Root Fracture In Immature Tooth: Report Of A Case, H. Cem Güngör, Bülent Büyükgüral, Serdar Uysal

Root fracture injuries affect 0.5–7% of permanent teeth. Although this type of injury is rarely seen in teeth with immature root formation, the prognosis is generally good depending on the site of the fracture. A case report of horizontal root fracture in maxillary central incisor of an 8-year and 3-month-old girl and its treatment was presented.

วัตถุประสงค์ เพื่อทดสอบความแข็งแรงตัดขวางสองแกนของวัสดุอินซีแรมและวัสดุไอพีเอสเอมเพรส 2 ที่มีอัตราส่วนความหนาของชั้นคอร์ต่อวีเนียร์แตกต่างกัน โดยวิธีทดสอบหาความแข็งแรงดัดขวางสองแกน วัสดุและวิธีการ ทําการขึ้นรูปชิ้นทดสอบเซรามิกอินซีแรมและไอพีเอสเอมเพรส 2 เป็นแผ่นกลมขนาดเส้นผ่าน ศูนย์กลาง 15 มิลลิเมตร หนา 1.2 มิลลิเมตร ตามคําแนะนําของบริษัทผู้ผลิตชนิดละ 50 ชิ้น แต่ละชนิดแบ่งออกเป็น 5 กลุ่ม กลุ่มละ 10 ชิ้น ตามอัตราความหนาของส่วนคอร์ต่อวีเนียร์ 1:0 2:1 1:1 1:2 และ 0:1 ตามลําดับ นําชิ้น ตัวอย่างทั้งหมดมาทดสอบหาความแข็งแรงดัดขวางสองแกนตามมาตรฐาน ISO 6872 ปี ค.ศ. 1995 โดยใช้เครื่อง ทดสอบสากลด้วยความเร็วหัวกด 1 มิลลิเมตร/นาที ผลการศึกษา ค่าเฉลี่ยและค่าเบี่ยงเบนมาตรฐานของอินซีแรมตามอัตราส่วนคอร์ต่อวีเนียร์จากมากไปน้อยของ อินซีแรม กลุ่มที่ 1 - 5 มีค่า 433.1 + 68.4, 338.9 + 22.6, 294.4 + 15.2, 259.9 + 14.5 และ 56.6 + 10.5 เมกะปาสกาล ตามลําดับ สําหรับกลุ่มของวัสดุไอพีเอสเอมเพรส 2 กลุ่มที่ 6-10 มีค่า 288.3 + 44.6, 246.3 + 28.2, 266.73 + 23.22, 226.6 + 26.4 และ 68.6 + 5.5 เมกะปาสกาล ตามลําดับ จากการวิเคราะห์ข้อมูลทาง สถิติโดยใช้การวิเคราะห์ความแปรปรวนและการทดสอบแบบแทมเฮน พบว่าอินซีแรมทั้งชิ้นมีค่าความแข็งแรง ดัดขวางสองแกนสูงกว่าแบบที่เคลือบวีเนียร์ โดยเมื่อความหนาของชั้นคอร์ลดลง ค่าความแข็งแรงดัดขวางสองแกน จะลดลงตามลําดับอย่างมีนัยสําคัญ (p …

