Radiographically-detected pneumoperitoneum in the setting of acute onset abdominal pain is a classic case among surgical emergencies. The spectrum of etiology ranges from benign to catastrophic, and the ability to differentiate depends in large part on the history and physical exam findings. Included among non-surgical types of pneumoperitoneum are (1) post-operative, (2) diagnostic/experimental, (3) gynecologic tract air entry, (4) the difference in intra- and extra-alveolar pressures, pneumothorax-associated and (5) miscellaneous. Jacuzzi-induced pneumoperitoneum is an extremely rare form of non-surgical pneumoperitoneum. As medical educators, we have a tremendous opportunity to transform how clinical skills related to breast care are taught and assessed and thereby, improve breast patient outcomes. This article reviews the current state of education and ideas for implementing a learner-specific, competency-based curriculum to teach breast care skills.

Goblet cell carcinoid of the appendix is a rare neoplasm with histological features of both neuroendocrine and adenocarcinomas. The combination of its aggressive behavior, infrequent occurrence, and variable clinical presentation convolutes the management of this tumor. We report the case of a 75-year-old female presenting with acute appendicitis. A laparoscopic appendectomy was performed. The pathology report showed goblet cell carcinoid at the base of the appendix with involvement of the proximal surgical margins. Fractures of the proximal humerus are commonly associated with osteoporosis and comprise 5.7% of all adult fractures [1]. Posterior dislocations of the shoulder comprise 2-5% of all shoulder dislocations, and when bilateral are even more infrequent, occurring less than 5%. [2,3]. Posterior dislocations are often associated with motor vehicle accidents, seizures, or electrical shock [4]. Fracture-dislocations of the proximal humerus occur in only 1% of all shoulder dislocations, thus making posterior fracture-dislocations off the proximal humerus exceedingly rare [5]. Pneumoperitoneum is most commonly associated with a perforated viscus. When free air is seen on imaging, the most common course of action is an emergent exploratory laparotomy. However, pneumoperitoneum may not always be indicative of a perforated bowel. In the case described below, we will discuss a patient who developed acute peritonitis and pneumoperitoneum after undergoing chest compressions during cardiopulmonary resuscitation (CPR) and positive pressure mechanical ventilation.

Systemic lupus erythematosus (SLE) is an autoimmune disease affecting all age groups and can manifest in various forms, often making the initial or successive presentations difficult to diagnose. Peritonitis secondary to lupus is a rare manifestation of this disease and the prevalence is said to be much lower in children. We present a case report of an adolescent male with a known history of lupus who presented to the emergency department with a clinical picture consistent with an acute surgical abdomen and underwent an appendectomy. Subsequent workup identified the culprit as a lupus-related peritonitis requiring corticosteroids for resolution.

Introduction:

Paralysis secondary to spinal cord ischemia following Endovascular repair of infrarenal abdominal aortic aneurysm is extremely rare complication, the reported incidence in the literature is 0.21%.

Case report:

Our patient is an 85-year old female patient who presented to the emergency department with abdominal pain radiating to the back. Abdominal examination showed mild epigastric tenderness. Laboratory work up was within the normal limits, and she had Computed Tomography Angiography of the abdomen and pelvis which showed impending rupture of 5.8 cm abdominal aortic aneurysm. The anatomic configuration of the AAA fulfilled the requirements for EVAR.

Traditional interview selection has several weaknesses limited often to a number, often USMLE/COMLEX that programs use to "weed out" potential residents then look at those who meet the mark. There is no single factor that indicates a good resident, yet we seek to analyze selection criteria from 9 hospitals with general surgery residencies to figure out what selection criteria are present in certain hospitals selection and determine which factors predict acceptance for general surgery residency.
