Abstract

Background:

In the U.S., late preterm (LP) births, which occur at 340/7-366/7 weeks gestation, make up the largest and fastest-growing group of preterm births. Although there is a trend of increased rates of LP infant readmissions compared to term infant readmissions, the relationship between LP infant morbidities and method of feeding has not been extensively studied.

Objective:

The purposes of this study were to 1) identify the primary causes of LP infant hospital readmissions and, 2) correlate the primary causes of LP infant hospital readmissions with the method of feeding.

This study looked at the impact ten minutes of daily yoga had on the behavior of elementary school children. Yoga was led by classroom teachers once daily for ten minutes over a sixteen-week period. At weeks one, eight, and sixteen, classroom teachers evaluated student behavior in six categories: personal awareness, attention, rapport with friends, rapport with teacher, following rules and transition between activities. The teachers rated the behaviors on a never, sometimes, mostly or always scale. The data was then analyzed and interpreted. Teachers are with their students an average of seven to 10 hours a day, five days a week. The substantial amount of time teachers spend with their students allows them to provide helpful insight into health problems children face. The purpose of this study was to identify elementary school teachers’ concerns for student health and to assess their knowledge of resources that address those health concerns. In a caring profession like nursing, the risk of sending newly-graduated nurses with low resilience into the workplace provided impetus for the study to explore the association between demographic, work environment and resilience characteristics among registered nurses employed in acute care setting. Select subscales from the PES-NWI were used to assess Nurse Manager Ability, Leadership and Support of Nurses; Staffing and Resource Adequacy; and Collegial Nurse-Physician Relations (Lake, 2002) and the Resilience Scale was used to measure resilience (Wagnild & Young, 1993). Introduction

Critical care nurses are responsible for administering sedative medications to mechanically ventilated patients. With significant advancements in the understanding of the impact of sedative exposure on physiological and psychological outcomes of ventilated patients, updated practice guidelines for assessment and management of pain, agitation, and delirium in the intensive care unit were released in 2013. The primary aim of this qualitative study was to identify and describe themes derived from critical care nurses' comments regarding sedation administration practices with mechanically ventilated patients.

The commercial sexual exploitation of children (CSEC) is a crime of abuse that is frequently unrecognized because mandated reporters may be unfamiliar with the indicators. Commercially sexually exploited minors participate in sex acts in exchange for money, goods, or services. At least 200,000 American children are believed to be victims of this crime each year (County Welfare Director Association of California, 2015). Victims often go undiscovered and experience significant health issues. The purpose of this study was to address the perceived and actual knowledge levels of sexually transmitted diseases (STDS) among college students at a southern university. The sample consisted of 138 participants over the age of 18 enrolled as undergraduate students at the University of Southern Mississippi (USM). Diabetes Mellitus is a disease that is becoming more prevalent in the world each day, and it can be deadly if it is not properly controlled. There are multiple aspects that go into self-management of diabetes such as diet, glucose control, physical activity, and lifestyle choices. This study focused on the physical activity aspect of diabetes self-management. Chronic pain is an ever present issue in the United States, with more people suffering from it than heart disease, cancer, and diabetes combined. Chronic pain is the most frequent complaint in primary care, and it poses significant challenges to both primary care providers (PCPs) and their patients. At the root of many of these challenges is the prescription and management of opioid prescription drugs used to treat chronic pain. โรคจิตเภท เป็นโรคที่มีความผิดปกติทางจิต ที่มีอาการของโรคเรื้อรังและซับซ้อน และมีอัตราการเพิ่มปริมาณของผู้ป่วยมากขึ้นในทุกปี ทั้งในระดับนานาชาติ และในประเทศไทย เป็นผลมากจากการเจ็บป่วยเรื้อรังซึ่งมีการดูแลรักษาที่จำกัด จนก่อให้เกิดความพิการทางด้านจิตใจ และด้านสังคมของผู้ป่วย ทำให้มีผู้ป่วยจำนวนมาก เข้ารับการรักษาในโรงพยาบาลจิตเวชทั้งแผนกผู้ป่วยใน และผู้ป่วยนอก ปัจจุบันแม้ว่าแผนพัฒนาสุขภาพแห่งชาติ ฉบับที่ 12 ซึ่งต่อเนื่องมาจาก แผนฉบับที่ 11 มีนโยบายให้ลดระยะเวลาการรักษาในโรงพยาบาล และให้ผู้ป่วยกลับสู่ชุมชน ภายใต้การดูแลของผู้ดูแล จากครอบครัว และชุมชน ในการดูแลผู้ป่วยที่บ้าน แต่จากการศึกษา พบว่า ครอบครัวและชุมชน ไม่มีความพร้อมในการรับผู้ป่วยกลับไปดูแลต่อที่บ้าน ดังนั้น พยาบาลสุขภาพจิต และจิตเวช จึงต้องแสดงบทบาทพยาบาลจิตเวช ในการเตรียมความพร้อมให้ครอบครัว สามารถให้การดูแลผู้ป่วยจิตเภทในชุมชนอย่างเหมาะสมต่อไป

