อัตมโนทัศน์ทางเพศเป็นปัจจัยด้านจิต-สังคมที่มีอิทธิพลต่อพฤติกรรมทางเพศของบุคคล เนื่องจากเป็นความรู้สึกที่มีต่อพฤติกรรมทางเพศที่เกี่ยวกับตนเองและมีอิทธิพลต่อการสร้างความสัมพันธ์ กับผู้อื่นรวมทั้งการรับรู้เกี่ยวกับคุณลักษณะความสามารถของตนต่อพฤติกรรมทางเพศ การรับรู้อัตมโนทัศน์ ทางเพศ (Sexual self-concept) ประกอบด้วย 4 ด้าน คือ 1) ความพึงพอใจในการมีเพศสัมพันธ์ (Sexual satisfaction) 2) ความวิตกกังวลในการมีเพศสัมพันธ์ (Sexual anxiety) 3) การรับรู้สมรรถนะแห่ง ตนในการมีเพศสัมพันธ์ (Perceived sexual self-efficacy) และ 4) ภาวะซึมเศร้าในการมีเพศสัมพันธ์ (Sexual depression) แนวคิดดังกล่าวได้ประยุกต์ใช้กับผู้ป่วยโรคหัวใจวายและโรคกล้ามเนื้อหัวใจตายพบว่า อัตมโนทัศน์ทางเพศมีความสัมพันธ์ทางบวกกับพฤติกรรมทางเพศ เมื่อพิจารณารายด้าน พบว่าความพึงพอใจในการมีเพศสัมพันธ์และการรับรู้สมรรถนะแห่งตนในการมีเพศสัมพันธ์มีความสัมพันธ์ทางบวกกับ พฤติกรรมทางเพศของผู้ป่วยโรคหัวใจวายและโรคกล้ามเนื้อหัวใจตาย ขณะที่ความวิตกกังวลในการมีเพศสัมพันธ์และภาวะซึมเศร้าในการมีเพศสัมพันธ์มีความสัมพันธ์ทางลบกับพฤติกรรมทางเพศของผู้ป่วย โรคหัวใจวายและโรคกล้ามเนื้อหัวใจตาย

วัตถุประสงค์: เพื่อศึกษาคุณภาพชีวิตของผู้ป่วยภาวะหัวใจล้มเหลว และทำนายความรู้สึกไม่แน่นอนใน ความเจ็บป่วย ความแตกฉานด้านสุขภาพ และการสนับสนุนทางสังคมที่มีต่อคุณภาพชีวิตของผู้ป่วย ภาวะหัวใจล้มเหลว \nรูปแบบการวิจัย: การวิจัยเชิงความสัมพันธ์พยากรณ์ \nวิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้ป่วยภาวะหัวใจล้มเหลวอายุมากกว่าหรือเท่ากับ 18 ปี ที่มา ติดตามรับการรักษา ณ คลินิกโรคหัวใจ โรงพยาบาลสรรพสิทธิประสงค์ จังหวัดอุบลราชธานี จำนวน 85 ราย เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูล คือ แบบสอบถามข้อมูลส่วนบุคคล แบบสอบถามความ รู้สึกไม่แน่นอนในความเจ็บป่วยฉบับชุมชน แบบประเมินความแตกฉานด้านสุขภาพขั้นพื้นฐานสำหรับ ผู้ป่วยภาวะหัวใจล้มเหลว แบบวัดการช่วยเหลือทางสังคมแบบพหุมิติ และแบบสอบถามวิถีชีวิตของ ผู้ป่วยภาวะหัวใจล้มเหลว ผ่านการตรวจสอบความตรงตามเนื้อหาและวิเคราะห์ความเที่ยงแล้ว มีค่า ความเที่ยงมากกว่า 0.80 วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา สัมประสิทธิ์สหสัมพันธ์ของเพียร์สัน \nและการวิเคราะห์ถดถอยแบบเข้าพร้อมกัน \nผลการวิจัย: \n1. ผู้ป่วยภาวะหัวใจล้มเหลวมีคุณภาพชีวิตอยู่ในระดับปานกลางร้อยละ 54.1 \n2. ผู้ป่วยภาวะหัวใจล้มเหลวมีความแตกฉานด้านสุขภาพอยู่ในระดับไม่เพียงพอร้อยละ 47.1\n3. ความรู้สึกไม่แน่นอนในความเจ็บป่วยเพียงตัวแปรเดียวที่สามารถทํานายคุณภาพชีวิตของ ผู้ป่วยภาวะหัวใจล้มเหลวได้อย่างมีนัยสำคัญทางสถิติ (B = 651, p < .001) \n4. ความรู้สึกไม่แน่นอนในความเจ็บป่วย ความแตกฉานด้านสุขภาพ การสนับสนุนทางสังคม สามารถร่วมกันทำนายคุณภาพชีวิตของผู้ป่วยภาวะหัวใจล้มเหลวได้ร้อยละ 46.8 อย่างมีนัยสําคัญทางสถิติ AWANG (R2 = .468, p < .001) \nสรุปและข้อเสนอแนะ: \nการศึกษาครั้งนี้พบว่าความรู้สึกไม่แน่นอนในความเจ็บป่วยสามารถทำนายคุณภาพ ชีวิตได้สูง จึงควรนำผลการวิจัยที่ได้มาพัฒนาโปรแกรมการดูแลผู้ป่วย และมีการทดสอบประสิทธิผลของ โปรแกรม เพื่อลดความรู้สึกไม่แน่นอนในความเจ็บป่วยที่จะส่งผลให้ผู้ป่วยภาวะหัวใจล้มเหลวมีคุณภาพชีวิต ที่ดีต่อไป

