Introduction: Orthodontic tooth movement requires the precise coordination of a multitude of biological factors in order for proper bone remodeling to occur. Cytokines, such as Interleukin-6 (IL-6), play a critical role in the communication necessary for both bone resorption and bone apposition. IL-6 reacts with receptor proteins, such as soluble IL-6 receptor (sIL-6R), on target cells in order to transmit signals important to both osteoclast and osteoblast activities. Retention is required in the majority of orthodontic patients throughout the remainder of their life. The two primary removable appliances are known as the traditional Hawley retainer or the vacuum formed retainer. These appliances were developed to maintain the position of the dentition without sacrificing oral health. The orthodontic population is at a higher risk for caries due to plaque accumulation from poor diet, suboptimal oral hygiene and often lack of motivation. Fixed orthodontic appliances (braces) are one of the most widely used appliances to correct malocclusion, however many patients prefer less metal display. To meet this demand more esthetically pleasing brackets and archwires have been developed. These esthetic components are often clear, white or tooth colored. Coatings of various types are commonly used to produce an esthetic archwire. However, these coating materials tend to wear and degrade, which may cause binding and increased resistance to friction, thereby potentially reducing the efficiency of tooth movement. Introduction.
The purpose of this study was to compare perceptions of efficacy and efficiency of Self-Ligating Brackets vs. conventional brackets between practicing Orthodontists and Orthodontic Residents.
Methods.
Cross-sectional survey research using a three-wave emailing of addresses from the current American Association of Orthodontists directory. The directors of all orthodontic residency programs in the US were asked to also distribute to all residents. The survey included five sections: (1) respondents' experience, (2) factors that influence, (3) positive or negative factors, (4) the perceived efficiency/efficacy, and (5) demographics. A 39% response rate (N=707) was obtained.

Instability of the occlusion is a common problem following orthodontic treatment. The purpose of the present research was to evaluate posttreatment relapse at long-term recall (> 10 years) in cases (A) treated with temporary fixed mandibular retention plus Hawley retainers compared to (B) a similar group retained with just removable retention (standard Hawley type retainers alone). Fixed retainers were removed after having been in place for about 2.5 years. Following this period, use of removable retention was left up to the discretion of the patient in both groups. Objective: To determine if American Orthodontics' (AO) new, experimental ligature demonstrates less friction in vitro when compared to four other ligatures on the market.
Methods: Four brackets were mounted on a custom metal fixture allowing an 0.018-in stainless steel wire attached to an opposite fixture with one bracket to be passively centered in the bracket slot. The wire was ligated to the bracket using one of five types of ligatures including the low friction test ligatures (AO), conventional ligatures (AO), Sili–TiesTM, Silicone Infused Ties (GAC), Synergy® Low-Friction Ligatures (RMO), and SuperSlick ligatures (TP Orthodontics). Introduction: The first-order toe-in angle of the maxillary first molar is an important aspect of orthodontics and occlusion. Many variables contribute to the amount of distal offset expressed such as bracket slot dimensions, the amount of toe in angle built into the prescription of the bracket, bracket placement, bracket deformation, and tooth morphology. The aim of this study was to evaluate how Asians, non-Asians, and orthodontists in America view esthetic soft tissue Asian lip profiles. The null hypothesis was that there was no difference in the perception of lip profile esthetics of Asian patients between Asian laypersons, non-Asians laypersons, and orthodontists. A survey was constructed using the profile photographs of one adult male and one adult female Asian American patient taken from the VCU Orthodontics Clinic records. OBJECTIVE: To establish cephalometric norms of a sample of aesthetically pleasing Pakistani faces and to analyze differences and similarities with accepted standards for Caucasians.
STUDY DESIGN: Analytical cross sectional study.
PLACE AND DURATION OF STUDY: Carried out at Alvi Dental Hospital, Karachi, from August 2007 to February 2008.

