การวิจัยติดตามย้อนหลังนี้ มีวัตถุประสงค์เพื่อศึกษาปัจจัยที่สัมพันธ์กับการติดเชื้อแผลผ่าตัดกลางหน้าอกในผู้ป่วยหลังทำผ่าตัดทางเบี่ยงหลอดเลือดหัวใจ ได้แก่ อายุ เพศ ภาวะอ้วน ภาวะขาดสารอาหาร ภาวะซีด ภาวะโรคร่วม ทัศนคติในการดูแลแผลผ่าตัด อัตราการกรองของไต การบีบตัวของหัวใจห้องล่างซ้าย ระดับน้ำตาลเฉลี่ยสะสมในเลือด และประเภทการผ่าตัด กลุ่มตัวอย่างคือ ผู้ป่วยหลังทำผ่าตัดทางเบี่ยงหลอดเลือดหัวใจ อายุตั้งแต่ 35 ปีขึ้นไป ทั้งเพศชายและหญิง ได้รับการผ่าตัดระยะตั้งแต่ 1 กรกฎาคม พ.ศ. 2564 ถึง 31 ธันวาคม พ.ศ. 2565 แบ่งเป็น 2 กลุ่ม จำนวน 81 คน คือ กลุ่มศึกษา หมายถึง กลุ่มผู้ป่วยที่พบการติดเชื้อแผลผ่าตัดกลางหน้าอก จำนวน 27 คน และ กลุ่มควบคุม หมายถึง กลุ่มผู้ป่วยที่ไม่พบการติดเชื้อแผลผ่าตัดกลางหน้าอก จำนวน 54 คน ณ โรงพยาบาลระดับตติยภูมิ ในกรุงเทพมหานคร เลือกกลุ่มตัวอย่างแบบหลายขั้นตอน เครื่องมือที่ใช้เก็บรวบรวมข้อมูล ได้แก่ 1) แบบบันทึกข้อมูลส่วนบุคคล 2) แบบบันทึกปัจจัยที่สัมพันธ์กับการติดเชื้อแผลผ่าตัดกลางหน้าอก 3) แบบประเมินทัศนคติในการดูแลแผลผ่าตัด และ 4) แบบประเมินการวินิจฉัยการติดเชื้อแผลผ่าตัดกลางหน้าอก เครื่องมือผ่านการตรวจสอบ โดยผู้ทรงคุณวุฒิ 5 คน ได้ค่าดัชนีความตรงตามเนื้อหาเท่ากับ 0.86 และค่าสัมประสิทธิ์แอลฟาของครอนบาค เท่ากับ 0.87 วิเคราะห์ข้อมูลโดยใช้สถิติพรรณนา และสถิติถดถอยแบบพหุลอจิสติก ผลการวิจัยพบว่า ปัจจัยที่สัมพันธ์กับการติดเชื้อแผลผ่าตัดกลางหน้าอกในผู้ป่วยหลังทำผ่าตัดทางเบี่ยงหลอดเลือดหัวใจ อย่างมีนัยสำคัญทางสถิติ ได้แก่ อัตราการกรองของไต Thai eGFR < 60 ml/min (aOR 3.29 [95 % CI 1.22-8.89], p = 0.02) และระดับน้ำตาลเฉลี่ยสะสมในเลือด HbA1C ≥ 6.5 % (aOR 2.96 [95 % CI 1.08-8.12] , p = 0.04)

การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อพัฒนาและศึกษาประสิทธิผลของการใช้การจัดอัตรากำลังพยาบาลในหอผู้ป่วยอายุรกรรมโดยใช้สมาร์ตโฟนแอปพลิเคชัน โรงพยาบาลสมเด็จพระปิ่นเกล้า โดยเป็นการวิจัยเชิงปฏิบัติการแบบมีส่วนร่วม (Participatory action research) ระหว่างผู้วิจัยและผู้ร่วม ซึ่งผู้ร่วมวิจัยเป็นพยาบาลวิชาชีพของหอผู้ป่วยอายุรกรรม 18 คน ดำเนินการวิจัยตามวงจรการวิจัยเชิงปฏิบัติการ (PAOR) ของ Kemmis และคณะ (2014) ตั้งแต่การวิเคราะห์สถานการณ์การจัดอัตรากำลังพยาบาลในหอผู้ป่วยอายุรกรรม การวางแผนการพัฒนา (Plan) การพัฒนาสมาร์ตโฟนแอปพลิเคชันสำหรับการจัดอัตรากำลัง (Act) การนำสมาร์ตโฟนแอปพลิเคชันสำหรับการจัดอัตรากำลังไปใช้จริง (Observe) และการประเมินผลการปฏิบัติ (Reflect) เพื่อให้ได้การจัดอัตรากำลังพยาบาลโดยใช้สมาร์ตโฟนแอปพลิเคชันที่เหมาะสมกับบริบทของหอผู้ป่วยและช่วยลดข้อจำกัดที่ปฏิบัติอยู่ วิเคราะห์ข้อมูลเชิงคุณภาพด้วยการวิเคราะห์เนื้อหาและวิเคราะห์ข้อมูลเชิงปริมาณด้วยสถิติเชิงพรรณนา ผลการวิจัยพบว่า 1) สมาร์ตโฟนแอปพลิเคชันสำหรับการจัดอัตรากำลังที่พัฒนาขึ้น ประกอบด้วย 5 ฟีเจอร์ ได้แก่ การรับและจำหน่ายผู้ป่วย การจำแนกประเภทผู้ป่วย ความต้องการการพยาบาล การคำนวณอัตรากำลัง และตารางเวรปฏิบัติงาน 2) ผู้ร่วมวิจัยมีความพึงพอใจในการใช้สมาร์ตโฟนแอปพลิเคชันการจัดอัตรากำลังพยาบาลที่ร่วมกันพัฒนาขึ้น รู้สึกเป็นส่วนหนึ่งของความสำเร็จในการวิจัย เนื่องจากการได้มีส่วนร่วมในกระบวนการพัฒนาทุกขั้นตอน สมาร์ตโฟนแอปพลิเคชันสำหรับการจัดอัตรากำลังที่พัฒนาขึ้นตอบโจทย์ความต้องการของผู้ใช้งานและสามารถนำไปปฏิบัติเพื่อแก้ไขข้อจำกัดการจัดอัตรากำลังที่มีอยู่ได้จริง ซึ่งแสดงให้เห็นถึงความเป็นไปได้ในการนำไปใช้และส่งผลต่อการพัฒนาการจัดอัตรากำลังอย่างต่อเนื่องในโอกาสต่อไป

