Increasing First Case On Time Starts In An Ambulatory Surgery Center, Diane Fecteau, Shannan Reid, Sydney Green, Ruth Hanselman, Suneela Nayak, Stephen Tyzik, Amy Sparks

Operations Transformation

In an ambulatory surgical center, first case on-time starts directly affects the patient experience. In addition, in order to treat as many patients as possible, delays of first case on-time starts negatively impacts the rest of scheduled surgical patients and increases staff overtime expenditures. An ambulatory surgical team within a large urban health care system initiated a performance improvement initiative to enhance the patient experience, increase staff accountability and care team well-being.

The goal of this project was to start 70% or more first cases on time. Baseline metrics demonstrated that patients and surgeons were the largest cause of delay. …

Race-Based Trauma Care Training For Veteran’S Health Administration Nurse Practitioners, Vanessa Loyd Jul 2019

Race-Based Trauma Care Training For Veteran’S Health Administration Nurse Practitioners, Vanessa Loyd

Dissertations

Abstract

Problem: Trauma informed care has become a priority for persons exposed to psychological, physiological, emotional consequences as a result of traumatic experiences. Veterans are from different cultures, including gender, race, religion, sexual orientation and other social groups. The Veterans Health Administration (VHA) is responsible for the health of this culturally diverse patient population. Healthcare providers without any prior military experience may experience difficulty obtaining an accurate history and physical exam due to the military’s cross-cultural environment. The project aim was to increase confidence and competence in race-based trauma care by incorporating training into a new acute care fellowship …

A Vaccine Administration Training Program To Increase Adolescent Vaccination Rates, Claire Jensen Jul 2019

A Vaccine Administration Training Program To Increase Adolescent Vaccination Rates, Claire Jensen

Dissertations

Abstract

Immunizations have been proven to control life-threatening diseases. Often Registered Nurses (RNs) administer vaccines but are unsure how to discuss vaccines with vaccine hesitant patients. Missouri’s adolescent vaccination rates are below its neighboring states. This initiative’s purpose was to increase adolescent vaccine administrations at a Missouri County Health Department (MCHD).

This quality improvement initiative had an observational, cross-sectional, descriptive design. RNs employed at three MCHD clinics attended an adolescent vaccine administration training program (VAT) and completed pre-/post-tests regarding the Advisory Council on Immunization Practices recommendations. Medical record reviews from March 1-April 30, 2018 and March 1-April 30, 2019 compared …

Examining Opioid Ordering Rates Of Advanced Practice Providers In An Emergency Department, Bridgette Thompson Jul 2019

Examining Opioid Ordering Rates Of Advanced Practice Providers In An Emergency Department, Bridgette Thompson

Dissertations

Problem Opioid abuse has become a major public health issue in America. Many Americans die daily from an opioid overdose. The Centers for Disease Control and Prevention (CDC) has identified prescription drug abuse and overdose as a major health threat.

Methods Descriptive cohort design with a retrospective record review of the number of opioids ordered by the advanced practice provider six weeks before and six weeks after a didactic and simulation educational experience.

Results The total sample was four (N=4). Using the paired samples t test there was no difference (t = 0.65, p = 0.56) between the number …

The Impact Of Calculators On A Test Of Clinician Numeracy: A Randomized Controlled Trial, Anne A. Taylor, Douglas R. Corfield, Lucie M. Byrne-Davis Jul 2019

The Impact Of Calculators On A Test Of Clinician Numeracy: A Randomized Controlled Trial, Anne A. Taylor, Douglas R. Corfield, Lucie M. Byrne-Davis

Numeracy

Clinician numeracy (CN), the ability to use and understand quantitative data in patient care, is an important skill for healthcare professionals. Nonetheless, it is recognized that many healthcare professionals, including doctors, have deficiencies in CN, and that this may affect patient safety. In our previous research using the Medical Interpretation and Numeracy Test (MINT), we found that many doctors in training in the UK had low CN. However, participants were not permitted to use calculators when taking the MINT, even though staff has access to calculators in clinical practice. Therefore, our original study may have underestimated doctors’ CN, compared to …

Dysphagia Screening To Prevent Hospital-Acquired Pneumonia Rates, Namneet Sekhon Jul 2019

Dysphagia Screening To Prevent Hospital-Acquired Pneumonia Rates, Namneet Sekhon

Dissertations

Problem Pneumonia is a leading cause of death worldwide and affects all ages. Pneumonia may be acquired by aspiration and a bedside nursing dysphagia screen may reduce hospital acquired pneumonia (HAP) rates for stroke patients. The aim of this study was to evaluate use of the dysphagia screen for all patients receiving a computed tomography (CT) scan of the head in the emergency department (ED).

Method An observational, descriptive design for all patients who received a CT of the head in the ED and admitted over a 60-day period. The rate of dysphagia screening with the DePaul Hospital Swallow Screen …

Faculty Entrustment Of Students In The Core Clerkships: A Comparison Between The Longitudinal Integrated Clerkship And The Block Clerkship, Robert L. Trowbridge, Marybeth D. Ford, Jenny L. Carwile, Eliza R. Bullis, Robert G. Bing-You

Journal of Maine Medical Center

INTRODUCTION: Entrustable Professional Activities (EPAs) have been proposed for use in undergraduate medical education. The ability of faculty to entrust students with EPAs may differ between Longitudinal Integrated Clerkships (LICs) and traditional block clerkships.

METHODS: Participants were core clerkship faculty, 64 in a LIC and 31 in a sequential block clerkship. We administered a web-based survey at the end of the core clerkship year to measure preceptors’ typical entrustment (on a scale of 0–10) in students for the 13 American Association of Medical Colleges Core EPAs. We compared entrustment between LIC and block faculty using a Mann-Whitney test.

RESULTS:LIC …

Maine Medical Center Quality And Safety Storyboards: Operational Excellence As Platform For Improvement

Journal of Maine Medical Center

Physician and other healthcare learners and clinicians must prepare to assume an active role in the design, implementation, and improvement of emerging models of health care delivery while concurrently improving quality, workflow efficiency and safety. While these expectations are building, few practicing clinicians have training or experience with these challenges. Maine Medical Center’s Operational Excellence Team builds on a framework of Lean Thinking, the Model for Improvement* and PDSA cycles** to advance improvement capacity among care teams across our organization. Interprofessional Teams are coached to use proven tools and techniques to identify and remove barriers to care and establish sustainable …

Rationale And Design Of An Endocrinology Education Program For Primary Care In Maine: Initiation Of Mainehealth Endo Echo Maine, Irwin Brodsky, Joan Ingram, Stacy O'Brien, Tho Ngo, Charles T. Winchell

Journal of Maine Medical Center

Introduction: Population health and quality of care initiatives for the management of common endocrine disorders often include algorithmic and protocol-driven workflows in primary care practices. Endo ECHO is a division of Project ECHO at University of New Mexico that engages primary care clinicians in didactic presentations and case discussions to augment conceptual (experiential, nuanced) rather than algorithmic (rote) learning.

Research Design and Methods: MaineHealth Endo ECHO was designed using the Project ECHO model. Project ECHO® is an interactive learning opportunity that uses video conferencing to connect specialty and primary care to share best practices for delivering care. Using …
