Despite this, rates of c-section have continued to rise in the US and around the world. Background

Globally, women experience disrespectful and abusive care from maternity healthcare providers at health facilities, committed intentionally or unintentionally, particularly during labor and delivery. Disrespectful care affects women’s childbirth experience and birth outcomes.

Methods

This study used a descriptive qualitative design to obtain thick and rich data on disrespect and abuse in maternity care in a low-resource setting in Tanzania. Three days workshop was conducted at the Aga Khan University comprising maternity healthcare providers from diverse settings. Background Implanted ports are frequently used for central venous access in the adult oncology population. Maintaining patency of these ports is important for continuing uninterrupted treatment and supportive care for oncologic patients. Historically, heparin solution has been used as a locking solution for implanted ports to prevent occlusion. However, evidence shows that 0.9% sodium chloride (normal saline or NS) as a lock solution is equally effective as heparin solution at maintaining implanted port patency and poses less risk to patients.

AbstractThe purpose of this quality improvement (QI) evaluation project was to decrease patient falls on a neurological stroke unit. This unit had the most falls throughout the hospital system. The unit averaged 50-60 falls per year with an average of 4.75 falls per month. วัยสูงอายุเป็นช่วงปลายของชีวิตมนุษย์ ซึ่งมีพัฒนาการเปลี่ยนแปลงในลักษณะที่เสื่อมลงตามธรรมชาติ การเปลี่ยนแปลงทางสรีรวิทยาอาจส่งผลให้เกิดกลุ่มอาการสูงอายุ (Geriatric syndromes) และเป็นสาเหตุที่อาจทำให้เกิดปัญหาสุขภาพที่ซับซ้อนและรุนแรงหลายระบบในร่างกายหากไม่ได้รับการรักษาอย่างทันท่วงที รวมถึงมีแนวโน้มการเกิดผลลัพธ์คลินิกที่ไม่ดี ส่งผลกระทบต่อคุณภาพชีวิตของผู้สูงอายุและผู้ดูแลเป็นอย่างมาก การคัดกรองกลุ่มอาการสูงอายุเป็นการประเมินสภาวะของผู้สูงอายุครอบคลุมทุกมิติสำคัญ ได้แก่ 1) ภาวะสุขภาพทางกาย 2) ภาวะสุขภาพจิตและการรู้คิด 3) สภาพแวดล้อมและสังคม และ 4) ความสามารถในการทำกิจวัตรประจำวัน เพื่อค้นหาปัญหาสุขภาพและแหล่งประโยชน์ที่จำเป็นสำหรับวางแผนให้การดูแลตามความต้องการและสภาพปัญหาสุขภาพของผู้สูงอายุได้อย่างครอบคลุม การคัดกรองกลุ่มอาการสูงอายุครอบคลุมตั้งแต่ระยะแรกจะช่วยป้องกันและเฝ้าระวังการเกิดกลุ่มอาการสูงอายุได้อย่างมีประสิทธิภาพ อย่างไรก็ตามบริบทการทำงานซึ่งมีผู้รับบริการจำนวนมาก ต้องคัดกรองภาวะเร่งด่วนที่คุกคามชีวิต และให้การพยาบาลในระยะฉุกเฉินก่อนเป็นลำดับแรก การคัดกรองกลุ่มอาการสูงอายุจึงมักถูกมองข้าม พยาบาลผู้สูงอายุถือเป็นบุคลากรทางด้านสุขภาพด่านแรกในการตั้งรับผู้รับบริการสูงอายุจึงจำเป็นต้องมีความรู้เกี่ยวกับแนวทางการคัดกรองกลุ่มอาการสูงอายุและการส่งต่อเพื่อดูแลรักษา บทความวิชาการนี้มีวัตถุประสงค์เพื่อเสนอบทบาทพยาบาลผู้สูงอายุในการคัดกรองกลุ่มอาการสูงอายุในคลินิกผู้สูงอายุ อันจะนำไปสู่เป้าหมายในการดูแลสุขภาพของผู้สูงอายุ ก่อให้เกิดคุณภาพชีวิตที่ดีต่อไป

วัตถุประสงค์: เพื่อศึกษาปัจจัยทำนายระยะเวลาการมาโรงพยาบาลเพื่อตรวจครั้งแรกของผู้ที่เป็นโรคมะเร็งเต้านม