วัตถุประสงค์:เพื่อศึกษาผลของโปรแกรมการควบคุมน้ำหนักต่อพฤติกรรมการบริโภคและพฤติกรรมการทำกิจกรรมทางกายของเด็กวัยเรียนที่มีภาวะโภชนาการเกิน\n\nรูปแบบการวิจัย: การวิจัยกึ่งทดลอง\n\nวิธีการดำเนินการวิจัย: การศึกษาครั้งนี้ใช้กรอบแนวคิดวงจรการเรียนรู้จากประสบการณ์ของ Kolb และแรงสนับสนุนทางสังคมของ House กลุ่มตัวอย่างคือเด็กวัยเรียนชั้นประถมศึกษาปีที่ 4 ที่มีภาวะโภชนาการเกินจำนวน 60 คน จาก 2 โรงเรียนๆ ละ 30 คน กลุ่มทดลองได้รับโปรแกรมการควบคุมน้ำหนัก 4 ครั้ง ในระยะเวลา 6 สัปดาห์ ประกอบด้วยการเรียนรู้จากการประเมินภาวะโภชนาการด้วยตนเองและการให้ความรู้ การบันทึกพฤติกรรมการบริโภคและการทำกิจกรรมทางกาย การอภิปรายกลุ่มและสะท้อนความคิด การสรุปแนวทางการควบคุมน้ำหนัก และการปฏิบัติจริงโดยใช้แรงสนับสนุนทางสังคมจากเพื่อน ผู้ปกครอง และครู กลุ่มควบคุมดำเนินชีวิตตามปกติ\n\nผลการวิจัย: คะแนนเฉลี่ยพฤติกรรมการบริโภคอาหารและพฤติกรรมการทำกิจกรรมทางกายระหว่างกลุ่มทดลองและกลุ่มควบคุมไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p> .05) เมื่อเปรียบเทียบก่อนและหลังทดลอง เด็กวัยเรียนกลุ่มทดลองมีพฤติกรรมการทำกิจกรรมทางกายสูงกว่าก่อนการทดลอง (d = 3.70, SD =1.36) และมีการเปลี่ยนแปลงภาวะโภชนาการในระดับอ้วนลดลงจากร้อยละ 46.70 เป็นร้อยละ 30 ซึ่งมีความแตกต่างอย่างไม่มีนัยสำคัญทางสถิติที่ระดับ .05\n\nสรุป:พยาบาลควรจัดโปรแกรมควบคุมน้ำหนักสำหรับเด็กวัยเรียนโดยการส่งเสริมการมีส่วนร่วม ของครอบครัวและโรงเรียนในการวางแผน การดำเนินกิจกรรม และการติดตามประเมินผล

