Objective: Every year, toxic exposures kill twelve hundred Americans. To aid in the timely diagnosis and treatment of such exposures, this research investigates the feasibility of a knowledge-based system capable of generating differential diagnoses for human exposures involving unknown toxins.
Methods: Data mining techniques automatically extract prior probabilities and likelihood ratios from a database managed by the Florida Poison Information Center. Using observed clinical effects, the trained system produces a ranked list of plausible toxic exposures. The resulting system was evaluated using 30,152 single exposure cases. In addition, the effects of two filters for refining diagnosis based on …
Quantitative Cherenkov Emission Spectroscopy For Tissue Oxygenation Assessment, Johan Axelsson, Adam K. Glaser, David J. Gladstone, Brian W. Pogue
Quantitative Cherenkov Emission Spectroscopy For Tissue Oxygenation Assessment, Johan Axelsson, Adam K. Glaser, David J. Gladstone, Brian W. Pogue
Dartmouth Scholarship
Measurements of Cherenkov emission in tissue during radiation therapy are shown to enable estimation of hemoglobin oxygen saturation non-invasively, through spectral fitting of the spontaneous emissions from the treated tissue. Tissue oxygenation plays a critical role in the efficacy of radiation therapy to kill tumor tissue. Yet in-vivo measurement of this has remained elusive in routine use because of the complexity of oxygen measurement techniques. There is a spectrally broad emission of Cherenkov light that is induced during the time of irradiation, and as this travels through tissue from the point of the radiation deposition, the tissue absorption and scatter …

Design And Experimental Validation Of Continuous Dna Separation Using Itp-Based Microfluidic Device, Nora C. M. Goscinski, Mary E. Rosa, Elizabeth K. Wheeler, Maxim Shusteff
Chemistry and Biochemistry
We have demonstrated a chip-based microfluidic device suitable for rapid and high-throughput DNA separation and purification for downstream analysis. The glass-based device incorporates a transverse free-flow isotachophoresis (tFF-ITP) that overcomes the volume limitations of CE approaches, and allows continuous sample processing in contrast to batch-mode solid-phase extraction (SPE). The device is able to focus DNA at flow rates up to 100 uL/min, and sample conductivity up to 2 mS/cm. Downstream of the chip, 30-40% of DNA from the input sample is recovered as a result of ITP focusing, and preliminary results indicate that this design is able to purify DNA …
Nanocrystals Of Chemotherapeutic Agents For Cancer Theranostics: Development And In Vitro And In Vivo Evaluation, Christin P. Hollis
Nanocrystals Of Chemotherapeutic Agents For Cancer Theranostics: Development And In Vitro And In Vivo Evaluation, Christin P. Hollis
Theses and Dissertations--Pharmacy
The majority of pharmacologically active chemotherapeutics are poorly water soluble. Solubilization enhancement by the utilization of organic solvents often leads to adverse side effects. Nanoparticle-based cancer therapy, which is passively targeted to the tumor tissue via the enhanced permeation and retention effect, has been vastly developed in recent years. Nanocrystals, which exist as crystalline and carry nearly 100% drug loading, has been explored for delivering antineoplastic agents. Additionally, the hybrid nanocrystal concept offers a novel and simple way to integrate imaging agents into the drug crystals, enabling the achievement of theranostics. The overall objective of this dissertation is to formulate …

This paper aims to investigate the effect of light emitting diode therapy (LEDT) on exercise-induced hand muscle fatigue by measuring the surface electromyography (sEMG) of flexor digitorum superficialis. Ten healthy volunteers were randomly placed in the equal sized LEDT group and control group. All subjects performed a sustained fatiguing isometric contraction with the combination of four fingertips except thumb at 30% of maximal voluntary contraction (MVC) until exhaustion. The active LEDT or an identical passive rest therapy was then applied to flexor digitorum superficialis. Each subject was required to perform a re-fatigue task immediately after therapy which was the same …

