More than 85% of Indonesian adults with Type 2 diabetes (T2DM) do not meet glycemic targets. More effective support from family members on diabetes self-management may improve diabetes management and outcomes. The study aimed to 1) compare glycated hemoglobin (HbA1c) levels and health-related quality of life (HRQoL) before and after participating in individual and family self-management programs (IFSM) and 2) compare HbA1c levels and HRQoL between adults with uncontrolled T2DM who received the IFSM program and those who received usual care. การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษาอนาคตภาพการบริหารจัดการองค์การพยาบาล โรงพยาบาลสังกัดกองทัพอากาศ ช่วงพุทธศักราช 2570-2574 โดยใช้เทคนิคการวิจัยแบบเดลฟาย ผู้ให้ข้อมูลคือผู้เชี่ยวชาญเกี่ยวกับการบริหารจัดการองค์การพยาบาล โรงพยาบาลสังกัดกองทัพอากาศ จำนวน 18 คน วิธีการดำเนินการวิจัยประกอบด้วย 3 รอบ คือ รอบที่ 1 สัมภาษณ์ความคิดเห็นของผู้เชี่ยวชาญเกี่ยวกับอนาคตภาพการบริหารจัดการองค์การพยาบาล โรงพยาบาลสังกัดกองทัพอากาศ ช่วงพุทธศักราช 2570-2574 รอบที่ 2 นำข้อมูลที่ได้จากการสัมภาษณ์มาวิเคราะห์เนื้อหาแล้วสร้างแบบสอบถาม เพื่อให้ผู้เชี่ยวชาญแสดงประมาณค่าแนวโน้มความน่าจะเป็นในแต่ละข้อ และรอบที่ 3 นำข้อมูลที่ได้จากรอบ 2 มาคำนวณหาค่ามัธยฐาน ค่าพิสัยระหว่างควอไทล์ และจัดทำแบบสอบถามให้ผู้เชี่ยวชาญยืนยันความคิดเห็น เพื่อสรุปเป็นอนาคตภาพการบริหารจัดการองค์การพยาบาล โรงพยาบาลสังกัดกองทัพอากาศ ช่วงพุทธศักราช 2570-2574 ผลการวิจัยพบว่า อนาคตภาพการบริหารจัดการองค์การพยาบาล โรงพยาบาลสังกัดกองทัพอากาศ ช่วงพุทธศักราช 2570-2574 ประกอบด้วย 7 ด้าน ดังนี้ 1) ด้านวางแผนการบริหารองค์การ ได้แก่ การวิเคราะห์ภาระงานยุทธการด้านการแพทย์ทหาร 2) ด้านรูปแบบการบริการพยาบาล ได้แก่ การให้บริการเชิงรุกแก่ข้าราชการและครอบครัว รวมถึงผู้ทำการในอากาศ 3) ด้านการจัดการความรู้และเทคโนโลยีสารสนเทศ ได้แก่ การนำเทคโนโลยีสารสนเทศมาใช้ในการบริหารจัดการ 4) ด้านนวัตกรรมและการพัฒนางานคุณภาพ ได้แก่ การยกระดับความสำเร็จในการผลิตผลงานวิจัย นวัตกรรมทางการพยาบาลและงานพัฒนาคุณภาพ 5) ด้านการจัดการทรัพยากรมนุษย์ ได้แก่ การพัฒนาทักษะที่จำเป็นให้กับบุคลากรพยาบาลอย่างต่อเนื่อง 6) ด้านสนับสนุนการจัดการศึกษา ได้แก่ การพัฒนาหลักสูตรพยาบาลศาสตรบัณฑิตให้มีคุณลักษณะและสมรรถนะเชิงวิชาชีพเป็นไปตามความต้องการขององค์การพยาบาล และ 7) ด้านการเตรียมพร้อมรับภารกิจด้านการแพทย์ทหาร สถานการณ์ฉุกเฉินและการบรรเทาสาธารณภัย ได้แก่ การพัฒนาศักยภาพของบุคลากรพยาบาลด้านการปฏิบัติการแพทย์ฉุกเฉิน