วัตถุประสงค์ : เพื่อ1)พัฒนาแบบประเมินสมรรถนะของพยาบาลวิชาชีพที่ปฏิบัติงานนอกเวลาราชการ โรงพยาบาลชุมชนขนาดเล็ก จังหวัดน่าน และ 2)ประเมินคุณภาพของแบบประเมินสมรรถนะฯ\n\nแบบแผนการวิจัย: การวิจัยแบบผสมผสาน\n\nวิธีการดำเนินการวิจัย: ผู้ให้ข้อมูลหลักมี 2 กลุ่ม (1) ผู้ให้ข้อมูลหลักสำหรับการพัฒนาแบบประเมินสมรรถนะของพยาบาลวิชาชีพที่ปฏิบัติงานนอกเวลาราชการ คัดเลือกแบบเฉพาะเจาะจงจำนวน 18 คน ประกอบด้วย หัวหน้าหอผู้ป่วย 3 คน พยาบาลเวชปฏิบัติทั่วไป 3 คน แพทย์ 2 คน ผู้ป่วย 5 คน และผู้ดูแลผู้ป่วย 5 คน และ (2) สำหรับการประเมินคุณภาพของแบบประเมินสมรรถนะที่พัฒนาขึ้น ประกอบด้วย ผู้ทรงคุณวุฒิสำหรับตรวจสอบความตรงของแบบประเมินสมรรถนะ 4 คน และผู้ให้ข้อมูลสำหรับตรวจสอบความเที่ยงของแบบประเมิน ได้แก่ ผู้บริหารการพยาบาล 2 คน และพยาบาลวิชาชีพที่ปฏิบัติงานนอกเวลาราชการ 5 คน เป็นผู้ประเมินและผู้ถูกประเมินตามลำดับ เครื่องมือการวิจัย คือ แบบวิเคราะห์สภาพการณ์และความคาดหวังเกี่ยวกับสมรรถนะของพยาบาลวิชาชีพที่ปฏิบัติงานนอกเวลาราชการที่ผู้วิจัยพัฒนาขึ้นตามแนวคิดของแมคคลีแลนด์ เฟย์และมิลท์เนอร์ และสภาการพยาบาล วิเคราะห์ข้อมูลโดยสถิติ ค่าเฉลี่ย ร้อยละ และการวิเคราะห์เนื้อหา\n\nผลการวิจัย: พบว่า 1) แบบประเมินสมรรถนะของพยาบาลวิชาชีพที่ปฏิบัติงานนอกเวลาราชการประกอบด้วยสมรรถนะหลัก 5 ข้อ ได้แก่ (1) มีคุณธรรมและจริยธรรมในการปฏิบัติการพยาบาล (2) สื่อสารอย่างมีประสิทธิภาพและมีมนุษยสัมพันธ์ (3) มีความรู้และทักษะในงานอุบัติเหตุและฉุกเฉิน (4) มีความรู้และทักษะในการดูแลผู้ป่วยทางสูติกรรม และ (5) มีความรู้และทักษะในงานหอผู้ป่วย และสมรรถนะย่อยรวม 26 ข้อ 2) แบบประเมินสมรรถนะมีค่าความตรงตามเนื้อหาของแบบประเมินเท่ากับ 0.97 และ ค่าความเที่ยงระหว่างผู้ประเมินเท่ากับ 0.80\n\nสรุป:จากผลการตรวจสอบคุณภาพของแบบประเมินที่พัฒนาขึ้นแสดงให้เห็นว่ามีคุณภาพและมีความเหมาะสมเพียงพอที่จะนำไปใช้ในการประเมินสมรรถนะของพยาบาลวิชาชีพที่ปฏิบัติงานนอกเวลาราชการ โรงพยาบาลชุมชนขนาดเล็กต่อไป

ปัจจัยทำนายพฤติกรรมการจัดการตนเองของผู้สูงอายุ กลุ่มอาการโรคหลอดเลือดหัวใจเฉียบพลัน, ภรปภา จันทร์ศรีทอง, ทัศนา ชูวรรธนะปกรณ์

