Purpura Arthralgia Weakness Mixed Cryoglobulinemia Syndrome Treated With Splenectomy: A Case Report, Carl R. Haeger

Weakness purpura mixed arthralgia cryoglobinemia (IgG - IgM) syndrome was first described by Meltzer in 1966. Such a case was recently reported after treatment with Cyclophosphamide and subsequent splenectomy for pancytopenia with beneficial effects on the cryocrit. The patient reported here underwent splenectomy without Cyclophosphamide and had subsequent amelioration of the clinical symptoms. Abnormal plasma cells found in the spleen presumably were responsible (at least in part) for the patient's symptomatology.

Malignant Histiocytosis Of The Robb-Smith Type (Histiocytic Medullary Reticulosis), Michael F. Macken, Ellis J. Van Slyck, Sheik M. Saeed

A 60-year-old woman developed high fever, jaundice, anemia with pancytopenia and hepatosplenomegaly. She deteriorated rapidly and died one month later, having shown no beneficial effect from high dose corticosteroids and splenectomy. An antemortem diagnosis of malignant histiocytosis of the Robb-Smith type was made and duly confirmed by autopsy study. This rare disorder of the reticuloendothelial system, heretofore designated "histiocytic medullary reticulosis," deserves recognition and inclusion among the histiocytic diseases and in the classification of the lymphomas. Its proper nosologic designation is suggested.

A Microtechnique For The Isolation Of Cell Wall Deficient Organisms From Very Small Samples, Lloyd T. Dutcheshen, Philip C. Hessburg

A miniaturized procedure using a modified microtiter plate is described for the isolation of cell wall deficient organisms. It is particularly useful when the volume of a liquid specimen is very small.

Kinesiology Of Knee Flexion Deformity Using Electromyography, Paul J. Roby

A kinesiological electromyographic study was completed to determine the relative changes in muscle activity as a person walks and stands with different degrees of knee flexion deformity. The ultimate aim was to establish criteria for appropriate preventive and corrective measures relating to knee flexion deformity.

Six different muscles were implanted with wire electrodes. The subjects performed a series of walking and standing experiments with the knee flexed at selected angles by a locking leg brace to simulate knee flexion deformity. Quantitative histologic measurements of haversian bone remodelling were made of mineralized rib cross sections in 13 healthy rhesus monkeys. With the aid of previous tetracycline labelling, coupled with the Villanueva Osteochrome stain, Frost's quantitative bone analysis revealed that: a) the mean haversian bone formation rate is 0.359 z 0.186 mm²/mm²/yr; b) the mean number of osteoid seams/mm² is 4.03 ± 1.64; c) the mean appositional rale is 1 33 ± 0 23 microns/day while the radial closure rate when all the labelled systems are corrected is 0.42 r 0.080 mm/year; d) the mean circumference of …

A review is presented of 36 cases of nonparasitic cysts of the liver. These are uncommon and had been difficult to diagnose clinically. Diagnosis has usually been made by post-mortem examination and by surgery. Attention is focused on the merits of modern diagnostic methods to differentiate cystic from solid tumors of the liver. These methods include radioisotope scanning, ultrasonography, and abdominal arteriography.

Two hundred and eighty patients with primary gout diagnosed between the years 1959-64 at Henry Ford Hospital were studied to determine the frequency of myocardial infarction in the fourth, fifth and sixth decades. The retrospective data correlate with previously published reports of accelerated platelet turnover and our own observation of increased platelet surface activation in patients with gout. The results suggest an increased risk (or myocardial infarction before age 50 in patients with primary gout; this merits prospective study. The finding of increased platelet surface activation in patients with gout may provide an insight into the pathophysiology of the apparent …

Two hundred patients have been reviewed in whom culdoscopy or laparoscopy was performed for infertility study. During these two procedures, methylene blue was injected through an endocervical cannula for detection of tubal patency under direct visualization. When compared with 89 patients tested by utero-tubal insufflation and hystero-salpingography, results indicated that direct visualization of the tubes and dye hydrotubation appears to be the most reliable method. HSG proved to be the second most reliable method with a failure rate of 19.2%. The least accurate procedure in this study was UTI with CO₂ gas, where the failure rate was 24.7%. The …