Objective(s): The purpose of this study was to explore the effectiveness of CHW/ nursing student teams in promoting secondary protection and improving access to care for residents of three urban underserved neighborhoods. The study also sought to measure CHW and resident satisfaction of such a program. Design and Sample: Quasi-experimental and nonexperimental designs were used for the study. Convenience samples consisted of residents who participated in the CHW program during 2005–2006, CHWs and residents who participated in the CHW program during 2005–2007 and a systematic random sample of residents across the three neighborhoods. Over 5 million older adults with diabetes are hospitalized each year. Though typically not the index condition that leads to hospitalization, diabetes control often decompensates during the course of an admission and necessitates changes in home self-management plans. The specific transitional care needs of older adults with diabetes have been largely unstudied. Transition theory provided the guiding framework for this research and proposes that each transition is a complex process created by the continuous interaction of personal, community, and societal-level conditions that facilitate or inhibit the health of a transition. Blast-induced traumatic brain injury (TBI) in military personnel may have long-lasting physical, emotional and sociocultural effects. The experiences of living with TBI as a military Service Member wounded during combat operations is not well understood. In this article, we intend to describe the lived experiences of sustaining a TBI and the impact of the injury. The method of inquiry that we selected was transcendental phenomenology, applying Edmund Husserl's principles for the data analysis. Cardiovascular disease (CVD) remains among the leading causes of death in the United States despite widespread knowledge about risk factors as well as effective primary prevention strategies. Risk perception is a complex phenomenon that plays an important role in how persons view disease and ultimately how they make health behavior choices. This study is supported by the knowledge that few studies have examined how persons perceive cardiovascular risk or the variables thought to contribute to the formation of risk perception.

The Role Of Self-Transcendence : A Missing Variable In The Pursuit Of Successful Aging?, Valerie Lander Mccarthy, Jiying Ling, Robert M. Carini

While successful aging is often defined as the absence of disease and disability or as life satisfaction, self-transcendence may also play an important role. The objective of this research was to test a nursing theory of successful aging proposing that transcendence and adaptation predict successful aging. In this cross-sectional exploratory study, a convenience sample of older adults (N = 152) were surveyed about self-transcendence, proactive coping, and successful aging. Using hierarchical multiple regression, self-transcendence, proactive coping, and all control variables (i.e., sex, race, perceived health, place of residence) together explained 50% of the variance in successful aging (p < 0.001). However, proactive coping alone was not a significant predictor of successful aging. Thus, this study did not support the theory that both self-transcendence and proactive coping predict successful aging. Self-transcendence was the only significant contributor to this multidimensional view of successful aging. Self-transcendence is an important variable in the pursuit of successful aging, which merits further investigation.

Surveillance of ventilator-associated pneumonia (VAP) has been the common outcome measurement used for internal and external benchmarking for mechanically ventilated patients; and although not a clinical definition, it is commonly used as an outcome measurement for research studies. Criteria in the VAP definition include both subjective and objective components, leading to questions of validity. In addition, recent legislation has mandated the public reporting of healthcare-associated infections, including VAP, in many states. Tibetan wild barley (Hordeum vulgare L. ssp. spontaneum), originated and grown in harsh enviroNment in Tibet, is well-known for its rich germpalsm with high tolerance to abiotic stresses. However, the genetic variation and genes involved in Al tolerance are not totally known for the wild barley. In this study, a genome-wide association analysis (GWAS) was performed by using four root parameters related with Al tolerance and 469 DArT markers on 7 chromosomes within or across 110 Tibetan wild accessions and 56 cultivated cultivars. Population structure and cluster analysis revealed that a wide genetic diversity was present in Tibetan wild barley. …

Background: Health Care Transition (HCT) describes the purposeful, planned movement of adolescents from child to adult-orientated care. The purpose of this phenomenological study is to uncover the meaning of transition to adult centered care as experienced by Young Adults with Cerebral Palsy (YA-CP) through the research question: What are the lived experiences of young adults with cerebral palsy transitioning from pediatric to adult healthcare?