วัตถุประสงค์ : เพื่อศึกษาพฤติกรรมการจัดการตนเอง และปัจจัยทำนายพฤติกรรมการจัดการตนเองของผู้สูงอายุกลุ่มอาการโรคหลอดเลือดหัวใจเฉียบพลัน ในโรงพยาบาลตติยภูมิ กรุงเทพมหานคร\n\nรูปแบบการวิจัย : การวิจัยแบบพรรณนาเชิงทำนาย\n\nวิธีดำเนินการวิจัย : กลุ่มตัวอย่าง คือ ผู้ที่อายุ 60 ปีขึ้นไป ที่ได้รับการวินิจฉัยว่ามีภาวะกลุ่มอาการโรคหลอดเลือดหัวใจเฉียบพลัน มารับการตรวจรักษาที่แผนกผู้ป่วยนอกอายุรกรรมโรคหัวใจและหลอดเลือด โรงพยาบาลจุฬาลงกรณ์ และโรงพยาบาลตำรวจ จำนวน 111 คน โดยการสุ่มอย่างง่าย เครื่องมือที่ใช้ในการวิจัย คือ แบบสอบถามข้อมูลส่วนบุคคล แบบสอบถามพฤติกรรมการจัดการตนเอง แบบสอบถามการรับรู้สมรรถนะแห่งตน แบบประเมินเรื่องความเปราะบาง และแบบสอบถามการสนับสนุนจากครอบครัว และหาค่าความเที่ยงของแบบสอบถามได้เท่ากับ 0.97, 0.91, 0.80 และ 0.99 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้การแจกแจงความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน ค่าสัมประสิทธิ์สหสัมพันธ์ของเพียรสัน และสถิติวิเคราะห์ถดถอยพหุคูณแบบถดถอยแบบใช้ทุกตัวแปรเป็นตัวทำนาย\n\nผลการวิจัย : \n1. ผู้สูงอายุกลุ่มอาการโรคหลอดเลือดหัวใจเฉียบพลัน มีพฤติกรรมการจัดการตนเองโดยรวมอยู่ในระดับมาก ( = 63.19, SD =11.68)\n2. การสนับสนุนจากครอบครัว, การรับรู้สมรรถนะแห่งตนมีความสัมพันธ์ทางบวกระดับสูงกับพฤติกรรมการจัดการตนเอง อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (r = 0.739 และ r = 0.738), เพศมีความสัมพันธ์ทางบวกระดับต่ำ กับพฤติกรรมการจัดการตนเอง อย่างมีนัยสำคัญทางสถิติ ที่ระดับ .05 (r=0.169) ความเปราะบาง มีความสัมพันธ์ทางลบระดับปานกลางกับพฤติกรรมการจัดการตนเองอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (r = -.599) และภาวะโรคร่วมไม่มีความสัมพันธ์กับพฤติกรรมการจัดการตนเอง\n3. การรับรู้สมรรถนะแห่งตน การสนับสนุนจากครอบครัว เพศ และความเปราะบาง สามารถร่วมทำนายพฤติกรรมการจัดการตนเองของกลุ่มตัวอย่างได้ ร้อยละ 69.6 (R2=.696) อย่างมีนัยสำคัญทางสถิติที่ระดับ .05\n\nสรุป : พยาบาลควรส่งเสริมพฤติกรรมการจัดการตนเองของผู้สูงอายุ โดยเน้นการเพิ่มสมรรถนะแห่งตน ร่วมกับการให้ครอบครัวเข้ามามีส่วนร่วม ทั้งนี้ต้องคำนึงถึงเพศ และภาวะเปราะบางของผู้สูงอายุแต่ละคนเพื่อนำไปสู่ผลลัพธ์คือคงไว้ซึ่งพฤติกรรมการจัดการตนเองที่เหมาะสมและการคงไว้ซึ่งคุณภาพชีวิตที่ดีต่อไป

