Chlorhexidine mouth rinses remove dental plaque from teeth, preventing dental caries, dental decay, and more serious systemic infections. Tooth discoloration due to extrinsic staining is the most prominent side effect of regular chlorhexidine use. Decreasing the concentration of chlorhexidine reduces staining area and severity. Staining can also be prevented and treated by tooth bleaching, a common cosmetic dental procedure that diminishes extrinsic staining by oxidizing chromagens adsorbed onto the tooth surface. In the proposed trial, common bleaching agents will be investigated for their efficacies in preventing chlorhexidine staining. Background: Lower incisor proclination has been shown to increase during Class II correction with appliances such as the Forsus. This lower incisor flaring shows great variability among patients. The purpose of this study was to evaluate the effects of Class II severity, mandibular growth, and occlusal plane rotation on lower incisor flaring in patients treated with the Forsus appliance. Materials and Methods: 121 records of Class II patients treated with the Forsus appliance were analyzed retrospectively. Cephalograms were traced at three time points. The Reporting of Supplement Use by Dental Patients on Their Medical History Questionnaire Objectives: The goals of the study were three fold. Estimate the prevalence of supplement use by dental patients. Determine if the design of a medical history form influences the prevalence of supplement use reported. Objective: The purpose of this study was to determine the fluoride recharging capability of Opal Seal, a fluoride releasing orthodontic primer, as compared to Transbond XT, the control. Material and Methods: 1mm x 5mm disks of Opal Seal and Transbond were prepared according to the respective manufacturer’s instructions. Initially, the samples were stored in deionized water (DI) for 8 weeks. The samples were then randomly divided into one of two groups: Over-the-counter (OTC) fluoride mouthwash and prescription strength (PS) fluoride mouthwash. ผู้ป่วยโรคตับแข็งควรได้รับการดูแลรักษาทางทันตกรรมอย่างระมัดระวัง เนื่องจากผู้ป่วยมีการเปลี่ยนแปลง ทางพยาธิสรีรวิทยา และมีภาวะแทรกซ้อนจํานวนมาก ซึ่งเป็นผลมาจากการทํางานของตับที่เสื่อมลง ทันตแพทย์จึงจําเป็นต้องมีความรู้ความเข้าใจเกี่ยวกับโรคตับแข็งเพื่อใช้ในการประเมินสภาวะร่างกายของผู้ป่วยโรคตับแข็งแต่ละรายก่อนการรักษาทางทันตกรรม เพื่อลดความเสี่ยงในการเกิดภาวะแทรกซ้อนจากการรักษา บทความนี้ได้ รวบรวมข้อมูลเกี่ยวกับโรคตับแข็งในประเด็นที่สําคัญ ได้แก่ สาเหตุ อาการและอาการแสดง อาการแสดงในช่องปาก ภาวะแทรกซ้อน การประเมินความรุนแรงของโรค ตลอดจนข้อควรระวังและข้อพิจารณาในการรักษาทาง ทันตกรรมในผู้ป่วยโรคตับแข็ง ได้แก่ การประเมินผู้ป่วยก่อนการรักษา การใช้ยาทางทันตกรรม ภาวะเลือดออก ผิดปกติและความเสี่ยงในการเกิดภาวะติดเชื้อจากการรักษาทางทันตกรรม เพื่อช่วยให้ทันตแพทย์สามารถปรับ เปลี่ยนแผนการรักษาทางทันตกรรมให้เหมาะสมกับผู้ป่วยโรคตับแข็งแต่ละรายได้ (ว ทันต จุฬาฯ 2557;37:241-50)