โพรไบโอติกส์คืออะไร?, จินตกร คูวัฒนสุชาติ

วัตถุประสงค์เพื่อทบทวนคําจํากัดความของโพรไบโอติกส์ และคําจํากัดความของจุลินทรีย์ที่จัดว่าเป็นโพรไบโอติกส์ ที่มาของข้อมูล ค้นหาจากการทดลองทางคลินิก และงานวิเคราะห์ผลงานย้อนหลังที่เกี่ยวกับโรคเด็กที่ใช้ โพรไบโอติกส์ รวมทั้งบทความปริทัศน์ที่เกี่ยวกับโรคเด็กที่ใช้โพรไบโอติกส์ สรุปงานที่ค้นคว้า วารสารทางวิทยาศาสตร์เกี่ยวกับเรื่องโพรไบโอติกส์เพิ่มขึ้นอย่างเด่นชัด ในรอบ 10 ปี ที่ผ่านมา มีการค้นหากลไกการทํางานของมันจากการศึกษาชี้ให้เห็นว่าโพรไบโอติกส์สามารถแข่งกันเพื่อความอยู่รอด กับจุลินทรีย์ที่ทําให้เกิดโรคได้ สามารถทําให้สภาพแวดล้อมในลําไส้มีความเป็นกรดที่เกิดจากผลิตผลที่มันผลิตออกมา สามารถเผชิญและปรับการตอบสนองต่อภูมิคุ้มกันและการอักเสบทั้งที่เป็นตําแหน่งเฉพาะที่กับที่เป็น ทั้งระบบได้ จากการศึกษาทางคลินิก และการศึกษาผลงานวิจัยย้อนหลัง แสดงให้เห็นว่า โพรไบโอติกส์ช่วยป้องกัน โรคท้องร่วงอย่างรุนแรง และโรคท้องร่วงที่เกิดจากการใช้ยาปฏิชีวนะได้ และยังลดอุบัติการณ์ของการเกิดผิวหนัง อักเสบเหตุภูมิแพ้ได้ สรุป คําว่าโพรไบโอติกส์มีการนํามาใช้กับเชื้อจุลินทรีย์ที่มีชีวิตตั้งแต่ปี ค.ศ. 1989 โดยจัดให้เป็นจุลินทรีย์มีชีวิต ที่ให้เสริมเพื่อประโยชน์ต่อสุขภาพ โดยการไปทําให้ความสมดุลของจุลินทรีย์ในลําไส้ดีขึ้น อย่างไรก็ตาม มันมีความ จําเป็นที่ว่า คําจํากัดความของโพรไบโอติกส์ในปัจจุบัน อาจต้องมีการปรับเปลี่ยนต่อไป เพื่อจะได้ช่วยให้สามารถ เลือกโพรไบโอติกส์ในแต่ละสถานะการณ์ในการป้องกันและรักษาโรคในทางคลินิกได้อย่างถูกต้อง สําหรับการศึกษาเกี่ยวกับบทบาทของโพรไบโอติกส์ทางด้านสุขอนามัยช่องปาก มีน้อย เมื่อเปรียบเทียบกับโพรไบโอติกส์ ที่เกี่ยวข้องกับระบบทางเดินอาหาร ดังเช่นเรื่องโพรไบโอติกส์ที่เกี่ยวกับกลิ่นปาก และที่เกี่ยวกับเรื่องฟันผุเป็นต้นบทความนี้อาจจุดประกายให้มีการศึกษาเกี่ยวกับแง่มุมทางด้านนี้มากขึ้นในอนาคต (ว ทันต จุฬาฯ 2550;30:205-18)

ผลของผงขัดที่ผสมในยาสีฟันต่อการสึกของซี่ฟันเทียมอะคริลิก, อรพินท์ แก้วปลั่ง, ปราณปรียา ใจธีรภาพกุล, ณัฐกาญจน์ ภาคยวงศ์, รจน์สุพรรณ แจ้งเจนกิจ, สาวิตร ทองสุข