วัตถุประสงค์: 1) เพื่อเปรียบเทียบภาวะซึมเศร้าในผู้เสพติดสุราก่อนและหลังได้รับโปรแกรมการบำบัดทางความคิดและพฤติกรรมที่ใช้การสนับสนุนทางอินเทอร์เน็ต 2) เพื่อเปรียบเทียบภาวะซึมเศร้าในผู้เสพติดสุราระหว่างกลุ่มที่ได้รับโปรแกรมการบำบัดทางความคิดและพฤติกรรมที่ใช้การสนับสนุนทางอินเทอร์เน็ตและกลุ่มที่ได้รับการพยาบาลตามปกติ\n\nแบบแผนงานวิจัย: การวิจัยครั้งนี้เป็นการวิจัยกึ่งทดลองแบบสองกลุ่มวัดก่อนและหลังการทดลอง\n\nวิธีดำเนินการวิจัย:กลุ่มตัวอย่าง คือ ผู้เสพติดสุราชายที่มีภาวะซึมเศร้าจำนวน 40 ราย ซึ่งมีคุณสมบัติตามเกณฑ์ ที่เข้ารับการบำบัดรักษาแบบผู้ป่วยนอก โรงพยาบาลธัญญารักษ์ขอนแก่น จังหวัดขอนแก่น ได้รับการจับคู่ด้วยระดับคะแนนภาวะซึมเศร้า และสุ่มเข้ากลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 20 ราย กลุ่มทดลองได้รับโปรแกรมการบำบัดทางความคิดและพฤติกรรมที่ใช้การสนับสนุนทางอินเทอร์เน็ต ส่วนกลุ่มควบคุมได้รับการพยาบาลตามปกติ เครื่องมือที่ใช้ในการวิจัยคือ 1) โปรแกรมการบำบัดทางความคิดและพฤติกรรมที่ใช้การสนับสนุนทางอินเทอร์เน็ต 2) แบบประเมินภาวะซึมเศร้าของเบค (Beck Depression Inventory) 3) แบบประเมินความคิดอัตโนมัติด้านลบ (Automatic Though Questionnaire) และ 4) แบบสอบถามข้อมูลส่วนบุคคล เครื่องมือทุกชุดผ่านการตรวจสอบความตรงเชิงเนื้อหาจากผู้ทรงคุณวุฒิจำนวน 5 ท่าน เครื่องมือ ชุดที่ 2 และ 3 มีค่าความเที่ยงอัลฟาของคอนบราคเท่ากับ .81 และ .89 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติร้อยละและใช้สถิติที\n\nผลการวิจัย: 1. ภาวะซึมเศร้าในผู้เสพติดสุราชายหลังได้รับโปรแกรมการบำบัดทางความคิดและพฤติกรรมที่ใช้การสนับสนุนทางอินเทอร์เน็ตต่ำกว่าก่อนได้รับโปรแกรมการบำบัดทางความคิดและพฤติกรรมที่ใช้การสนับสนุนทางอินเทอร์เน็ตอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2. ภาวะซึมเศร้าในผู้เสพติดสุราชายที่ได้รับโปรแกรมการบำบัดทางความคิดและพฤติกรรมที่ใช้การสนับสนุนทางอินเทอร์เน็ตต่ำกว่ากลุ่มที่ได้รับการพยาบาลตามปกติ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05

