การมีแผลพุพองในช่องปากที่เกี่ยวข้องกับโรคทางภูมิคุ้มกันนั้น ก่อให้เกิดความรำคาญ และอาการเจ็บปวดแก่ผู้ป่วยอย่างยิ่ง แม้ว่าปัจจุบันยังไม่มีการรักษาให้หายขาด แต่ก็สามารถ ควบคุมและบรรเทาอาการของรอยโรคเหล่านั้นได้โดยใช้ยาประเภทอิมมูโนเรคคิวเลเตอร์ บทความ นี้จึงได้รวบรวมลักษณะธรรมชาติการเกิดของรอยโรค ยาที่ใช้รักษา โดยเฉพาะอย่างยิ่งคอร์ติ โคสเตียรอยด์ อะซาไธโอพริน และลีวามิโซล เพื่อใช้เป็นประโยชน์ต่อการพิจารณาให้การ รักษาผู้ป่วยเหล่านั้นต่อไป

การวิเคราะห์ภูมิหลังและความต้องการของผู้ใช้บริการ ทันตกรรม ที่ คณะทันตแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย, สุนทร ระพิสุวรรณ, ประกอบ บูรณสิน, ธนสิทธิ์ เสรีรัตน์

ได้ศึกษาถึงภูมิหลังและความต้องการรักษาทางทันตกรรมของผู้ใช้บริการทันตกรรม คณะทันตแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย โดยใช้แบบสอบถาม และสำรวจจากบัตร ตรวจจำนวน 467 คน พบว่า 60.4 เปอร์เซ็นต์ เป็นหญิง 77.52 เปอร์เซ็นต์ เป็นคนที่อายุ ไม่เกิน 33 ปี 63.17 เปอร์เซ็นต์ มีการศึกษาสูงกว่า ม.ศ. 3, 60.81 เปอร์เซ็นต์ มีอาชีพเป็น นักเรียน นิสิต นักศึกษา ฐานะทางการเงินของผู้ใช้บริการเทียบโดยอิงรายได้ของครอบครัว ต่อ เดือน จํานวนสมาชิกในครอบครัวและค่าใช้จ่ายต่อคนของคนกรุงเทพฯ พบว่า 65.91 เปอร์เซ็นต์ มีรายได้พอใช้เดือนต่อเดือน หรือไม่ค่อยจะพอใช้ ระยะทางโดยเฉลี่ยจากบ้าน หรือสถานที่ทํางาน มาถึงคณะทันตแพทยศาสตร์ประมาณ 7 ก.ม. เวลาที่ใช้ในการเดินทางไปรักษาเฉลี่ยประมาณ 45 นาที ความต้องการรักษา หรือบริการทันตกรรมพบว่า 74.15 เปอร์เซ็นต์ต้องการอุดฟัน รอง ลงมา 49.78 เปอร์เซ็นต์ต้องการถอนฟัน และความต้องการแก้ไขระบบบดเคี้ยวมีน้อยที่สุดเพียง 6.84 เปอร์เซ็นต์ ค่าเฉลี่ยของผู้ใช้บริการถอนฟันต่อนิสิตที่ขึ้นคลินิกเพื่อฝึกหัดถอนฟัน โดยอิงจำนวนคนไข้ใหม่ 50 คน จะเท่ากับ 0.63 คน ต่อนิสิต 1 คน

การใช้คอนสตรัคชันเพลท (Reconstruction plate) ช่วยยึดกระดูกเป็นวิธีการ อย่างหนึ่งที่นำมาใช้ช่วยยึดกระดูกที่ปลูกให้ติดแน่น ง่าย และการติดเชื้อลดลง ได้รายงาน ผู้ป่วย 2 ราย ที่สูญเสียกระดูกบริเวณขากรรไกรล่าง รายแรกเนื่องจากถูกปืนยิง ส่วนรายที่สอง ได้รับการผ่าตัดไฟบรัสดีสเพลเซียบริเวณกระดูกขากรรไกรออก ทั้งสองรายได้ใช้กระดูกมาปลูกแทนที่โดยใช้เพลทช่วยยึด

Pain from mastication can be experienced as a discomfort about the face and mouth induced by function and parafunction of the jaws. Phosphate bonded investments are widely used in casting the high- fusing alloys. After casting it always have positively cast nodules on the surface of casting, which cause the fitness of the casting on the die. The purpose of this article is examine the effect of a number of investing variable on the incidence of bubbles cast to wax patterns invested in a phasphate bonded investment. Noma,which is derived from the Greek meaning the devour or galloping horse gangrene, was known in the times of Hippocrates, Galen and Celsen.(1) Lund(2), who after studying 11 cases of gangrene of the mouth in young children, proposed the name noma (necrosis). Furthermore, it also refered to as gangrenous stomatitis, cancrum oris or in Chiness, dzo-ma-gan (running horse gangrene). A review of 580 cases of facial injuries from the Department of Surgery, Faculty of Dentistry, Chulalongkorn University, during 1973-1983 revealed that motorcycle and car accidents were the major causes of facial bone fractures. The male-to-female ratio was 4.3 to 1. The site distribution in descending order of frequency was symphysis, body of the mandible, angle of the mandible, condylar process, zygomatic complex, alveolar process of maxilla and palate. The highest incidence recorded was in the group of patients between the ages of 20 and 29 years. Simple fracture was found to be the most common type of fractures. Most …