วัตถุประสงค์ เพื่อเปรียบเทียบความแตกต่างของการสึกของฟันเทียมอะคริลิกภายหลังการแปรงร่วมกับ ยาสีฟันประเภทต่างๆ และเป็นแนวทางแนะนําผู้ป่วยในการยืดอายุการใช้งานฟันเทียมจากการทําความสะอาด ซี่ฟันเทียมนั้น วัสดุและวิธีการ นําฟันหน้าเทียมอะคริลิก จํานวน 30 แผง มาแบ่งเป็น 5 กลุ่มตามชนิดของยาสีฟันที่ใช้ขัด กลุ่มละ 6 แผงคือ กลุ่มที่ 1 ยาสีฟันควบคุมการเกิดคราบหินน้ําลาย กลุ่มที่ 2 ยาสีฟันลดคราบจุลินทรีย์ กลุ่มที่ 3 ยาสีฟันป้องกันฟันผุ กลุ่มที่ 4 ยาสีฟันทําให้ฟันขาว กลุ่มที่ 5 ยาสีฟันลดอาการเสียวฟัน นําแต่ละซี่ฟันเทียมไป ชั่งน้ําหนักก่อนแปรงด้วยเครื่องชั่งดิจิทัล จากนั้นยึดแต่ละซี่ฟันด้วยพลาสเตอร์หิน วัดความหยาบพื้นผิวก่อนแปรง ด้วยเครื่องโปรไฟโลมิเตอร์ แล้วจึงนําไปแปรงร่วมกับยาสีฟันในแต่ละกลุ่มด้วยเครื่องแปรงฟัน จํานวน 20,000 รอบ โดยแปรงครั้งละ 8 จนครบทุกซี่ จากนั้นนําฟันมาชั่งน้ําหนักและวัดความหยาบพื้นผิวหลังแปรง นําค่าเฉลี่ย ของผลต่างในแต่ละกลุ่มที่ได้มาวิเคราะห์ผลทางสถิติด้วย ครูสคัล-วาลิส ผลการศึกษา การสึกของฟันเทียมอะคริลิกหลังจากแปรงร่วมกับยาสีฟันที่มีคุณสมบัติต่างกัน มีความแตกต่าง อย่างมีนัยสําคัญทางสถิติ (P < 0.05) และพบว่าซี่ฟันเทียมที่แปรงร่วมกับยาสีฟันที่ทําให้ฟันขาวมีการสึกของฟัน มากที่สุด (∆Wwh= 4.10 ± 0.27 mg, ∆Rawh = 0.261 ± 0.064 ไมโครเมตร) สรุป ซี่ฟันเทียมอะคริลิกเมื่อแปรงร่วมกับยาสีฟันที่มีผงขัดมากหรือผงขัดหยาบ เช่น ยาสีฟันทําให้ฟันขาวนั้นจะ ทําให้มีการสึกของฟันมากที่สุด ดังนั้นเพื่อลดการหายไปของน้ําหนักเฉลี่ยของซี่ฟันเทียมและความหยาบพื้นผิว ฟันควรหลีกเลี่ยงการใช้ยาสีฟันดังกล่าวข้างต้นกับฟันอะคริลิกมากที่สุด (ว ทันต จุฬาฯ 2550;30:157-68)

การกระจายตัวเชิงพื้นที่และความคิดเห็นต่อการเลือกพื้นที่ปฏิบัติงานของบัณฑิตทันตแพทย์จุฬาลงกรณ์มหาวิทยาลัย ภายใต้นโยบายทันตแพทย์ใช้ทุนและนโยบายหลักประกัน, ชาญชัย ให้สงวน

วัตถุประสงค์ เพื่อประเมินสถานการณ์การกระจายตัวเชิงพื้นที่ของบัณฑิตทันตแพทย์ แห่งจุฬาลงกรณ์ มหาวิทยาลัยภายใต้นโยบายทันตแพทย์ใช้ทุนและนโยบายหลักประกันสุขภาพถ้วนหน้า และเพื่อประเมินปัจจัยที่มีส่วนสัมพันธ์กับการตัดสินใจเลือกพื้นที่ปฏิบัติงาน วัสดุและวิธีการ ใช้แบบแผนการวิจัยเชิงสํารวจภาคตัดขวาง สุ่มตัวอย่างแบบเป็นระบบจากบัณฑิตทันตแพทย์ จุฬาลงกรณ์มหาวิทยาลัยที่สําเร็จการศึกษาระหว่างปี พ.ศ. 2518-2547 ใช้แบบสอบถามทางไปรษณีย์รวม 2 รอบ เก็บรวบรวมข้อมูลเกี่ยวกับพื้นที่ปฏิบัติงานและปัจจัยที่มีส่วนในการตัดสินใจเลือกพื้นที่ปฏิบัติงาน มีทันตแพทย์ ส่งแบบสอบถามกลับคืน 375 คน เปรียบเทียบการกระจายตัวระหว่างทันตแพทย์ 3 กลุ่ม ซึ่งแบ่งกลุ่มตามปีที่ สําเร็จการศึกษาให้สอดคล้องกับระยะเวลาดําเนินการตามนโยบายทั้งสอง ดังนี้ กลุ่มที่ 1: พ.ศ. 2518-2531 กลุ่มที่ 2: พ.ศ. 2532-2543 และกลุ่มที่ 3: พ.ศ. 2544-2547 ผลการศึกษา โดยรวมแล้ว บัณฑิตทันตแพทย์ จุฬาลงกรณ์มหาวิทยาลัยมากกว่าครึ่งปฏิบัติงานในกรุงเทพมหานคร ทันตแพทย์กลุ่มที่ 2 ซึ่งสําเร็จการศึกษาหลังมีนโยบายทันตแพทย์ใช้ทุนและได้ผ่านพ้นระยะเวลาผูกพันไปแล้ว มี สัดส่วนที่กระจายตัวออกต่างจังหวัดมากขึ้นเมื่อเทียบกับกลุ่มที่ 1 ซึ่งสําเร็จการศึกษาก่อนมีนโยบายดังกล่าว แบบแผน การกระจายตัวตามภูมิภาคต่าง ๆ มีความผันแปรค่อนข้างสูง ทันตแพทย์กลุ่มที่ 3 มีสัดส่วนที่ตั้งใจจะโยกย้ายงาน ออกนอกพื้นที่ปฏิบัติงานเดิมค่อนข้างสูง ปัจจัยที่มีน้ําหนักความสําคัญสูงสุด 3 ลําดับแรกต่อการตัดสินใจเลือกพื้นที่ ปฏิบัติงานเป็นปัจจัยส่วนบุคคล ได้แก่ การมีการคมนาคมเดินทางได้สะดวก เป็นภูมิลําเนาเดิม และมีโอกาสทํารายได้ดี สรุป การศึกษานี้สะท้อนให้เห็นถึงอิทธิพลของนโยบายแห่งรัฐและปัจจัยส่วนบุคคลที่ส่งผลกระทบต่อการกระจายตัวเชิงพื้นที่ของบัณฑิตทันตแพทย์ จุฬาลงกรณ์มหาวิทยาลัย (ว ทันต จุฬาฯ 2550;30:85-102)