รูปแบบการวิจัย: การวิจัยย้อนหลังแบบพรรณนาเชิงทำนาย

วิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้ที่เป็นโรคมะเร็งเต้านมรายใหม่ที่ได้รับการวินิจฉัยว่าเป็นมะเร็งเต้านมทุกระยะของโรคที่มารับการรักษาที่โรงพยาบาลสมเด็จพระนางเจ้าสิริกิติ์ และโรงพยาบาลมะเร็งชลบุรี จำนวน 110 คน เครื่องมือวิจัย คือ แบบสอบถามข้อมูลส่วนบุคคล แบบบันทึกระยะเวลาการมาโรงพยาบาล แบบประเมินความรู้เกี่ยวกับโรคมะเร็งเต้านม มีค่า KR-20 เท่ากับ .70 แบบสอบถามความกลัวการวินิจฉัยและการรักษา แบบสอบถามการได้รับแรงสนับสนุนทางสังคม ค่าสัมประสิทธิ์แอลฟ่าของครอนบาค เท่ากับ .88 และ .81 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติวิเคราะห์ถดถอยพหุคูณ

ผลการศึกษา: 1) ระยะเวลาการมาโรงพยาบาลของผู้ที่เป็นโรคมะเร็งเต้านมตั้งแต่พบอาการผิดปกติ ครั้งแรกจนกระทั่งมาโรงพยาบาลเพื่อตรวจครั้งแรกมีระยะเวลาเฉลี่ย 119.45 วัน 2) ความรู้เกี่ยวกับโรคมะเร็งเต้านม (Beta=-.447) และการได้รับแรงสนับสนุนทางสังคม (Beta=-.236) สามารถร่วมกันทำนายระยะเวลาการมาโรงพยาบาลเพื่อตรวจครั้งแรกของผู้ที่เป็นโรคมะเร็งเต้านมได้ ร้อยละ 24.70

สรุป: ผลการศึกษานี้สามารถใช้ข้อมูลไปใช้เป็นแนวทางให้กับพยาบาลในการพัฒนาโปรแกรมเพื่อลดระยะเวลาการตรวจครั้งแรกของผู้ที่เป็นโรคมะเร็งเต้านมที่มารับการตรวจที่โรงพยาบาล

การตัดสินใจเชิงจริยธรรมเป็นกระบวนการที่ช่วยให้พยาบาลได้ตรวจสอบประเด็นขัดแย้งทางจริยธรรมในการปฏิบัติการพยาบาลอย่างเป็นระบบและเป็นองค์รวม โดยยึดหลักเหตุผลเชิงจริยธรรม พยาบาลผู้ตัดสินใจ  จะต้องรู้องค์ประกอบและปัจจัยที่เกี่ยวข้องกับการตัดสินใจและสามารถเลือกกระทำหรือไม่กระทำในสถานการณ์ที่เกิดขึ้นนั้นได้ เนื่องจากพยาบาลถือว่าเป็นบุคลากรทางด้านสุขภาพที่มีความใกล้ชิดกับผู้ป่วยมากที่สุด ในหลายสถานการณ์ที่พยาบาลต้องเผชิญกับประเด็นทางจริยธรรม โดยเฉพาะอย่างยิ่งในบทความนี้แสดงประเด็นที่เกี่ยวข้องกับการการยื้อหรือยุติการมีชีวิตอยู่ในผู้ป่วยมะเร็งระยะท้าย ถือได้ว่าเป็นมิติใหม่ที่ท้าทายบทบาทของพยาบาล พยาบาลจึงมีบทบาทสำคัญที่จำเป็นต้องมีทักษะ ความรู้ และประสบการณ์หลายด้านในการดูแลผู้ป่วยมะเร็งระยะท้าย รวมทั้งทีมบุคลากรด้านสุขภาพต้องให้ความสำคัญกับกระบวนการตัดสินใจเชิงจริยธรรมในการดูแลประคับประคองด้านข้อมูลและด้านจิตใจของผู้ป่วยและครอบครัว เพื่อเพิ่มความมั่นใจและศักยภาพในการดูแลผู้ป่วยมะเร็งระยะท้ายแบบองค์รวม ส่งผลให้ผู้ป่วยได้รับการดูแลอย่างสมศักดิ์ศรีความเป็นมนุษย์และเกิดประโยชน์สูงสุดต่อผู้ป่วยและครอบครัว

วัตถุประสงค์: เพื่อศึกษาผลของโปรแกรมการจัดการตนเองและครอบครัวตามวิถีใหม่ต่อพฤติกรรมป้องกันการป่วยซ้ำของบุคคลที่เป็นจิตเภทในชุมชน