การวิจัยครั้งนี้เป็นการศึกษาความสัมพันธ์เชิงทำนายเพื่อศึกษาปัจจัยทำนายความรู้สึกกลัวความก้าวหน้าของความเจ็บป่วยในบุคคลภายหลังเกิดภาวะกล้ามเนื้อหัวใจตายเฉียบพลัน กลุ่มตัวอย่าง คือ บุคคลที่ได้รับการวินิจฉัยจากแพทย์ว่ามีภาวะกล้ามเนื้อหัวใจตายเฉียบพลัน จำนวน 214 คน ที่มารับบริการที่แผนกผู้ป่วยนอกอายุรกรรมโรคหัวใจและหลอดเลือด โรงพยาบาลพระมงกุฎเกล้าและโรงพยาบาลนพรัตนราชธานี คัดเลือกกลุ่มตัวอย่างด้วยวิธีการสุ่มแบบเจาะจง เก็บข้อมูลโดยใช้แบบสอบถามทั้งหมด 9 ส่วน คือ 1) แบบสอบถามข้อมูลส่วนบุคคล 2) แบบสอบถามความรู้เกี่ยวกับโรคหลอดเลือดหัวใจ 3) แบบสอบถามการรับรู้อาการเจ็บหน้าอก 4) แบบสอบถามการรับรู้ความเจ็บป่วยฉบับย่อ 5) แบบสอบถามความรู้สึกกลัวความก้าวหน้าของโรคฉบับย่อ 6) แบบสอบถามความวิตกกังวลในผู้ป่วยโรคหัวใจ 7) แบบประเมินความรู้สึกไม่แน่นอนในความเจ็บป่วย 8) แบบสอบถามการรับรู้สมรรถนะแห่งตน และ 9) แบบสอบถามการสนับสนุนทางสังคม ผ่านการตรวจสอบความตรงตามเนื้อหาจากผู้ทรงคุณวุฒิ 5 ท่าน ได้ค่าดัชนีความตรงตามเนื้อหาเท่ากับ .70, 1, .77, .83, .94, .91, .92 และ 1 ตามลำดับ และผ่านการตรวจสอบความเที่ยงของเครื่องมือเท่ากับ .70, .71, .84, .80, .81, .84 และ .89 ตามลำดับ วิเคราะห์ข้อมูลโดยคำนวณสัมประสิทธิ์สหสัมพันธ์พหุคูณ ผลการศึกษาพบว่า 1. บุคคลภายหลังเกิดภาวะกล้ามเนื้อหัวใจตายเฉียบพลันมีความรู้สึกกลัวความก้าวหน้าของความเจ็บป่วยในระดับน้อยคิดเป็นร้อยละ 23.8 มีความรู้สึกกลัวความก้าวหน้าของความเจ็บป่วยในระดับปานกลางคิดเป็นร้อยละ 57.9 มีความรู้สึกกลัวความก้าวหน้าของความเจ็บป่วยในระดับมากคิดเป็นร้อยละ 18.2 และบุคคลภายหลังเกิดภาวะกล้ามเนื้อหัวใจตายเฉียบพลันมีความรู้สึกกลัวความก้าวหน้าของความเจ็บป่วยโดยรวมอยู่ในระดับปานกลาง (Mean = 36.92, SD = 9.89) 2. ปัจจัยทำนายความรู้สึกกลัวความก้าวหน้าของความเจ็บป่วยในบุคคลภายหลังเกิดภาวะกล้ามเนื้อหัวใจตายเฉียบพลัน ประกอบด้วย 4 ตัวแปร ได้แก่ ความวิตกกังวล (Beta = .369) ความรู้สึกไม่แน่นอนในความเจ็บป่วย (Beta = .281) การรับรู้ความเจ็บป่วย (Beta = .119) และการรับรู้สมรรถนะแห่งตน (Beta = - .309) สามารถทำนายความรู้สึกกลัวความก้าวหน้าของความเจ็บป่วยในบุคคลภายหลังเกิดภาวะกล้ามเนื้อหัวใจตายเฉียบพลันคิดเป็นร้อยละ 69.6 (Adjusted R2 …