ฟันเทียมติดแน่นชนิดเรซินบอนด์เป็นการใส่ฟันเชิงอนุรักษ์วิธีหนึ่ง ซึ่งมีข้อดีหลายประการเมื่อเทียบ กับฟันเทียมติดแน่นแบบสัญนิยม อย่างไรก็ตาม ปัญหาที่มักพบได้มากสําหรับฟันเทียมชนิดนี้คือการหลุด ของชิ้นงาน จึงมีการพัฒนาเทคนิคต่าง ๆ เพื่อเพิ่มการยึดติด ได้แก่ การออกแบบโครงโลหะ การกรอแต่ง ฟัน องค์ประกอบของการยึดด้วยเรซินซีเมนต์ ซึ่งได้แก่การเลือกชนิดของโลหะผสม การปรับสภาพผิวโลหะ รวมทั้งวิธีการดัดแปลงการใช้ฟันเทียมชนิดนี้เพื่อให้ประสบผลสําเร็จทางคลินิกที่มากขึ้นในแบบต่าง ๆ เช่นการใช้หลักยึดข้างเดียวหรือการใช้วัสดุเรซินเสริมเส้นใยหรือใช้เซรามิกแทนโลหะผสมเพื่อความสวยงาม เพิ่มขึ้น จุดประสงค์ของบทความนี้เพื่อรวบรวมปัจจัยสําคัญที่มีอิทธิพลต่ออายุการใช้งานทางคลินิกของ ฟันเทียมติดแน่นชนิดเรซินบอนด์ การดัดแปลงการออกแบบหรือวัสดุ รวมทั้งข้อดีข้อด้อยของแต่ละวิธีการเพื่อเป็นข้อมูลให้แก่ทันตแพทย์ในการตัดสินใจเลือกวิธีการรักษาที่เหมาะสมให้กับผู้ป่วย (ว ทันต จุฬาฯ 2550:30:189-204)