วัตถุประสงค์ : เพื่อเปรียบเทียบภาวะซึมเศร้าในผู้สูงอายุโรคซึมเศร้าก่อนและหลังได้รับโปรแกรมการส่งเสริมความเข้มแข็งทางใจแบบครอบครัวมีส่วนร่วม และเพื่อเปรียบเทียบภาวะซึมเศร้าในผู้สูงอายุโรคซึมเศร้าระหว่างกลุ่มที่ได้รับโปรแกรมการส่งเสริมความเข้มแข็งทางใจแบบครอบครัวมีส่วนร่วมกับกลุ่มที่ได้รับการพยาบาลตามปกติ\n\nแบบแผนงานวิจัย : การวิจัยแบบกึ่งทดลอง\n\nวิธีการดำเนินการวิจัย : กลุ่มตัวอย่าง คือ ผู้สูงอายุโรคซึมเศร้าและผู้ดูแลหลักที่มารับบริการในแผนกจิตเวช ห้องตรวจผู้ป่วยนอกกองอายุรกรรม โรงพยาบาลภูมิพลอดุลยเดช กรมแพทย์ทหารอากาศ จำนวน 40 ครอบครัว จับคู่ให้มีลักษณะคล้ายคลึงกันด้วยอายุและคะแนนภาวะซึมเศร้า สุ่มเข้ากลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 20 ครอบครัว เครื่องมือวิจัยประกอบด้วย 1) โปรแกรมการส่งเสริมความเข้มแข็งทางใจแบบครอบครัวมีส่วนร่วม 2) แบบประเมินภาวะซึมเศร้าของเบค เครื่องมือทุกชุดได้ผ่านการตรวจสอบความตรงตามเนื้อหาจากผู้ทรงคุณวุฒิ จำนวน 5 ท่าน มีค่าความเที่ยงสัมประสิทธิ์อัลฟาของครอนบาค ของเครื่องมือแบบประเมินภาวะซึมเศร้าเท่ากับ .90 วิเคราะห์ข้อมูลโดยใช้ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และสถิติที\n\nผลการวิจัย :\n1) ค่าเฉลี่ยของคะแนนภาวะซึมเศร้าของผู้สูงอายุโรคซึมเศร้า หลังได้รับโปรแกรมการส่งเสริมความเข้มแข็งทางใจแบบครอบครัวมีส่วนร่วมลดลงกว่าก่อนทดลองอย่างมีนัยสำคัญทางสถิติที่ระดับ.05 (t=7.9)\n\n2) ค่าเฉลี่ยของคะแนนภาวะซึมเศร้าของผู้สูงอายุโรคซึมเศร้ากลุ่มที่ได้รับโปรแกรมการส่งเสริมความเข้มแข็งทางใจแบบครอบครัวมีส่วนร่วมลดลงกว่าผู้สูงอายุโรคซึมเศร้าที่ได้รับการพยาบาลตามปกติอย่างมีนัยสำคัญทางสถิติที่ระดับ.05 (t=-2.73)\n\nสรุป : โปรแกรมการส่งเสริมความเข้มแข็งทางใจแบบครอบครัวมีส่วนร่วม ทำให้ผู้สูงอายุโรคซึมเศร้ามีความเข้มแข็งทางใจสูงขึ้นและช่วยให้ภาวะซึมเศร้าของผู้สูงอายุโรคซึมเศร้าลดลง

Purpose: Nursing students are prone to stress-related diseases, such as depression and anxiety, which are associated with chronically elevated cortisol levels due to the excessive firing of the Hypothalamic Pituitary Adrenal (HPA) axis. This study assessed three speeds of yoga and how they acutely affected salivary cortisol levels within female nursing students. Methods: Students participated in three yoga sessions cadenced at different speeds in a non-randomized order: (i) standard-speed yoga (SSY), (ii) high-speed yoga (HSY), and (iii) slow-speed yoga (SLSY). The administration of opioids leads to postsurgical complications. Overview: Abilities Care Experts™ (ACE™) is an educational training program for nursing staff who care for individuals with dementia in skilled nursing facilities (SNF). The objective of the program is to provide the nursing staff with knowledge, and methods to engage residents’ remaining abilities in order to maximize occupational participation using a stage-specific dementia care approach. ACE™ training has yet to be evaluated for effectiveness in a controlled study design.
