บทบาทของยีนพีพีเออาร์-แกมมาในการควบคุมการเกิดเซลล์สลายกระดูก, วรรณธิดา ศรีอาจ

เซลล์สลายกระดูกเป็นเซลล์ที่ทําหน้าที่สําคัญในการปรับรูปของกระดูก เซลล์เหล่านี้มีกําเนิดจากเซลล์ สร้างเม็ดเลือดในกลุ่มโมโนไซต์-แมคโครแฟจในไขกระดูก โดยปฏิสัมพันธ์ระหว่างแรงค์-แรงค์ไลแกน จะเป็นกลไก สําคัญในการกระตุ้นพัฒนาการของเซลล์สลายกระดูก อย่างไรก็ตามจากรายงานการศึกษาได้นําเสนอหน้าที่ใหม่ ของยีนเพอร์รอกซิโซม โพรลิเฟอเรเตอร์ แอคทิเวเท็ด รีเซ็นเตอร์ แกมมา หรือพีพีเออาร์-แกมมาในการควบคุม การเกิดเซลล์สลายกระดูก บทความฉบับนี้จึงมีจุดประสงค์ในการนําเสนอและอภิปรายบทบาทของยีนพีพีเออาร์- แกมมาในการเกิดเซลล์สลายกระดูก โดยทั่วไปหน้าที่ของยีนพีพีเออาร์-แกมมานั้น จะเกี่ยวข้องกับเมแทบอลิซึม ของน้ําตาลและไขมัน รวมทั้งทําหน้าที่ในการเหนี่ยวนําการเกิดเซลล์ไขมัน และยับยั้งการแปรสภาพของเซลล์สร้าง กระดูก แต่หลักฐานจากงานวิจัยเมื่อเร็ว ๆ นี้ แสดงให้เห็นว่า พีพีเออาร์-แกมมา ยังทําหน้าที่ร่วมกับปฏิสัมพันธ์ ระหว่างแรงค์-แรงค์ไลแกนในการควบคุมเกิดเซลล์สลายกระดูกด้วยหน้าที่ใหม่ของของพีพีเออาร์-แกมมานี้ นอกจากจะเพิ่มความชัดเจนในกระบวนการสร้างเซลล์สลายกระดูกแล้ว ของกลูโคสกับการเกิดเซลล์สลายกระดูกด้วยยังแสดงถึงความเกี่ยวเนื่องระหว่างเมแทบอลิซึม (ว ทันต จุฬาฯ 2556;36:207-20)

ภาวะสะสมแร่ธาตุน้อยเกินในฟันกรามและฟันตัดเป็นความผิดปกติของเคลือบฟันแท้ที่เกิดจาก พัฒนาการของฟันที่ผิดปกติ เคลือบฟันมีการสะสมแร่ธาตุน้อยกว่าเคลือบฟันปกติบริเวณรอบๆ พบในชุดฟันแท้ บริเวณฟันกรามแท้ซี่แรกจํานวน 1 ซี่ หรือมากกว่า และมักพบร่วมกับฟันตัดบน ปัจจุบันยังไม่ทราบสาเหตุการเกิด โรคที่แน่ชัด แต่มีแนวโน้มที่จะพบมากขึ้นในเด็กทั้งในประเทศไทยและทั่วโลก เคลือบฟันที่มีภาวะดังกล่าวมักกร่อน แตกหักง่าย ทําให้เกิดฟันผุและรอยผุลุกลามอย่างรวดเร็ว การวินิจฉัยรอยโรคตั้งแต่ระยะแรกทันทีที่ฟันขึ้นใน ช่องปากเป็นสิ่งสําคัญ เพื่อให้ผู้ป่วยได้รับการรักษาทางทันตกรรมป้องกันและการบูรณะฟันที่มีรอยโรค เป็นการ ป้องกันการสูญเสียฟันกรามแท้ซี่แรก แต่ในรอยโรคที่รุนแรงมากจนไม่สามารถบูรณะได้ หรือมีการพยากรณ์ผล การรักษาในระยะยาวที่ไม่ดี การถอนฟันกรามแท้ซี่แรกในช่วงเวลาที่เหมาะสมอาจเป็นอีกทางเลือกหนึ่งในการรักษา รายงานฉบับนี้เสนอการรักษาในผู้ป่วย 3 ราย ที่มีภาวะดังกล่าวระดับรุนแรงมากที่มาพบทันตแพทย์ในช่วงอายุที่ แตกต่างกัน โดยติดตามผลการรักษาเป็นเวลา 3 ปี รายงานนี้อาจช่วยเป็นแนวทางในการจัดการทางทันตกรรมในผู้ป่วยที่มีภาวะสะสมแร่ธาตุน้อยเกินในฟันกรามและฟันตัดระดับรุนแรงต่อไป (ว ทันต จุฬาฯ 2556;36:189-206)

