Cerebral palsy (CP) is a common developmental neurological disorder affecting about 2-3 children out of 1,000. CP is the result of infant brain damage or abnormal development resulting in impaired muscle control, coordination, tone, reflex, posture, and balance. These patients are unable to control motor movements of their muscles of mastication and facial expression, causing excessive drooling, clenching, bruxism, and other oral health-related issues. This lack of motor control affects their ability to swallow and often limits these patients to a liquid diet. This can lead to vitamin deficiencies and result in further developmental problems. Objective: To provide an analysis of the association between the longevity of breastfeeding and development of ECC. Determine the optimal time frame in which mothers should cease breastfeeding to reduce ECC development. Methods: Dr. Brickhouse, PubMed, Google Scholar and other scholarly databases were utilized to find current scientific evidence on the effects of breast milk on ECC. Relevant articles were summarized to write a review of literature. 16 articles published from 2015 to the present date were reviewed and cited. Background: Dental caries is a common chronic disease among children and adults alike, posing a substantial health burden. Caries is affected by multiple genetic and environmental factors, and prior studies have found that a substantial proportion of caries susceptibility is genetically inherited.
Methods: To identify such genetic factors, we conducted a genome-wide linkage scan in 464 extended families with 2616 individuals from Iowa, Pennsylvania and West Virginia for three dental caries phenotypes: (1) PRIM: dichotomized as zero versus one or more affected primary teeth, (2) QTOT1: age-adjusted quantitative caries measure for both primary and permanent dentitions including pre-cavitated lesions, and …

การศึกษาย้อนหลังของการรักษาพัลเพคโตมีในฟันน้ำนม : การวิเคราะห์การอยู่รอด, เมธาพร ทรงเวชเกษม

การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาอัตราการอยู่รอดของฟันน้ำนมที่ได้รับการรักษาพัลเพคโตมี โดยใช้เครื่องรูทซีเอ็กซ์กำหนดความยาวทำงาน ภายใต้การดมยาสลบและการใช้ยาชาเฉพาะที่ และศึกษาปัจจัยที่มีผลต่อการอยู่รอดของฟันน้ำนมที่ได้รับการรักษาพัลเพคโตมี โดยทำการศึกษาย้อนหลังจากข้อมูลจากเวชระเบียนของผู้ป่วยเด็กอายุต่ำกว่า 5 ปี ที่มารับการรักษาที่คณะทันตแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย ระหว่างปี พ.ศ. 2550 - 2559 และมีฟันที่ได้รับการรักษาพัลเพคโตมีภายใต้การดมยาสลบหรือการใช้ยาชาเฉพาะที่อย่างน้อย 1 ซี่ในช่องปาก โดยกำหนดให้ความล้มเหลวของการรักษาพัลเพคโตมีคือฟันต้องได้รับการถอนหรือรักษารากฟันซ้ำในภายหลังเนื่องจากการรักษาพัลเพคโตมีล้มเหลว และวันที่ได้รับการวินิจฉัยให้ถูกถอนหรือรักษารากฟันซ้ำเป็นวันที่สิ้นสุดการติดตามการรักษา การศึกษาครั้งนี้ประเมินอัตราการอยู่รอดโดยใช้สถิติแคแพลน-ไมเออร์และวิเคราะห์ปัจจัยที่มีผลต่อการอยู่รอดโดยใช้สถิติล็อค-แรงค์และตัวแบบถดถอยค็อกซ์ ผลการศึกษาพบว่ามีฟันน้ำนมที่ถูกคัดเลือกเข้ามาในการศึกษาทั้งหมด 272 ซี่ และเมื่อติดตามผลการรักษาเป็นเวลา 5 ปีหลังการรักษาพบว่ามีฟันน้ำนมทั้งหมด 25 ซี่ที่ถูกถอนหรือรักษารากฟันซ้ำเนื่องจากการรักษาพัลเพคโตมีล้มเหลว และมีอัตราการอยู่รอดของฟันที่ได้รับการรักษาภายใต้การดมยาสลบและการใช้ยาชาเฉพาะที่เท่ากับร้อยละ 81.4 และ 87.4 ตามลำดับ ซึ่งไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p≥0.05) ผลการวิเคราะห์ตัวแบบถดถอยค็อกซ์พบว่าการมีเงาดำบริเวณปลายรากหรือง่ามรากฟันในภาพรังสีก่อนการรักษาเป็นปัจจัยที่ส่งผลต่อการอยู่รอดของฟันที่ได้รับการรักษาพัลเพคโตมีที่ระยะเวลา 5 ปีหลังการรักษา (HRadj= 3.95, 95%CI = 1.31-11.86) การศึกษานี้แสดงให้เห็นว่าการรักษาฟันน้ำนมด้วยวิธีพัลเพคโตมีเป็นวิธีที่สามารถเก็บรักษาฟันน้ำนมที่ผุลุกลามถึงเนื้อเยื่อในไว้ในช่องปากได้นานและมีอัตราการอยู่รอดที่สูงทั้งในการรักษาภายใต้การดมยาสลบและการใช้ยาชาเฉพาะที่