วัตถุประสงค์ เพื่อศึกษาสมบัติทางกายภาพของวัสดุเคลือบหลุมร่องฟันที่เสริมเส้นใยอีสเตอร์ของสารไคโตซาน วัสดุและวิธีการ ทําการทดสอบความแข็งและค่ามอดูลัสของสภาพยืดหยุ่นของวัสดุเคลือบหลุมร่องฟันพรีโวแคร์ (คณะทันตแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย, ไทย) และวัสดุเคลือบหลุมร่องฟันพรีโวแคร์ที่เสริมเส้นใย วีสเกอร์ไคโตซานในปริมาณร้อยละ 3.3 5.0 และ 6.5 โดยน้ําหนัก โดยเครื่องทดสอบความแข็งในระดับนาโน (UMIS, ออสเตรเลีย) หลังจากนั้นนําวัสดุเคลือบหลุมร่องฟันพรีโวแคร์และวัสดุเคลือบหลุมร่องฟันพรีโวแคร์ที่มี สารไคโตซานในปริมาณร้อยละ 3.3 โดยน้ําหนัก มาทดสอบมาตรฐานไอเอสโอ 6874:1988 ของวัสดุเคลือบ หลุมร่องฟันชนิดเรซินในเรื่องความลึกของการแข็งตัวและความหนาของชั้นผิวที่ไม่แข็งตัว ทําการวิเคราะห์ ความแตกต่างของความแข็งและค่ามอดูลัสของสภาพยืดหยุ่นของวัสดุที่ทดสอบ โดยใช้การวิเคราะห์ความแปรปรวนทางเดียวและวิเคราะห์ความแตกต่างระหว่างกลุ่มของวัสดุที่นํามาทดสอบมาตรฐานไอเอสโอโดยการใช้สถิติอินดิเพนท์เดนท์ แซมเปิล ที- เทส ผลการศึกษา วัสดุพรีโวแคร์ที่มีสารไคโตซานในปริมาณร้อยละ 3.3 5.0 และ 6.5 โดยน้ําหนัก มีค่าความแข็ง ที่ไม่มีความแตกต่างกันอย่างมีนัยสําคัญกับกลุ่มควบคุม จึงเลือกวัสดุพรีโวแคร์ที่มีสารไคโตซานร้อยละ 3.3 มาทดสอบตามมาตรฐานไอเอสโอ ซึ่งพบว่าไม่มีความแตกต่างกันในเรื่องค่าความลึกของการแข็งตัวและความหนา ของชั้นผิวที่ไม่แข็งตัวของวัสดุพรีโวแคร์และวัสดุพรีโวแคร์ที่มีสารไคโตซานร้อยละ 3.3 โดยน้ําหนัก สรุป วัสดุเคลือบหลุมร่องฟันที่เสริมเส้นใยอีสเตอร์ของสารไคโตซานไม่ทําให้ความแข็งของวัสดุลดลง และมีคุณสมบัติผ่านมาตรฐานไอเอสโอ 6874:1988 (ว ทันต จุฬาฯ 2550;30:129-40)

วัตถุประสงค์ การวิจัยนี้เป็นการศึกษาในห้องปฏิบัติการถึงประสิทธิภาพของฟลูออไรด์เฉพาะที่ 3 ชนิด ได้แก่ แอซิดเลตเทดฟอสเฟตฟลูออไรด์เจลความเข้มข้นร้อยละ 1.23 ฟลูออไรด์วานิชความเข้มข้นร้อยละ 0.1 และ ยาสีฟันผสมฟลูออไรด์ความเข้มข้นร้อยละ 0.11 ในการส่งเสริมการคืนกลับของแร่ธาตุบนรอยโรคจุดขาวจําลอง ที่ผิวเคลือบฟันด้านเรียบในฟันน้ํานม วัสดุและวิธีการ ทําการศึกษาในฟันกรามน้ํานมจํานวน 30 ซี่ ตัดแบ่งครึ่งในแนวแก้ม-ลิ้นเป็น 30 คู่ เพื่อเป็น ชิ้นควบคุมและชิ้นทดลอง นําไปทําให้เกิดรอยโรคจุดขาวจําลอง แล้วแบ่งออกเป็น 3 กลุ่ม กลุ่มละ 10 คู่ คือ กลุ่มที่ 1 ชิ้นทดลองทาแอซิดูเลตเทดฟอสเฟตฟลูออไรด์เจล 4 นาที กลุ่มที่ 2 ชิ้นทดลองทาฟลูออไรด์วานิช 1 นาที และกลุ่มที่ 3 ชิ้นทดลองแช่ในฟองยาสีฟันผสมฟลูออไรด์ วันละ 2 ครั้ง นําฟันทุกกลุ่มไปผ่านกระบวนการจําลอง การเปลี่ยนแปลงสภาวะความเป็นกรดด่างในช่องปาก เป็นเวลา 14 วัน จากนั้นนํามาคํานวณพื้นที่รอยโรคจุดขาว จําลองด้วยกล้องจุลทรรศน์แสงโพลาไรส์ ผลการศึกษา พบว่าพื้นที่รอยโรคจุดขาวจําลองในชิ้นทดลองและชิ้นควบคุมของทั้งสามกลุ่มทดลองมีความ แตกต่างกันอย่างมีนัยสําคัญทางสถิติ (p < 0.01) โดยพื้นที่รอยโรคจุดขาวจําลองในชิ้นทดลองของทั้งสาม กลุ่มลดลงในขณะที่ชิ้นควบคุมมีพื้นที่เพิ่มขึ้น อย่างไรก็ตาม พบว่าไม่มีความแตกต่างกันอย่างมีนัยสําคัญ ทางสถิติในการลดพื้นที่รอยโรคจุดขาวจําลองของฟลูออไรด์เฉพาะที่แต่ละชนิด (p > 0.22) โดยที่พื้นที่รอยโรคจุด ขาวจําลองเริ่มต้นของฟลูออไรด์เฉพาะที่ทั้งสามกลุ่มไม่แตกต่างกันอย่างมีนัยสําคัญทางสถิติ (p > 0.17) สรุป ฟลูออไรด์เฉพาะที่ทั้งสามชนิดมีประสิทธิภาพในการส่งเสริมการคืนกลับแร่ธาตุของรอยโรคจุดขาว ในฟันน้ํานม (ว ทันต จุฬาฯ 2550;30:169-80)

