Injury to an immature permanent tooth may result in cessation of dentin deposition and root maturation leaving an open root apex and thin dentinal walls that are prone to fracture. Endodontic treatment is often complicated and protracted with an uncertain prognosis frequently resulting in pre-mature tooth loss. There are several advantages of promoting apexogenesis in immature teeth with open apices. It encourages a longer and thicker root to develop thus decreasing the propensity of long term root fracture. Minimal treatment time is a primary goal for orthodontic therapy. Reducing treatment time decreases risk of caries, periodontal disease, root resorption, and increases patient satisfaction. Orthodontic tooth movement (OTM) is a result of bone resorption and apposition; factors influencing the rate at which these processes take place may affect OTM. Various attempts have been made to shorten treatment time. The regional acceleratory phenomenon (RAP) has been used to modify the balance between resorption and apposition of bone through selectively injuring the cortical plate of the alveolus to accelerate tooth movement. Apical periodontitis is the most common pathological process in the periapical region. Disciplined clinical and radiographic evaluations and appropriate diagnostic tests can detect lesions related to apical periodontitis. Aside from this, lesions mimicking pulp-related pathology but unrelated to pulpal infection and necrosis are occasionally discovered in the periapical region. The objective of this study was two-fold: (1) to determine the prevalence of diverse periapically located pathological entities, and (2) to apply that information to evaluate the rationale for routine submission of surgically obtained tissue for histological examination and diagnosis. Background: To understand the relationship between our bacterial microbiome and health, it is essential to define the microbiome in the absence of disease. The digestive tract includes diverse habitats and hosts the human body’s greatest bacterial density. We describe the bacterial community composition of ten digestive tract sites from more than 200 normal adults enrolled in the Human Microbiome Project, and metagenomically determined metabolic potentials of four representative sites.
Results: The microbiota of these diverse habitats formed four groups based on similar community compositions: buccal mucosa, keratinized gingiva, hard palate; saliva, tongue, tonsils, throat; sub- and supra-gingival plaques; …

Periodontal Resident Self-Assessment Of Ergonomics Before And After Videotaped Surgeries, Corin Marantz

Objective: To examine whether self-assessment of videotaped surgeries helps improve periodontal residents’ ergonomics. Methods: Residents (n=8) provided self-assessments of their own ergonomics while performing periodontal surgery using a questionnaire with open and closed items. Results were analyzed using quantitative and qualitative means. Results: Comparison of responses resulted in a change between Pre-video Surgery 1 and PSV1 (p<0.05) and between the three occasions for flat foot and horizontal shoulder positions (p<0.05). Resident goals were most numerous for improving positions of shoulder, back and neck and most notable responses for failure to achieve goals were the need for surgical access and being too focused the procedure. Conclusions: Videotape review is a valid means of self-assessment. Intervention solely in the form of a questionnaire and videotape review was insufficient in its ability to change the residents’ ergonomics. Barriers to implementation of proper ergonomics were identified.

Objectives: This study was designed to investigate the dental health status of several communities in Honduras in terms of the DMFT index (decayed missing and filled teeth). Specifically the study focused on comparisons of the Garifuna (a maroon community in Tela) with the people of San Jose, Copan, and others who traveled from other regions to come to a medical clinic. These results were also compared to other populations of developing and developed countries. Through looking at each participant’s DMFT score, many possible correlations to oral health were explored including region, age, gender, snacking, tobacco use, and oral hygiene. …