Impact Of Sensory Adapted Dental Environment On Children With Developmental Disabilities, Gail Kim

Purpose: This was a pilot study assessing the impact of a sensory adapted dental environment (SADE) on children with developmental disabilities (DD) receiving routine dental care.
Methods: A crossover study of 22 children with DD, aged 6 through 21, was conducted at Virginia Commonwealth University Pediatric Dental clinic. Each participant was randomized to a sequence of two dental cleanings, one regular dental environment (RDE) and one SADE with a 3-4-month recall. Outcomes included physiological measures (heart rate and oxygen saturation) and cooperation (Frankl scores).

Aim: To evaluate the effect of disking technique and resin modified glass ionomer cement (RMGI) restoration on proximal caries of primary upper incisors in tribal preschool children, aged 3-5 years, attending child development centers of Doi Tung Development Project, Chiang Rai. Methods: Twenty tribal preschool children were enrolled in this match-paired design study. Twenty-six proximal carious surfaces were randomized for RMGI restoration (control group), and 26 proximal carious surfaces were randomized for disking technique (experimental group). การศึกษานี้มีวัตถุประสงค์เพื่อเปรียบเทียบประสิทธิภาพของซิลเวอร์ไดเอมีนฟลูออไรด์ที่มีจำหน่ายในท้องตลาด ในการคืนแร่ธาตุในรอยผุชั้นเนื้อฟันของฟันน้ำนม การศึกษานี้เป็นการศึกษาในห้องปฏิบัติการโดยใช้ชิ้นฟันตัวอย่างจากฟันกรามน้ำนมที่มีรอยผุตามธรรมชาติชั้นเนื้อฟันจำนวน 36 ชิ้น วัดความหนาแน่นแร่ธาตุของรอยผุเริ่มต้นด้วยเครื่องถ่ายภาพรังสีคอมพิวเตอร์ระดับไมโครเมตรแล้วนำมาคำนวณความลึกรอยผุและความหนาแน่นแร่ธาตุเฉลี่ย แบ่งชิ้นฟันเป็น 3 กลุ่ม กลุ่มที่ 1 ทา Saforide® (Toyo Seiyaku Kesei, Japan) กลุ่มที่ 2 ทา Advantage ArrestTM (Elevate Oral Care, USA) และกลุ่มที่ 3 ทาน้ำปราศจากไอออน (กลุ่มควบคุม) นำไปผ่านกระบวนการสลับกรด-ด่างโดยใช้เชื้อแบคทีเรียเพื่อจำลองสภาวะในช่องปากเป็นเวลา 5 วัน จากนั้นนำมาวัดความหนาแน่นแร่ธาตุหลังการทดลองด้วยเครื่องถ่ายภาพรังสีคอมพิวเตอร์ระดับไมโครเมตร คำนวณความลึกรอยผุ ความหนาแน่นแร่ธาตุเฉลี่ยและร้อยละการเปลี่ยนแปลงความหนาแน่นแร่ธาตุหลังการทดลอง ผลการทดลองพบว่าชิ้นฟันตัวอย่างกลุ่มที่ทา Saforide® และ Advantage ArrestTM มีความลึกรอยผุหลังการทดลองลดลงอย่างมีนัยสำคัญทางสถิติ (p= 0.033 และ 0.021 ตามลำดับ) และมีความหนาแน่นแร่ธาตุเฉลี่ยหลังการทดลองเพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติ (p= 0.002 และ 0.002 ตามลำดับ) พบว่าร้อยละการเปลี่ยนแปลงความหนาแน่นแร่ธาตุระหว่างกลุ่มทดลองทั้งสองกลุ่มไม่แตกต่างกันแต่แตกต่างจากกลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (p< 0.001) สรุปได้ว่า Saforide® และ Advantage ArrestTM มีประสิทธิภาพในการคืนแร่ธาตุในรอยผุชั้นเนื้อฟันของฟันน้ำนมใกล้เคียงกัน