วัตถุประสงค์ เพื่อศึกษาผลของเครื่องฉายแสง 3 ชนิด ที่มีความเข้มแสงต่างกันต่อความแข็งแรงของพันธะเฉือนในช่วงแรกของวัสดุยึดติดทางทันตกรรมจัดฟันชนิดหนึ่ง วัสดุและวิธีการ เตรียมฟันกรามน้อยบนจํานวน 60 ปี แบ่งเป็น 4 กลุ่ม ๆ ละ 15 ซี เตรียมผิวฟันด้วยผงพัมมิซ และกรดฟอสฟอริก 30 วินาที ล้างน้ําและเป่าให้แห้งนําแบร็กเกตติดลงบนผิวฟันด้วยวัสดุยึดติดชนิดบ่มด้วยแสง โดย กลุ่ม 1 เครื่องฉายแสงฮาโลเจน (เดเมตรอน แอลซี) 410 มิลลิวัตต์/ตารางเซนติเมตร 20 วินาที (กลุ่มควบคุม) กลุ่มที่ 2 เครื่องฉายแสงแอลอีดี (เดเมตรอน เอทู) 1,140 มิลลิวัตต์/ตารางเซนติเมตร 20 วินาที กลุ่มที่ 3 เครื่อง ฉายแสงแอลอีดีชนิดเดียวกับกลุ่มที่ 2 1,140 มิลลิวัตต์/ตารางเซนติเมตร 6 วินาที และกลุ่มที่ 4 เครื่องฉายแสง แอลอีดี (วาโล) 2,230 มิลลิวัตต์/ตารางเซนติเมตร 6 วินาที นําตัวอย่างทั้งหมดไปวัดค่าความแข็งแรงของพันธะ เฉือนหลังการฉายแสง 4 นาที ด้วยเครื่องทดสอบสากลเปรียบเทียบค่าเฉลี่ยของความแข็งแรงพันธะเฉือนใน แต่ละกลุ่ม โดยใช้การวิเคราะห์ทางสถิติค่าความแปรปรวนแบบทางเดียววัดค่าดัชนีการเหลืออยู่ของวัสดุติดยึดและ ทดสอบด้วยค่าทางสถิติไคสแควร์ที่ระดับความเชื่อมั่นร้อยละ 95 ผลการศึกษา ค่าเฉลี่ยและค่าเบี่ยงเบนมาตราฐานของความแข็งแรงของพันธะเฉือนกลุ่มที่ 1 2 3 และ 4 เท่ากับ 4.76 ± 0.99 5.35 ± 1.47 3.98 + 1.07 และ 5.69 + 1.11 เมกะปาสคาลตามลําาดับ โดยค่าเฉลี่ย ความแข็งแรงของ พันธะเฉือน กลุ่มที่ 2 และ 4 มีค่ามากกว่ากลุ่มที่ 3 อย่างมีนัยสําคัญทางสถิติ …

ความชอบของทันตแพทย์ไทยต่อสัดส่วนความกว้างและอัตราส่วนความกว้างต่อความสูงของฟันหน้าบน, ชยพร ศุภชาติวงศ์, เฉลิมพล ลี้ไวโรจน์

วัตถุประสงค์ เพื่อจัดหาข้อมูลมาตรฐานของสัดส่วนและอัตราส่วนของฟันหน้าที่ทันตแพทย์ไทยสามารถนํามาใช้ในการออกแบบฟันหน้า วัสดุและวิธีการ รูปรอยยิ้มซึ่งถูกนํามาตกแต่งด้วยโปรแกรมตกแต่งภาพโดยปรับภาพอัตราส่วนความกว้างต่อ ความยาวของฟันหน้า ให้มีลักษณะ 3 รูปแบบ ได้แก่ ฟันปกติ (ร้อยละ 78) ฟันยาว (ร้อยละ 71) และฟันสั้น (ร้อยละ 87) ซึ่งในแต่ละอัตราส่วนความกว้างต่อความยาวนั้น สัดส่วนฟันหน้าต่อฟันข้างต่อฟันเขี้ยวจะถูกปรับแต่ง เป็น 3 ลักษณะ ได้แก่ สัดส่วนทองคํา สัดส่วน 70 อาร์อีดี และสัดส่วนเพรสตัน รูปรอยยิ้มที่แตกต่างกันถูกนํามา ทําเป็นแบบสํารวจเป็นคู่ 18 คู่และจัดให้ทันตแพทย์ไทย 242 คน ทําแบบสอบถามเลือกรูปที่ชอบระหว่างคู่นั้น ๆนําผลการสารวจที่ได้มาวิเคราะห์ผลการทดสอบแบบทวินามและการทดสอบฟิชเชอร์ ผลการศึกษา ไม่พบความแตกต่างของความชอบของทันตแพทย์ไทยต่อสัดส่วนฟันหน้าบนทั้งสามลักษณะอย่างมีนัยสําคัญในกลุ่มอัตราส่วนความกว้างต่อความยาวฟันแบบปกติและแบบยาว เมื่อเปรียบเทียบในกลุ่มสัดส่วนฟันหน้าบนลักษณะเดียวกัน พบว่าฟันที่มีอัตราส่วนความกว้างต่อความยาวแบบฟันปกติได้รับความชอบสูงที่สุด และเมื่อนําปัจจัยอื่นได้แก่ สาขาวิชาที่ศึกษาต่อ เพศของทันตแพทย์ และอายุการทํางาน มาวิเคราะห์ร่วมพบว่า มีผลต่อความชอบลักษณะอัตราส่วนและสัดส่วนของฟันในบางคู่ สรุป ในกลุ่มทดลองทันตแพทย์ไทย อัตราส่วนความกว้างต่อความยาวฟันหน้า (โดยเฉพาะในกลุ่มฟันสั้น) มี อิทธิพลต่อความชอบในรอยยิ้มมากกว่าสัดส่วนฟันหน้าต่อฟันข้างต่อฟันเขี้ยว นอกจากนี้สาขาวิชาที่ศึกษาต่อ เพศ และอายุการทํางาน ก็อาจมีผลต่อความชอบในรอยยิ้มเช่นกัน (ว ทันต จุฬาฯ 2557;37:183-96)

