A Case Report Of Concurrent Cryptococcal And Tuberculous Meningitis In An Immunosuppressed Renal Transplant Patient, Linda Barasa, Ahmed Sokwala, Felix Riunga, Dilraj Sokhi

Internal Medicine, East Africa

Infections after renal transplant are a common cause of morbidity and are commonly due to Cytomegalovirus (CMV), Cryptococcus, Mycobacterium tuberculosis, and Aspergillus. Concurrent infections with both cryptococcal and tuberculous aetiologies are rare within the central nervous system (CNS). We present a case of a 67-year-old male patient who presented with three weeks of headaches, confusion, unsteady gait, and seizures. He had type 2 diabetes mellitus and hypertension. He had a kidney transplant three years prior and was on three immunosuppressive agents. He was HIV-negative. He was evaluated and found to have cryptococcal meningitis and received appropriate treatment with liposomal amphotericin …

Perceived Barriers To Peritoneal Dialysis Among Kenyan Nephrologists: A Cross-Sectional Descriptive Study, Saleem Abdul Karim Sokwala, John Otieno Odhiambo, Ahmed Sokwala Jun 2022

Perceived Barriers To Peritoneal Dialysis Among Kenyan Nephrologists: A Cross-Sectional Descriptive Study, Saleem Abdul Karim Sokwala, John Otieno Odhiambo, Ahmed Sokwala

Internal Medicine, East Africa

Background: Peritoneal dialysis (PD) is a well-recognized technique of renal replacement therapy (RRT), with similar efficacy as well as survival outcomes as hemodialysis (HD). Despite its advantages including prolonged preservation of residual renal function, potentially lower cost and advances with automated techniques, and commercialization of more biocompatible solutions, the overall prevalence of patients treated with PD is still very low in developed countries and even more so in Africa and low-middle income countries like Kenya. According to our knowledge, no local studies have been done on prevalence of peritoneal dialysis or on potential barriers to utilization of PD as an …

Adequacy Of Hemodialysis And Its Associated Factors Among Patients Undergoing Chronic Hemodialysis In Dar Es Salaam, Tanzania, Samina Somji, Pascal Ruggajo, Sibtain Moledina Feb 2020

Adequacy Of Hemodialysis And Its Associated Factors Among Patients Undergoing Chronic Hemodialysis In Dar Es Salaam, Tanzania, Samina Somji, Pascal Ruggajo, Sibtain Moledina

The worldwide prevalence of maintenance hemodialysis continues to rise. An adequate delivery of hemodialysis dose as measured by Kt/V or urea reduction ratio is a crucial determinant of clinical outcome for chronic hemodialysis patients. )e aim of this study was to assess the adequacy of hemodialysis and its associated factors among patients undergoing chronic hemodialysis in Dar es Salaam. )is was a cross-sectional study done on patients undergoing chronic hemodialysis in four dialysis centers in Dar es Salaam. Sociodemographic information and treatment characteristics were collected. Urea reduction rate and single-pool Kt/V were calculated to determine the adequacy of hemodialysis. )e …

Antiretroviral Therapy Dose Adjustments Based On Calculated Creatinine Clearance, J. Shavadia, G. Tesfaldet, Majid Twahir Apr 2009

Antiretroviral Therapy Dose Adjustments Based On Calculated Creatinine Clearance, J. Shavadia, G. Tesfaldet, Majid Twahir

Internal Medicine, East Africa

Background: Whereas therapy for HIV is dependent on level of creatinine clearance, most laboratories locally only report an absolute creatinine value. There is likelihood that the patients already on antiretroviral therapy (ART) may have required dosage adjustment at the time of initiation of therapy or sometime during ongoing therapy. This paper explores a group of patients who are already on ART to determine their creatinine clearance and assess the need for ART dose adjustment.

Objective: To determine the proportion of stable HIV outpatients who have a documented creatinine clearance (CrCI) and the proportion requiring antiretroviral drug dose adjustments depending on …
