The 1.5T hybrid MRI/linear accelerator (MR-linac) has recently been introduced into clinical practice and used for the treatment of head and neck cancers (HNC). This device enables on-line adaptive radiation therapy (ART) based on anatomical changes throughout treatment and variations in patient position. This novel technology also has the potential for advanced ART strategies such as dose-optimized ART, in which the treatment plan is optimized based on the accumulated dose over previous fractions, or biological image-guided ART, in which the plan is adapted based on individual tumor response as measured through quantitative imaging techniques such as diffusion-weighted imaging (DWI). The aim of the clinical component of this work was to determine whether the currently available clinical imaging tools can be integrated with radiotherapy (RT) platforms for monitoring and adaptation of radiation dose, prediction of tumor response and disease outcomes, and characterization of patterns of failure and normal tissue toxicity in head and neck cancer (HNC) patients with potentially curable tumors. Targeted radionuclide therapy (TRT) uses unsealed radiation sources that are accumulated in the target tissues to deliver a damaging radiation dose to the cancerous cells, and a part of the radiopharmaceutical will end up in non-targeted tissues. To avoid treatment-related side effects, dosimetry is mandatory for TRT for each treatment cycle. Alanine dosimeters are generally used in high-dose dosimetry, while accurate and precise dose delivery verification procedures are important in stereotactic radiosurgery (SRS). Hence, an end-to-end test is required. In addition, no end-to-end SRS test employing an alanine dosimeter has been established in Thailand. The objective of this study was to determine the dose accuracy of SRS VMAT plans in end-to-end test phantom using alanine dosimeter. This study aims at evaluating the accuracy of W2 plastic scintillation detector (PSD) compared with the calculation from algorithms for small-field high energy photon in heterogenous media. In proton therapy treatment planning, dose calculation requires CT calibration curve by relating CT number (HU) to relative stopping power to water (RSP). The CT calibration curve and the RSP validation are described for single-energy CT (SECT) and dual-energy CT (DECT). BACKGROUND: The purpose of this study was to describe the topography, extension (volume), and timing of severe osteoradionecrosis (ORN) that required mandible resection in patients previously treated for head and neck cancer at a high-volume Veterans Affairs Medical Center.
MATERIALS AND METHODS: The records from a reference hyperbaric oxygen clinic were retrospectively analyzed (n = 50, 2018-2021). Inclusion criteria were: I) severe ORN defined as progressive ORN that required resection; II) pathologic confirmation of ORN; and III) availability of pre-operative CT-imaging. Abdominal computed tomography (CT) using a single energy protocol is a common imaging procedure in hospitals. As CT technology has continued to evolve, dual-energy protocols (DECT) have emerged as a new option. A spectral CT scanner with fast kVp switching was installed at King Chulalongkorn Memorial Hospital in 2017, and its clinical utility in emergency patients has not yet been studied. The Mobius3D software addresses limitations lacking in measurement-based methods in patient-specific QA. The objective of this study is to validate its dosimetric performance against conventionally used portal dose measurements using gamma analysis and confidence limits. First, a total of 240 patient-specific QA plans for the Varian Halcyon linear accelerator were collected. Then, the EPID was characterized and the Mobius3D software commissioned through plan verification. Then all patient-specific plans underwent QA using the Portal Dosimetry software and the Mobius3D. This study aimed to automatically determine the corrected effective diameter (Deffcorr) using convolutional neural network (CNN) for simplified SSDE calculation in thoracic CT. Transaxial images of 108 adult patients underwent thoracic CT without contrast at King Chulalongkorn Memorial Hospital (KCMH) were collected. To calculate the Deffcorr according to Mihailidis et al., the average relative electron densities for lung tissue (ρelung=0.3), for other tissues (ρetissue=1.0) and for bone (ρebone=1.2) were employed to correct the lateral and anteroposterior dimension. B-mode ultrasound imaging is the standard method for hepatocellular carcinoma (HCC) screening. If a lesion has a unique appearance, a conventional detector can be applied. However, if the lesion shares its appearance with other lesion, a large training dataset is required, which may be unavailable. Therefore, a two-stage method is proposed. In the first stage, lesions are detected but not differentiated into any particular class. PURPOSE: We investigated the feasibility of biology-guided radiotherapy (BgRT), a technique that utilizes real-time positron emission imaging to minimize tumor motion uncertainties, to spare nearby organs at risk.
METHODS: Volumetric modulated arc therapy (VMAT), intensity-modulated proton (IMPT) therapy, and BgRT plans were created for a paratracheal node recurrence (case 1; 60 Gy in 10 fractions) and a primary peripheral left upper lobe adenocarcinoma (case 2; 50 Gy in four fractions).
RESULTS: For case 1, BgRT produced lower bronchus V40 values compared to VMAT and IMPT. For case 2, total lung V20 was lower in the BgRT case compared to VMAT …

PURPOSE: In a prospective multicenter study, gemcitabine monotherapy followed by stereotactic body radiation therapy (SBRT) was well tolerated with outcomes comparable to chemoradiation for locally advanced pancreatic cancer (LAPC). Recent trials have reported improved survival with multiagent chemotherapy (MA-CTX) alone. This prospective trial explored whether SBRT could be safely delivered after MA-CTX. Herein, we report the long-term outcomes of adding SBRT after MA-CTX in LAPC patients and evaluate whether genetic profiles of specimens obtained before SBRT influence outcomes.

The standard of care in the treatment for women diagnosed with breast cancer includes identification and biopsy of the sentinel lymph nodes (SLN) of the breast (Chaterjee et al., 2017; Kneece, 2017). The process of identifying the SLNs involves radioisotope injections into or near the areola. Because this area is highly sensitive, women report this procedure to be considerably painful. Radiopharmaceuticals are reemerging as attractive anticancer agents, but there are no universally adopted guidelines or standardized procedures for evaluating agent validity before early-phase trial implementation. To validate a radiopharmaceutical, it is desirous for the radiopharmaceutical to be specific, selective, and deliverable against tumors of a given, molecularly defined cancer for which it is intended to treat. In this article, we discuss four levels of evidence—target antigen immunohistochemistry, in vitro and in vivo preclinical experiments, animal biodistribution and dosimetry studies, and first-in-human microdose biodistribution studies—that might be used to justify oncology therapeutic radiopharmaceuticals in a drug-development sequence involving early-phase trials. The intermediate dose spill for a stereotactic radiosurgery (SRS) plan can be quantified with the metric R50%, defined as the 50% isodose cloud volume (VIDC50%) divided by the volume of the planning target volume (PTV). By coupling sound physical principles with the basic definition of R50%, we derive an analytical expression for R50% for a spherical PTV. Our analytical expression depends on three quantities: the surface area of PTV (SAPTV), the volume of PTV (VPTV), and the distance of dose drop-off to 50% (Δr). Background: The COVID-19 pandemic has created a need to prioritize care because of limitation of resources. Owing to the heterogeneity and high prevalence of breast cancers, the need to prioritize care in this vulnerable population is essential. While various medical societies have published recommendations to manage breast disease during the COVID-19 pandemic, most are focused on the Western world and do not necessarily address the challenges of a resource-limited setting.

The purpose of this study is to determine the hepatic lesion detectability in abdomen from various phantom sizes by dual-energy computed tomography (DECT) imaging and to investigate the factors affecting the detectability of hepatic lesion in abdomen by dual-energy CT in phantom study. The anthropomorphic liver with nodule inserted phantom with extension rings simulating the small, medium, and large patients were scanned under DECT acquisitions by varying three kVp combinations (80/-,90/-, and 100/Sn150-kVp).