วัตถุประสงค์ เพื่อประเมินค่ากําลังแรงยึดแบบผลักของเดือยฟันเส้นใยควอตซ์ที่ผ่านการปรับสภาพพื้นผิวเดือย ฟินด้วยสารเคมี วัสดุและวิธีการ นําฟันกรามน้อยล่างแท้ซี่ที่หนึ่งของมนุษย์ที่ถอนออกมาจํานวน 45 ซี่ มาตัดส่วนตัวฟันออก โดยติดตั้งฉากกับแนวแกนฟันที่บริเวณรอยต่อของเคลือบฟันกับเคลือบรากฟัน เตรียมคลองรากฟันและเตรียม ช่องว่างสําหรับใส่เดือยฟันเส้นใยควอตซ์ดีทีไลท์โพสต์เบอร์ 1 แบ่งเดือยฟันออกเป็น 9 กลุ่ม กลุ่มที่ 1 กลุ่มควบคุม ไม่ได้ปรับสภาพพื้นผิวเดือยฟันด้วยสารเคมี กลุ่มที่ 2 ถึง กลุ่มที่ 7 แช่เดือยฟันในไฮโดรเจนเปอร์ออกไซด์ที่ความ เข้มข้น 3 แบบ (ความเข้มข้นร้อยละ 24 30 และ 35) และระยะเวลาการแช่ 2 แบบ (นาน 5 นาที และ 10 นาที) กลุ่มที่ 8 แช่เดือยฟันในโซเดียมไฮโปคลอไรต์ที่ความเข้มข้นร้อยละ 10 นาน 10 นาที และกลุ่มที่ 9 แช่เดือยฟันใน กรดไฮโดรฟลูออริกที่ความเข้มข้นร้อยละ 5 นาน 5 วินาที แล้วทาพื้นผิวเดือยฟันทุกชิ้นด้วยสารคู่ควบไซเลน จากนั้นยึดเดือยฟันกับคลองรากฟันที่เตรียมไว้ด้วยสารยึดติดเอ็กไซท์ ดีเอสซี ร่วมกับเรซินคอมโพสิตเหลวมัลติ- คอร์โฟลว์ นารากฟันแต่ละรากที่เตรียมไว้ติดบริเวณที่ใส่เดือยฟันรากละ 6 ชิ้น โดยแต่ละชิ้นมีความหนา 1 มิลลิเมตร เพื่อเป็นตัวแทนของรากฟันส่วนใกล้ตัวฟัน ส่วนกลาง และส่วนใกล้ปลายรากฟัน แล้วนํามาทดสอบค่ากําลังแรงยึด แบบผลักด้วยเครื่องทดสอบเอนกประสงค์ชนิดอินสตรอน วิเคราะห์ผลด้วยสถิติความแปรปรวนแบบสองทาง และ วิเคราะห์ความแตกต่างของค่าเฉลี่ยแบบจับคู่พหุคูณ แบบทรีย์ที่ระดับความเชื่อมั่นร้อยละ 95 ผลการศึกษา การปรับสภาพพื้นผิวเดือยฟันด้วยไฮโดรเจนเปอร์ออกไซด์ที่ความเข้มข้นร้อยละ 30 นาน 10 นาที กรดไฮโดรฟลูออริกที่ความเข้มข้นร้อยละ 5 นาน 5 วินาที โซเดียมไฮโปคลอไรต์ที่ความเข้มข้นร้อยละ 10 นาน 10 นาทีและไฮโดรเจนเปอร์ออกไซด์ที่ความเข้มข้นร้อยละ 35 นาน 5 นาที ช่วยเพิ่มค่ากําลังแรงยึดแบบผลักแตกต่างจาก กลุ่มควบคุมอย่างมีนัยสําคัญทางสถิติ …

ปัจจุบันนี้ วัสดุชนิดบ่มตัวด้วยแสงหลายชนิดได้รับการพัฒนาอย่างกว้างขวาง เช่น วัสดุบูรณะ สารยึดติด และเรซินซีเมนต์ เครื่องฉายแสงจึงเป็นเครื่องมือที่มีความสําคัญในงานทันตกรรม ทําให้มีการพัฒนาเครื่องฉาย แสงหลายชนิดซึ่งมีความแตกต่างกันทั้งความยาวคลื่นและความเข้มแสง ความเข้มแสงที่เหมาะสมเป็นปัจจัยพื้น ฐานของการเกิดพอลิเมอร์ของวัสดุ ทําให้วัสดุมีคุณสมบัติทางกายภาพที่เหมาะสม และใช้ประเมินประสิทธิภาพ ของเครื่องฉายแสงด้วย บทความปริทัศน์นี้กล่าวถึงความเข้มแสง เครื่องฉายแสงทางทันตกรรมชนิดต่าง ๆ รวมถึง ปัจจัยที่มีผลต่อความเข้มแสงของเครื่องฉายแสงทางทันตกรรม (ว ทันต จุฬาฯ 2556;36:221-32)

