Introduction: Current cognitive assessments suffer from floor/ceiling and practice effects, poor psychometric performance in mild cases, and repeated assessment effects. This study explores the use of digital speech analysis as an alternative tool for determining cognitive impairment. The study specifically focuses on identifying the digital speech biomarkers associated with cognitive impairment and its severity.

Methods: We recruited older adults with varying cognitive health. Their speech data, recorded via a wearable microphone during the reading aloud of a standard passage, were processed to derive digital biomarkers such as timing, pitch, and loudness. Introduction: Nearly, a quarter of older adults suffer from frequent foot pain, impacting their quality of life. While proper footwear can alleviate this, design issues often hinder regular use. This study evaluated novel therapeutic footwear, designed for aesthetics and custom fit, to reduce foot pain. We hypothesized that older adults would experience less foot pain and favor the new footwear over their own.

Methods: This 12-week crossover randomized controlled trial evaluated the effectiveness of OrthoFeet therapeutic footwear on reducing foot pain in older adults (n = 50, age = 65 ± 5, 18% male) with moderate to severe pain. Participants …

Transcription factor EB (TFEB) mediates gene expression through binding to the coordinated lysosome expression and regulation (CLEAR) sequence. TFEB targets include subunits of the vacuolar ATPase (v-ATPase), which are essential for lysosome acidification. Single-nucleus RNA sequencing of wild-type and PS19 (Tau) transgenic mice expressing the P301S mutant tau identified three unique microglia subclusters in Tau mice that were associated with heightened lysosome and immune pathway genes. To explore the lysosome-immune relationship, we specifically disrupted the TFEB-v-ATPase signaling by creating a knock-in mouse line in which the CLEAR sequence of one of the v-ATPase subunits, Atp6v1h, was mutated. Functional roles of SIGLEC15 in hepatocellular carcinoma (HCC) were not clear, which was recently found to be an immune inhibitor with similar structure of inhibitory B7 family members. SIGLEC15 expression in HCC was explored in public databases and further examined by PCR analysis. SIGLEC15 and PD-L1 expression patterns were examined in HCC samples through immunohistochemistry. SIGLEC15 expression was knocked-down or over-expressed in HCC cell lines, and CCK8 tests were used to examine cell proliferative ability in vitro. Emerging evidence finds similar cardiovascular risks resulting from electronic cigarettes (E-cig) as that seen from smoking. Lung cancer remains the most frequently diagnosed malignant neoplasm shared in men and women worldwide, second only to the sexually dimorphic prostate cancer in men and breast cancer in women. Overall, lung cancer was responsible for 12.5% of new cancer cases globally in the year 2022. Of these cases, 80-85% are non-small cell lung cancer (NSCLC), a highly genotypically diverse sub-type of lung cancer. Chronic inflammation plays a role in the development and progression of airway diseases such as chronic obstructive pulmonary disease (COPD) and cystic fibrosis (CF). COPD is caused primarily by chronic cigarette smoke exposure. Conversely, CF is a single gene disorder caused by mutations in the cystic fibrosis transmembrane conductance regulator gene (CFTR). In recent years, nanobodies have become important theranostic molecules, and research has focused on improving their pharmacokinetics and binding for novel targets. Nanobodies are characterized by their high kidney uptake, leading to potentially unsafe exposure of the kidneys when delivering cytotoxic payloads. Researchers hypothesize that positively charged amino acids contribute to this kidney uptake. To address this, we assessed the impact of substituting lysine residues within the 2Rs15d HER2 nanobody scaffold to alanine on its in vivo pharmacokinetics using positron emission tomography (PET) imaging. The sympathetic nervous system is a key regulator of energy homeostasis, in part, through the control of white adipose tissue lipolysis. Sympathetic nerves modulate adipose tissue physiology through the release of catecholamines, particularly norepinephrine, onto alpha- and beta-adrenergic receptors (a-AR; B-AR). Lipolysis refers to the breakdown of triglycerides into glycerol and free fatty acids, often in response to activation of B2/3-AR. The neuropathology includes the formation of extracellular amyloid-β plaques and intracellular neurofibrillary tangles of phosphorylated tau. Patients receiving antibiotic treatment experience dysbiosis of gut microbiota, and C. difficile, normally held in check by the various other organisms, takes this opportunity to propagate. Symptoms of infection generally include diarrhea, colitis, dehydration, and fever. Turner Syndrome (TS) is a chromosomal disorder from conception characterized by the partial or complete absence of the second X chromosome in females. Chromosomal abnormalities, both numerical and structural, contribute to a significantly higher prevalence of fractures (30.5-32.2%) compared to non-TS postmenopausal women (14.9%). This highlights the intrinsic bone abnormalities associated with TS and increased fracture risk. Peripheral quantitative computed tomography (pQCT) is commonly used to assess bone mineral density (BMD). In recent years, hypersomnia patients and researchers alike have increasingly discussed the possibility of heightened comorbidities, or co-occurrences, of autism spectrum disorder (ASD) and attention-deficit/hyperactivity disorder (ADHD) with the central disorders of hypersomnolence: idiopathic hypersomnia (IH) and narcolepsy (N). While a few scarce studies have investigated a link between ADHD and N, there has been no research on the co-occurrence of these conditions with IH. The Prognostic Impact Of Various Circulating Cytokine Levels On Response To Egfr-Tkis Treatment In Advanced Nsclc Patients, Warissara Khuntharuks

