Cardiac arrest, unanticipated admissions to the intensive care unit (ICU), and mortality account for about 50% of serious inpatient adverse events (Ko et al., 2020). Efforts to prevent in-hospital cardiopulmonary resuscitation (CPR) require a system to identify deteriorating patients and include an appropriate interventional response, such as a rapid response team (Andersen et al., 2019). A well-structured rapid response team (RRT) has five categories that include team structure, organizational culture, expertise, communication, and teamwork (Jackson, 2017). The national epidemic of boarding hours in the emergency departments prompted the implementation of a discharge bundle in an acute care hospital. This bundle included an ADT nurse, discharge orders written prior to 11AM, and EVS shifting employee hours. Initial findings indicated a reduction in boarding time by 126 minutes.

Background: Although beneficial to patient outcomes, inpatient ambulation is not a prioritized care intervention, and a task often deferred to physical therapy. Patient immobility is a complex problem that requires a multifaceted approach.

Local Problem: In a rural, critical access hospital, on an 18-bed medical surgical unit, nursing staff lacked ownership of patient mobility. There was no standardized process to document ambulation and mobility tasks were not scheduled, falling to the end of the nursing work list. การวิจัยเชิงคุณภาพเชิงพรรณนานี้ มีวัตถุประสงค์เพื่อบรรยายประสบการณ์การบริหารทรัพยากรสุขภาพของพยาบาลกลุ่มงานประกันสุขภาพ โรงพยาบาลศูนย์ ผู้ให้ข้อมูล คือ พยาบาลวิชาชีพที่ปฏิบัติงานบริหารทรัพยากรสุขภาพ ในกลุ่มงานประกันสุขภาพ โรงพยาบาลศูนย์ไม่น้อยกว่า 5 ปี ผ่านการอบรมหลักสูตรที่เกี่ยวข้องกับการบริหารทรัพยากรสุขภาพ และมีความยินดีในการเข้าร่วมการวิจัย จำนวนทั้งสิ้น 12 ราย เก็บรวบรวมข้อมูลโดยใช้วิธีการสัมภาษณ์เชิงลึก (In-dept interview) ร่วมกับการสังเกต (Observation) และการจดบันทึกภาคสนาม (Field notes) วิเคราะห์ข้อมูลแบบแก่นสาระตามวิธีการของ Braun and Clarke และตรวจสอบความน่าเชื่อถือของข้อมูลตามเกณฑ์ของ Guba and Lincoln ผลการวิเคราะห์ข้อมูลประสบการณ์การบริหารทรัพยากรสุขภาพของพยาบาลกลุ่มงานประกันสุขภาพ โรงพยาบาลศูนย์ ประกอบด้วย 5 ประเด็นหลัก และ 19 ประเด็นย่อย ดังต่อไปนี้ 1. การบริหารองค์ความรู้และศักยภาพบุคลากร ประกอบด้วย 4 ประเด็นย่อย ได้แก่ 1.1) การเรียนรู้และเข้าใจเกณฑ์กองทุนต่าง ๆ 1.2) การใช้ความรู้ความชำนาญด้านการพยาบาล 1.3) การพัฒนาทักษะการให้รหัสโรคและหัตถการ และ 1.4) การติดตามความรู้ด้านนโยบายและการเปลี่ยนแปลงระบบสุขภาพ 2. การทบทวนการใช้ทรัพยากรสุขภาพ ประกอบด้วย 3 ประเด็นย่อย ได้แก่ 2.1) การทบทวนความสมบูรณ์ของข้อมูลเพื่อการเบิกจ่าย 2.2) การทบทวนความสมเหตุสมผลของการใช้ทรัพยากร และ 2.3) การจัดทำรายงานและบันทึกผลการทบทวน 3. การบริหารงบประมาณและความคุ้มค่าของทรัพยากรสุขภาพ ประกอบด้วย 5 ประเด็นย่อย ได้แก่ 3.1) การวิเคราะห์ต้นทุนและผลตอบแทน 3.2) การควบคุมงบประมาณ 3.3) การบริหารมาตรฐานการดูแลผู้ป่วยให้สอดคล้องกับสิทธิ 3.4) การพัฒนากระบวนการให้เอื้อต่อการเบิกจ่าย และ 3.5) การสร้างนวัตกรรมหรือบริการใหม่ที่คุ้มค่า 4. การประสานงานเพื่อสร้างความร่วมมือใช้ทรัพยากรสุขภาพอย่างเหมาะสม ประกอบด้วย 3 ประเด็นย่อย ได้แก่ 4.1) การประสานงานภายในองค์กร 4.2) การประสานงานกับหน่วยงานภายนอก และ 4.3) การประสานงานกับผู้รับบริการ 5. …