วัตถุประสงค์ เพื่อศึกษาความสัมพันธ์ของอายุฟันของฟันเขี้ยวแท้ ฟันกรามน้อยซี่ที่หนึ่งและสอง ฟันกรามแท้ซี่ ที่สองด้านล่างขวากับอายุกระดูกที่ประเมินจากกระดูกคอ และกําหนดซี่ฟันที่มีอายุฟันสัมพันธ์กับอายุกระดูกมากที่สุดในเพศชายและเพศหญิง วัสดุและวิธีการ เลือกกลุ่มตัวอย่างจากผู้ป่วยของภาควิชาทันตกรรมจัดฟัน คณะทันตแพทยศาสตร์ จุฬาลงกรณ์ มหาวิทยาลัย จํานวน 320 คน แบ่งเป็น 8 กลุ่ม กลุ่มละ 40 คน (ชาย 20 คน หญิง 20 คน) มีอายุเฉลี่ยและ ค่าเบี่ยงเบนมาตรฐานของอายุปฏิทินในแต่ละกลุ่มของเพศชาย ได้แก่ 9.08 + 0.27 9.94 + 0.35 11.04 + 0.26 11.94 + 0.33 13.10 + 0.37 13.99 + 0.28 14.97 + 0.39 และ 16.02 + 0.36 ปี ตามลําดับ และแต่ละกลุ่มในเพศ หญิง ได้แก่ 9.21 + 0.26 9.95 + 0.25 17.05 + 0.36 12.01 + 0.27 12.94 + 0.33 13.83 + 0.37 14.99 + 0.42 และ 15.99 + 0.26 ปี ตามลําดับ ทําการอ่านและบันทึกค่าอายุฟันด้วยการพิจารณาลักษณะของหน่อฟันแท้ของฟันเขี้ยว ฟันกรามน้อยซี่ที่หนึ่งและสอง และฟันกรามซี่ที่สองด้านล่างขวา จากภาพรังสีแพโนรามา และประเมินอายุกระดูก จากกระดูกคอในภาพรังสีเซฟฟาโลเมทริกด้านข้างของแต่ละคนพร้อมกันโดยผู้วิจัยสามคน และใช้การทดสอบ ไคสแควร์ ชนิดสเปียร์แมน เพื่อวิเคราะห์ความสัมพันธ์ของอายุฟันซี่ต่าง ๆ กับอายุกระดูก ผลการศึกษา อายุฟันของฟันซี่ต่างๆ …

Materials & Methods: A Sirona digital lateral cephalogram and a NewTom synthesized lateral cephalogram of a phantom in the orthogonal and perspective projections were created. Introduction: The objective of this study was to determine the anterior/posterior position of the mandible at which an orthodontist and oral surgeon would advise bilateral sagittal split osteotomy advancement or set back when evaluating adult female photos and silhouettes. Material & Methods: Fifty-one adult female profile photos and lateral cephalograms were collected. Subjects were selected who met three criteria: 1) ANB of 0-2, 2) balanced profile image, and 3) age greater than 18. Background

Photodynamic therapy with aminolevulinic acid (ALA PDT) for oral leukoplakia has shown promising effects in regression of oral leukoplakia. Although ALA has been extensively studied and is an ideal photosensitizer, the optimal light dose for treatment of oral leukoplakia has not been determined. We conducted a phase I study to determine MTD and DLT of PDT in patients treated with ALA for leukoplakia.

Methods

Patients with histologically confirmed oral leukoplakia received a single treatment of ALA PDT in cohorts with escalating doses of light (585 nm). Clinical, histologic, and biologic markers were assessed.

Owre served as dean of dentistry at both the University of Minnesota, 1905-27, and Columbia University, 1927-33. Safety net clinics across the country struggle with a lack of resources to tackle the needs presented. Screening programs set up for children and elderly have proven to be effective in triaging need, prioritizing care, and maximizing resources. These programs do not currently exist for working uninsured adults. Research was initiated to answer the question: Does the screening process improve patient care for the community clinic? Evidence suggests differences in both the quality and quantity of the dental pulp-derived mesenchymal stem cells (DPSC) obtained from different sources, such as primary or "baby teeth" and adult, permanent teeth. Background

Researchers have documented an association between waiting times in emergency departments (EDs) and quality of care for medical conditions, but little is known about trends and factors associated with waiting times for ED visits related to nontraumatic dental conditions (NTDCs). The authors examined trends in waiting time and associated factors for NTDC-related ED visits in the United States.

Methods

Purpose: To assess a caregiver’s oral health attitudes, habits, and behaviors pre and post intervention, and to determine whether a particular delivery-style (verbal-only or with visual supplementation) of a motivational interviewing session is more effective in improving oral health behaviors as well as improving success of a chosen preventive goal. Methods: N=140 caregivers of pediatric dental patients were given questionnaires to assess readiness to change and current preventive oral health behaviors.