Objective To evaluate and compare the effects of two desensitizing toothpastes and a regular fluoride toothpaste on microtensile bond strength of two adhesive agents to dentin. Materials and methods The labial surfaces of forty bovine incisor crowns were ground flat, exposing dentin. The teeth were then randomly divided into four groups corresponding to the toothpaste used: 1) Sensodyne® Rapid Relief (GlaxoSmithKline, UK), 2) Colgate Sensitive Pro-ReliefTM (Colgate- Palmolive, Thailand), 3) Colgate® Regular Flavor (Colgate-Palmolive, Thailand), and 4) immersed in artificial saliva (control). วัตถุประสงค์ เพื่อเปรียบเทียบการตอบสนองของเด็กขณะฉีดยาชาเฉพาะที่ระหว่างการเห็นและไม่เห็นเข็ม โดยประเมินจากระดับพฤติกรรมและการเปลี่ยนแปลงอัตราการเต้นของหัวใจ วัสดุและวิธีการ เด็กก่อนวัยเรียน 60 คน แบ่งออกเป็นสองกลุ่มเท่ากัน กลุ่มไม่เห็นเข็มและกลุ่มเห็นเข็มขณะฉีดยา ประเมินระดับพฤติกรรมจากวิดีทัศน์โดยใช้เกณฑ์การประเมินที่ปรับปรุงจากเกณฑ์ของแฟรงค์และเปรียบเทียบการเปลี่ยนแปลงอัตราการเต้นของหัวใจของแต่ละบุคคล วิเคราะห์ความแตกต่างของระดับพฤติกรรมระหว่างกลุ่มด้วยสถิติของฟิชเชอร์ เปรียบเทียบการเปลี่ยนแปลงอัตราการเต้นของหัวใจระหว่างกลุ่มด้วยสถิติแมนวิทนีย์ ข้อมูลพื้น ฐานของกลุ่มตัวอย่างจะถูกวิเคราะห์ด้วยสถิติไคสแควร์หรือสถิติฟิชเชอร์ ที่ระดับความเชื่อมั่นร้อยละ 95 ผลการศึกษา ระดับพฤติกรรมของกลุ่มไม่เห็นเข็มและกลุ่มเห็นเข็มขณะฉีดยาชาไม่แตกต่างกัน (ฟิชเชอร์ p = .000) และการเปลี่ยนแปลงอัตราการเต้นของหัวใจไม่แตกต่างกัน ระหว่างกลุ่มที่ไม่เห็นเข็มและเห็นเข็มเช่นกัน (แมนวิทนีย์p = 0.609) สรุป ในเด็กอายุ 4-5 ปี สามารถให้เด็กเห็นหรือไม่เห็นเข็มขณะฉีดยาชาได้ทั้งสองวิธี โดยต้องมีการเตรียมตัวเด็ก ที่ดีและการจัดการพฤติกรรมที่เหมาะสมเป็นปัจจัยสําคัญเพื่อให้ประสบความสําเร็จในการฉีดยาชา (ว ทันต จุฬาฯ 2557;37:137-48)

Objective To compare the chewing ability of mandibular implant-retained overdenture in Thai elderly patients using the subjective and objective assessments. Materials and methods Thirty-three participants were recruited. Their chewing ability were evaluated twice: 1 month after implant placement and 3 months after the insertion of a mandibular implantretained overdenture. The subjective chewing ability was evaluated using a developed questionnaire consisting of 14 common food types. For the objective chewing ability, it was assessed by a wax cube analysis method. Intrusion Phenomenon And Rebound Of A Natural Tooth Located Between Two Implant Supported Crowns: Case Report, Ketkarn Sakultap, Pravej Serichetaphongse

This case report describes the intrusion of a natural tooth located between two implantsupported crowns after 7 months in function. The intruded first molar was 2.5-3 mm infraoccluded and tilted lingually. After 4 months of serial proximal contact adjustment, the intruded tooth was gradually rebounded to its normal position in complete occlusion with the opposing tooth. A new crown was fabricated for the molar to obtain the optimal proximal contact with the adjacent crowns. At the six-month follow up the tooth was still in its normal position. Objective To present an alternative treatment method using a minimally invasive approach employing ceramic veneers to mask staining of moderately tetracycline-stained teeth. Materials and methods A 39-year-old Thai female patient had a moderate type tetracycline-staining of both upper and lower anterior teeth, and 11 and 21 were fractured. A resin composite veneer had been placed on 14. The patient presented with a high smile line and an unsymmetrical gingival balance. Her oral hygiene was good and the gingival tissue was healthy. The treatment plan to create a harmonious gingival balance for 11, 12, and 13 was an esthetic crown lengthening. …

