวให้สามารถดูแลผู้ป่วยมะเร็งระยะลุกลามที่บ้านได้

วัตถุประสงค์: เพื่อศึกษากระบวนการและผลลัพธ์การจัดการความปวดของผู้ป่วยที่ได้รับการผ่าตัดศัลยกรรมจักษุหลังผ่าตัดศัลยกรรมจักษุ 24, 48 และ 72 ชั่วโมง

รูปแบบการวิจัย: การวิจัยย้อนหลังเชิงพรรณนา

วิธีดำเนินการวิจัย: ศึกษาเวชระเบียนย้อนหลัง 3 ปี ตั้งแต่ พ.ศ 2562-2564 จำนวน 80 ราย ในผู้ป่วยที่มารับการผ่าตัดศัลยกรรมจักษุ โดยการสุ่มแบบเจาะจง รวบรวมข้อมูลโดยแบบบันทึกข้อมูล ประกอบด้วย ข้อมูลส่วนบุคคลและด้านสุขภาพ ผลลัพธ์การจัดการความปวด และกระบวนการจัดการความปวด เครื่องมือการวิจัยครั้งนี้ผ่านการตรวจสอบความตรงตามเนื้อหาจากผู้ทรงวุฒิ ทั้งหมด 3 ท่าน ได้ค่าความตรงตามเนื้อหา เท่ากับ 1.0, .85 และ .96 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติเชิงบรรยาย และสถิติ One sample t-test

ผลการวิจัย: ค่าเฉลี่ยคะแนนความปวดกับเกณฑ์มาตรฐานควบคุมความปวด (ค่าคะแนนความปวด ≤  3 คะแนน) หลังจากผู้ป่วยผ่าตัดทางจักษุที่มีคะแนนความปวดมากที่สุด ด้วยสถิติ One sample t-test พบว่าระยะเวลา 24 ชั่วโมงหลังผ่าตัด ผู้ป่วยมีคะแนนความปวดสูงกว่าเกณฑ์มาตรฐานควบคุมความปวด อย่างไม่มีนัยสำคัญ (p= .244) และระยะเวลา 48 และ72 ชั่วโมงหลังผ่าตัด พบว่าผู้ป่วยมีคะแนนความปวดอยู่ในระดับเกณฑ์ควบคุมความปวดอย่างมีนัยสำคัญทางสถิติ (p= .001 และ .001 ตามลำดับ)

สรุป: หลังผ่าตัดทางจักษุในระยะเวลา 24 ชั่วโมงแรกหลังผ่าตัด ผู้ป่วยจะมีคะแนนความปวดมากกว่า 3 คะแนน เนื่องจากเป็นการผ่าตัดทางจักษุ ทำให้เนื้อเยื่อบริเวณรอบๆดวงตาถูกทำลายจากการผ่าตัด ดังนั้นพยาบาลหรือแพทย์ผู้รักษา ควรมีการจัดการความปวดอย่างมีประสิทธิภาพให้เหมาะสมกับผู้ป่วยรายนั้น

วัตถุประสงค์การวิจัย: เพื่อศึกษาผลของการใช้โปรแกรมการบำบัดความคิดและพฤติกรรมต่ออาการซึมเศร้าในผู้สูงอายุโรคไม่ติดต่อเรื้อรัง

