Cryolipolysis (CoolSculpting®) is a Food and Drug Administration-cleared noninvasive fat-reduction technique that has gained popularity in recent years given its reduced cost, risk of adverse effects, and recovery time relative to traditional invasive methods, such as liposuction. Despite this low incidence of adverse effects, there have been increasing reports of a rare aesthetically displeasing phenomenon termed paradoxical adipose hyperplasia (PAH). PAH presents several months after cryolipolysis as a painless and well-demarcated enlargement of the tissue limited to the treatment area.1 With a reported incidence ranging from 0.005% to 2%, all known cases of PAH have been localized to the treatment …

Background and study aims Endoscopic ultrasound-directed transgastric ERCP (EDGE) is a new endoscopic procedure to perform ERCP in Roux-en-y gastric bypass (RYGB) patients. The aim of this study was to conduct a systematic review and meta-analysis to evaluate technical success, clinical success and adverse effects of EDGE and compare it to laparoscopic ERCP (LA-ERCP) and balloon ERCP (BE-ERCP). Patients and methods We conducted a comprehensive search of several databases and conference proceedings including PubMed, EMBASE, Google-Scholar, LILACS, SCOPUS, and Web of Science databases to identify studies reporting on EDGE, LA-ERCP, and BE-ERCP. The primary outcome was to evaluate technical and …

Purpose: Despite federally mandated insurance coverage and clear benefits of breast reconstruction after mastectomy, reported rates remain low at 23-63%. We hypothesize that rates and types of reconstruction would be disproportionately lower in the rural state of Nevada. The goal of this study was to identify and understand disparities in breast reconstruction utilization in Nevada compared to the nation. Methods: Breast cancer patient information (2008-2013) was aggregated from AHRQ’s HCUP Nationwide Inpatient Sample (NIS) and the Nevada State Inpatient Database (NV SID). Chi-square analysis for two population proportions was used to determine differences between the NV SID (n=7,466) and NIS …

Background: Fat grafting is a growing field within plastic surgery. Adipose-derived stem cells (ASCs) and stromal vascular fracture (SVF) may have a role in fat graft survival. Our group previously demonstrated a detrimental effect on ASC survival by the lidocaine used in tumescent solution. Sodium bicarbonate (SB) buffers the acidity of lidocaine. The purpose of this study was to determine whether SB buffering is a practical method to reduce ASC and SVF apoptosis and necrosis seen with common lidocaine-containing tumescent solution. Methods: Human patients undergoing bilateral liposuction for any indication were included in this study. An internally controlled, split-body design …