Background: UK HealthCare transitioned to a new enterprise electronic health record (EHR) system, offered by Epic Systems Corporation, in June 2021. Approximately 2,000 inpatient nurses use the EpicCare Inpatient Module in the 1,086 licensed bed facilities. Compared to other academic medical centers, UK HealthCare nurses take more time documenting in this EHR inpatient module’s Basic Assessment Flowsheet (documentation burden) and have a longer delay between assessment and documentation (timeliness) potentially contributing to nursing dissatisfaction with using this new EHR.

การวิจัยเชิงบรรยาย (Descriptive research) โดยใช้เทคนิคแบบเดลฟาย (Delphi Technique) เพื่อศึกษาสมรรถนะพยาบาลผู้ประกอบการโรงเรียนบริบาล เครื่องมือที่ใช้ในการศึกษาได้แก่ 3 ชุด ประกอบด้วย 1) แบบสอบถามเพื่อแสดงความคิดเห็นเกี่ยวกับสมรรถนะพยาบาลผู้ประกอบการ โรงเรียนบริบาล 2) แบบสอบถามเรื่องสมรรถนะพยาบาลผู้ประกอบการโรงเรียนบริบาล เพื่อให้ผู้เชี่ยวชาญได้ให้ระดับความสำคัญของข้อรายการสมรรถนะพยาบาลผู้ประกอบการโรงเรียนบริบาล และ 3) แบบสอบถามผู้เชี่ยวชาญเรื่องสมรรถนะพยาบาลผู้ประกอบการโรงเรียนบริบาล เพื่อให้ผู้เชี่ยวชาญยืนยันคงคำตอบเดิม หรือเปลี่ยนแปลงคำตอบที่มีอยู่เดิม ผู้เชี่ยวชาญที่ใช้ในการวิจัยครั้งนี้ จำนวน 19 คน ประกอบด้วย กลุ่มผู้เชี่ยวชาญ 3 กลุ่ม ได้แก่ 1) กลุ่มผู้ประกอบการโรงเรียนบริบาล จำนวน 5 คน 2) กลุ่มคณาจารย์ทางการศึกษา จำนวน 5 คน และ 3) กลุ่มพยาบาลผู้ประกอบการโรงเรียนบริบาล จำนวน 9 คนผลการวิจัยพบว่า กลุ่มผู้เชี่ยวชาญมีความคิดเห็นสอดคล้องกันเกี่ยวกับสมรรถนะพยาบาลผู้ประกอบการโรงเรียนบริบาล โดยสมรรถนะพยาบาลผู้ประกอบการโรงเรียนบริบาล มีทั้งหมด 9 ด้าน ประกอบด้วย ด้านธุรกิจการจัดการศึกษา จำนวน 4 ข้อ ด้านกลยุทธ์เชิงธุรกิจการจัดการศึกษา จำนวน 7 ข้อ ด้านการบริหารองค์กรสถานศึกษา จำนวน 8 ข้อ ด้านการบริหารจัดการทรัพยากรสถานศึกษา จำนวน 7 ข้อ ด้านการจัดการส่วนผสมการตลาดทางการศึกษา จำนวน 5 ข้อ ด้านสัมพันธภาพระหว่างบุคคล จำนวน 5 ข้อ ด้านภาวะผู้นำและคุณลักษณะส่วนบุคคล จำนวน 8 ข้อ ด้านจริยธรรม กฎหมายธุรกิจ และกฎหมายที่เกี่ยวข้องกับการศึกษา จำนวน 6 ข้อ และด้านการ บูรณาการวิชาชีพทางการพยาบาล จำนวน 5 ข้อ โดยสมรรถนะพยาบาลผู้ประกอบการโรงเรียนบริบาล มีค่าระดับความสำคัญอยู่ในระดับมาก และมากที่สุด (Md = 4.29-4.82, IR = …

การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาพฤติกรรมการทำงานที่เน้นนวัตกรรมของพยาบาลวิชาชีพ โรงพยาบาลมหาวิทยาลัยของรัฐ ศึกษาความสัมพันธ์ระหว่างการเรียนรู้ตลอดชีวิต ภาวะผู้นำเชิงนวัตกรรม และบรรยากาศการส่งเสริมนวัตกรรมในองค์การกับพฤติกรรมการทำงานที่เน้นนวัตกรรมของพยาบาลวิชาชีพ โรงพยาบาลมหาวิทยาลัยของรัฐ และศึกษาตัวแปรพยากรณ์ที่สามารถร่วมทำนายพฤติกรรมการทำงานที่เน้นนวัตกรรมของพยาบาลวิชาชีพ โรงพยาบาลมหาวิทยาลัยของรัฐ กลุ่มตัวอย่าง คือ พยาบาลวิชาชีพระดับปฏิบัติการ โรงพยาบาลมหาวิทยาลัยของรัฐ ที่มีประสบการณ์ทำงานตั้งแต่ 1 ปีขึ้นไป จำนวน 210 คน เครื่องมือที่ใช้ในการวิจัย คือ แบบสอบถามข้อมูลส่วนบุคคล แบบสอบถามการเรียนรู้ตลอดชีวิต แบบสอบถามภาวะผู้นำเชิงนวัตกรรม แบบสอบถามบรรยากาศการส่งเสริมนวัตกรรมในองค์การ และแบบสอบถามพฤติกรรมการทำงานที่เน้นนวัตกรรม ซึ่งผ่านการตรวจสอบความตรงเชิงเนื้อหาจากผู้ทรงคุณวุฒิ จำนวน 5 ท่าน ได้ค่า CVI เท่ากับ .94, .89, .88 และ .93 ตามลำดับ และทดสอบค่าความเที่ยง โดยคำนวณหาค่าสัมประสิทธิ์แอลฟ่า ได้ค่าความเที่ยง เท่ากับ .90, .97, .95 และ .96 ตามลำดับ วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา สถิติสหสัมพันธ์เพียร์สัน และสถิติถดถอยพหุคูณ ผลการวิจัยพบว่า พฤติกรรมการทำงานที่เน้นนวัตกรรมของพยาบาลวิชาชีพในโรงพยาบาลมหาวิทยาลัยของรัฐ อยู่ในระดับปานกลาง (M = 3.49, S.D. = 0.76) การเรียนรู้ตลอดชีวิต ภาวะผู้นำเชิงนวัตกรรม และบรรยากาศการส่งเสริมนวัตกรรมในองค์การ มีความสัมพันธ์ทางบวกกับพฤติกรรมการทำงานที่เน้นนวัตกรรมของพยาบาลวิชาชีพ โรงพยาบาลมหาวิทยาลัยของรัฐ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (r = .683, .524 และ .499 ตามลำดับ) ปัจจัยที่สามารถร่วมทำนายพฤติกรรมการทำงานที่เน้นนวัตกรรมของพยาบาลวิชาชีพ โรงพยาบาลมหาวิทยาลัยของรัฐ คือ การเรียนรู้ตลอดชีวิต บรรยากาศการส่งเสริมนวัตกรรมในองค์การ และภาวะผู้นำเชิงนวัตกรรม โดยมีอำนาจในการทำนายร้อยละ 53.6 (Adjusted R² = .536) สามารถเขียนสมการทำนายในรูปคะแนนมาตรฐาน ดังนี้ ZIWB = .540 ZLLL + .192 ZOIC + .149 ZIL

This quantitative correlational study explored the relationships between perceived psychological safety, conflict management styles, and perceived conflict management success among nurse practitioners (NPs) operating in interprofessional acute or critical care teams in the United States. Despite existing literature on conflict management and psychological safety, there is a noticeable gap concerning the interplay of these factors, specifically among NPs. A multi-item survey was developed from validated measures to assess psychological safety, conflict management success, and conflict management style. The final sample included 944 NPs.
