วัตถุประสงค์: เพื่อเปรียบเทียบการเปลี่ยนแปลงของปริมาณไขมันตับหลังจากเปลี่ยนยาจากเอฟาเวียเรนซ์เป็นริลพิวิรีน ในผู้ป่วยที่รักษาด้วยยาเอฟาเวียเรนซ์ร่วมกับยาต้านไวรัสกลุ่ม NRTI อีก 2 ชนิด เทียบกับผู้ป่วยที่ได้รับยาเอฟาเวียเรนซ์เดิมต่อ วิธีการวิจัย: การศึกษาแบบสุ่มชนิดมีกลุ่มควบคุม สุ่มอาสาสมัครเป็นอัตรา 1:1 เป็นผู้ติดเชื้อเอชไอวีอายุตั้งแต่ 18 ปี และรับยาต้านไวรัสเอฟาเวียเรนซ์ร่วมกับทีโนโฟเวียร์/เอ็มตริไซตาบีน หรืออะแบ็กคาเวียร์/ลามิวูดีน และเป็นโรคตับคั่งไขมันที่ไม่ได้เกิดจากแอลกอฮอล์ แล้วเปลี่ยนยาจากเอฟาเวียเรนซ์เป็นริลพิวิรีน หรือยังให้เอฟาเวียเรนซ์เดิมต่อ โดยก่อนเข้าร่วมวิจัยต้องมีค่าไขมันตับวัดด้วยวิธี CAP มากกว่า 248 เดซิเบล/เมตร ขึ้นไป ผลการศึกษา: อาสาสมัครเข้าร่วมทั้งหมด 30 คน และ 17 คน ถูกสุ่มเข้ากลุ่มเปลี่ยนยาเป็นริลพิวิรีน พบค่ามัธยฐานของความต่างของค่าไขมันตับระหว่างเวลาตั้งต้นและสัปดาห์ที่ 24 ลดลง -20 เดซิเบล/เมตร ในผู้ป่วยกลุ่มเปลี่ยนยา RPV และลดลง -2 เดซิเบล/เมตร ในผู้ป่วยกลุ่มใช้ยาเดิม EFV โดยกลุ่มผู้ป่วยกลุ่มเปลี่ยนยา RPV มีการลดลงของความต่างของค่าไขมันตับมากกว่า แต่ไม่มีความแตกต่างอย่างมีนัยสำคัญทางสถิติ (P = .202) สรุป: การศึกษานี้ แสดงให้เห็นว่าหลังสัปดาห์ที่ 24 ผู้ป่วยติดเชื้อเอชไอวีที่เป็นโรคตับคั่งไขมันที่ไม่ได้เกิดจากแอลกอฮอล์ ที่ได้รับการเปลี่ยนยาต้านไวรัสจากเอฟาเวียเรนซ์เป็นริลพิวิรีน เทียบกับผู้ป่วยที่ได้รับยาต้านไวรัสเป็นเอฟาเวียเรนซ์เดิม ไม่พบความแตกต่างของการเปลี่ยนแปลงของปริมาณไขมันตับ ผลการศึกษาเช่นนี้ อาจเกิดจากขนาดตัวอย่างน้อยกว่าเป้าหมายและต้องการศึกษาขนาดใหญ่มากขึ้น

ที่มาของงานวิจัย: ภาวะทุพโภชนาการเป็นปัจจัยเสี่ยงที่สำคัญต่อการเสียชีวิตในผู้ป่วยล้างไตทางช่องท้องที่มีภาวะเยื่อบุช่องท้องอักเสบ ข้อมูลของการให้โปรตีนเสริมในผู้ป่วยที่มีภาวะติดเชื้อเยื่อบุช่องท้องยังค่อนข้างจำกัด ระเบียบวิธีวิจัย: เป็นการศึกษาแบบการทดลองหลายแหล่งวิจัยแบบเปิดที่ศึกษาในศูนย์ล้างไตทางช่องท้องทั้งหมด 9 แห่งในประเทศไทย ที่ศึกษาผู้ป่วยล้างไตทางช่องท้องที่อายุมากกว่าหรือเท่ากับ 18 ปีที่ล้างไตทางช่องท้องและได้รับการวินิจฉัยว่ามีภาวะติดเชื้อเยื่อบุช่องท้อง ร่วมกับมีระดับแอลบูมินในกระแสเลือดต่ำกว่า 3.5 กรัม/ดล. และคัดผู้ป่วยที่ติดเชื้อจากวัณโรค เชื้อรา ติดเชื้อแบบทุติยภูมิ หรือมีภาวะติดเชื้อในกระแสโลหิตรุนแรงออกจากการศึกษา โดยทำการสุ่มผู้ป่วยในอัตรา 1:1 เพื่อเข้าสู่กลุ่มศึกษา และกลุ่มควบคุม ในกลุ่มที่ศึกษาผู้ป่วยจะได้รับโปรตีนเวย์ปริมาณ 30 กรัม/วัน เป็นระยะเวลา 30 วัน โดยที่กลุ่มควบคุมจะไม่ได้รับโปรตีนเสริม ผู้ป่วยในทั้งสองกลุ่มจะได้รับคำแนะนำทางโภชนาการจากนักกำหนดอาหารตามแนวทางการดูแลผู้ป่วยติดเชื้อเยื่อบุช่องท้อง โดยศึกษาผลลัพธ์ของการศึกษาหลักเป็นผลลัพธ์รวมของการเสียชีวิตจากภาวะติดเชื้อเยื่อบุช่องท้องและการติดเชื้อเยื่อบุช่องท้องซ้ำแบบ relapsing และ repeat และผลลัพธ์ของการศึกษารองคือระดับแอลบูมินในกระแสเลือดระหว่างทั้งสองกลุ่ม ผลการศึกษา: ผู้ป่วยทั้งหมด 74 คนจากเดือนมิถุนายน พ.