Hypertension (HTN) prevalence in African Americans contribute to higher rates of disabilities and deaths from stroke, myocardial infarction, and end stage renal disease than all other racial groups in the United States. The major reason documented for these poor health outcomes is related to lower HTN control rates among African Americans compared to other racial/ethnic groups. Though overall HTN awareness, pharmacological treatments and control have significantly improved for all populations, studies found that rates of HTN control and adherence with anti-hypertensive medications are lower for African Americans compared to other subgroups.

Academic failure has been described as endemic in nursing education. Although, associate degree nursing programs graduate the largest number of nurses each year, the on-time graduation rate is 59%. Every semester students fail nursing courses and are required to successfully repeat the course before they can progress in the nursing program. Students who re-take a failed course are often called "repeaters." This qualitative Grounded Theory research explored the process of overcoming failure and becoming a successful student repeater. In the United States, 40% of the contemporary nursing workforce is comprised of women of childbearing age, 65% of whom are employed full-time. Hence, the likelihood of pregnancy occurring for this population at some point in their employment is high. A holistic exploration of how nurses integrate pregnancy and full-time employment has been lacking. The purpose of this research was to explore how primiparous nurses managed pregnancy and full-time employment. วัตถุประสงค์: เพื่อศึกษาสมรรถนะผู้จัดการหอผู้ป่วยวิกฤต โรงพยาบาลเอกชน เขตกรุงเทพมหานคร \nรูปแบบการวิจัย: การวิจัยเชิงพรรณา \nวิธีดำเนินการวิจัย: ใช้เทคนิคเดลฟาย ผู้ให้ข้อมูลคือ ผู้เชี่ยวชาญด้านการบริหารหอผู้ป่วยวิกฤต จำนวน 18 คน วิธีการดำเนินการวิจัยประกอบด้วย 3 ขั้นตอน ดังนี้ ขั้นตอนที่ 1 ศึกษาและวิเคราะห์เอกสาร เพื่อนำสาระที่ได้มาบูรณาการกำหนดเป็นกรอบในการสร้างคำถามสัมภาษณ์ และทำการสัมภาษณ์ ผู้เชี่ยวชาญ ขั้นตอนที่ 2 นำข้อมูลที่ได้มาวิเคราะห์เนื้อหาร่วมกับสมรรถนะที่ได้จากการทบทวนวรรณกรรม นํามาสร้างแบบสอบถามเพื่อให้ผู้เชี่ยวชาญ ได้แสดงความคิดเห็นเกี่ยวกับระดับความสำคัญของ ข้อรายการสมรรถนะในแต่ละด้าน ขั้นตอนที่ 3 นำข้อมูลที่ได้มาคำนวณหาค่ามัธยฐาน และค่าพิสัย ระหว่างควอไทล์ จัดทำแบบสอบถามส่งให้ผู้เชี่ยวชาญยืนยันความคิดเห็นอีกครั้ง นำข้อมูลที่ได้มา วิเคราะห์หาค่ามัธยฐาน และค่าพิสัยระหว่างควอไทล์ เพื่อสรุปผลการวิจัย \nผลการวิจัย: สมรรถนะผู้จัดการหอผู้ป่วยวิกฤต โรงพยาบาลเอกชน เขตกรุงเทพมหานคร ผู้เชี่ยวชาญ ให้ระดับความสำคัญของสมรรถนะสอดคล้องกันทุกข้อ ประกอบด้วยสมรรถนะ 8 ด้าน รวม 44 ข้อ ได้แก่ 1) การบริหารจัดการความเสี่ยง 6 ข้อ 2) การพยาบาลขั้นสูง 10 ข้อ 3) การบริหารบุคลากรในหอ ผู้ป่วยวิกฤต 6 ข้อ 4) การบริหารจัดการเทคโนโลยีสารสนเทศ อุปกรณ์และเครื่องมือทางด้านการแพทย์ 5 ข้อ 5) การตัดสินใจในสถานการณ์วิกฤต 3 ข้อ 6) การบริหารจัดการข้อขัดแย้งและการเจรจาต่อรอง 4 ข้อ 7) การประสานความร่วมมือในการทํางาน 5 ข้อ และ 8) การบริหารจัดการเชิงกลยุทธ์ 5 ข้อ สรุปและข้อเสนอแนะ: จากผลการวิจัยครั้งนี้เป็นการศึกษาเกี่ยวกับสมรรถนะผู้จัดการหอผู้ป่วยวิกฤต ซึ่งสามารถนำผลการวิจัยนี้ไปใช้เป็นแนวทางในการสรรหา วางแผน และพัฒนาสมรรถนะพยาบาลผู้ที่ จะเข้ามาดำรงตำแหน่งผู้จัดการหอผู้ป่วยวิกฤต และเป็นแนวทางในการฝึกอบรมผู้จัดการหอผู้ป่วยวิกฤต