การศึกษานี้มีวัตถุประสงค์เพื่อเปรียบเทียบผลของซีเมนต์ชนิดซิงค์ฟอสเฟตซีเมนต์ (ZC) ซิงค์โพลีคาร์บอกซิเลตซีเมนต์ (ZPC) กลาสไอโอโนเมอร์ซีเมนต์ชนิดดั้งเดิม (GI) เรซินมอดิฟายด์กลาสไอโอโนเมอร์ซีเมนต์ชนิดผงและน้ำ (RMGI-P/L) เรซินมอดิฟายด์กลาสไอโอโนเมอร์ซีเมนต์ชนิดสองหลอด (RMGI-P/P) และโพลีแอซิดมอดิฟายด์คอมโพสิตเรซิน (PMCR) ต่อการยับยั้งการสูญเสียแร่ธาตุที่ผิวเคลือบฟันบริเวณขอบของแถบรัดฟัน เป็นการศึกษาในห้องปฏิบัติการโดยใช้ฟันกรามน้อยแท้ของมนุษย์จำนวน 70 ซี่ ทำการขัดผิวเคลือบฟัน และสร้างช่องหน้าต่างทดลองบริเวณกึ่งกลางของด้านไกลกลางขนาด 1 x 2 ตารางมิลลิเมตร แบ่งฟันเป็น 7 กลุ่ม เพื่อยึดด้วย; (1) ZC (Zinc Cement Improved®), (2) ZPC (Hy-Bond Polycarboxylate Cement®), (3) GI (Hy-Bond Glasionomer CX®), (4) RMGI-P/L (GC Fuji Ortho LC®), (5) RMGI-P/P (GC Fuji Ortho Band Paste Pak®), (6) PMCR (Ultra Band-Lok®) และ (7) กลุ่มควบคุมที่ไม่ได้ยึดแถบรัดฟัน (Control) จากนั้นจำลองภาวะอุณหภูมิร้อนเย็นในช่องปาก 24 ชั่วโมง และภาวะการสูญเสียและคืนกลับแร่ธาตุ 21 วัน ทำการรื้อแถบรัดฟันและกำจัดซีเมนต์ก่อนนำชิ้นงานทั้งหมดวัดค่าร้อยละการสูญเสียฟลูออเรสเซนส์บริเวณช่องหน้าต่างทดลอง (∆F), ค่าร้อยละการสูญเสียฟลูออเรสเซนส์สูงสุดบริเวณช่องหน้าต่างทดลอง (∆Fmax) ค่าพื้นที่ที่เกิดการสูญเสียฟลูออเรสเซนส์ (Area) และปริมาตรของเคลือบฟันที่สูญเสียแร่ธาตุ (∆Q) ด้วยเทคนิควิเคราะห์การเรืองแสงเชิงปริมาณด้วยการใช้แสงกระตุ้นชนิดดิจิตอล จากการวิเคราะห์ด้วยสถิติ Kruskal-Wallis test พบความแตกต่างของค่าเฉลี่ย ∆F, ∆Fmax, Area และ ∆Q ของซีเมนต์แต่ละกลุ่มอย่างมีนัยสำคัญทางสถิติ (P<0.001) เมื่อหาความแตกต่างทางสถิติในแต่ละคู่โดยใช้สถิติ Pairwise comparisons ไม่พบความแตกต่างกันอย่างมีนัยสำคัญทางสถิติของค่าเฉลี่ย ∆F, ∆Fmax, Area และ ∆Q เมื่อจับคู่เปรียบเทียบระหว่างกลุ่ม Control:PMCR, ZP:ZPC, ZP:GI, ZP:RMGI-P/L, ZP:RMGI-P/P, ZPC:GI, ZPC:RMGI-P/L, ZPC:RMGI-P/P, GI:RMGI-P/L, GI:RMGI-P/P, RMGI-P/L:RMGI-P/P และ RMGI-P/L:PMCR แต่พบความแตกต่างอย่างมีนัยสำคัญทางสถิติเมื่อจับคู่เปรียบเทียบระหว่างกลุ่ม Control:ZP, Control:ZPC, Control:GI, Control:RMGI-P/L และ Control: RMGI-P/P, PMCR:ZC, PMCR:ZPC, PMCR:GI และ PMCR:RMGI-P/P การศึกษาในครั้งนี้พบว่าฟันซึ่งยึดแถบรัดฟันด้วย ZC, ZPC, GI และ RMGI-P/P ไม่เกิดการสูญเสียแร่ธาตุที่ผิวเคลือบฟันบริเวณขอบของแถบรัดฟัน ส่วนกลุ่ม RMGI-P/L, PMCR และกลุ่มควบคุมเกิดการสูญเสียแร่ธาตุที่ผิวเคลือบฟัน โดยที่กลุ่ม RMGI-P/L เกิดการสูญเสียแร่ธาตุน้อยกว่ากลุ่มควบคุม แต่กลุ่ม PMCR เกิดการสูญเสียแร่ธาตุไม่แตกต่างจากกลุ่มควบคุม