This article is a case report of conventional endodontic therapy for a left maxillary second premolar with three root canals. The incidence of three canals at the apex of the maxillary second premolar has been reported to be 0.3-1.0%. This anatomical variation of root canal system can be challenging in endodontic treatment. การติดตามผลการรักษาผู้ป่วยเท็มโพโรแมนดิบิวลาร์ดิสออเดอร์ที่ยุติการรักษา, วันทนี มุทิรางกูร, ประภาพร หอมจันทร์จีรัง, สัจจพร พรรคอนันต์

วัตถุประสงค์ เพื่อติดตามผลการรักษาและหาสาเหตุที่ผู้ป่วยเท็มโพโรแมนดิบิวลาร์ดิสออเดอร์ (ทีเอ็มดี) ยุติการรักษา วัสดุและวิธีการ ใช้แบบสอบถามทางไปรษณีย์ส่งไปยังผู้ป่วยที่เอ็มดีที่มารับการรักษาที่คณะทันตแพทยศาสตร์จุฬาลงกรณ์มหาวิทยาลัยที่ยุติการรักษาไป 316 คน ผลการศึกษา มีผู้ตอบแบบสอบถามกลับมาจํานวน 77 คน (อัตราการตอบกลับร้อยละ 24.3) เป็นชาย 17 คน (ร้อยละ 22.1) หญิง 60 คน (ร้อยละ 77.9) โดยกลุ่มนี้มี 58 คน (ร้อยละ 75.3) เป็นผู้มารับการรักษาด้วย อาการมากกว่าหนึ่งอย่าง และมีเสียงข้อต่อขากรรไกรขณะอ้า-หุบปากเป็นอาการสําคัญอันดับแรกที่ต้องการ รักษา (ร้อยละ 31.1) ผู้ป่วยทุกคนได้รับการรักษาเชิงอนุรักษ์โดยใช้เฝือกสบฟันเป็นหลัก พบว่า 1. หลังจากยุติการ รักษา ผู้ป่วยยังมีอาการอยู่ 55 คน (ร้อยละ 71.4) โดยอาการที่ยังคงมีไม่แตกต่างอย่างมีนัยสําคัญจากก่อน รักษา ยกเว้นอาการอ้าปากจํากัด (p = 0.013) มีผู้ที่ต้องการรักษาต่อ 43 คน (ร้อยละ 78.2) และผู้ที่ไม่ต้องการ รักษาต่อ 12 คน (ร้อยละ 21.8) 2. เหตุผลที่ทําให้ผู้ป่วยยุติการรักษาไปมีดังนี้ รู้สึกทนอาการที่มีอยู่ได้ (ร้อยละ 37.7) คิดว่าอาการนั้นรักษาไม่หาย (ร้อยละ 32.5) ไม่สะดวกมารับการรักษาในเวลาราชการ (ร้อยละ 31.2) ไม่สะดวกในการเดินทาง (ร้อยละ 26.0) ไม่ได้รับการติดต่อ (ร้อยละ 20.8) และอื่น ๆ สรุป การศึกษานี้ชี้ให้เห็นว่า ผู้ป่วยที่เอ็มดีอาจยังมีอาการอยู่ภายหลังยุติการรักษาไป และมีเหตุผลหลายประการ ที่เป็นสาเหตุของการยุติการรักษา (ว ทันต จุฬาฯ 2550;30:117-28)