Comprehensive evaluation of PTSD includes diagnostic interviews, self-report testing, and physiological reactivity measures. It is often difficult and costly to diagnose PTSD due to patient access and the variability in symptoms presented. Additionally, potential patients are often reluctant to seek help due to the stigma associated with the disorder. A voice-based automated system that is able to remotely screen individuals at high risk for PTSD and monitor their symptoms during treatment has the potential to make great strides in alleviating the barriers to cost effective PTSD assessment and progress monitoring. In this paper we present a voice-based automated Tele-PTSD Monitor …
Biomedical Image Signal Processing For Reflection-Based Imaging, Paul Jansz, Steven Richardson, Graham Wild, Steven Hinckley
Biomedical Image Signal Processing For Reflection-Based Imaging, Paul Jansz, Steven Richardson, Graham Wild, Steven Hinckley
Research outputs 2012
Optical coherence tomography (OCT), ultrasound and other reflection based biomedical imaging technologies involve image signal processing that is primarily a filtering, digitizing and summing process so that the tissue cross-section can be visualized. In particular, in OCT, a series of adjacent one dimensional in-vivo axial interferograms (A-scan) are summed to form a two dimensional (B-scan) reflection map or reflectogram. Further graphical combinations can add adjacent B-scan together to form three dimensional C-scans. A physician can make a subjective interpretation and evaluation from the B and/or C-scans that may lead to actions impacting on the patient’s prognosis. More objective information can …
Targeted Multistage Delivery Of Nanoparticles To The Bone Marrow, Aman Mann
Targeted Multistage Delivery Of Nanoparticles To The Bone Marrow, Aman Mann
Dissertations and Theses (Open Access)
Bone marrow is a target organ site involved in multiple diseases including myeloproliferative disorders and hematologic malignancies and metastases from breast and prostate. Most of these diseases are characterized with poor quality of life, and the treatment options are only palliative due to lack of delivery mechanisms for systemically injected drugs which results in dose limitation to protect the healthy hematopoietic cells. Therefore, there is a critical need to develop effective therapeutic strategies that allow for selective delivery of therapeutic payload to the bone marrow. Nanotechnology-based drug delivery systems provide the opportunity to deliver drugs to the target tissue while …
In-Vivo Ct Dosimetry During Virtual Colonoscopy, Jonathon W. Mueller
In-Vivo Ct Dosimetry During Virtual Colonoscopy, Jonathon W. Mueller
Dissertations and Theses (Open Access)
Virtual colonoscopy (VC) is a minimally invasive means for identifying colorectal polyps and colorectal lesions by insufflating a patient’s bowel, applying contrast agent via rectal catheter, and performing multi-detector computed tomography (MDCT) scans. The technique is recommended for colonic health screening by the American Cancer Society but not funded by the Centers for Medicare and Medicaid Services (CMS) partially because of potential risks from radiation exposure. To date, no in‐vivo organ dose measurements have been performed for MDCT scans; thus, the accuracy of any current dose estimates is currently unknown.
In this study, two TLDs were affixed to the inner …
Evaluating The Capacity Of A Proton Therapy Facility, Ridvan Gedik
Evaluating The Capacity Of A Proton Therapy Facility, Ridvan Gedik
Graduate Theses and Dissertations
A relatively new consideration in proton therapy planning is the requirement that the mix of patients treated satisfy desired percentages. Since it is very dicult to satisfy an integer number of patients in light of these requirements, deviations from patient mix preferences and their impacts on operational capabilities are of particular interest to healthcare planners. Therefore, we propose a bicriteria mathematical programming model that determines an outpatient schedule maximizing the number of fractions and minimizing the deviations from the patient mix ratios over the planning horizon. The tradeos between the two objectives are identied through analysis of ecient frontiers. Our …

