This study was designed to investigate whether the ingestion of ibuprofen prior to or after a 30-min bout of downhill (-10%) treadmill running would result in reduced sensation of soreness. A secondary purpose was to investigate whether ibuprofen would augment the diminished muscle performance following downhill running. Subject consisted of 21 males (age= 20.3 ± 1.7 years; weight= 77.4 ± 7.8 kg). Each subject was randomly assigned to either a control group, a prophylactic group which received 600 mg of ibuprofen 1 hour prior to the 30-min treadmill run and (200 mg every 3 hr postexercise up to 24 hours), …

Evaluation Of Oral Midazolam As Pre-Medication In Day Care Surgery In Adult Pakistani Patients, N Ahmed, F A. Khan

A placebo controlled randomized double blind study was designed to assess the suitability of oral Midazolam as a premedication in day care surgery in adult Pakistani patients. Fifty ASA I and II patients aged between 20-60 years received either Midazolam 7.5 mg or a placebo approximately one hour prior to surgery. Midazolam 7.5 mg produced significant anxiolysis and sedation (p < 0.001) in comparison to placebo after one hour of premedication. There was a significant difference (p < 0.001) in the mean heart rate and blood pressure in both groups after 1 hour of premedication with a lesser rise in blood pressure and heart rate in the Midazolam group. Psychomotor performance assessed by 'n' deletion test was impaired by Midazolam (p < 0.001) and recall of pictures revealed differences (p < 0.05) in the groups at one hour after premedication. However, at four hours after surgery there were no differences in both groups. These findings indicate that rapidly acting oral Midazolam in doses of 7.5 mg provided safe and effective premedication in terms of anxiolysis, sedation, amnesia and psychomotor performance and is suitable for day surgery.

An unusual behavioral and cardiovascular reaction was observed during opioid blockade with naltrexone in a 32-year-old male who met DSM III-R criteria for post-traumatic stress disorder (PTSD). As part of an ongoing placebo-controlled investigation of the effects of naltrexone on laboratory and ambulatory blood pressure reactivity, this participant reported experiencing feelings of rage, explosive behavior, and other unpleasant symptoms. When compared to all other subjects (N=24), this individual showed significantly greater effects of naltrexone on blood pressure reactivity during the laboratory stressor. Trismus associated with mandibular fractures is thought to increase the risks of general anesthesia. Nasotracheal intubation is necessary since the dental arches will be occluded at the end of the procedure. To establish an airway, an awake nasal intubation or fiberoptic endoscopy is required if direct laryngoscopy cannot be performed due to trismus. A prospective analysis of fifty-one patients with various types of mandibular fractures indicated that trismus was for the most part due to two factors, pain induced guarding and infection. The pharmacological effects of suxamethonium and pancuronium on the cardiovascular system may vary and therefore alter the haemodynamic response to intubation. The arterial blood pressure, the heart rate and the rate pressure product were measured as parameters of haemodynamic change in forty adult ASA. I and II patients undergoing laryngoscopy and endotracheal intubation in a randomised controlled study. The patients were induced with either thiopentone/suxamethonium (Group A) or thiopentone/pancuronium (Group B). There was no significant difference between the groups on comparison of systolic and diastolic blood pressure changes. The purpose of this study was to identify factors that influence a Prepared Childbirth couple's decision to utilize epidural anesthesia for labor and delivery. Review of the literature indicated a high correlation between epidural use for uncomplicated labors and deliveries and a high incidence of maternal complications and low neonatal neurological and behavioral scores.

A sample of twenty-nine couples was used in this prospective, non-experimental field study for the research. Data were collected utilizing two developed demographic and factual event tools, Rotter's Internal-External Locus of Control Tool and Spanier's Dyadic Marital Adjustment Tool.

The potential for secondary complications associated with retching and vomiting such as aspiration pneumonitis strain of suture line with possible hemorrhage, and potential fluid and electrolyte imbalance makes prevention of retching and vomiting a primary concern in the management of the postoperative patient.

This paper demonstrates the value of oxygen in maintaining arterial blood gas in an adequate range during outpatient oral surgery. Dental extractions were completed on ten patients under an anesthetic technique utilizing Innovar and methohexital with supplemental nasal oxygen. Blood gas samples were drawn at five stages during the procedure.

The mean of the PaO2 samples drawn with the patient at the point of deepest sedation was approximately three times the normal value.

A study of 800 patients ranging in age from seven to one hundred years was conducted in the development of an anesthetic technique which combines the desirable qualities of a neuroleptic agent with the requirements of an outpatient oral surgical anesthetic. These requirements included rapidity of onset, effectiveness of action, ease of controlling duration, and rapid postoperative recovery.

Patients who were cleared medically for outpatient procedures were given Innovar 1.5 cc/150 lb of body weight, atropine 0.4 mg, and fentanyl 1.0 cc/150 lb of body weight as an initial dose within 30-45 seconds in a D5W drip by means of …

The identification of anxiety as a causative factor in post operative vomiting was the purpose of this study. It was hypothesized that the more anxiety a patient experienced before surgery, the greater would be his chances of vomiting immediately after surgery.

The establishment of an underlying basis and need for a study of the relationship of anxiety and postoperative vomiting were made through a review of literature. To determine if the woman who receives the continuous attendance of a nurse during the first stage of labor requires less analgesic medication than the woman who is given interrupted or intermittent nursing care, a study was made employing the experimental method with the parallel-group technique.
