Effects Of Functional Appliances On Pharyngeal Airway Dimensions: A Cone Beam Computed Tomography Study, Melissa K. Lee

Introduction: Functional appliances are intended to correct by incremental advancement of the mandible for correction of skeletal Class 11 relationships. Similar in design to functional appliances, mandibular advancing devices (MAD) are used for obstructive sleep apnea (OSA) patients as they increase the size of the airway by drawing the tongue and soft palate forwards, and thus maintain its patency during sleep. It has been shown that significant increases in airway dimensions and decreases in airway resistance were gained in OSA patients while evaluated with a MAD in place. Study objectives: The association of sleep-disordered breathing (SDB) and blunting of normal nocturnal lowering of blood pressure (BP) (nondipping) has only been examined cross-sectionally. The purpose of this study is to investigate whether SDB is prospectively associated with nondipping.

Methods: The longitudinal association between SDB and incident nondipping was examined in a subsample of 328 adults enrolled in the Wisconsin Sleep Cohort Study who completed 2 or more 24-hour ambulatory BP studies over an average of 7.2 years of follow-up. Research examining the impact of children’s autism and the sleep problems frequently associated with the disorder on parents’ stress has been quite limited. Studies have been constrained by methodological problems, including small sample sizes and the examination of children of autism within larger groups of children with a range of developmental disabilities. Typically, diagnostic criteria used to select children for an autism study is not indicated and no measure of the severity of children’s symptomology is included. Sleep disturbances are important correlates of depression, with epidemiologic research heretofore focused on insomnia and sleepiness. This epidemiologic study’s aim was to investigate, in a community sample, depression’s relationships to other sleep disturbances: sleep paralysis (SP), hypnagogic/hypnopompic hallucinations (HH), cataplexy – considered rapid eye movement-related disturbances – and automatic behavior (AB). Although typical of narcolepsy, these disturbances are prevalent, albeit under-studied, in the population. The purpose of this study was to validate a sleep apnea questionnaire from the Loma Linda University Sleep Disorders Center, and then to utilize the questionnaire in order to identify potential sleep apnea patients from a pre-orthodontic patient sample. Study patients were selected from the Loma Linda University Sleep Disorders Center and the Loma Linda University Orthodontics Clinic, both in Loma Linda, California, USA.

The fatigue and cognitive performance deficits associated with sleep loss and stress have motivated the search for effective nonpharmacological countermeasures. The purpose of the present study was to examine the potential behavioral effects of tyrosine, an amino-acid presursor to dopamine and norepinephrine, during an episode of continuous nighttime work involving one night of sleep loss.

Sleep is necessary for physical and psychological restoration of the body. Studies indicate that tissue repair is augmented during sleep, showing that rates of protein synthesis and of mitotic division are higher at times of rest and sleep. Studies evaluating sleep-deprived subjects reveal that after loss of normal sleep, some types of memory and learning processes are impaired. These subjects generally become anxious and irritable, especially after loss of dreaming sleep.

The prevalence of heart disease in the population over age 65 has contributed to the increasing number of aged patients hospitalized in critical care units (Gsell, 1977). Changes which occur in the process of aging and in cardiac disease should be considered when providing nursing care to these patients. A disturbance in the sleep wakefulness cycle resulting in a decrease of restorative sleep is a major change occurring at age 65 and after (Feinberg, 1967). The cardiac patient finds that it takes longer to go to sleep, and then awakens often during the night (Karacan et al., 1969).

Postpartum depression was thought to be an important component of maternity blues and was the factor explored in this study. Sleep was hypothesized as a factor affecting either the occurrence or severity of postpartum depression. Hence, the purpose of this study was to determine the relationship between sleep deprivation and postpartum depression.

Twenty patients were interviewed one to three weeks prior to their expected dates of delivery. The Minnesota Multiphasic Personality Inventory Depression scale was administered during the interview and each patient kept a record of her hours of sleep during the following week.