วัตถุประสงค์: เพื่อศึกษาความยึดมั่นผูกพันต่อองค์การ และความสัมพันธ์ระหว่างสถานภาพโรงพยาบาล การเป็นโรงพยาบาลดึงดูดใจกับความยึดมั่นผูกพันต่อองค์การของพยาบาลวิชาชีพโรงพยาบาลมหาวิทยาลัย \nรูปแบบการวิจัย: การวิจัยแบบพรรณนา \nวิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ พยาบาลวิชาชีพ ที่ปฏิบัติงานในโรงพยาบาลมหาวิทยาลัย จำนวน 380 คน เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูล คือ แบบสอบถามการเป็นโรงพยาบาลดึงดูดใจ ประกอบด้วย ด้านภาวะผู้นำการเปลี่ยนแปลง ด้านการเสริมสร้างพลังอำนาจ ด้านการเป็นแบบอย่าง ของการปฏิบัติการเชิงวิชาชีพ ด้านการพัฒนาคุณภาพ นวัตกรรมและองค์ความรู้ใหม่ และด้านผลลัพธ์ คุณภาพเชิงประจักษ์ กับความยึดมั่นผูกพันต่อองค์การ ตรวจสอบความตรงตามเนื้อหา และหาค่าความ เที่ยงโดยสูตรสัมประสิทธิ์แอลฟาของครอนบาคได้ 91, 94, 97, 96, 90 และ 95 ตามลำดับ วิเคราะห์ ข้อมูลโดยการแจกแจงความถี่ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน สถิติไคสแควร์ และค่าสัมประสิทธิ์ สหสัมพันธ์เพียร์สัน \nผลการวิจัย: ผลการวิจัยมีดังนี้ \n1) พยาบาลวิชาชีพ โรงพยาบาลมหาวิทยาลัยมีความยึดมั่นผูกพันต่อองค์การ อยู่ในระดับมาก (X= 4.01, SD = 0.61) \n2) สถานภาพโรงพยาบาล ไม่มีความสัมพันธ์กับความยึดมั่นผูกพันต่อองค์การของพยาบาลวิชาชีพ \n3) การเป็นแบบอย่างของการปฏิบัติการเชิงวิชาชีพ การเสริมสร้างพลังอำนาจ ผลลัพธ์ คุณภาพเชิงประจักษ์ การพัฒนาคุณภาพ นวัตกรรมและองค์ความรู้ใหม่ และ ภาวะผู้นำการเปลี่ยนแปลง มีความสัมพันธ์ทางบวก กับความยึดมั่นผูกพันต่อองค์การของพยาบาลวิชาชีพ โรงพยาบาลมหาวิทยาลัย (r = .686, 680, .584, 563 และ 556 ตามลำดับ) \nสรุปและข้อเสนอแนะ: จากผลการวิจัย จะเห็นได้ว่า การเป็นโรงพยาบาลดึงดูดใจเป็นปัจจัยที่ทำให้ พยาบาลมีความยึดมั่นผูกพันต่อองค์การมากขึ้น ดังนั้นผู้บริหารการพยาบาลจึงควรธำรงรักษาการเป็น โรงพยาบาลดึงดูดใจของฝ่ายการพยาบาลต่อไป

The nurse manager role has experienced an explosion of responsibilities and expectations over the last decade. As a result, competency and skill acquisition for the successful nurse manager has become the focus of many nurse managers, nurse executives, researchers and organizations. The three manuscripts contained in this final capstone report will explore the acquisition of nurse manager competency, propose coaching as a methodology for acceleration and improvement of nurse manager skill acquisition and describe the results of three case studies using coaching as an intervention to improve the performance improvement skill acquisition of nurse managers.

Military readiness, employer productivity and individual health are all significantly impacted by high rates of tobacco use. Current research, to date, suggests that use of brief, individually tailored behavioral modification strategies through group therapy may be most effective in smoking cessation. The objective of this Practice Inquiry Project was to explore aspects of tobacco use the U.S. military population and examine potential relationships between type of treatment, smoking abstinence, and maintenance of body weight (e.g. avoid weight gain) among those seeking tobacco use cessation.