วิจัยนี้เป็นการวิจัยกึ่งทดลองแบบสองกลุ่มวัดแบบอนุกรมเวลามีการให้สิ่งทดลองซ้ำ มีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมการนวดกดจุดสะท้อนฝ่าเท้าต่ออาการปวดหลังส่วนล่างในผู้ป่วยภายหลังได้รับการตรวจสวนหัวใจ กลุ่มตัวอย่าง คือ ผู้ป่วยโรคหลอดเลือดหัวใจทั้งเพศชายและเพศหญิง อายุ 40 ปีขึ้นไป ที่ได้รับการตรวจสวนหัวใจผ่านหลอดเลือดแดงบริเวณขาหนีบ ณ โรงพยาบาลธรรมศาสตร์เฉลิมพระเกียรติ จังหวัดปทุมธานี เลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจงตามคุณสมบัติที่กำหนด แบ่งเป็นกลุ่มควบคุมและกลุ่มทดลองกลุ่มละ 19 คน โดยจับคู่กลุ่มตัวอย่างให้มีคุณสมบัติคล้ายคลึงในด้านเพศ อายุ ชนิดและปริมาณยาลดปวดที่ได้รับ กลุ่มควบคุมได้รับการพยาบาลตามปกติ และกลุ่มทดลองได้รับโปรแกรมการนวดกดจุดสะท้อนฝ่าเท้า พัฒนาขึ้นโดยใช้กรอบแนวคิดทางสรีระวิทยาการเกิดอาการปวดหลังส่วนล่างในผู้ป่วยภายหลังได้รับการตรวจสวนหัวใจร่วมกับการทบทวนวรรณกรรม เครื่องมือที่ใช้ในงานวิจัย ได้แก่ แบบบันทึกข้อมูลส่วนบุคคล และแบบประเมินอาการปวดหลังส่วนล่างโดยใช้มาตรวัดความปวดด้วยสายตา (Visual analog scale) ผ่านการตรวจสอบความตรงตามเนื้อหาจากผู้ทรงคุณวุฒิ 5 คน และได้ค่าความเที่ยงด้วยวิธีการวัดซ้ำเท่ากับ 0.83 วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา สถิติการวิเคราะห์ความแปรปรวนแบบวัดซ้ำ และสถิติทดสอบค่าที ผลการวิจัยสรุปได้ดังนี้ 1. ผู้ป่วยภายหลังได้รับการตรวจสวนหัวใจในกลุ่มที่ได้รับโปรแกรมการนวดกดจุดสะท้อนฝ่าเท้า หลังได้รับโปรแกรมในชั่วโมงที่ 2, 4 และ 6 มีคะแนนอาการปวดหลังส่วนล่างน้อยกว่าก่อนได้รับโปรแกรมอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05 2. ผู้ป่วยภายหลังได้รับการตรวจสวนหัวใจในกลุ่มที่ได้รับโปรแกรมการนวดกดจุดสะท้อนฝ่าเท้ามีคะแนนอาการปวดหลังส่วนล่างน้อยกว่ากลุ่มที่ได้รับการพยาบาลตามปกติ ในชั่วโมงที่ 2, 4 และ 6 อย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05