Background: The University of Connecticut (UConn) School of Dental Medicine (SODM) routinely recruits dental students and faculty volunteers to conduct no-cost dental health screenings for transient residents at South Park Inn Homeless Shelter in Hartford, CT. Once screened, patients are referred to UConn or another hospital-based dental clinic in the area for treatment. The purpose of this study was to determine the demographics and oral health needs of the homeless population at South Park Inn and referrals to UConn SODM were followed to verify patient presentation at UCONN SODM. Tooth enamel is inherently weak, with fracture toughness comparable with glass, yet it is remarkably resilient, surviving millions of functional contacts over a lifetime. We propose a microstructural mechanism of damage resistance, based on observations from ex situ loading of human and sea otter molars (teeth with strikingly similar structural features). Section views of the enamel implicate tufts, hypomineralized crack-like defects at the enamel–dentin junction, as primary fracture sources. We report a stabilization in the evolution of these defects, by ‘‘stress shielding’’ from neighbors, by inhibition of ensuing crack extension from prism interweaving (decussation), and by self-healing. Mechanical and chemical debridement plays an important role in reducing intracanal microorganisms. Effective root canal irrigation depends on both the root canal irrigant and irrigation system. The objective of this study was to evaluate the debridement efficiency of four root canal irrigation systems, Endovac®, PiezoFlow™, EndoActivator® and traditional needle irrigation using saline as an irrigant. Seventy-five, single canal, extracted, mature teeth were selected. Teeth were standardized to canal lengths of 15 mm and instrumented to Master Apical File size #40 with 4% taper. A functional orthopedic appliance is a popular type of Class II corrector that is thought to correct Class II dental relationships both dentally and skeletally by enhancing mandibular growth. วัตถุประสงค์ เพื่อศึกษาประสิทธิภาพของสารต้านอนุมูลอิสระต่อความแข็งแรงพันธะของสารยึดติดระบบเซลฟ์เอทซ์ 2 ขั้นตอน (เคลียร์ฟิล เอสอี บอนด์) กับเนื้อฟันบริเวณผนังโพรงเนื้อเยื่อในภายหลังการฟอกสีด้วยไฮโดรเจน เพอร์ออกไซด์ความเข้มข้นสูง (โอพัลเลสเซนส์ บูชท์) วัสดุและวิธีการ ใช้ฟันกรามน้อยบนสภาพสมบูรณ์ ไม่มีรอยโรค และไม่เคยได้รับการบูรณะใด ๆ มาก่อน โดยตัด รากฟันออกและแบ่งครึ่งส่วนตัวฟันในแนวด้านแก้ม-ด้านลิ้น ใช้หัวกรอกากเพชรทรงสอบกรอผนังโพรงเนื้อเยื่อใน เพื่อลอกเลียนขั้นตอนการเปิดทางเข้าสู่โพรงเนื้อเยื่อในในการรักษาคลองราก จากนั้นแบ่งชิ้นฟันทั้งหมดเป็น 4 กลุ่ม ทดลองได้แก่ (1) กลุ่มควบคุมซึ่งไม่ทําการฟอกสี (2) กลุ่มที่ทําการฟอกสีด้วยโอพัลเลสเซนส์ บูชท์ (3) กลุ่มที่ใช้ สารละลายโซเดียมแอสคอร์เบทภายหลังการฟอกสีฟัน และ (4) กลุ่มที่ใช้สารละลายคะตะเลสภายหลังการฟอกสีฟัน บูรณะวัสดุเรซินคอมโพสิตบนผิวเนื้อฟันด้วยสารยึดติดผลิตภัณฑ์เคลียร์ฟิล เอสอี บอนด์ เก็บชิ้นงานไว้ภายใต้ ความชื้นสัมพัทธ์ร้อยละ 100 ที่อุณหภูมิ 37 องศาเซลเซียส เป็นเวลา 24 ชั่วโมง ก่อนนํามาทดสอบความแข็งแรง พันธะเฉือนระดับจุลภาคของสารยึดติดกับเนื้อฟัน ตรวจสอบลักษณะพื้นผิวของชิ้นงานภายหลังการแตกหักด้วยกล้องถ่ายภาพในช่องปากร่วมกับโปรแกรมแสดงภาพผ่านระบบคอมพิวเตอร์ รวมทั้งตรวจสอบลักษณะผิวเนื้อฟันของกลุ่มทดลองต่าง ๆ ภายใต้กล้องจุลทรรศน์อิเล็กตรอนชนิดส่องกราด ผลการศึกษา จากการวิเคราะห์ทางสถิติพบว่ากลุ่มที่ไม่ได้ทําการฟอกสีมีค่าความแข็งแรงพันธะเฉือนกับเนื้อฟัน สูงที่สุด (p < 0.001) รองลงมาเป็นกลุ่มที่ใช้สารละลายโซเดียมแอสคอร์เบท และกลุ่มที่ใช้สารละลายคะตะเลส ภายหลังการฟอกสีฟัน (p < 0.001 และ p < 0.007 ตามลําดับ) ส่วนกลุ่มที่ทําการฟอกสีฟันและบูรณะด้วยเรซิน คอมโพสิตทันที มีค่าเฉลี่ยความแข็งแรงพันธะเฉือนน้อยกว่ากลุ่มทดลองอื่น ๆ อย่างมีนัยสําคัญ (p < 0.0035) ซึ่งส่วนใหญ่พบการแตกหักของชิ้นงานภายในชั้นสารยึดติดหรือเรซินคอมโพสิต นอกจากนี้ในกลุ่มทดลองดังกล่าว ยังพบการหลุดของเรซินคอมโพสิตจากชิ้นงานก่อนการทดสอบถึงร้อยละ 40 การตรวจสอบด้วยกล้องจุลทรรศน์อิเล็กตรอนชนิดส่องกราดพบว่าชั้นสเมียร์และสเมียร์พลัคของทุกกลุ่มทดลองที่ผ่านการปรับสภาพด้วยไพรเมอร์ ของสารยึดติดผลิตภัณฑ์เคลียร์ฟิล เอสอี บอนด์นั้นถูกละลายออกไปเป็นส่วนใหญ่ รวมทั้งพบการเผยของเส้นใย คอลลาเจนได้อย่างชัดเจน สรุป สารละลายโซเดียมแอสคอร์เบทและสารละลายคะตะเลสสามารถเพิ่มประสิทธิภาพการยึดของสารยึดติด ผลิตภัณฑ์เคลียร์ฟิล เอสอี บอนด์กับเนื้อฟันที่ผนังโพรงเนื้อเยื่อในภายหลังการฟอกสีด้วยโอพัลเลสเซนส์ บูชท์ได้ แต่ไม่เท่ากับกรณีเนื้อฟันที่ไม่ได้ทําการฟอกสี โดยสารละลายโซเดียมแอสคอร์เบทให้ผลที่ดีกว่าสารละลายคะตะเลสอย่างมีนัยสําคัญ (ว ทันต จุฬาฯ 2555;35:79-92)