Cyberknife Radiation Modeling: Effects Of Relaxed Translational And Rotational Constraints On Healthy Tissue Irradiation, Sudha Bhavaroop Potineni
Electrical & Computer Engineering Theses & Dissertations
The Cyberknife system is a robotic radio surgery instrument that is used to treat cancers in different parts of the human body. Because of its accuracy and precision, it has become one of the best radio surgery systems in the world. However, even with Cyberknife the treatment time for one session takes from 30 minutes up to 120 minutes depending on the size, shape, and position of the tumor in the human body. In this research, a solution is presented to aid in reducing this long treatment time. An algorithm is developed which simulates the amount of the healthy tissue …

Determining A Method For Rendering Low Cost Cdse(Zns) Core(Shell) Quantum Dots Aqueous Soluble Via Amphiphilic Polymer Wrapping, Patrick Mcbride
Materials Engineering
Herein is described the procedure of two amphiphilic polymer wrapping techniques that may be employed for obtaining aqueous soluble quantum dots (QDs) for use in biological fluorescent imaging applications. The advent of QDs has led to new nanoscale fluorescent materials that exhibit unparalleled quantum yields (QYs), high resistance to photobleaching, tunable emissions, and
absorption over a large optical range. However, the QD synthesis employed here at Cal Poly to obtain bright, photostable CdSe(ZnS) core(shell) QDs involves the use of organic solvents and surfactants, leading to hydrophobic QDs. Since all of biology relies on aqueous solubility, this hydrophobicity creates a major …

The Use Of Motion Analysis Technology As An Alternative Means Of Assessing Spinal Deformity In Patients With Adolescent Idiopathic Scoliosis, Matthew J. Solomito
Master's Theses
Adolescent Idiopathic Scoliosis (AIS) is a three dimensional deformity of the spine that causes abnormal curvatures in the coronal and sagittal planes and abnormal vertebral rotations. The gold standard for diagnosing and tracking the progression of the deformity is through the use of repeated radiography and a patient can receive upwards of fifteen radiographic series over the course of treatment, which exposes this population to an increased risk of radiation-dependent complications.
This thesis presents the necessary calculations and the marker set required to make the motion capture technology a viable alternative to radiographs, in an effort to reduce the patient’s …

Master's Theses
This thesis encompasses a feasibility study of using low-cost materials to manufacture microfluidic chips that can perform the same functions as chips manufactured using traditional methods within an acceptable range of efficiency of chips created with more exotic methods and materials. The major parts of the project are the selection and characterization of the fabrication methods for creating the channels for fluid flow, the methods for sealing the channels to create a usable chip and the electrophoretic separations of carboxylated microspheres of different potentials. In this work we seek to answer the question if laser-etched PMMA microfluidic chips are comparable …

This paper describes an integrated system for dental implantation including both preoperative planning utilizing computer-aided technology and automatic robot operation during the intra-operative stage. A novel two-step registration procedure was applied for transforming the preoperative plan to the operation of the robot, with the help of a Coordinate Measurement Machine (CMM). Experiments with a patient-specific phantom were carried out to evaluate the registration error for both position and orientation. After adopting several improvements, registration accuracy of the system was significantly improved. Sub-millimeter accuracy with the Target Registration Errors (TREs) of 0.38±0.16 mm (N=5) was achieved. The target orientation errors after …

Electrical & Computer Engineering Faculty Publications
In this paper, we present a texture analysis based method for diagnosing the Basal Cell Carcinoma (BCC) skin cancer using optical images taken from the suspicious skin regions. We first extracted the Run Length Matrix and Haralick texture features from the images and used a feature selection algorithm to identify the most effective feature set for the diagnosis. We then utilized a Multi-Layer Perceptron (MLP) classifier to classify the images to BCC or normal cases. Experiments showed that detecting BCC cancer based on optical images is feasible. The best sensitivity and specificity we achieved on our data set were 94% …

In a recent study [1], we investigated the feasibility of predicting brain tumor progression based on multiple MRI series and we tested our methods on seven patients' MRI images scanned at three consecutive visits A, B and C. Experimental results showed that it is feasible to predict tumor progression from visit A to visit C using a model trained by the information from visit A to visit B. However, the trained model failed when we tried to predict tumor progression from visit B to visit C, though it is clinically more important. Upon a closer look at the MRI scans …