การวิจัยแบบกึ่งทดลองครั้งนี้มีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมพัฒนาความรอบรู้ด้านสุขภาพต่อพฤติกรรมป้องกันภาวะน้ำเกินในผู้ที่มีภาวะหัวใจล้มเหลว กลุ่มตัวอย่างเป็นผู้ที่มีภาวะหัวใจล้มเหลว เพศชายและหญิง อายุตั้งแต่ 18 ปีขึ้นไป คัดเลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจงตามคุณสมบัติที่กำหนด โดยจับคู่ให้มีลักษณะคล้ายคลึงกันในเรื่องเพศ อายุ ระดับการศึกษา แบ่งเป็นกลุ่มควบคุมและกลุ่มทดลอง กลุ่มละ 30 คน โดยกลุ่มควบคุมได้รับการพยาบาลตามปกติและกลุ่มทดลองได้รับโปรแกรมพัฒนาความรอบรู้ด้านสุขภาพร่วมกับการพยาบาลตามปกติ เครื่องมือที่ใช้ในการวิจัย ได้แก่ แบบสอบถามพฤติกรรมป้องกันภาวะน้ำเกิน และแบบสอบถามประเมินความรอบรู้ด้านสุขภาพเกี่ยวกับหัวใจล้มเหลวและภาวะน้ำเกิน ผ่านการตรวจสอบจากผู้ทรงคุณวุฒิ 5 คน ได้ค่าดัชนีความตรงตามเนื้อหาเท่ากับ .95 และ .80 และมีค่าความเชื่อมั่นเท่ากับ .91 และ .93 ตามลำดับ วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนาและเปรียบเทียบความแตกต่างด้วยสถิติทดสอบที ผลการวิจัยสรุปได้ดังนี้ 1. คะแนนเฉลี่ยพฤติกรรมป้องกันภาวะน้ำเกินในผู้ที่มีภาวะหัวใจล้มเหลวในกลุ่มทดลองหลังได้รับโปรแกรมพัฒนาความรอบรู้ด้านสุขภาพสูงกว่าก่อนได้รับโปรแกรมพัฒนาความรอบรู้ด้านสุขภาพอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2. คะแนนเฉลี่ยพฤติกรรมป้องกันภาวะน้ำเกินในผู้ที่มีภาวะหัวใจล้มเหลวที่ได้รับโปรแกรมพัฒนาความรอบรู้ด้านสุขภาพสูงกว่ากลุ่มที่ได้รับการพยาบาลตามปกติอย่างมีนัยสำคัญทางสถิติที่ระดับ .05

Background
The experiences of mothers enrolled in Maternal, Infant and Early Childhood Home Visiting (MIECHV) program with virtual home visiting (VHV) during the pandemic remain mostly unknown. This study aimed to describe in detail the experience of home visitors and mothers with VHV during COVID-19 pandemic. This is a prerequisite for guiding future efforts to optimize MIECHV services that are provided through virtual operation.

Melanomas are outstandingly aggressive cancers, still their incidence is increasing. They can occur in the skin as cutaneous melanoma or in the eye as ocular melanoma. Their pathogenesis is generally similar; however, the prognosis of ocular melanoma patients is still poor, due to its special genetic profile and tumor microenvironment. Here we review the similarities and differences between cutaneous and ocular melanoma, with a focus on their prognostic significance, their molecular signature, and their tumor microenvironment, in order to understand the strategies adopted to escape immune response. Background: Children in the US spend an average of 7.5 hours a day in front of a screen, well above the recommended 2 hours per day for entertainment. Screen time of greater than 3 hours per day is associated with higher body mass indices and insulin resistance that can lead to type 2 diabetes. Providers should review and reinforce age-appropriate screen time guidelines at well-child visits. Screen time education and behavior contracts can be used to facilitate age-appropriate screen time.

This systematic review explores the published literature and determines the impact of educating nursing staff on the indication of urinary catheter, insertion of foley catheters, maintenance of urinary catheters, and periodical communication between nurses and physicians for changing foley catheters can reduce the CAUTI rates in adult patients in the acute care hospital setting. Purpose: This systematic review seeks to explore whether utilizing Eat, Sleep, Console (ESC) over the Finnegan Neonatal Abstinence Scale (FNASS) in the neonatal abstinence syndrome (NAS) population decreases length of stay or pharmacological interventions.
Method: Systematic literature review utilizing CINAHL, PubMed and Google Scholar databases.
Results: Utilizing Eat Sleep Console, over FNASS decreased length of stay and pharmacologic intervention.
Conclusions: There is compelling research suggesting a change in NAS treatment from FNASS to ESC with a decrease in length of stay and pharmacologic intervention. However, there is an opportunity for further studies providing higher levels of evidence as the current …

