Our objective is to determine if the structure of Graduate Medical Education teaching clinics is associated with how well ophthalmology residents are prepared to meet the workload demands of independent clinical practice. Resident preparedness to enter independent practice was measured by the Readiness Index. Part of the Department of Veterans Affairs’ new Workload-based Resident Academic Performance measures (WRAP), resident readiness is computed from electronic health records for residents by clinic and service-date. The index compares resident productivity net of supervision and adjusted for care quality to the average productivity of non-supervising ophthalmologists. Background: Postpartum depression (PPD) significantly impacts mothers and fathers, with paternal PPD (PPPD) affecting 28.3% of fathers in Pakistan. PPPD exacerbates maternal symptoms and disrupts family dynamics but remains underdiagnosed due to cultural stigmas and gender-specific presentations, such as aggression and stress. Socio-economic challenges, marital dissatisfaction, and limited support further heighten PPPD risks. Mobile health (mHealth) technologies offer accessible, stigma-free solutions for mental health support. This study explores PPPD health needs and evaluates an mHealth intervention for improving detection, support, and relationship outcomes among postpartum couples in Pakistan.

Delayed cord clamping (DCC) benefits mothers and infants by improving iron levels and neurodevelopmental outcomes while reducing neonatal complications. It is a crucial intervention, especially where iron-rich foods are scarce. Despite its advantages, DCC is often neglected due to medical complications, inconsistent policies, and clinical time constraints. Promoting DCC requires comprehensive education, clear protocols, interdisciplinary collaboration, and adequate skilled staffing. This paper emphasizes the importance of consistent DCC practices to enhance maternal and infant health and urges healthcare providers and parents to prioritize DCC in birth plans.

Background: This study identified major determinants of Cesarean-sections and the outcomes in one referral center.
Methods: A retrospective study was conducted at a tertiary healthcare facility and all women who had cesarean section between January 2020 to July 2021 were enrolled. A chi-square or independent t-test was used to compare maternal and pregnancy characteristics, and neonatal outcomes between emergency c-sections (EmCS) and elective c-sections (ElCS) groups. A multivariable logistic regression analysis compared indications and outcomes between the two groups.

Houge-Janssens syndrome 1 is a rare autosomal dominant disorder with approximately 100 case reports worldwide. Here, we present a case of a Pakistani toddler who presented with epilepsy and developmental delay. Family history was significant with reported and documented developmental delays in both father and elder sister, with subsequent improvement. Trio-whole-genome sequencing (WGS) and Sanger sequencing were performed, revealing a heterozygous LP variant NM_006245.4: c.626A>C, p.(His209Pro) in the girl. High Dose Methylprednisolone In Children With Severe Headache In Viral Encephalitis: A Case Series, Abia Abdullah, Jay Kumari, Prem Chand

Severe headache is a common symptom in children with viral encephalitis (VE), yet effective symptom-specific therapies are limited. In this case series, we present three children of ages 15, 14, and seven diagnosed with VE who presented with severe headaches unresponsive to standard analgesics but showed significant clinical improvement following high-dose methylprednisolone therapy. Although this is not first line treatment, these cases suggest a possible role for methylprednisolone in alleviating severe VE-associated headaches. Further controlled studies are needed to establish its effectiveness and safety in such settings.

Enterovirus is a member of the Picornaviridae family, an enterovirus. It is suspected of causing a polio-like disorder called acute flaccid myelitis (AFM). This case report aims to present a 10-year-old boy who presented with neurological manifestations of weakness of lower limbs and urinary retention. He had reduced tone, absent reflexes, and palpable bladder. MRI spine revealed inflammation in grey matter of spinal cord. He was treated with high dose steroids and IVIG but no improvement was observed. Enterovirus was isolated from his stool sample. Hyperbaric oxygen therapy (HBOT) can accelerate the regeneration of skeletal muscle after injury and improve overall recovery. Certain cells of the spleen are increased during muscle regeneration; however, the effect of HBOT on splenic cells during muscle regeneration is not well understood. In other experimental models, HBOT proliferates the proportion of splenic cells. In this study, eight male and nine female mice sustaining crush muscle injuries underwent HBOT at 1.5 atmospheres absolute for 60 min or normobaric hyperoxia (100% O₂) for 60 min within 2-6 hours of injury for 4 days. There are many ways to propel a wheelchair, including four main propulsion patterns. However, there are no definitive criteria for a “correct” form. This study investigated differences in wheelchair propulsion between experienced and inexperienced wheelchair users to determine any differences in propulsion patterns, joint kinematics, or speed between the two groups, indicating what contributes to a more mature form. Subjects were recorded pushing a wheelchair for twenty feet, then each video was analyzed using Kinovea, a software for tracking body movements. Research in the Weidanz laboratory is focused on cellular molecules which inhibit immune activation, known as immune checkpoints. One example is HLA-E, which can modulate immune responses at homeostasis or in the context of cancer. HLA-E typically acts by presenting short canonical peptides (VL9s) and binding to CD94/NKG2A receptors on NK cells and T cells. The Weidanz laboratory’s research has found that in the absence of these VL9 peptides, HLA-E binds to another immune checkpoint, VISTA, instead of CD94/NKG2A.