รูปแบบการวิจัย: การวิจัยกึ่งทดลอง

วิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ บุคคลที่เป็นจิตเภทในชุมชนและผู้ดูแลหลักในครอบครัวจำนวน 42 ครอบครัว ซึ่งมีคุณสมบัติตามเกณฑ์ที่กำหนดและเข้ารับบริการที่คลินิกจิตเวช โรงพยาบาลจุฬาลงกรณ์ฯ แบ่งเป็นกลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 21 ครอบครัว เครื่องมือที่ใช้ในการวิจัย ได้แก่ 1) โปรแกรมการจัดการตนเองและครอบครัวตามวิถีใหม่ 2) แบบสอบถามพฤติกรรมป้องกันการป่วยซ้ำของบุคคลที่เป็นจิตเภทในชุมชน 3) แบบประเมินความสามารถในการจัดการตนเองของบุคคลที่เป็นจิตเภทในชุมชน และ 4) แบบประเมินความสามารถในการจัดการตนเองและผู้ป่วยของผู้ดูแล วิเคราะห์ข้อมูลโดยใช้สถิติ เชิงพรรณนาและสถิติทดสอบที

ผลการศึกษา: พฤติกรรมป้องกันการป่วยซ้ำของกลุ่มทดลองหลังได้รับโปรแกรมการจัดการตนเองและครอบครัวตามวิถีใหม่สูงกว่าก่อนได้รับโปรแกรมอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 และพฤติกรรมป้องกันการป่วยซ้ำของกลุ่มทดลองสูงกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติที่ระดับ .05

สรุป: บุคลากรทางสุขภาพควรนำรูปแบบของโปรแกรมการจัดการตนเองและครอบครัวตามวิถีใหม่ไปใช้ในการส่งเสริมพฤติกรรมป้องกันการป่วยซ้ำของบุคคลที่เป็นจิตเภทในชุมชน

วัตถุประสงค์การวิจัย: เพื่อศึกษาปัจจัยที่มีความสัมพันธ์กับความรู้สึกไม่แน่นอนในความเจ็บป่วยของผู้รอดชีวิตจากมะเร็งเต้านม

รูปแบบการวิจัย: การวิจัยแบบพรรณนาเชิงความสัมพันธ์

วิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้รอดชีวิตจากมะเร็งเต้านมเพศหญิงอายุ 20- 60 ปี จำนวน 135 คน เก็บข้อมูลด้วยแบบสอบถาม ได้แก่ แบบสอบถามข้อมูลส่วนบุคคล ความรู้สึกไม่แน่นอนในความเจ็บป่วย ความกลัวการกลับเป็นซ้ำของโรคมะเร็ง ความรอบรู้ทางสุขภาพ การสนับสนุนทางสังคม และแบบสอบถามความทุกข์ทรมานจากอาการ วิเคราะห์ข้อมูลด้วยสถิติสหสัมพันธ์แบบเพียร์สัน

ผลการวิจัย: ความรู้สึกไม่แน่นอนในความเจ็บป่วยของผู้รอดชีวิตจากมะเร็งเต้านม อยู่ในระดับปานกลาง

(=69.04, SD =11.78) ระดับการศึกษามีความสัมพันธ์ทางลบ กับความรู้สึกไม่แน่นอนในความเจ็บป่วยอย่างมีนัยสำคัญทางสถิติที่ระดับ.05 (r=-.400) ความทุกข์ทรมานจากอาการ มีความสัมพันธ์ทางบวกกับความรู้สึกไม่แน่นอนในความเจ็บป่วย อย่างมีนัยสำคัญทางสถิติ ที่ระดับ .05 (r=.324) และความกลัวการ กลับเป็นมะเร็งซ้ำมีความสัมพันธ์ทางบวก กับความรู้สึกไม่แน่นอนในความเจ็บป่วย อย่างมีนัยสำคัญทางสถิติที่ ระดับ .05 (r=.380)

รูปแบบการวิจัย: การวิจัยกึ่งทดลอง แบบสองกลุ่มวัดผลก่อนและหลังการทดลอง

วิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้สูงอายุโรคซึมเศร้าและครอบครัวที่รับบริการในแผนกยาเสพติดและจิตเวชโรงพยาบาลองครักษ์ จำนวน 40 ครอบครัว จับคู่ให้มีลักษณะคล้ายคลึงกันด้วยอายุและคะแนนอาการซึมเศร้า สุ่มเข้ากลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 20 ครอบครัว โดยกลุ่มทดลองได้รับการดูแลตามโปรแกรมฯ และกลุ่มควบคุมได้รับการดูแลตามปกติ เครื่องมือวิจัย ประกอบด้วย 1) โปรแกรมสุขภาพจิตศึกษาครอบครัวที่เน้นการมีส่วนร่วมในการดูแล 2) แบบประเมินอาการซึมเศร้าของBeck 3) แบบประเมินความรู้ความเข้าใจเรื่องโรคซึมเศร้า 4) แบบวัดสัมพันธภาพในครอบครัว โดยเครื่องมือทุกชุดได้รับการตรวจสอบความตรงเชิงเนื้อหาโดยผู้ทรงคุณวุฒิ มีค่าดัชนีความตรงตามเนื้อหาเท่ากับ .81 ,1 และ 1 ตามลำดับ และมีค่าความเที่ยงสัมประสิทธิ์ของแอลฟาครอนบาคเท่ากับ .78, .86 และ .85 ตามลำดับ และนำไปวิเคราะห์ข้อมูลโดยใช้สถิติ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐานและการทดสอบ t-test

