Nemastrina monkeys with multiple electrodes implanted in the amygdaloid area of the brain were used to study the relationship of electrical seizures and overt behavior. The electrical seizures were produced by long duration electrical pulses administered through the implanted electrodes. There appears to be no relationship between this kind of electrical seizure and lever pressing performance. Relationships between depth EEG, depth impedance, DC potential and surface EEG are observed and as a consequence some ideas concerning the genesis of the electroencephalogram are put forth.

A Preliminary Evaluation Of Azathioprine (Imuran®) In The Treatment Of Multiple Sclerosis, William G. Tucker, K. H. Kapphahn

A theoretical approach to the unknown etiology of multiple sclerosis is presented in this preliminary study which reports the results of empiric treatment with azathioprine. Imuran® therapy produced stabilization of the disease, neurological improvement, and no progress of neurological dysfunction or loss of any regained function. Evidence points toward a viral-induced immunological defect. Abnormal gamma globulins found in the cerebral spinal fluid suggest an immunological etiology.

Sarcoidosis And Idiopathic Thrombocytopenic Purpura: First Report Of Coexistence, Ellis J. Vanslyck, Dipankarsri Dasgupta

A patient is described in whom sarcoidosis of the mediastinal lymph nodes was found during the course of acute idiopathic thrombocytopenic purpura. The association of these two diseases should be differentiated from the more commonly seen thrombocytopenia of hypersplenic origin, occurring during the disseminated stage of sarcoidosis, in which granulomatous involvement of an enlarged spleen is found. An elevated serum IgM immunoglobulin level was present. These observations possibly strengthen the concept that an immunological disorder plays a significant role in sarcoidosis.

Etiology Of Congenital Heart Disease, Jami G. Shakibi

The different factors involved in the etiology of congenital heart disease are discussed with particular attention lo the teratogenic agents. Experimental production of congenital heart disease in animals is reviewed. This seems to be a very promising field for investigations, which may eventually lead to an understanding of the etiology of congenital heart disease and its prevention.

Several cases are presented which illustrate the indications and results of a relatively new procedure of intrathecal injection of radioiodinated human seruin albumin I 131 to demonstrate cerebrospinal fluid dynamics. This procedure has been used in conjunction with pneumoencephalography in the diagnosis of communicating hydrocephalus. A proper dose as well as adequate volume are necessary to obtain diagnostic scans.

Indigo carmine only recently was found to have a vasopressor effect. Intravenous injection in anesthetized and unanesthetized patients resulted in blood pressure elevation and bradycardia. This effect appears analogous to that of serotonin which indigo carmine structurally resembles. This drug should be used with caution in patients with cardiovascular disease.

Slipped Capital Femoral Epiphysis (Follow-Up Study), C. Leslie Mitchell

Epiphyseolysis, or slipping of the capital femoral epiphysis, remains a controversial problem from the standpoint of etiology and treatment. This report is limited to results of treatment of severely slipped epiphyses at the Henry Ford Hospital from 1937 lo 1967. The surgical technique and postoperative care are described. Of a total of 137 hips treated surgically in this period, 38 were severe, requiring transcervical osteotomy and internal fixation. The follow-up results of these 38 cases are reported.

Generally satisfactory experience is reported in a 10-year study and evaluation of a modified surgical technique for correction of anteversion and associated gait abnormalities in cerebral palsy. The 25 patients so treated represent only 2% of those seen during the period.

Experience gained in the study of 307 patients by pulmonary angiograms and/or isotope lung scans has led to concepts regarding the limitation and usefulness of each. The scan is ideally useful when the patient with suspected pulmonary embolism has a negative chest film. Angiograms should be performed when there are parenchymal infiltrations on the chest radiograph or when a definite diagnosis is necessary. Either study should be performed early in the course of the patient's illness to permit diagnostic study.

