INTRODUCTION: Depression has been well studied as a risk factor for postoperative complications following total shoulder arthroplasty (TSA). However, little is known about new-onset depression (NOD) following TSA. The purpose of this study is to determine the incidence of NOD following TSA and identify risk factors for its occurrence. INTRODUCTION: Among surgical interventions for proximal humerus fractures, RSA has been shown to have superior or noninferior outcomes to hemiarthroplasty or open reduction internal fixation. Cemented humeral fixation in RSA has been the traditional treatment method for proximal humerus fractures, however, there is a growing interest in uncemented humeral fixation. INTRODUCTION: A fully 3D printed prosthetic may offer numerous potential advantages including improved pore precision, overall porosity, topology optimization, spatial variability in stiffness, communication between pores, and strut size. However, there are numerous theoretical concerns surrounding inferior mechanical performance with fully 3D printed implants (e.g. potentially higher rates of fatigue, increased in vivo particle release, etc.) compared to conventionally fabricated ones. Objective: Congenital heart defects (CHDs) are a leading cause of infant morbidity and mortality partly due to low prenatal detection rates. We evaluated whether an artificial intelligence (AI) system can improve the detection of CHDs on fetal ultrasound exams among both general OBGYNs and MFM specialists. Study Design: The AI system analyzes all grayscale 2D ultrasound cines of an exam and detects 8 morphological findings associated with severe CHDs. Objective: Late-onset fetal growth restriction (LO-FGR), defined as onset 3 32 weeks gestation, is the more common subgroup of FGR. The umbilical artery Doppler is usually unaffected. There are no subgroup-specific guidelines for monitoring pregnancies affected by LO-FGR. We compared the utility of the cerebroplacental Doppler ratio (CPR) to the Biophysical Profile (BPP), a widely endorsed FGR monitoring tool, for the prediction of adverse perinatal outcomes (APO). Study Design: This is a retrospective cohort study of LO-FGR, defined as estimated fetal weight < 10th%, from 2013-2018. Using logistic regression analysis, we determined whether low CPR < 5th% and BPP score £ 6 significantly independently predicted the composite APO defined as one or more complications: cord pH < 7.20, 5-minute Apgar score of < 5, operative or cesarean delivery secondary to non-reassuring fetal heart tracing, assisted respiration at birth, NICU admission, and/ or perinatal death. Secondarily, we also evaluated prediction of severe neonatal morbidity (SNM) defined in Table 1 (legend). Results: There were 233 cases of LO-FGR of which 76 (34%) had a low CPR percentile and 28 (13%) had a low BPP score. There were 22 (9.9%) cases with SNM of which 5 (2.1%) had a low CPR percentile and 4 (1.8%) had a low BPP score. A low CPR was associated with a significantly higher rate of individual and composite APO than cases with normal CPR percentile (p < 0.001) [Table 1]. Both a low CPR percentile and a low BPP score significantly predicted APO overall. CPR appeared to be a better predictor of the APO, controlling for gestational age at testing [Table 2]. Only BPP significantly predicted SNM: CPR (OR = 4.64, p = 0.13) and BPP (OR = 4.11, p = 0.038). Conclusion: A low CPR percentile was a significant independent predictor of perinatal complications in LO-FGR, including abnormal fetal heart rate tracing, smaller birthweight percentile, and NICU admission. CPR appeared superior to BPP overall. CPR should be considered as a fetal surveillance tool in LO-FGR.

Objective: Late diagnosis of cyanotic congenital heart defects (CCHD) significantly increases newborn morbidity and mortality. However, a significant percentage of CCHD cases remain undetected prenatally. Our objectives were therefore to evaluate the accuracy of maternal serum lipidomic markers for the prenatal detection of CCHD and also to elucidate the pathogenesis of these disorders. Introduction: Cervical total disc replacement (cTDR) has gained acceptance as a treatment for symptomatic disc degeneration with symptoms of radiculopathy with/without myelopathy. While clinical outcomes of cTDR have consistently been reported to be similar or superior to those of cervical discectomy and fusion, there are limited reports addressing differences related to patient age. The purpose of this study was to compare the 4-year results of two-level PEEKon-ceramic cervical TDR for patients ≥60 years old to those < 60. Materials and Methods: Data were from the prospective Food and Drug Administration (FDA) Investigational Device Exemption (IDE) trial for the Simplify cervical disc. The study included 145 patients who completed 4-year follow-up, of which 124 were < 60 years old at the time of surgery and 21 were ≥60 years. All patients were treated for two-level cervical disc degeneration with radiculopathy and/or myelopathy. Clinical outcome was based on the Neck Disability Index (NDI), neck pain and arm pain intensity scores (10-point scale), and neurologic status. Radiographic measures included flexion/extension (F/E) range of motion (ROM), disc space height, heterotopic ossification, and adjacent-level degeneration based on Kellgren-Lawrence grading. Mean 48-month outcomes for patients ≥60 years old were compared to those of patients < 60. Radiographs were evaluated by an independent lab specializing in image assessment. Results: Of the outcomes reviewed, only one statistically significant difference was detected based on age group. The mean change in F/E ROM from baseline to 48 months at the inferior index level was 3.33° (< 60) and -0.19° (≥60) (p=0.01). At the superior index level, the difference was not significant (1.80° (< 60) vs. 0.12° (≥60); p=0.20). The difference in change in global ROM was also not significant (5.67° (< 60) vs. 0.77° (≥60); p=0.07). Statistically significant differences were not seen for the other outcomes assessed: mean change in NDI, mean change in neck pain, mean change in arm pain. Neurologic status was normal in 93.6% of the < 60 age group and 100.0% of the ≥60 group (p=0.24). The mean change in average disc height from baseline to 48 months was similar in both groups. The distribution of Kellgren-Lawrence grades (none, doubtful, minimal, moderate, and severe) was also similar for each age group for the level above and below the surgical levels (p=0.88 above, p=0.49 below) and the rate of radiographically relevant HO (grade 3 or 4) was comparable between age groups (superior level (p=0.49); inferior level (p=0.20)). Through latest follow-up, there were zero secondary surgical interventions in the ≥60 cohort and 9 in the < 60 cohort. Conclusion: This study found that patients ≥60 years of age who were treated with PEEK-on-ceramic TDR at two levels had comparable clinical and radiographic outcomes to patients < 60 years of age through 4-year follow-up. These results affirm that cervical TDR performs similarly in patients ≥60 versus those < 60, although the small sample size of patients ≥60 years of age is a limitation.

Introduction Obesity is associated with an increased risk of developing renal cell carcinoma (RCC) but paradoxically correlates with improved outcomes in metastatic cases. Moreover, the data on the relationship between obesity and postoperative outcomes following nephrectomy are inconsistent.