การศึกษาความสัมพันธ์เชิงทำนายนี้มีวัตถุประสงค์เพื่อศึกษาระยะเวลาการตัดสินใจในการ มารับการรักษาและปัจจัยทำนายระยะเวลาการตัดสินใจในการมารับการรักษาของผู้ป่วยภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันชนิด ST-segment ไม่ยก กลุ่มตัวอย่าง คือ ผู้ป่วยภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันชนิด ST-segment ไม่ยก เพศชายและหญิง อายุ 40 ปีขึ้นไป ที่มารับการรักษาในหอผู้ป่วยสามัญอายุรกรรม หอผู้ป่วยวิกฤตอายุรกรรม และหอผู้ป่วยวิกฤตโรคหัวใจ โรงพยาบาลสระบุรี และโรงพยาบาลพระนครศรีอยุธยา จำนวน 100 ราย เลือกกลุ่มตัวอย่างแบบหลายขั้นตอน เครื่องมือที่ในการใช้เก็บข้อมูล ได้แก่ 1) แบบสอบถามข้อมูลส่วนบุคคลและการเจ็บป่วย 2) แบบสอบถามภาวะโรคร่วม 3) แบบสอบถามการรับรู้ความเจ็บป่วย 4) แบบสอบถามความวิตกกังวล และ 5) แบบสอบถามการสนับสนุนทางสังคม เครื่องมือมีค่าดัชนีความตรงตามเนื้อหาในชุดที่ 2 – 5 เท่ากับ 0.95, 1.00, 0.75, และ 1.00 ตามลำดับ และค่าสัมประสิทธิ์แอลฟาของครอนบาคเท่ากับ 0.84, 0.87, 0.85, และ 0.91 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติบรรยายและสถิติถดถอยพหุคูณแบบขั้นตอน ผลการศึกษาพบว่า 1. ระยะเวลาที่ใช้ในการตัดสินใจในการมารับการรักษาของผู้ป่วยภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันชนิด ST-segment ไม่ยก มีค่ามัธยฐานเท่ากับ 90 นาที ( x = 122.35, SD = 102.86) 2. ปัจจัยทำนายระยะเวลาการตัดสินใจในการมารับการรักษาของผู้ป่วยภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันชนิด ST-segment ไม่ยก ได้สูงสุดอย่างมีนัยสำคัญทางสถิติ คือ การรับรู้ความเจ็บป่วย (β = -.294, p < .01) รองลงมาคือ ความวิตกกังวล (β = -.237, p < .05) ภาวะโรคร่วม (β = .229, p < .05) การสนับสนุนทางสังคม (β = -.207, p < .05) การใช้บริการการแพทย์ฉุกเฉิน (β = .125, p < .05) และประสบการณ์การเข้ารับการรักษาในโรงพยาบาล (β = .117, p < .05) ตามลำดับ โดยปัจจัยทั้ง 6 ปัจจัย สามารถร่วมทำนายระยะเวลาการตัดสินใจ ในการมารับการรักษาของผู้ป่วยภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันชนิด ST-segment ไม่ยก ได้ร้อยละ 75.70 อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (Adjusted R2 = .757, p < .05)

ปัจจัยทำนายภาวะสับสนเฉียบพลันในผู้สูงอายุที่เข้ารับบริการในแผนกฉุกเฉิน, สุภาวดี แก้วสินธุ์

การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อ ศึกษาอัตราการเกิดภาวะสับสนเฉียบพลันในผู้สูงอายุที่เข้ารับบริการในแผนกฉุกเฉิน และเพื่อศึกษาปัจจัยทำนายภาวะสับสนเฉียบพลันในผู้สูงอายุที่เข้ารับบริการในแผนกฉุกเฉิน ได้แก่ ภาวะการรู้คิดบกพร่อง ประวัติโรคหลอดเลือดสมอง ระยะเวลาการเข้ารับบริการในแผนกฉุกเฉิน ความปวด ภาวะเลือดออกในสมอง และภาวะซึมเศร้า กลุ่มตัวอย่าง คือ ผู้ที่มีอายุเกิน 60 ปีบริบูรณ์ขึ้นไป ที่เข้ารับบริการในแผนกฉุกเฉินและอุบัติเหตุโรงพยาบาลของรัฐระดับตติยภูมิ จำนวน 3 แห่งในเขตกรุงเทพมหานคร จำนวน 220 คน โดยการคัดเลือกกลุ่มตัวอย่างตามคุณสมบัติที่กำหนดไว้ เครื่องมือที่ใช้ในการวิจัย คือ แบบประเมินข้อมูลส่วนบุคคล แบบประเมินภาวะสับสนเฉียบพลัน แบบคัดกรองภาวะสมองเสื่อม แบบประเมินความปวด โดยใช้มาตรวัดแบบตัวเลข และในกรณีผู้สูงอายุที่ไม่สามารถบอกระดับความเจ็บปวดได้ ใช้แบบประเมินความเจ็บปวดสำหรับผู้ที่มีภาวะสมองเสื่อม ผ่านการตรวจสอบการหาค่าความเที่ยง โดยการหาค่าสัมประสิทธิ์แอลฟาครอนบาค มีความเที่ยงเท่ากับ .83 และ .79 ตามลำดับ และหาค่าความเที่ยงของแบบประเมินที่ใช้ในการวิจัยโดยวิธีการวัดซ้ำ มีค่าความเที่ยงเท่ากับ .77 และ .97 ตามลำดับ วิเคราะห์ข้อมูลโดยการหาค่าเฉลี่ย ค่าร้อยละ ค่าเบี่ยงเบนมาตรฐาน ค่าสถิติไคสแคว์ (Chi-square) ค่าสัมประสิทธิ์สหสัมพันธ์แบบพอยท์ไบซีเรียล (Point biserial correlation coefficient) และ สถิติถดถอยโลจิสติกทวิ (binary logistic regression) ผลการวิจัยสรุปได้ดังนี้ 1. ผู้สูงอายุที่เข้ารับบริการในแผนกฉุกเฉิน มีภาวะสับสนเฉียบพลัน จำนวน 34 คน จากจำนวนกลุ่มตัวอย่างทั้งหมด 220 คน คิดเป็นร้อยละ 15.45 2. ภาวะการรู้คิดบกพร่อง ประวัติโรคหลอดเลือดสมอง ระยะเวลาการเข้ารับบริการในแผนกฉุกเฉิน ระดับความปวด ภาวะเลือดออกในสมอง และภาวะซึมเศร้า มีความสัมพันธ์เชิงบวกกับภาวะสับสนเฉียบพลันในผู้สูงอายุที่เข้ารับบริการในแผนกฉุกเฉิน อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (X2= 9.277, 10.020, rpb= .216, X2= 9.000, 9.568, 3.874 ตามลำดับ) 3. ตัวแปรที่สามารถร่วมกันทำนายภาวะสับสนเฉียบพลันในผู้สูงอายุที่เข้ารับบริการในแผนกฉุกเฉินได้อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 คือ ภาวะการรู้คิดบกพร่อง (AOR=7.25, 95% CI …

การวิจัยนี้เป็นการวิจัยแบบบรรยายเชิงความสัมพันธ์ มีวัตถุประสงค์เพื่อศึกษาการทำพินัยกรรมชีวิตของผู้ที่เป็นมะเร็ง และศึกษาความสัมพันธ์ของ อายุ ระยะของโรค ความรุนแรงของอาการ ความรู้เกี่ยวกับการทำพินัยกรรมชีวิต ความวิตกกังวลเกี่ยวกับความตาย การสนับสนุนทางสังคม กับการทำพินัยกรรมชีวิตของผู้ที่เป็นมะเร็ง กลุ่มตัวอย่าง คือ ผู้ป่วยที่ได้รับการวินิจฉัยจากแพทย์ว่าเป็นโรคมะเร็ง และรับบริการในแผนกหอผู้ป่วยนอกโรคมะเร็งของโรงพยาบาลระดับตติยภูมิในเขตกรุงเทพมหานคร จำนวน 180 คน เครื่องมือที่ใช้งานวิจัย ได้แก่ แบบสอบถามข้อมูลส่วนบุคคล แบบสอบถามการทำพินัยกรรมชีวิต แบบประเมินความรุนแรงของอาการ แบบประเมินความรู้เกี่ยวกับการทำพินัยกรรมชีวิต แบบสอบถามความวิตกกังวลเกี่ยวกับความตาย และแบบประเมินการสนับสนุนทางสังคม โดยค่าความเที่ยงแบบประเมินความรุนแรงของอาการ แบบสอบถามความวิตกกังวลเกี่ยวกับความตาย และแบบประเมินการสนับสนุนทางสังคมมีค่าสัมประสิทธิ์แอลฟาของครอนบาค เท่ากับ .88, .90 และ .89 ตามลำดับ ส่วนแบบประเมินความรู้เกี่ยวกับการทำพินัยกรรมชีวิต มีค่า KR-20 เท่ากับ .81 วิเคราะห์ข้อมูลโดยสถิติเชิงพรรณนา และการถดถอยโลจิสติค ผลการวิจัยพบว่า ผู้ที่เป็นมะเร็งมีการทำพินัยกรรมชีวิตเพียงร้อยละ 37.2 โดยส่วนใหญ่ทำด้วยวิธีการบอกกล่าวร้อยละ 49.2 โดยปัจจัยที่ทำให้ผู้ที่เป็นมะเร็งทำพินัยกรรมชีวิตมากขึ้น ได้แก่ ผู้ที่มีอายุ 60 ปีขึ้นไป เป็นมะเร็งระยะลุกลาม ความรุนแรงของอาการ และ ความรู้เกี่ยวกับการทำพินัยกรรมชีวิต (OR= 2.63, 7.59, 1.05, 1.79 ตามลำดับ, p<.05) และปัจจัยที่ทำให้ผู้ที่เป็น มะเร็งทำพินัยกรรมชีวิตลดลง ได้แก่ ความวิตกกังวลเกี่ยวกับความตาย (OR= 0.87, p<.05)

