© 2018 Elsevier B.V. Background: The purpose of this study is to examine the conditions under which daily stressful and positive events are associated with alcohol use among heavy drinking sexual minority men (SMM). Specifically, we examined the moderating effects of two indicators of alcohol addiction severity (i.e., alcohol dependence severity and negative drinking consequences) on the associations between daily stressful events and alcohol use and between daily positive events and alcohol use among heavy drinking SMM. Procedures: Secondary data analyses were performed using data from a randomized controlled trial of heavy drinking and treatment seeking SMM who were assigned …

Shared Polygenicity Detection Using Elastic Nets: Sharpen, M. John, T. Lencz
Journal Articles
Current research suggests that hundreds to thousands of single nucleotide polymorphisms (SNPs) with modest effect sizes contribute to the genetic basis of many disorders, a phenomenon labeled as polygenicity. Additionally, many such disorders demonstrate polygenic overlap, in which risk alleles are shared at associated genetic loci. However, there are currently no well-developed statistical methods that can be utilized to detect specific subsets of SNPs involved in the shared polygenicity of phenotypes. In this paper, we illustrate how elastic nets, with appropriate adaptation in selecting the penalty parameter, can be utilized for narrowing the range of SNPs involved in shared polygenicity. …

© 2018 American Academy of Child and Adolescent Psychiatry Our traditional names for psychotropic medication classes lead to unnecessary confusion. As clinicians, we have grown comfortable with idiosyncratic names of psychotropic medications and have forgotten how unclear and misleading they can be. For example, evidence shows that serotonin reuptake inhibitors help in pediatric anxiety disorders, but a parent with an anxious child might ask, “If you diagnosed my son with separation anxiety, why are you giving him an antidepressant?” Another parent might object to the use of a “stimulant” medication, “My daughter never slows down, the last thing she needs …

© The Author(s) 2018. Background There is no countable biomarker for opioid dependence treatment responses thus far. In this study, we recruited Taiwanese methadone maintenance treatment patients to search for genes involving the regulatory mechanisms of methadone dose by genome-wide association analyses. Methods A total of 344 Taiwanese methadone maintenance treatment patients were included in a genome-wide association study. The involvement of GRK5 in opioid dependence was then further confirmed by gene expression study on lymphoblastoid cell lines derived from 3 independent age- and gender-matched groups: Methadone maintenance treatment patients, medication-free former heroin abusers, and normal controls. Results The results …

More than 20 years after the clinical high risk syndrome for psychosis (CHR) was first articulated, it remains controversial whether the CHR syndrome predicts onset of psychosis with diagnostic specificity or predicts pluripotential diagnostic outcomes. Recently, analyses of observational studies, however, have suggested that the CHR syndrome is not pluripotential for emergent diagnostic outcomes. The present report conducted additional analyses in previously reported samples to determine (1) whether comorbid disorders were more likely to persist in CHR patients compared to a comparison group of patients who responded to CHR recruitment efforts but did not meet criteria, termed help-seeking comparison subjects …

© 2018 Elsevier B.V. Objective: Most studies of pediatric bipolar disorder (BP) combine youth who have manic symptoms, but do not meet criteria for BP I/II, into one “not otherwise specified” (NOS) group. Consequently, little is known about how youth with cyclothymic disorder (CycD) differ from youth with BP NOS. The objective of this study was to determine whether youth with a research diagnosis of CycD (RDCyc) differ from youth with operationalized BP NOS. Method: Participants from the Course and Outcome of Bipolar Youth study were evaluated to determine whether they met RDCyc criteria. Characteristics of RDCyc youth and BP …

© 2018 The Author(s). Memory reminders can return a memory into an unstable state such that it will decay unless actively restabilized into long-term memory through reconsolidation. Exposure to a memory reminder, however, does not always lead to destabilization. The 'trace dominance' principle posits that the extent of exposure to memory reminders governs memory susceptibility to disruption. Here, we provide a first systematic investigation of reminder duration effects on threat memory modification in humans. Reminder duration was parametrically varied across 155 participants in a three-day protocol. We found that short reminders (1 s and 4 s) made the memory prone …

Alzheimer's disease (AD) is the most common type of dementia, which has a substantial genetic component. AD affects predominantly older people. Accordingly, the prevalence of dementia has been rising as the population ages. To date, there are no effective interventions that can cure or halt the progression of AD. The only available treatments are the management of certain symptoms and consequences of dementia. The current state-of-the-art medical care for AD comprises three simple principles: prevent the preventable, achieve early diagnosis, and manage the manageable symptoms. This review provides a summary of the current state of knowledge of risk factors for …

Benzodiazepines I: Upping The Care On Downers: The Evidence Of Risks, Benefits And Alternatives, Jeffrey Guina, Brian Merrill
Psychiatry Faculty Publications
Benzodiazepines are some of the most commonly prescribed medications in the world. These sedative-hypnotics can provide rapid relief for symptoms like anxiety and insomnia, but are also linked to a variety of adverse effects (whether used long-term, short-term, or as needed). Many patients take benzodiazepines long-term without ever receiving evidence-based first-line treatments (e.g., psychotherapy, relaxation techniques, sleep hygiene education, serotonergic agents). This review discusses the risks and benefits of, and alternatives to benzodiazepines. We discuss evidence-based indications and contraindications, and the theoretical biopsychosocial bases for effectiveness, ineffectiveness and harm. Potential adverse effects and drug-drug interactions are summarized. Finally, both fast-acting/acute …