ศ. 2564 ถึงเดือนกุมภาพันธ์ 2565 เข้าร่วมการศึกษาและได้รับการสุ่มไปยังกลุ่มศึกษา 37 คน และกลุ่มควบคุม 37 คน ลักษณะผู้ป่วยในทั้งสองกลุ่มไม่ได้มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ จากผลการศึกษาพบว่าผู้ป่วยในกลุ่มศึกษามีอัตราการเสียชีวิตหรือการติดเชื้อเยื่อบุช่องท้องซ้ำแบบ relapsing หรือ repeat ที่ 15 คน (ร้อยละ 41) และ 19 คน (ร้อยละ 51) (hazard ratio 0.84; ช่วงเชื่อมั่นร้อยละ 95 0.43-1.67) แต่พบว่าระดับแอลบูมินในกระแสเลือดในกลุ่มศึกษามีระดับสูงกว่ากลุ่มควบคุมอย่างมีนัยสำคัญเมื่อวิเคราะห์แบบตลอดการศึกษา (P<0.001) โดยมีค่าแตกต่างที่วันที่ 120 เฉลี่ยอยู่ที่ 0.35 กรัม/ดล. A novel monopolar electroporation system and methodologies were developed for in vivo electroporation intended for potential clinical applications such as gene therapy. We hypothesized that an asymmetric anode/cathode electrode applicator geometry could produce favorable electric fields for electroporation, without the typical drawback associated with traditional needle and parallel plate geometries. Three monopolar electrode applicator prototypes were built and tested for gene delivery of reporter genes to the skin in a guinea pig model. Cold physical plasma is a partially ionized gas generating various reactive oxygen and nitrogen species (ROS/RNS) simultaneously. ROS/RNS have therapeutic effects when applied to cells and tissues either directly from the plasma or via exposure to solutions that have been treated beforehand using plasma processes. This review addresses the challenges and opportunities of plasma-treated solutions (PTSs) for cancer treatment. These PTSs include plasma-treated cell culture media in experimental research as well as clinically approved solutions such as saline and Ringer’s lactate, which, in principle, already qualify for testing in therapeutic settings. Autosomal genetic analyses of blood lipids have yielded key insights for coronary heart disease (CHD). However, X chromosome genetic variation is understudied for blood lipids in large sample sizes. We now analyze genetic and blood lipid data in a high-coverage whole X chromosome sequencing study of 65,322 multi-ancestry participants and perform replication among 456,893 European participants. Common alleles on chromosome Xq23 are strongly associated with reduced total cholesterol, LDL cholesterol, and triglycerides (min P = 8.5 × 10-72), with similar effects for males and females. The Gram-positive anaerobic bacterium Cutibacterium acnes (C. acnes) is a commensal of the human skin, but also an opportunistic pathogen that contributes to the pathophysiology of the skin disease acne vulgaris. C. acnes can form biofilms; cells in biofilms are more resilient to antimicrobial stresses. Acne therapeutic options such as topical or systemic antimicrobial treatments often show incomplete responses. Biofilms formed by multidrug-resistant bacteria are a major cause of hospital-acquired infections. Cold atmospheric-pressure plasma (CAP) is attractive for sterilization, especially to disrupt biofilms formed by multidrug-resistant bacteria. However, the underlying molecular mechanism is not clear. In this study, CAP effectively