A Monocular Marker-Free Gait Measurement System, Jane Courtney, Annraoi M. De Paor
Articles
This paper presents a new, user-friendly, portable motion capture and gait analysis system for capturing and analyzing human gait, designed as a telemedicine tool to monitor remotely the progress of patients through treatment. The system requires minimal user input and simple single-camera filming (which can be acquired from a basic webcam) making it very accessible to nontechnical, nonclinical personnel. This system can allow gait studies to acquire a much larger data set and allow trained gait analysts to focus their skills on the interpretation phase of gait analysis. The design uses a novel motion capture method derived from spatiotemporal segmentation …

Macrophages Loaded With Gold Nanoshells For Photothermal Ablation Of Glioma: An In Vitro Model, Amani Riad Makkouk
UNLV Theses, Dissertations, Professional Papers, and Capstones
The current median survival of patients with glioblastoma multiforme (GBM), the most common type of glioma, remains at 14.6 months despite multimodal treatments (surgery, radiotherapy and chemotherapy). This research aims to study the feasibility of photothermal ablation of glioma using gold nanoshells that are heated upon laser irradiation at their resonance wavelength. The novelty of our approach lies in improving nanoshell tumor delivery by loading them in macrophages, which are known to be recruited to gliomas via tumor-released chemoattractive agents. Ferumoxides, superparamagnetic iron oxide (SPIO) nanoparticles, are needed as an additional macrophage load in order to visualize macrophage accumulation in …
Bioelectric Applications For Treatment Of Melanoma, Stephen J. Beebe, Karl H. Schoenbach, Richard Heller
Bioelectric Applications For Treatment Of Melanoma, Stephen J. Beebe, Karl H. Schoenbach, Richard Heller
Bioelectrics Publications
Two new cancer therapies apply bioelectric principles. These methods target tumor structures locally and function by applying millisecond electric fields to deliver plasmid DNA encoding cytokines using electrogene transfer (EGT) or by applying rapid rise-time nanosecond pulsed electric fields (nsPEFs). EGT has been used to locally deliver cytokines such as IL-12 to activate an immune response, resulting in bystander effects. NsPEFs locally induce apoptosis-like effects and affect vascular networks, both promoting tumor demise and restoration of normal vascular homeostasis. EGT with IL-12 is in melanoma clinical trials and nsPEFs are used in models with B16F10 melanoma in vitro and in …
Automatic Exposure Control And Estimation Of Effective System Noise In Diffuse Fluorescence Tomography, Dax L. Kepshire, Hamid Dehghani, Frederic Leblond, Brian W. Pogue
Automatic Exposure Control And Estimation Of Effective System Noise In Diffuse Fluorescence Tomography, Dax L. Kepshire, Hamid Dehghani, Frederic Leblond, Brian W. Pogue
Dartmouth Scholarship
A diffuse fluorescence tomography system, based upon time-correlated single photon counting, is presented with an automated algorithm to allow dynamic range variation through exposure control. This automated exposure control allows the upper and lower detection levels of fluorophore to be extended by an order of magnitude beyond the previously published performance and benefits in a slight decrease in system effective noise. The effective noise level is used as a metric to characterize the system performance, integrating both model-mismatch and calibration bias errors into a single parameter. This effective error is near 7% of the reconstructed fluorescent yield value, when imaging …
Statistical Hypothesis Testing For Postreconstructed And Postregistered Medical Images, Eugene Demidenko
Statistical Hypothesis Testing For Postreconstructed And Postregistered Medical Images, Eugene Demidenko
Dartmouth Scholarship
Postreconstructed and postregistered medical images are typically treated as the raw data, implicitly assuming that those operations are error free. We question this assumption and explore how the precision of reconstruction and affine registration can be assessed by the image covariance matrix and confidence interval, called the confidence eigenimage, using a statistical model-based approach. Various hypotheses may be tested after image reconstruction and registration using classical statistical hypothesis testing vehicles: Is there a statistically significant difference between images? Does the intensity at a specific location or area of interest belong to the “normal” range? Is there a tumor? Does the …
Investigation Of Magnetic Resonance Imaging And Spectroscopy For The Detection Of Breast Cancer, Robert Thomas Etnire
Investigation Of Magnetic Resonance Imaging And Spectroscopy For The Detection Of Breast Cancer, Robert Thomas Etnire
UNLV Theses, Dissertations, Professional Papers, and Capstones
Magnetic resonance imaging (MRI) of the breast offers an alternative to screening mammography which may benefit those women at high risk for breast cancer, women under the age of 40, and those with dense breast tissue. One concern with MRI is the number of high false positives. Coupling MRI with magnetic resonance spectroscopy(MRS) may lower the number of false positives, and thus improve the diagnostic capabilities of MRI for the clinician. MRS for breast imaging focuses on the total choline containing compounds in the spectra in the suspected breast lesion to analyze areas of concern. The results of the study …