วัตถุประสงค์ การวิจัยนี้มีวัตถุประสงค์เพื่อประเมินอิทธิพลของภูมิลําเนาที่มีต่อพื้นที่ปฏิบัติงานของบัณฑิตทันตแพทย์แห่งจุฬาลงกรณ์มหาวิทยาลัย วัสดุและวิธีการ ดําเนินการสํารวจกลุ่มตัวอย่างบัณฑิตทันตแพทย์ จุฬาลงกรณ์มหาวิทยาลัยที่สําเร็จการศึกษา ในรอบ 30 ปีที่ผ่านมา จํานวน 1,289 คน ซึ่งถูกคัดเลือกด้วยวิธีสุ่มตัวอย่างแบบเป็นระบบ เก็บข้อมูลเกี่ยวกับ ภูมิลําเนาและพื้นที่ปฏิบัติงานด้วยแบบสอบถามทางไปรษณีย์ มีทันตแพทย์ส่งแบบสอบถามกลับคืนซึ่งเป็น ทันตแพทย์ที่ยังปฏิบัติงานอยู่ 375 คน วิเคราะห์ความสัมพันธ์ระหว่างภูมิลําเนากับพื้นที่ปฏิบัติงานโดยควบคุม ตัวแปรอื่นที่เกี่ยวข้องด้วยสถิติแมนเทล-แฮนเซลไคสแควร์ ผลการศึกษา บัณฑิตทันตแพทย์ จุฬาลงกรณ์มหาวิทยาลัยส่วนใหญ่ร้อยละ 60.5 มีภูมิลําเนาในกรุงเทพมหานคร พบว่าภูมิลําเนามีความสัมพันธ์กับพื้นที่ปฏิบัติงานอย่างมีนัยสําคัญ ทันตแพทย์จากกรุงเทพมหานครและปริมณฑล ส่วนใหญ่ปฏิบัติงานตรงตามภูมิลําเนาของตน ส่วนทันตแพทย์จากภูมิภาคอื่นมีการกระจายตัวแบ่งเป็น 2 กลุ่มใหญ่ กลุ่มหนึ่งเลือกกลับไปปฏิบัติงานในภูมิลําเนาเดิม แต่อีกกลุ่มหนึ่งมุ่งเข้ากรุงเทพมหานคร หลังจากที่ได้ควบคุม อิทธิพลของตัวแปรอื่นที่เกี่ยวข้องแล้ว พบว่าทันตแพทย์จากต่างจังหวัดมีความน่าจะเป็นสูงกว่า 2.79 เท่าตัวที่จะ ปฏิบัติงานนอกพื้นที่จังหวัดภูมิลําเนา เมื่อเทียบกับผู้ที่มีภูมิลําเนาในกรุงเทพมหานครและปริมณฑล สรุป การศึกษานี้บ่งชี้ว่าภูมิลําเนาเป็นปัจจัยกําหนดที่สําคัญประการหนึ่งในการเลือกพื้นที่ปฏิบัติงานของบัณฑิต ทันตแพทย์ จุฬาลงกรณ์มหาวิทยาลัย แต่การพัฒนานโยบายเพื่อแก้ไขปัญหาการกระจายโดยอิงเกณฑ์ภูมิลําเนา ควรคํานึงถึงปัจจัยอื่นที่เกี่ยวข้องร่วมด้วย (ว ทันต จุฬาฯ 2550;30:103-16)

The efficacy of a novel synthetic antimicrobial peptide (WLBU2) was evaluated against three oral microorganisms (grown planktonically): Streptococcus gordonii, Fusobacterium nucleatum, and Porphyromonas gingivalis. WLBU2 killed all three species, with F. nucleatum being the most susceptible. WLBU2 also reduced the bacterial burden of S. gordonii and F. nucleatum biofilms.

