Through evidence based research, the use of supplemental probiotics has been shown to have a positive effect in preventing secondary infections in hospital patients. Although there are other techniques that have decreased the incidence of infections, such as reducing invasive procedures and aseptic techniques, these are external procedures designed to prevent infection. Probiotics are an effective method of internally preventing infection. Retention of advanced practice providers (APPs) is needed for ongoing high quality comprehensive care. Administrators at a large urban pediatric institute in the Midwest identified turnover and retention as an issue for APPs in the first one to two years of service. Even in years when turnover rates were not as high, the greatest number of APPs leaving the institution where those in their first five years of tenure. This project explores the implementation of Student Success Coaches at Augusta Technical College (Augusta Tech) and their potential impact on the rates of overall program completion and the rate of diverse student program completion. Framed within the GROW model and Sociocultural Learning Theory, the project aims to determine if coaching interventions address barriers to completion and foster a supportive learning environment. A mixed-methods exploratory approach was employed, combining empirical data such as retention rates and demographics with qualitative information from surveys. Background: COVID-19 has dramatically changed human-to-human interactions from in-person to the nonhuman domain of virtual space. In January 2021, every 28 seconds one person died of COVID-19 in the United States. An average of 3,100 Americans died each day during the deadliest month of the pandemic. As of January 2023, the number of COVID-19 deaths in the USA surpassed 1.000.000 people. In hospitals, human-to-human interactions were often limited to seeing one’s eyes behind the face shields, and respirator masks, and hearing a muffled voice, or sounds of the ventilators. In New York State 452 nurses died during the pandemic.

Antibiotics have revolutionized modern medicine; however, the overuse in urgent care and primary care significantly contributes to the global burden of infections resistant to available antimicrobial medicines. Approximately 30% of the antimicrobials prescribed in acute care settings are unnecessary. Patient demand for antibiotics has seemingly skyrocketed following the recent COVID-19 pandemic. Practitioners must be knowledgeable about antibiotic stewardship initiatives, such as the wait-and-see approach, to reduce antibiotic overuse and improve the trajectory of antibiotic resistance and patient health outcomes. การวิจัยเชิงพรรณนานี้ ศึกษาสมรรถนะตามบันไดวิชาชีพของพยาบาลหอผู้ป่วยวิกฤตโรคหลอดเลือดสมอง โรงพยาบาลตติยภูมิขั้นสูง ด้วยเทคนิคการวิจัยแบบเดลฟาย ผู้ให้ข้อมูลคือ กลุ่มผู้เชี่ยวชาญจำนวน 18 คน แบ่งเป็น 5 กลุ่ม ได้แก่ 1) พยาบาลผู้เชี่ยวชาญการดูแลผู้ป่วยวิกฤตโรคหลอดเลือดสมองจำนวน 6 คน 2) หัวหน้าหอผู้ป่วยวิกฤตโรคหลอดเลือดสมองจำนวน 4 คน 3) แพทย์ผู้เชี่ยวชาญการรักษาผู้ป่วยวิกฤตโรคหลอดเลือดสมอง จำนวน 3 คน 4) พยาบาลผู้ปฏิบัติการขั้นสูงด้านอายุรศาสตร์และศัลยศาสตร์จำนวน 3 คน และ 5) อาจารย์พยาบาลผู้เชี่ยวชาญการพยาบาลผู้ป่วยวิกฤตโรคหลอดเลือดสมองจำนวน 2 คน ดำเนินการวิจัย 3 ขั้นตอน ได้แก่ 1) สัมภาษณ์เกี่ยวกับสมรรถนะพยาบาลตามบันไดวิชาชีพ ซึ่งประกอบด้วย พยาบาลระดับปฏิบัติการ ชำนาญการ ชำนาญการพิเศษ และเชี่ยวชาญ 2) วิเคราะห์ข้อมูล และสร้างแบบสอบถามเพื่อให้ผู้เชี่ยวชาญประเมินระดับความสำคัญ และ 3) คำนวณค่ามัธยฐานและค่าพิสัยระหว่างควอไทล์เพื่อสรุปผล ผลการศึกษาพบว่า สมรรถนะตามบันไดวิชาชีพของพยาบาลหอผู้ป่วยวิกฤตโรคหลอดเลือดสมอง โรงพยาบาลตติยภูมิขั้นสูง ประกอบด้วย 8 ด้าน ดังนี้ 1) การปฏิบัติการพยาบาลในระยะวิกฤต 2) การบริหารยาความเสี่ยงสูงที่ใช้ในผู้ป่วยวิกฤต 3) การป้องกันภาวะแทรกซ้อน 4) การฟื้นฟูสภาพหลังพ้นวิกฤต 5) การสื่อสารและประสานงานเพื่อการรักษาพยาบาล 6) การพยาบาลระยะท้าย 7) การใช้เทคโนโลยีและเครื่องมือดิจิทัลทางการแพทย์ และ 8) การจัดการและพัฒนาคุณภาพการดูแลผู้ป่วย ซึ่งพยาบาลที่ปฏิบัติงานในหอผู้ป่วยประกอบด้วยพยาบาลระดับปฏิบัติการ ชำนาญการ ชำนาญการพิเศษ และเชี่ยวชาญที่มีสมรรถนะทั้ง 8 ด้าน แต่สมรรถนะย่อยมีความแตกต่างกัน ดังนี้ 1) ระดับปฏิบัติการ เน้นงานพื้นฐานภายใต้การดูแล 2) ระดับชำนาญการ สามารถดูแลผู้ป่วยซับซ้อนและทำงานเป็นทีมได้ 3) ระดับชำนาญการพิเศษ สามารถแก้ปัญหาเฉพาะหน้าและใช้หลักฐานเชิงประจักษ์ และ 4) ระดับเชี่ยวชาญ สามารถพัฒนาองค์ความรู้ใหม่ทำให้คุณภาพการดูแลผู้ป่วยดีขึ้น การศึกษานี้สามารถใช้เป็นแนวทางในการประเมินสมรรถนะและมอบหมายงานตามความสามารถของพยาบาลแต่ละระดับ