Objective This study aimed to evaluate the fracture resistance of enamel-boned ceramic with variations in cement and ceramic thickness. Materials and methods Leucite-reinforced and lithium-disilicate porcelain laminates (0.5 and 1 mm thick) were fabricated and cemented to human enamel using bonding agent and resin cement with the thicknesses of 30 and 100 μm. Non-cemented porcelain laminate was used as control. Fracture load (Newton) was obtained by pressing a 2 mm-diameter indenter rod against ceramic until the laminates fractured. Artifacts due to high-density objects (HDO) such as metallic dental restorations on maxillofacial cone beam computed tomographic (CBCT) images can render certain areas unsuitable for diagnosis. It was hypothesized that image quality due to HDO artifacts was affected by CBCT acquisition parameters and the number and configuration of HDO. Simulated complete maxillary and mandibular dental arches were constructed using dental stone and extracted teeth. Conservative coronal dental amalgam (MOD) restorations on premolar and molar teeth were used as HDOs. Background: Mutations in the APC gene cause familial adenomatous polyposis (FAP), an autosomal dominant colorectal cancer predisposition associated with the development of hundreds to thousands of adenomatous colorectal polyps beginning in childhood or adolescence. Both malignant and non-malignant extracolonic manifestations are associated with APC gene mutations, including approximately 17% of individuals with various dental anomalies. The availability of dental anomaly information in the medical record remains to be evaluated.

The proposed dually licensed advanced dental therapy program, a graduate level curriculum created by the American Dental Hygienists’ Association (ADHA) encompassing both dental hygiene and basic restorative procedures, creates an innovative career path in dental hygiene and increases the standing of the dental hygienist from an auxiliary role to an independent midlevel dental practitioner. Data were gathered via an online anonymous survey tool from Massachusetts registered dental hygienists to assess support of this proposed curriculum by practicing hygienists. Dental hygiene students may struggle in dental hygiene curriculum in regards to hand function. Currently, this is not an aspect dental hygiene programs screen for or have protocol in place to help students. The research in the study examined if hand function could improve with hand function exercises and if exercises improved instrumentation scores. During a 6-week pilot study, an occupational therapist tested the hand function of a cohort of dental hygiene students. Head and neck cancer (HNC), also known as oral cavity and pharyngeal cancer, comprises a group of cancers that arise in the head or neck region, such as the nasal cavity, sinuses, lips, mouth, salivary glands, and pharynx. Use of tobacco, alcohol, or a combination of the two, are major risk factors for head and neck cancer. Approximately 75% of oral cancers are attributable to smoking and/or alcohol consumption. A gingival crevice model (epithelial cell-Porphyromonas gingivalis-neutrophil) was established and used to profile gingipain, matrix metalloproteinase (MMP), MMP mediators [neutrophil gelatinase-associated lipocalin (NGAL) and tissue inhibitor of metalloproteinases 1 (TIMP-1)] and cytokine networks. Smoking is the primary environmental risk factor for periodontitis. Therefore, the influence of cigarette smoke extract (CSE) was also monitored in the same model. Porphyromonas gingivalis alone induced low levels of interleukin-1b and interleukin-8 from epithelial cells, but high levels of both cytokines were produced on the addition of neutrophils. Introduction: This study compared the clinical efficacy of panoramic imaging and Cone Beam Computed Tomography (CBCT) in the optimal placement of virtual dental implants in the posterior edentulous bounded regions of the jaws. Material and methods: From a retrospective audit of patient records, fifty-two subjects were recruited with sixty-one dental implant sites in the maxilla and mandible. Physical measurements of the residual alveolar ridge were performed and consensus decisions of optimal implant length and bone graft necessity were obtained using reformatted panoramic alone (RP) or cross-sectional imaging (XS). Purpose: The purpose of this report was to review an emerging alternative treatment to pulpectomies and extractions for nonvital primary teeth called lesion sterilization and tissue repair (LSTR) and provide the results of three clinical case applications. Purpose: The purpose was to assess and understand operating room (OR) utilization and efficiency in a pediatric dental residency program. Methods: A retrospective study was performed using chart extraction from 778 patients completed by both pediatric dentistry faculty (n=7) and residents (n=17) in an ambulatory care setting over a 32 month period (between July 2010 and March 2013). The current understanding of the distribution and frequency of periapical pathoses include biopsies submitted by all specialists and general dentists. As a result, they do not accurately reflect the distribution seen by endodontists. This retrospective chart review aims to determine the prevalence of periapical pathoses and associated demographics from biopsies submitted by endodontists over 30 years. All biopsy reports submitted to the Virginia Commonwealth University Oral Pathology Diagnostic Service from January 1, 1983 to December 31, 2012 were reviewed.