Advanced NSCLC patients are routinely treated with EGFR-TKIs but often eventually develop acquired resistance, influenced by circulating cytokines. This study investigated cytokine associations with disease control in EGFR-mutant NSCLC patients (N=123) by analyzing plasma samples at pre-treatment and 6–12 weeks post-treatment using a 27-bioplex cytokine panel, compared with healthy controls (N=30). Patients were classified as durable responders (16.3%) (PFS ≥ 104 weeks) and non-durable responders (83.7%) (PFS<104 weeks). Notably, IL-1Ra, IL2, IL-6, IL-7, IL-12p(70), TNF- alpha, IL-4, IL-5, IL-8, IL-13, Eotaxin, IP-10, MIP-1 alpha, RANTES, Basic FGF, GM-CSF and PDGF-BB were significantly presented with high levels in the patient’s pre-treatment plasma compared to healthy controls (p<0.05). High IL-6 and IL-10 levels were significant in non-durable responders (p = 0.0076 and p = 0.0166, respectively). At second-time points, non-durable responders exhibited significantly lower IL-6 levels (p=0.0019), whereas durable responders showed increased IL-10 (p = 0.0234). High pre-treatment IL-6 correlated with poor PFS (p=0.018), OS (p<0.001), and TTF (p<0.001). Consistently, high IL-10 was also correlated with shorter TTF (p<0.005). IL-6 and IL-10 showed strong diagnostic performance (AUC with 0.755 and 0.738, respectively). These cytokines may serve as prognostic biomarkers for evaluating durable EGFR-TKI responses in NSCLC patients.

วัตถุประสงค์ : เพื่อศึกษาผลการรักษาความผิดปกติของเส้นประสาทขนาดใหญ่ด้วยวิตามินดีในผู้ป่วยเบาหวานที่มีโรคเส้นประสาท และมีภาวะพร่องวิตามินดี วิธีวิจัย : ศึกษาแบบสุ่มเปรียบเทียบผลการรักษาระหว่างกลุ่มวิตามินดีและกลุ่มยาหลอกในโรงพยาบาลจุฬาลงกรณ์ โดยคัดเลือกผู้ป่วยที่มีอาการหรืออาการแสดงของเส้นประสาทขนาดใหญ่จากเบาหวาน และมีภาวะพร่องวิตามินดี แบ่งเป็นสองกลุ่ม ได้แก่ กลุ่มที่ได้รับวิตามินดีสอง ขนาด 60,000 IU ต่อสัปดาห์ และกลุ่มที่ได้รับยาหลอก เป็นระยะเวลา 12 สัปดาห์ ผลลัพธ์หลักคือ การเปลี่ยนแปลงของคะแนน modified NDS หลังได้รับการรักษา 12 สัปดาห์ ผลลัพธ์รองคือ การเปลี่ยนแปลงของการนำไฟฟ้าของเส้นประสาท การรับรู้การสั่นสะเทือน และประเมินคะแนนความรู้สึกของอาการชา ผลการศึกษา: ผู้เข้าร่วมการศึกษาครบ 12 สัปดาห์ จำนวน 60 ราย แบ่งเป็นกลุ่มละ 30 ราย ผลการรักษาที่ 12 สัปดาห์พบว่าคะแนน modified NDS ดีขึ้นอย่างมีนัยสำคัญทางสถิติในกลุ่มวิตามินดี (P = 0.045) อย่างไรก็ตาม เมื่อเปรียบเทียบระหว่างกลุ่ม ไม่พบความแตกต่างอย่างมีนัยสำคัญ (P = 0.060) เช่นเดียวกับการเปลี่ยนแปลงการนำไฟฟ้าของเส้นประสาท การรับรู้การสั่นสะเทือน และคะแนนความรู้สึกของอาการชา ไม่พบความแตกต่างกันระหว่างกลุ่ม สรุป: การรักษาด้วยวิตามินดีสองในผู้ป่วยเบาหวานที่มีโรคเส้นประสาทขนาดใหญ่ และมีภาวะพร่องวิตามินดี ไม่ทำให้คะแนน modified NDS ดีขึ้นอย่างมีนัยสำคัญทางสถิติ