Objective/Aims: Assess the prevalence of sibling recurrent dental general anesthesia (DGA) at VCU Pediatric Dentistry. Assess factors that contribute to sibling recurrent dental general anesthesia. Methods: The guardian of patients with siblings were provided a questionnaire to assess the prevalence and factors associated with recurrent DGA. Results: A total of 40 families with a child presenting for GA and at least one sibling were included in the study. Of these, 45% had sibling-recurrent GA treatment (20% in one sibling; 25% in 2 or more siblings). Additionally, 13% of the children currently presenting for GA had already been treated under GA, …

Assessment Of Pediatric-Focused Brief Motivational Interviewing Training Of Dental Students And Pediatric Dental Residents, Victoria M. Onesty

Purpose: To assess dental students and pediatric dental residents’ knowledge and attitudes towards motivational interviewing and evaluate the efficacy of a pediatric-focused brief motivational interviewing training program.
Methods: A total of 66 participants were enrolled; 35% were third year dental students, 46% were fourth year dental students and 18% were pediatric dental residents. Participants completed three questionnaires: the first before the training, the second immediately after the training and the third approximately 3.5 months after the training.
Results: A significant increase in correct responses was found for 4 of the 5 questions assessing participants’ knowledge. Participants were satisfied with the …

A Comparison Of Moderate Oral Sedation Drug Regimens For Pediatric Dental Treatment: A Pilot Study, Ojas A. Parikh

Purpose: Compare moderate oral sedation of pediatric patients using Hydroxyzine and Meperidine with either Diazepam or Midazolam in management of pediatric dental patients.
Methods: Randomized, double-blind, crossover pilot study of patients 3 to 7 years of age requiring two sedation visits. Frankl and Houpt behavior scores recorded at injection time, initiation of treatment and 100% oxygen at end of treatment. Postoperative phone call surveys conducted within eight hours and within 24 hours of discharge. Wilcoxon Signed-Rank tests, Fisher’s Exact Chi-squared test and 0.10 significance level.
Results: 25 subjects completed 35 sedations. Eight participants completed both treatments and demonstrated significantly higher …