Intraperitoneal administration in rats of a single large injection of the potent nephrotogenic aminonucleoside,6-dimethylamino-9-(3'-amino-3' =deoxy-β-D-ribofuranosyl)-purine, resulted in striking reductions in kidney cortex mitochondrial monoamine oxidase (MAO) activity, assayed on tyramine substrate The alterations in enzymatic activity correlated remarkably well with the onset and development of the massive proteinuria characteristic of the disease. The inhibitory effects were also produced in vitro, and Lineweaver-Burk reciprocal plots were clearly indicative of non-competitive inhibition. It is suggested that aminonucleoside inhibition of kidney mitochondrial MAO activity may be a biochemical lesion in the primary step of GBM collagen cross-link formation, whereby collagen fibrillogenesis is impaired …

Biopsy-Proven Amebic Colitis Treated With Metronidazole, Frederick R. Bode, Earl J. Van Volkinburg

Amebiasis was diagnosed in a Vietnam veteran and treated in a conventional manner with diodoquin and tetracycline. Symptoms persisted over the next three and one-half years despite repeated courses of diodoquin. Amebic colitis was confirmed by biopsy through a sigmoidoscope. The patient then received a ten-day course of metronidazole (Flagyl). Studies are cited proving this single agent is the drug of choice in the oral therapy of all clinical forms of amebiasis.

C-Reactive Protein: Potential Significance Of Quantitation In Patients With Chronic Diseases, Hajime Hayashi, Gerald A. Logrippo

Serum C-reactive protein (CRP) levels in 351 patients with certain chronic diseases were assayed by a micro-double immunodiffusion technique developed and used in this hospital. Elevation of CRP values and severity of disease were evident in the following clinical entities: 31 of 54 patients (57%) with liver cirrhosis; 122 of 204 (60%) hepatitis patients; 34 of 52 (65%) patients with gastro-intestinal surgery; 8 of 21 (38%) uremic patients; and all of 20 patients with renal transplantation. There was good correlation between markedly elevated serum CRP values and degree of inflammation and/or physiologic stress during clinical course and recovery.

This report describes a project in a large urban, predominantly black, high school which combined a hemoglobinopathy survey with an educational and genetic counseling emphasis. The faculty and students received information on sickle cell disease explaining its genetic and personal implications. About 29% of the 2,700 students received parental permission and volunteered for blood tests in the initial year. Blood was examined by starch gel electrophoresis during the first year and by cellulose acetate electrophoresis during the second year. Of the 1300 total individuals studied, 7% were found to have sickle cell trait and 2% hemoglobin C trait. The cellulose …

Epidermoid Carcinoma Arising In An Epidermal Inclusion Cyst, Talmadge V. Hayes, Joseph Ponka

Mostly because of adequate early treatment, large epidermoid carcinomas are not frequently seen today. The case which we documented is unusual because of the huge size of the tumor and the resulting deformity. The photographs illustrate the pre-operative and post-operative appearance. The fact that these tumors can reach enormous size and become malignant bears repeating. Radical excision and skin grafting, we believe, resulted in a cure for this patient. Incidentally, there was a great change in her mental outlook after the removal of this tumor.

Fifty-nine consecutive patients who were bleeding from the upper gastrointestinal tract were examined endoscopically with fiberoptic instruments. The site and nature of the bleeding were identified in 84% of the patients without significant morbidity or mortality. Endoscopic examination of the esophagus, stomach and duodenum in the bleeding patient should be done within 12 to 24 hours after hospital admission and before barium study of the upper gastrointestinal tract.

Research findings in the last 20 years concerning senile and postmenopausal osteoporosis indicate that the skeletal condition associated with these diseases arises because some systemic factor acts directly upon the bone marrow tissues. There is a secondary tendency to accelerate loss of bone greater than in the normal aging process. The loss occurs only on those surfaces in physical contact with the bone marrow tissue.

The main purpose of this investigation was to test the endodontic sealing abilities of silver cones, N2, warm gutta percha, chemically softened gutta percha, laterally condensed gutta percha and Cavit under carefully controlled conditions. Evaluations were done by radiographic, macroscopic, microscopic, and autoradiographic examinations.

The four points which must be considered when evaluating the efficacy of sealing root canal systems, in vivo, were presented and discussed.

The cytology of rectal imprints of 105 control subjects and 79 patients with various rectosigmoid diseases has been studied. Our modified slide applicator was employed and found to be efficient, safe and easy to use. The cytological method has been more accurate than evaluation of the macroscopic appearance of the rectal mucosa only and therefore more efficient in the early diagnosis and follow-up of inflammatory bowel diseases. If used during routine proctosigmoidoscopy, it can help to determine the true incidence of proctocolitis in the population at large.