วัตถุประสงค์ เพื่อเปรียบเทียบการสึกและความหยาบผิวของเรซินคอมโพสิต 4 ชนิด หลังการแปรงด้วยยาสีฟัน เพื่อฟันขาว 4 ผลิตภัณฑ์ วัสดุและวิธีการ เตรียมชิ้นตัวอย่างเรซินคอมโพสิตชนิดนาโนฟิลล์ (ฟิลเทคซี 350 เอกซ์ที) นาโนไฮบริด (เฮอร์คู ไลท์อัลตรา) ไมโครฟิลล์ (ดูราฟิลวีเอส) และไมโครไฮบริด (ฟิลเทคซี 250) ชนิดละ 64 ชิ้น จากแบบหล่อโลหะ แบ่ง ชิ้นตัวอย่างเรซินคอมโพสิตแต่ละชนิดออกเป็น 4 กลุ่ม กลุ่มละ 16 ชิ้น ตามชนิดของยาสีฟันเพื่อฟันขาว 4 ผลิตภัณฑ์ ได้แก่ สปาร์คเคิลไวท์ ฟลูโอคารีลเฮลท์ตี้ไวท์เทนนิ่ง คอลเกตแอดวานส์ไวท์เทนนิ่ง และเซ็นโซดายน์ไวท์เทนนิ่งวัดปริมาตรและความหยาบผิวโดยใช้เครื่องโปรไฟโลมิเตอร์ก่อนและหลังการแปรงด้วยเครื่องแปรงอัตโนมัติ เมื่อใช้ แรงในการแปรง 300 กรัม ความเร็ว 90 รอบต่อนาที จํานวน 20,000 รอบ วิเคราะห์ผลการทดลองที่ได้ด้วยสถิติ ความแปรปรวนแบบสองทาง และสถิติแทมเฮนที่ระดับนัยสําคัญ 0.05 ผลการศึกษา หลังการแปรงด้วยยาสีฟันเพื่อฟันขาวทุกผลิตภัณฑ์พบว่าดูราฟิลวีเอสเกิดการสึกและความหยาบ ผิวต่างจากเฮอร์คูไลท์อัลตรา ฟิลเทคซี 350 เอกซ์ที และฟิลเทคซี 250 อย่างมีนัยสําคัญทางสถิติ (p < 0.05) เซ็นโซดายน์ไวท์เทนนิ่งทําให้เรซินคอมโพสิตทั้ง 4 ชนิด เกิดการสึกมากกว่าสปาร์คเคิลไวท์ คอลเกตแอดวานส์ไวท์- เทนนิ่ง และฟลูโอคารีลเฮลท์ตี้ไวท์เทนนิ่ง เซ็นโซดายน์ไวท์เทนนิ่ง สปาร์คเคิลไวท์ และคอลเกตแอดวานส์ไวท์เทนนิ่งทําให้เรซินคอมโพสิตเกิดความหยาบผิวมากกว่าฟลูโอคารีลเฮลท์ตี้ไวท์เทนนิ่ง สรุป ดูราฟิลวีเอสเกิดการสึกและความหยาบผิวมากที่สุดหลังแปรงด้วยยาสีฟันเพื่อฟันขาวทุกผลิตภัณฑ์เซ็นโซดายน์ ไวท์เทนนิ่ง สปาร์คเคิลไวท์ และคอลเกตแอดวานส์ไวท์เทนนิ่ง ทําให้เรซินคอมโพสิตส่วนใหญ่เกิดการสึกและ ความหยาบผิวมากกว่าฟลูโอคารีลเฮลท์ตี้ไวท์เทนนิ่ง (ว ทันต จุฬาฯ 2555;35:105-18)