“Early” or two-phase orthodontic treatment of Class II malocclusions is a highly debated topic in the orthodontic literature. We report here on a retrospective cephalometric study of patients with Class II, division 1 malocclusions. One group consisted of 32 consecutively treated patients who received “early” treatment with a Fränkel II appliance followed by treatment with full fixed appliance. These subjects were “matched” by sex and cephalometric value, to a subject treated with standard edgewise appliances in a single phase. Matching criteria focused on the bony facial characteristics, notably ANB, NAP, Y Axis, AOBO, FMA, overjet, and overbite. The question was …

Purpose: The objective of this study was to evaluate the osseointegration between plasma sprayed (PS) hydroxyapatite (HA) coated implants and electrophoresis deposited (EPD) nano hydroxyapatite coated implants in an animal model. The quantity of osseointegration was inferred by interfacial strength and percent tissue-contact length.Materials and Methods: Thirty-six cylindrical implants (18 PS and 18 EPD) were placed into dog mandibles. Nine implants from each group were evaluated with pull out testing and histological studies at 12 weeks after implantation. Malocclusions can involve arch-size tooth-size discrepancies that have to be resolved by premolar extractions. Treatment planning in such cases usually involves removing first (P1) or second premolars (P2). Choosing an extraction pattern is a learned skill that incorporates many diagnostic factors. This study compared the amounts of in-treatment tooth movement in two common extraction patterns (4/4 and 5/5) in order to gain insight into the criteria used for differential diagnosis and treatment planning. PURPOSE: The effectiveness of many commercially available composite finishing and polishing devices have been studied and compared. However, no studies have directly compared the effectiveness among the polishing brushes. This study set out to (1) investigate the surface roughness reduction and gloss value increase of two universal dental composites polished by three polishing brushes and two well-studied finishing and polishing systems, and (2) to evaluate resulting surface morphology using scanning electron microscopy (SEM).

Purpose: The aim of this study was to evaluate the reproducibility of mineral density measurement expressed in Hounsfield Units (HU) using a phantom device with known mineral densities and the NewTom 3G Cone Beam Computerized Tomography (CBCT).

Materials and Methods: A phantom device made of 21 HA cylindrical disks with known densities was fabricated. The phantom was scanned in 6, 9, and 12 inch fields of view using a NewTom 3G CBCT. Bacterial lipopolysaccharide (LPS) has been implicated in the pathogenesis of pulpal and periapical diseases. Several studies have evaluated the ability of various medicaments and irrigants used during root canal therapy to chemically detoxify LPS. The purpose of this study was to evaluate the ability of some endodontic sealers to detoxify LPS. This investigation compared the quantities of TNF-α released by human monocytes exposed to LPS or LPS pre-treated with sealers. The clinical success of dental implants is determined mostly by surface properties of implants and interactions with surrounding tissues. Furthermore, clinical success of dental implants is directed by implant surfaces and bone cell responses that promote osseointegration and long-term stability. Hydroxyapatite (HA)-coated Titanium (Ti) implants have been widely used due to its effect on bone response. However, microbial infection is common. Objective: To compare as-received copper-nickel-titanium (CuNiTi) archwires to those used in patients by means of differential scanning calorimetry (DSC). Also, the thermal or phase properties of 27°C, 35°C, and 40°C CuNiTi archwires were studied to ascertain if their properties match those indicated by the manufacturer.

Materials and Methods: Six wires of 27°C, 35°C, and 40°C CuNiTi were tested as-received, and six each of the 27°C and 35°C wires were examined after use in patients for an average of approximately 9 and 7 weeks, respectively. Traumatic Neuroma Following Sagittal Split Osteotomy Of The Mandible, R. H. Kallal, F. G. Ritto, Luis Eduardo Almeida, D. J. Crofton, G. P. Thomas

A 16-year-old male underwent bilateral sagittal split osteotomy of the mandible to correct a mandibular deficiency. Twenty-one years later, a routine panoramic radiograph revealed a radiolucent lesion on the left side of the mandible. The lesion was biopsied. As the patient did not have symptoms and the lesion was connected to the inferior alveolar nerve, the lesion was not totally excised in order to preserve nerve function. The histological features were consistent with traumatic neuroma, and no further surgical procedure was planned.