reduced the living cells in the biofilms formed by methicillin-resistant Staphylococcus aureus, and 6 min treatment with CAP reduced the S. aureus cells in biofilms by 3.5 log10. Interleukin 15 (IL-15) has been evaluated as a potential treatment for solid tumors in clinical trials, but the effectiveness of systemic IL-15 administration as a monotherapy has not been realized. Gene-based approaches for protein replacement therapies have the potential to reduce the number of administrations. Our previous work demonstrated that expression could be enhanced and/or the applied voltage reduced by preheating the tissue prior to pulse administration. In the current study, we utilized our 16-pin multi-electrode array (MEA) and incorporated nine optical fibers, connected to an infrared laser, between each set of four electrodes to heat the tissue to 43 °C. An estimated 350 million people are living with chronic Hepatitis B virus (HBV) worldwide. Preventative HBV vaccination in infants has reduced the disease burden; however, insufficient immunization programs and access obstacles leave vulnerable populations at risk for infection in endemic regions. Gene electrotransfer (GET) using a noninvasive multielectrode array (MEA) provides an alternative platform for DNA vaccination in the skin. Visceral stimuli transmitted through primary afferent neurons in the dorsal root ganglia (DRG) induce a nociceptive response. Anxiety disorders disproportionately impact women and are more prevalent in people with a history of chronic stress. Post-traumatic stress disorder (PTSD) is a type of stress and trauma-related disorder that is largely characterized by hyperarousal to fear-based cues. The inability to dissociate the fear-response from a non-threatening cue is known as impaired safety learning. Given that N-methyl-D-aspartate (NMDA) receptors are key mediators of learning behaviors, we examined the role of NMDAr function in deficient safety learning. Traumatic brain injury (TBI) is a common phenomenon, accounting for significant cost and adverse health effects. While there is information about focal pathologies following TBI, knowledge of more diffuse processes is lacking, particularly regarding how analgesics affect this pathology. Cognitive processes involve precisely coordinated neuronal communications between multiple cerebral cortical structures in a task specific manner. Rich new evidence now implicates the cerebellum in cognitive functions. There is general agreement that cerebellar cognitive function involves interactions between the cerebellum and cerebral cortical association areas. Traditional views assume reciprocal interactions between one cerebellar and one cerebral cortical site, via closed-loop connections. We offer evidence supporting a new perspective that assigns the cerebellum the role of a coordinator of communication.