รูปแบบงานวิจัย: การวิจัยกึ่งทดลอง

วิธีการดำเนินการวิจัย: กลุ่มตัวอย่างคือผู้ที่มีอายุ 60 ปีขึ้นไป ซึ่งได้รับการวินิจฉัยจากแพทย์ว่าเป็นโรคใดโรคหนึ่งหรือเป็นโรคร่วมตั้งแต่สองโรคขึ้นไปในกลุ่มของโรคไม่ติดต่อเรื้อรัง จำนวน 60 ราย เครื่องมือวิจัยคือแบบบันทึกข้อมูลส่วนบุคล โปรแกรมการบำบัดความและพฤติกรรมโดยผ่านการตรวจสอบความตรงตามเนื้อหาจากผู้ทรงคุณวุฒิ 3 ท่านมีค่าดัชนีความตรงตามเนื้อหาเท่ากับ .90 และแบบแบบวัดภาวะซึมเศร้าในผู้สูงอายุของไทย (TGDS) มีค่าความเที่ยงของเครื่องมือค่าสัมประสิทธิ์แอลฟ่าของครอนบาดเท่ากับ .91  จัดกลุ่มตัวอย่างด้วยการจับคู่พิจารณาจากเพศและระดับคะแนนอาการซึมเศร้า แบ่งเป็นกลุ่มละ 30 ราย กลุ่มควบคุมได้รับการพยาบาลโดยการให้คำปรึกษาตามปัญหาที่พบ และกลุ่มทดลอง ได้รับโปรแกรมการบำบัดความคิดและพฤติกรรมจำนวน 9 กิจกรรม ประกอบด้วยการเริ่มต้นสร้างสัมพันธภาพ  การจัดกิจกรรมสิ่งที่มีอิทธิพลต่อความรู้สึกและการฝึกทักษะการผ่อนคลาย   การปรับโครงสร้างทางการรู้คิดโดยค้นหาความเชื่อที่ผิดและความคิดอัตโนมัติด้านลบ การปรับโครงสร้างทางความคิดและความเชื่อที่ไม่เหมาะสมเกี่ยวกับอาการซึมเศร้า การสร้างเสริมรูปแบบการจัดการอาการซึมเศร้าที่เหมาะสม จำนวน 3 กิจกรรม  การใช้เทคนิคการพูดกับตนเอง (self-talk) และทบทวนการเรียนรู้ในการฝึกทักษะที่เข้าร่วมในโปรแกรมร่วมกับการวางแผนการนำทักษะไปใช้ในอนาคต วิเคราะห์ข้อมูลโดยใช้สถิติบรรยาย และทดสอบค่าเฉลี่ยโดยใช้สถิติทดสอบที (t-test)

ผลการศึกษา: คะแนนเฉลี่ยอาการซึมเศร้าในกลุ่มทดลองที่ได้รับโปรแกรมการบำบัดความคิดและพฤติกรรมต่ำกว่าก่อนได้รับโปรแกรมอย่างมีนัยสำคัญทางสถิติ (p‹.05) และพบว่าคะแนนเฉลี่ยอาการซึมเศร้าในกลุ่มทดลองภายหลังได้รับโปรแกรมการบำบัดความคิดและพฤติกรรมต่ำกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (p‹.05)

สรุป: โปรแกรมการบำบัดความคิดและพฤติกรรมช่วยให้ผู้สูงอายุที่ป่วยด้วยโรคไม่ติดต่อเรื้อรังให้มีอาการซึมเศร้าลดลง ควรนำโปรแกรมนี้ไปเผยแพร่กับผู้สูงอายุกลุ่มอื่น และควรศึกษาวิจัยในบุคคลที่มีความเสี่ยงต่อการเกิดอาการซึมเศร้าต่อไป

BACKGROUND: Young adults experiencing homelessness (YAEH) experience more stressors compared to housed peers, yet little is known about the impact of the COVID-19 pandemic on these youth. The purpose of this qualitative study was to explore how YAEH perceived the pandemic's impact on their well-being and coping.

METHODS: YAEH were recruited from those participating in an HIV prevention study. Semi-structured interviews were conducted and analysis was guided by Lazarus and Folkman's transactional theory of stress and coping.

Purpose: The purpose of this project was to evaluate nursing staff perceptions of safety and workplace violence and increase safety awareness and knowledge in the emergency department of a level 1 trauma center.

Methods: This study was a single site project that used a descriptive non-experimental survey design to evaluate staff perceptions on workplace violence and safety before and after educational intervention.