Pacemaker Induced Ventricular Tachycardia: Report Of A Case With The "Runaway Pacemaker", Remigio Garcia, Eduardo Arciniegas

The "runaway pacemaker" is a serious pacing failure resulting from battery exhaustion or malfunction of some of the pulse generator components. Hypotension, rapid rate, dyspnea and palpitations were the main clinical features of one example of this iatrogenic arrhythmia presented here. When diagnosed, this condition must be treated as an emergency by disconnecting the subcutaneous wire electrodes from the unit and attaching them to a new pulse generator. The importance of frequent examinations with electrocardiographic recordings for the early recognition of this complication is emphasized.

Calorie-free carbonated beverages, or "diet pops," are becoming increasingly popular and many households no longer contain the older, more familiar, sugar containing, carbonated beverages. Because physicians frequently make use of "soda pop" when prescribing treatment for common pediatric conditions such as febrile episodes or bouts of vomiting or diarrhea, the following case report is presented to illustrate the fact that physicians must ascertain that the beverage to be used to maintain hydration is not calorie free.

An antibody chemically and immunologically active against bovine testicular hyaluronidase was produced in rabbits by injecting them with purified hyaluronidase in complete Freund's adjuvant. Nonspecific antibodies, reacting with human and bovine proteins which had little or no hyaluronidase activity, were also detected by the double agar immunodiffusion technique using antiserum against these proteins. After absorption of the antiserum with bovine heart and liver and removal of the precipitation bands against human and bovine tissue proteins, the antiserum inhibited the chemical activity of the purified hyaluronidase and exhibited immunologic activity against the purified hyaluronidase as shown by the agar double immunodiffusion …

During the past 10 years, the collection and microscopic examination of human temporal bones has provided the opportunity to correlate clinical symptomatology and findings with histopathology. The case findings in this report support the clinical suspicion of glandular mucosal disease in some forms of secretory otitis media.

Sjögren's Syndrome In Childhood: Report Of A Case, Howard Duncan, Bruce N. Epker, Gerald M. Sheldon

This report documents a rare case of Sjögren's Syndrome which began in a child with juvenile rheumatoid arthritis at the age 10 years, followed by the symptoms of xerostomia with intermittent salivary gland swelling 15 months later. By age 12 the patient had developed keratoconjunctivitis sicca. Variations in the intensity of each component of Sjögren's syndrome were noted to be independent of each other.

Serum immunoglobulin levels are reported in 75 patients with jaundice: 32 with jaundice due to virus hepatitis and 43 with jaundice due to obstruction. The degrees and variations of serum immunoglobulins (Ig), (IgM, IgG and IgA) in virus hepatitis (IH and SH) with jaundice are compared with those found in obstructive jaundice. In 32 of the patients with jaundice from virus hepatitis, serum IgM (100%) and IgG (75%) were elevated above 2 Standard Deviations from the mean values. Two simplified quantitative urine culture techniques, suitable for use in the physician's office, were shown to be reliable and practical. The miniature culture method (Testuria) and flood-plate technique gave comparable quantitative results, and the latter also yielded prompt information of organism specificity which was helpful in judging the significance of the culture findings.

Histochemical enzymatic survey of 25 "normal" human carotid bodies removed at autopsy from patients three days to 83 years of age revealed a variety of oxidative and hydrolytic enzymes in the lobular and interlobular cells. The intensity of the reactions was greater in the lobular cells. Acid phosphatase was the only enzyme that was limited to the cells comprising the lobules.

Massive hemoptysis demands immediate investigation and treatment. Bronchoscopy done while the bleeding is active is essential to accurate localization of the site of origin and the cause of hemorrhage. Emergency pulmonary resection should be carried out if death is to be prevented by suffocation or exsanguination. Illustrative cases are presented.

Endometrial Carcinoma: Report Of 441 Cases From Henry Ford Hospital 1923 Through 1963, Roy B. Patton

The study here presented is an historical examination of endometrial carcinoma at Henry Ford Hospital. A modified life table was constructed to estimate survivorship, and selected clinical features related to the patient's endocrine status were examined. Pathological findings associated with endocrine abnormalities in the ovaries were enumerated. The results of histologic grading of the tumors are presented, and the incidence and location of other primary malignant tumors in these patients are discussed.