วัตถุประสงค์: เพื่อเปรียบเทียบคะแนนเฉลี่ยความปวดของผู้ป่วยโรคระบบทางเดินอาหารหลังผ่าตัดเปิดช่องท้อง ระหว่างกลุ่มทดลองที่ได้รับการใช้หมอนข้าวไรซ์เบอรี่ประคบเย็นและกลุ่มควบคุมที่ได้รับการพยาบาลปกติในวันที่ 1, 2 และ 3 หลังผ่าตัด
รูปแบบการวิจัย: การวิจัยเชิงทดลองรูปแบบสุ่มที่มีกลุ่มควบคุมและมีการวัดซ้ำ
วิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้ป่วยที่ได้รับการวินิจฉัยว่าเป็นโรคระบบทางเดินอาหารเกี่ยวกับส่วนของกระเพาะอาหาร ลำไส้เล็ก ไส้ติ่ง ถุงน้ำดี ลำไส้ใหญ่และทวารหนัก และรอรับการผ่าตัดเปิดช่องท้อง ที่รับไว้เป็นผู้ป่วยในหอผู้ป่วยศัลยกรรม โรงพยาบาลระยอง จำนวน 40 คน จับฉลากเข้ากลุ่มควบคุมและกลุ่มทดลอง กลุ่มละ 20 คน เครื่องมือที่ใช้ในการวิจัย คือ โปรแกรมการประคบเย็นด้วยหมอนข้าวไรซ์เบอรี่ แบบประเมินความปวดมาตรวัดตัวเลข มีค่าสัมประสิทธิ์สหสัมพันธ์ของเพียร์สัน r = .80 และแบบบันทึกการใช้นวัตกรรมหมอนข้าวไรซ์เบอรี่ประคบเย็น วิเคราะห์ข้อมูลโดยใช้สถิติ Independent t-test และ Repeated Measured ANOVA
ผลการศึกษา: ผู้ป่วยหลังผ่าตัดเปิดช่องท้องที่ได้รับการประคบเย็นด้วยหมอนข้าวไรซ์เบอรี่ ในวันที่ 1, 2 และ 3 หลังผ่าตัด มีคะแนนเฉลี่ยความปวดต่ำกว่าผู้ป่วยหลังผ่าตัดเปิดช่องท้องที่ได้รับการพยาบาลตามปกติ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (p <.05)

Introduction: Clostridium difficile infection (CDI) is the most prevalent nosocomial infection pathogen responsible for antibiotic associated diarrhea. CDI is a significant cause of morbidity and mortality rates of medical patients and has increased due to frequent broad spectrum antibiotic usage. CDI is associated with a mortality rate of 25% amongst medical patients. At this catholic hospital in the Northeast of the United States, there is currently no official intervention implemented to prescribe a probiotic when patients are started on antibiotics. Background: Implementation of a universal suicidality screening is considered best practice as it is associated with improved the detection of occult, or latent, suicide risk and can reduce subsequent risk. This quality improvement (QI) project evaluates the implementation of the Columbia-Suicide Severity Rating Scale (C-SSRS) to screen patients over the age of twelve at a regional healthcare system.

Objectives: To investigate the incidence of fatphobic behaviors among the healthcare team and how nursing students’ and recent graduates’ observations of fatphobic behaviors impact the provision of affirming care.
Background: Weight stigma among healthcare providers can lead to fatphobia (e.g., hurtful or stigmatizing language, dismissal of symptoms). These experiences can negatively impact patients’ health and experiences with healthcare, leading some patients to avoid or delay seeking healthcare services. Objective: The next generation of nurses have increased educational preparation to provide affirming care to lesbian, gay, bisexual, transgender, and queer (LGBTQ+) people, but an evaluation of the effectiveness of this teaching has not been performed. The purpose of this study was to identify nursing student and recent nursing graduate observations of affirming and non-affirming care of LGBTQ+ people in healthcare settings during clinical experiences.