ผลการวิจัย: 1) คะแนนอาการซึมเศร้าในผู้สูงอายุโรคซึมเศร้าที่ได้รับโปรแกรมฯ น้อยกว่าก่อนการทดลองอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2) คะแนนอาการซึมเศร้าในผู้สูงอายุโรคซึมเศร้าที่ได้รับโปรแกรมฯ น้อยกว่ากลุ่มที่ได้รับการพยาบาลตามปกติอย่างมีนัยสำคัญทางสถิติที่ระดับ .05

ข้อสรุป: โปรแกรมสุขภาพจิตศึกษาครอบครัวที่เน้นการมีส่วนร่วมในการดูแลสามารถลดอาการซึมเศร้าของผู้สูงอายุโรคซึมเศร้าในชุมชนได้อย่างมีประสิทธิภาพ

วัตถุประสงค์ : ศึกษาผลของโปรแกรมการจัดการอาการร่วมกับการนวดกดจุดสะท้อนฝ่าเท้าต่ออาการ ท้องอืดในผู้ป่วยที่ได้รับการผ่าตัดนิ่วในถุงน้ำดีผ่านกล้อง

รูปแบบการวิจัย: วิจัยกึ่งทดลองแบบสองกลุ่มวัดครั้งเดียว

วิธีการดำเนินการวิจัย: กลุ่มตัวอย่างคือ ผู้ป่วยที่ได้รับการผ่าตัดนิ่วในถุงน้ำดีผ่านกล้องในโรงพยาบาลสุราษฎร์ธานี คัดเลือกแบบเจาะจง เข้ากลุ่มควบคุมและกลุ่มทดลองกลุ่มละ 22 คน จับคู่ อายุ จำนวนยาแก้ปวด และระยะเวลาในการผ่าตัด กลุ่มทดลองได้รับโปรแกรมฯ หลังผ่าตัด กลุ่มควบคุมได้รับการพยาบาลตามปกติ เครื่องมือวิจัยคือ 1) แบบสอบถามข้อมูลส่วนบุคคล 2) แบบประเมินอาการท้องอืด 3) โปรแกรมการจัดการอาการร่วมการนวดกดจุดสะท้อนฝ่าเท้าและเครื่องมือกำกับการทดลอง แบบประเมินอาการ      ท้องอืดมีค่าความเชื่อมั่นสัมประสิทธิ์แอลฟาครอนบาค .84 วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา และ สถิติการทดสอบความแปรปรวนแบบเกี่ยวข้อง (Repeated – measures ANOVA)

ผลการวิจัย: กลุ่มทดลองที่ได้รับโปรแกรมการจัดการอาการร่วมกับการนวดกดจุดสะท้อนฝ่าเท้ามีอาการท้องอืดหลังการผ่าตัดน้อยกว่ากลุ่มควบคุมที่ได้รับการพยาบาลตามปกติอย่างมีนัยสำคัญทางสถิติที่ ระดับ .05

สรุป: โปรแกรมฯ นี้สามารถลดอาการท้องอืดหลังการผ่าตัดนิ่วในถุงน้ำดีผ่านกล้องได้

รูปแบบการวิจัย: การวิจัยกึ่งทดลอง

วิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ประชาชนทั่วไป อายุมากกว่า 20 ปี ที่เข้ามารับบริการมุมสุขภาพ ตึกผู้ป่วยนอก โรงพยาบาลศิริราช จำนวนกลุ่มทดลองและกลุ่มควบคุมกลุ่มละ 103 คน กลุ่มทดลองได้เข้าร่วมกิจกรรมการแลกเปลี่ยนเรียนรู้แบบกลุ่มร่วมกับสื่อการสอนด้วยวีดิทัศน์ กลุ่มควบคุมได้เข้าร่วมกิจกรรมส่งเสริมสุขภาพตามปกติของโรงพยาบาล เครื่องมือวิจัย คือ แบบสอบถามข้อมูลส่วนบุคคล แบบสอบถามการรับรู้ความรุนแรง แบบสอบถามโอกาสเสี่ยงต่อการเกิดโรคหลอดเลือดสมอง แบบสอบถามความคาดหวังในประสิทธิผลของการตอบสนอง และแบบสอบถามความคาดหวังในความสามารถของตนในการปฏิบัติเบื้องต้นเมื่อมีอาการโรคหลอดเลือดสมอง วิเคราะห์ข้อมูลโดยใช้ สถิติทดสอบที