Early or two-phase orthodontic treatment of Class II malocclusions is a debated topic in orthodontic circles primarily because the benefits of early intervention have not been consistently reproduced among researchers. The present study was a retrospective analysis of cephalograms from patients with Class II, division 1 malocclusions at the start of treatment. These were 50 consecutively treated youths who received phase 1 (early) treatment with a Bionator appliance and later treatment with full appliances (all treated by a single clinician). The purpose of this study was to compare the shear bond strength of orthodontic brackets to enamel in four bonding protocols: SEP without prior pumicing (None), Pre-etch and SEP without prior pumicing (Pre-etch), control: SEP with prior pumicing (Pumice), and Pre-etch and SEP with prior pumicing (Both). 80 extracted bovine incisors were randomly divided into 4 groups of 20, and brackets were bonded under the different experimental conditions. Debonding force was measured with an Instron universal testing machine. A two-way ANOVA comparing the four groups indicated that there was a significant difference in debonding force (P = 0.001). Medicaid orthodontic patients have been shown to miss more appointments and break more appliances than self-pay patients, indicating a greater tendency toward non-compliance. Lack of compliance during the post-treatment phase can be detrimental to retention. The purpose of this study was to determine whether there is a difference in PAR score changes between Medicaid and non-Medicaid patients during and after orthodontic treatment. “Early” or two-phase orthodontic treatment of Class II malocclusions is a highly debated topic in the orthodontic literature. We report here on a retrospective cephalometric study of patients with Class II, division 1 malocclusions. One group consisted of 32 consecutively treated patients who received “early” treatment with a Fränkel II appliance followed by treatment with full fixed appliance. These subjects were “matched” by sex and cephalometric value, to a subject treated with standard edgewise appliances in a single phase. Malocclusions can involve arch-size tooth-size discrepancies that have to be resolved by premolar extractions. Treatment planning in such cases usually involves removing first (P1) or second premolars (P2). Choosing an extraction pattern is a learned skill that incorporates many diagnostic factors. The goal of this study was to determine if a relationship exists between pretreatment facial attractiveness and posttreatment occlusal outcome in orthodontic patients. A randomized sample of forty-seven patients (26 males, 21 females) was selected for inclusion in this study. Orthodontic evaluators rated pretreatment patient photographs (frontal smile, frontal, profile) using a 100 mm visual analog scale (VAS). Occlusal outomes were evaluated using percentage reduction in weighted Peer Assessment Rating (PAR) index scores and the American Board of Orthodontics objective grading system (ABO-OGS). The aim was to evaluate whether a whitening system, when used during the final stages of orthodontics, would yield results comparable to whitening alone. Patients were assigned to either the control (n=20) or experimental (n=26) groups. At T1, patients were given a ten day supply of Trèswhite⢠by Opalescence®, instructions on use, and initial shade determination was made and photographs were taken. At T2 and T3, shade determination was accomplished and updated photographs were taken. Whitening of the teeth occurred in both groups on average, but significantly more whitening was experienced in the experimental group (p < 0.004). An average of 87% of teeth whitened during orthodontic treatment compared to 97% of control teeth (p < 0.01) were judged to be uniform in appearance. In conclusion, the data do not contraindicate the concurrent accomplishment of teeth whitening during orthodontic treatment.

The effect on shear bond strength of bracket movement after seating the brackets and before light curing has not been reported. The purpose of this study was to determine the effect of linear and rotational pre-cure bracket movement on the shear bond strength of orthodontic brackets. 100 extracted human maxillary premolars were divided into 5 groups of 20 teeth each. The control group was bonded with no pre-cure bracket movement, and test groups were bonded with pre-cure bracket movement of 2 mm, 4 mm, 45¢ª or 180¢ª. The purpose of this study was to help predict the enamel thickness of mandibular incisors. At least two direct digital periapical radiographs were made for each of the 80 subjects. Radiographs were scaled to control for magnification errors using dental study models and computer software. Mesiodistal incisor width and mesial and distal enamel thicknesses were measured. Lateral incisors were determined to be wider than central incisors and distal enamel thicknesses were larger than mesial enamel thicknesses on average. The African American group demonstrated wider incisors and enamel thicknesses than the Caucasian group on average. Enamel thickness positively correlated with tooth …

Orthodontic programs spend considerable amounts of time and energy to attract, recruit, and interview the best and brightest applicants. Applicants and programs submit ranked preferences, and resident positions are filled by a computerized matching system (Match). The specific aims of this study were to determine the relative importance of certain factors in applicants' Match ranking of orthodontic programs and to determine differences between orthodontic Program Directors perceptions and actual factors cited by applicants influencing their ranking of orthodontic programs.Surveys were mailed to 55 orthodontic Program Directors and 478 applicants participating in the 2002 orthodontic Match. State Medicaid programs were established to care for the poor by eliminating financial barriers and increasing their ability to be treated within the mainstream of the heath care system. The number of children eligible for Medicaid services is increasing, yet the number of Medicaid providers remains low. There is a shortage of faculty in academic dentistry and orthodontics in particular. The purpose of this study was to quantify real and perceived differences in income, workload, and satisfaction between full-time academic and private practice orthodontists. Surveys were returned by 119 (70%) faculty, 79 (36%) private practitioners, and 160 (59%) orthodontic residents. Average annual income for faculty was less than half that of private practitioners. Faculty also reported working significantly more hours per week and taking less vacation time. Though both faculty and practitioners perceived their own jobs to be more satisfying overall, faculty felt academics was more stressful, …