Background: Antioxidant insufficiency, elevated inflammatory markers, and poor health-related quality of life (HRQOL) are prevalent in patients with heart failure (HF). Objective: The objective of this study was to examine the associations among dietary antioxidant intake, inflammatory markers, and HRQOL in patients with HF. Methods: This was a secondary analysis of 265 patients with HF who completed a 4-day food diary. We assessed intake of 10 antioxidants: alpha carotene, beta carotene, beta cryptoxanthin, lutein, zeaxanthin, lycopene, vitamins C and E, zinc, and selenium. Introduction: The cumulative number of COVID-19 cases has surpassed 579 million globally. Symptoms during and after COVID-19 infection vary from mild cold symptoms to severe multisystem illness. Given the wide range of symptom presentations and complications post COVID-19, the purpose of this study was to describe the lived experience of American adults surviving COVID-19.

Background: Family caregivers are at a high risk for low quality of life due to caregiving-related stress. Caregivers' stress is commonly assessed using self-reported measures, which reflect relatively subjective and long-term stress related to caregiving, but objective biological markers of stress are rarely used for caregivers. To describe the lived experience of nursing staff and nurse leaders working in COVID-19 devoted units (intensive care or medical unit) prior to vaccine availability.

Research Design

Qualitative phenomenological design with a focus group approach.

Retaining nursing students in courses is key to increasing the number of new nurses. This study examined factors associated with nursing course or program withdrawal among students with and without prior military experience. The purpose of this study was to uncover differences between groups of students who withdrew compared to those who did not. The COVID-19 pandemic has required educators to develop innovative methods for IPE student engagement. Recent events have underlined the critical role of mother-infant-linked longitudinal surveillance to characterize and assess the impacts of emerging, re-emerging, and persistent threats, including infectious diseases, on these populations. Approximately 20% of patients with an acute ischemic stroke (AIS) will need an intensive care unit (ICU) admission, and less than 20% of these patients have a good neurological outcome at six months. The objective of the study was to explore how older people living in a residential aged care facility perceived that their oral health influenced their food preferences and attitudes towards food, their social interactions and their self-esteem.

Preterm birth is the leading cause of perinatal morbidity and mortality and is associated with adverse developmental and long-term health outcomes, including several cardiometabolic risk factors and outcomes. However, evidence about the association of preterm birth with later body size derives mainly from studies using birth weight as a proxy of prematurity rather than an actual length of gestation. The bioethical principles: non maleficence, beneficence justice and autonomy, that govern health care practice have been criticised as abstract, patriarchal and even rhetorical in maternity care practice (MacLellan, 2014) and consequently may be insufficient in guiding care of childbearing women.