ผลการวิจัย: กิจกรรมการแลกเปลี่ยนเรียนรู้แบบกลุ่มร่วมกับสื่อการสอนด้วยวีดิทัศน์ ทำให้กลุ่มทดลองมีการรับรู้ความรุนแรง การรับรู้โอกาสเสี่ยง ความคาดหวังในประสิทธิผลของการตอบสนอง และความ คาดหวังในความสามารถของตนเองในผู้ป่วยโรคหลอดเลือดสมองสูงกว่ากลุ่มควบคุม (ทันทีหลังการทดลอง) และสูงกว่ากลุ่มควบคุมเมื่อติดตาม 6 เดือน อย่างมีนัยสำคัญทางสถิติที่ระดับ.05

สรุป: กิจกรรมการแลกเปลี่ยนเรียนรู้แบบกลุ่มร่วมกับสื่อการสอนด้วยวีดิทัศน์ สามารถนำไปประยุกต์ใช้ในหน่วยบริการด้านส่งเสริมสุขภาพอื่น ๆ เพื่อให้ประชาชนทั่วไปได้เข้าถึงบริการ และมีความรู้ความเข้าใจอาการความรุนแรงของโรคหลอดเลือดสมอง มีพฤติกรรมสุขภาพที่เหมาะสม

วิธีการดำเนินการวิจัย : กลุ่มตัวอย่างคือ พยาบาลวิชาชีพที่มีบทบาทหน้าที่ในการพยาบาลผู้ป่วยโรคหัวใจ และหลอดเลือดไร้รอยต่อเขต 12 จำนวน 40 คน เก็บข้อมูลโดยใช้แบบสอบถามข้อมูลทั่วไป และ แบบประเมินสมรรถนะการพยาบาลตามบทบาทหน้าที่ ในการพยาบาลผู้ป่วยโรคหัวใจไร้รอยต่อของ สมาคมพยาบาลโรคหัวใจ และทรวงอก (ประเทศไทย) มีค่าความเชื่อมั่นเท่ากับ 0.84 วิเคราะห์ข้อมูล  ทั่วไปของกลุ่มตัวอย่าง และสมรรถนะการพยาบาลตามบทบาทหน้าที่โดยใช้สถิติเชิงพรรณนา

ผลการศึกษา : สมรรถนะตามบทบาทหน้าที่ของพยาบาลวิชาชีพในการพยาบาลผู้ป่วยโรคหัวใจ และ หลอดเลือดแบบไร้รอยต่อมีค่าเฉลี่ยในระดับปานกลางทุกด้านตามลำดับดังนี้ 1. ความสามารถในด้าน การประเมินผู้ป่วย 2. ความสามารถในด้านการเสริมสร้างพลังอำนาจ 3. ความสามารถด้านปฏิบัติการ ช่วยฟื้นคืนชีพ (CPR) 4.ความสามารถการบริหารยาผู้ป่วย และ 5. ความสามารถด้านการให้การพยาบาล และช่วยทำหัตถการในการพยาบาลผู้ป่วยโรคหัวใจ และหลอดเลือดแบบไร้รอยต่อ

Purpose: China employed a unique volunteerism system where health care providers outside of Hubei Province, the epicentre, travelled to reverse the devastation wrought by coronavirus disease 2019 (COVID-19) at its global onset. The aim is to study the unique circumstances of Chinese volunteerism in the context of continuing pandemic threats, specifically exploring the experiences of 20 Chinese nurse and physician volunteers fighting COVID-19 during the outbreak. Methods: Interviews were done through video calling. Newly licensed nurse turnover can be costly, causing a mismatch between supply and demand for nurse staffing needs, lowering quality outcomes, and decreasing patient satisfaction. This quantitative study was conducted to identify the relationship between why newly licensed nurses selected their first position and their retention in that initial position. AbstractVaccination hesitancy is high among African Americans and other people of color. The practice gap addressed in this project was the lack of a culturally sensitive clinical guideline for vaccination education relevant to the African American community. The practice-focused question was answered by developing an evidence-based Clinical Vaccination Education Guideline for the African American Community (CVEGAAC) that was validated by a group of four content experts using the Appraisal of Guidelines, Research, and Evaluation tool.