แอคทิโนไมโคซิสปลายรากฟันเป็นการติดเชื้อนอกรากฟันรูปแบบหนึ่งซึ่งเชื่อกันว่าเป็นสาเหตุหนึ่งที่ทําให้การรักษาคลองรากฟันล้มเหลว จากการศึกษาพบว่าแอคทิโนไมโคซิสปลายรากฟันเกิดจากการติดเชื้อชนิดแอคทิโนไมเซส และเชื้อโปรปิโอนิแบคทีเรียม โปรปิโอนิคม เป็นหลัก เชื้อแอคทิโนไมเซสสามารถตรวจพบได้ทั้ง ในเนื้อเยื่อรอบปลายรากฟัน และภายในคลองรากฟันที่ติดเชื้อ ซึ่งเชื่อว่าเป็นช่องทางของเชื้อที่จะเข้าไปในบริเวณ เนื้อเยื่อรอบปลายรากฟันได้ โดยที่เชื้อโรคเหล่านี้มีความสามารถในการรุกรานเข้าไปอาศัยอยู่ในเนื้อเยื่อรอบ ปลายรากฟันโดยไม่ถูกร่างกายทําลายให้หมดไป บทความปริทัศน์นี้จึงทําการรวบรวมเนื้อหาที่เกี่ยวข้องกับความสามารถในการก่อโรคของเชื้อแอคทิโนไมเซสที่เป็นสาเหตุของแอคทิโนไมโคซิสปลายรากฟัน รวมทั้งอุบัติการณ์การวินิจฉัย ลักษณะทางจุลพยาธิวิทยา แนวทางการรักษาและแนวทางการป้องกัน (ว ทันต จุฬาฯ 2555;35:141-48)

Objective To present techniques to mask discolored teeth before porcelain veneer cementation using resin-based color modifier as a subopaque layer in severely tetracycline-stained teeth. Materials and methods A female patient had severely tetracycline-stained teeth on both upper and lower anterior teeth, and space between teeth #11 and #21. Her oral hygiene was good. Spacing in the esthetic area results in an unconfident smile. To solve this problem, many alternative treatments can be used with multidisciplinary knowledge: for example, orthodontic treatment and restorative treatment. The treatment plan should be performed under conservative consideration, while the esthetic outcome should persist in the long term. Objective This case report showed the alternative management of anterior spacing by the use of orthodontic elastomeric ligatures to redistribute the spaces at the maxillary anterior teeth of one patient and followed by closing all the spaces between teeth with composite resin restoration. Orthodontic tooth movement is mediated by interactions between PDL cells and those of the alveolus. One protein—the receptor activator nuclear factor kappa B ligand (RANKL)—is critical for osteoclastogenesis, and osteoprotegerin (OPG) is a decoy ligand that competitively inhibits RANKL. A higher RANKL/OPG ratio is associated with areas of bone resorption, while a lower ratio occurs in areas of bone deposition and homeostasis. There have been almost no clinical studies of RANKL and OPG expression in human subjects undergoing orthodontic tooth movement. Introduction: Improved environments in the U.S. have resulted in taller, larger, and heavier children and adults compared to past generations. Studies of other skeletal-dental dimensions have shown increases across generations, and our perception is that teeth now are forming faster. The purpose of this study is to test for a secular trend towards faster tempos of tooth mineralization in a sample of U.S. white adolescents over the past quarter century (1980-85 to 2005-10).

The cranberry (species: Vaccinium macrocarpon, family: Ericaceae) is a polyphenolicrich berry fruit native to North America and exhibits numerous beneficial properties for human health, such as inhibition of human cancer cell line proliferation and prevention of urinary tract colonization by bacterial pathogens.