การวิจัยนี้เป็นการศึกษาในช่องปากร่วมกับห้องปฏิบัติการ มีวัตถุประสงค์เพื่อประเมินผลการคืนกลับแร่ธาตุของวัสดุเคลือบหลุมร่องฟันต่างชนิด บนรอยผุจำลองระยะแรกที่ผิวด้านเรียบของฟัน โดยตัดชิ้นฟันจากด้านประชิดของฟันกรามน้อยจำนวน 24 ซี่ จำนวน 3 ชิ้นต่อซี่ ทำให้เกิดรอยผุจำลองระยะแรกบริเวณผิวเคลือบฟัน แบ่งชิ้นฟันเป็น 3 หน้าต่าง ได้แก่ หน้าต่างทดลอง หน้าต่างรอยผุจำลองระยะแรกก่อนการทดลอง และหน้าต่างควบคุมที่ไม่ได้เคลือบวัสดุ บริเวณหน้าต่างทดลองจะถูกเคลือบด้วยวัสดุเคลือบหลุมร่องฟันเรซินที่มีฟลูออไรด์ (เดลตัน-เอฟเอสพลัส) หรือวัสดุเคลือบหลุมร่องฟันเรซินที่ไม่มีฟลูออไรด์ (เดลตัน) หรือกลาสไอโอโนเมอร์ (ฟูจิเซเว่น) ติดชิ้นฟันบนแบร็กเกตที่ยึดกับแถบรัดจัดฟัน และสุ่มชิ้นฟันตัวอย่างติดบนฟันกรามแท้บนซี่ที่หนึ่งทั้งสองข้างของอาสาสมัครจำนวน 24 คน เป็นระยะเวลา 28 วัน มีช่วงพักการทดลอง 7 วัน หลังจากนั้นใช้ชิ้นฟันตัวอย่างที่เหลือติดในการทดลองช่วงที่ 2 ก่อนการทดลอง 7 วันจนสิ้นสุดการทดลองอาสาสมัครใช้ยาสีฟันที่มีส่วนผสมของฟลูออไรด์ วันละ 2 ครั้ง วัดค่าความหนาแน่นแร่ธาตุทั้ง 3 หน้าต่าง ด้วยเครื่องไมโครคอมพิวเตตโทโมกราฟี ภายหลังการทดลอง พบว่าวัสดุเคลือบหลุมร่องฟันทั้ง 3 ชนิด เดลตัน เดลตัน-เอฟเอสพลัส และฟูจิเซเว่น มีค่าความหนาแน่นแร่ธาตุเฉลี่ยเพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติ (p=0.018 และ p=0.001 และ p=0.000 ตามลำดับ) และเดลตัน-เอฟเอสพลัสกับฟูจิเซเว่นมีค่าร้อยละการเปลี่ยนแปลงความหนาแน่นแร่ธาตุเฉลี่ยมากกว่ากลุ่มควบคุมในช่องปากอย่างมีนัยสำคัญทางสถิติ (p=0.003 และ p=0.001 ตามลำดับ) นอกจากนี้ยังพบว่ากลุ่มที่เคลือบด้วยฟูจิเซเว่น มีค่าร้อยละการเปลี่ยนแปลงความหนาแน่นแร่ธาตุเฉลี่ยสูงสุด และมีค่ามากกว่าเดลตันอย่างมีนัยสำคัญทางสถิติ (p=0.01) อย่างไรก็ตามไม่พบความแตกต่างระหว่างกลุ่มฟูจิเซเว่นกับเดลตัน-เอฟเอสพลัส อย่างมีนัยสำคัญทางสถิติ (p=0.625) จึงสรุปได้ว่า การเคลือบหลุมร่องฟันสามารถเพิ่มการคืนกลับแร่ธาตุให้กับรอยผุได้ โดยวัสดุเคลือบหลุมร่องฟันกลาสไอโอโนเมอร์ สามารถเพิ่มการคืนกลับแร่ธาตุได้ดีกว่าวัสดุเคลือบหลุมร่องฟันชนิดเรซินที่มีและไม่มีฟลูออไรด์ ตามลำดับ

This study investigated the effect of curcumin on the expression of wound healing-related genes including transforming growth factor beta 1 (TGF-β1), transforming growth factor beta receptor type I (TGFβR I), transforming growth factor beta receptor type II (TGFβR II) and vascular endothelial growth factor (VEGF) in human gingival fibroblasts. The cytotoxicity of curcumin was determined by MTT assay. Then, cells were treated with non-cytotoxic concentrations of curcumin for 24 hours and the level of gene expression was determined by quantitative polymerase chain reaction (qPCR). Curcumin at 0.1-20 µM caused no significant change in cell viability while higher concentrations of curcumin …

Objective: To investigate factors associated with dental caries experience in preschool children with cerebral palsy attending physical therapy programs in non-governmental rehabilitation centers in Bangkok. Materials and Methods: This research is a cross sectional study of 60 children with cerebral palsy, aged 3-6 years, mean age 5.01 ± 1.23 years old. Dental caries experience was determined by dmfs index, numbers of surfaces of dental caries, extracted tooth, and filled tooth. The other variables were collected including: dental plaque index (OHI-S), type of cerebral palsy, frequency of carbohydrate intake events, dietary consistency, assisting with toothbrushing, and frequency of toothbrushing.