กลาสไอโอโนเมอร์ซีเมนต์ มีใช้ในวงการทันตแพทย์มาเกือบ 10 ปีแล้ว เป็นวัสดุ บูรณะฟันชนิดแรกที่มีแรงยึดทางเคมีกับผิวเคลือบฟันและเนื้อฟัน (Enamel and dentin) ใช้ได้ดีในการบูรณะฟันที่เกิดจากการสึกและกร่อนที่คอฟัน (class V) และฟันหน้าที่เกิด การทางด้านข้างและบริเวณคอฟันใกล้กับขอบเหงือก (class III และ class V) วิธีบูรณะ ที่แนะนําไว้ในบทความนี้ได้มาจากการติดตามผลงานการบูรณะฟันผู้ป่วยของผู้เขียน และ ศึกษาจากผลงานของผู้อื่น การใช้ซีเมนต์ชนิดนี้ด้วยความระมัดระวังทั้งการผสม การนําเข้าไป บูรณะและการขัดจะทําให้การบูรณะประสบผลสําเร็จอย่างดียิ่ง

รายงานผู้ป่วยที่เกิดปฏิกริยาแพ้ยาอมบ้วนปาก 0.2% คลอเฮกซิดีน ที่ทันตแพทย์ ใช้ในการรักษาไลเคน พลานัส (lichen planus)ในช่องปาก เพื่อผลในการป้องกันการ เกิดแผ่นคราบฟัน รอยโรคที่ปรากฏแสดงการหลุดลอกของเยื่อบุข้างแก้ม เมื่อเช็ดออกจะเป็นแผล สีแดงจัดในบริเวณมิวโคบัคคัลโฟลด์ (mucobuccal fold)มีอาการปวดแสบปวดร้อน แต่ เมื่อหยุดการใช้ยานี้ รอยโรคหายไปหมดในระยะเวลาเพียง 5 วัน

The association of fluoride and the reduction of dental caries is well established and the usefulness of water fluoridation as a community health measure has been acknowledged and proved. This article considers the metabolism of fluoride by the tooth physiologically and therapeutically and examines the theories and evidence of fluoride action with the aim of instituting a rationalised preventive programme for patients with dental caries.

การรายงานผู้ป่วยคนไทย 6 ราย เป็นโรคอยู่ในกลุ่มอาการของมัลติเปิลบาซาลเซลล์นีวัส เป็นการศึกษา และติดตามผู้ป่วยอย่างแท้จริงซึ่งเริ่มต้นด้วยผู้ป่วยชายรายแรกมีถุงน้ำในกระดูกขากรรไกรหลาย ๆ ถุง พร้อมกับมี บาซาลเซลล์ คาร์ซิโนมา และ ซีเบซัส ซิสต์ ที่ บริเวณใบหน้า ต่อมาสามารถติดตาม และศึกษาผู้ที่มีความเกี่ยวพันทางสายเลือดกับผู้ป่วย รายแรกได้อีก 5 ราย รวมทั้งบุตรชายของเขา ผู้ป่วยทุกรายมีถุงน้ำหลายถุงเกิดในกระดูก ขากรรไกรพร้อมกับมี บาซาลเซลล์ คาร์ซิโนมา นีวัส ซีเบซัส ซิสต์ อยู่ตามผิวหนัง การสืบประวัติย้อนหลังมีหลักฐานว่าโรคเกิดได้รวมแล้วถึง 3 ชั่วอายุคนรวมผู้ป่วยทั้งหมดได้ 15 ราย

ภายหลังการผ่าตัดเอา superior cervical ganglion ทางด้านขวาของหนู (mouse) ออกเป็นเวลา 3 เดือนมีผลทําให้จํานวนของ acini ใน submandibular salivary gland ลดลงเนื่องจากขาด sympathetic nerve มาเลี้ยง ในขณะเดียวกันต่อมน้ำลายได้สร้าง granular ducts เพิ่มขึ้นเป็นจํานวนมากเพื่อช่วยผลิตน้ำลายขึ้นมาชดเชยกับปริมาณน้ำลาย ซึ่งลดลงจากการที่ acini ฝ่อไป การเพิ่มจํานวนขึ้นอย่างรวดเร็วของ granular ducts ทําให้ไปเบียดที่อยู่ของ stroma ดังนั้นจํานวนของ stroma จึงลดลงอย่างมีนัยสําคัญทางสถิติ (P < 0.03) ไม่มีการเปลี่ยนแปลงทางจุลกายวิภาคศาสตร์หรือจำนวนส่วนประกอบของ major sublingual salivary gland อย่างเห็นได้ชัดเมื่อเปรียบเทียบระหว่างกลุ่มทดลอง(experi-mental animals) และกลุ่มเปรียบเทียบ(sham-operated animals)