Clinical MRI images are generally corrupted by random noise during acquisition with blurred subtle structure features. Many denoising methods have been proposed to remove noise from corrupted images at the expense of distorted structure features. Therefore, there is always compromise between removing noise and preserving structure information for denoising methods. For a specific denoising method, it is crucial to tune it so that the best tradeoff can be obtained. In this paper, we define several cost functions to assess the quality of noise removal and that of structure information preserved in the denoised image. Strength Pareto Evolutionary Algorithm 2 (SPEA2) …

Prostate cancer is the most common type of cancer and the second leading cause of cancer death among men in US1. Quantitative assessment of prostate histology provides potential automatic classification of prostate lesions and prediction of response to therapy. Traditionally, prostate cancer diagnosis is made by the analysis of prostate-specific antigen (PSA) levels and histopathological images of biopsy samples under microscopes. In this application, we utilize a texture analysis method based on the run-length matrix for identifying tissue abnormalities in prostate histology. A tissue sample was collected from a radical prostatectomy, H&E fixed, and assessed by a pathologist …

Combined Magnetic Resonance (MR) and Near Infrared Spectroscopy (NIRS) has been proposed as a unique method to quantify hemodynamics, water content, and cellular size and packing density of breast tumors, as these tissue constituents can be quantified with increased resolution and overlaid on the structural features identified by the MR. However, the choices in how to reconstruct and visualize this information can have a dramatic impact on the feasibility of implementing this modality in the clinic. This is especially true in 3 dimensions, as there is often limited optical sampling of the breast tissue, and methods need to accurately reflect …

During the last few decades of the twentieth century, medical imaging has been playing a prominent role in many fields of biomedical research and clinical practice. Image modalities such as x-rays, computed tomography (CT), and magnetic resonance images (MRI) have all been valuable additions to the radiologist's arsenal of imaging tools. Medical images assure quality diagnosis and patient safety by gathering valuable information without invading the human body. Apart from clinical diagnosis, medical images are used as tools for education where they are used for training individuals before operating on a patient. Many medical educators tum to simulation based training …

The detection, recognition and classification of features in a digital image is an important component of quality control systems in production and process engineering and industrial systems monitoring, in general. In this paper, a new pattern recognition system is presented that has been designed for the specific task of monitoring the quality of sheet-steel production in a rolling mill. The system is based on using both the Euclidean and Fractal geometric properties of an imaged object to develop training data that is used in conjunction with a supervised learning procedure based on the application of a fuzzy inference engine. Thus, …

This paper discusses a new approach to the processes of object detection, recognition and classification in a digital image. The classification method is based on the application of a set of features which include fractal parameters such as the Lacunarity and Fractal Dimension. Thus, the approach used, incorporates the characterisation of an object in terms of its texture.
The principal issues associated with object recognition are presented which includes two novel fast segmentation algorithms for which C++ code is provided. The self-learning procedure for designing a decision making engine using fuzzy logic and membership function theory is also presented and …
