Using Adult Learning Characteristics And The Humanities To Teach Undergraduate Healthcare Students About Social Determinants Of Health, Elizabeth A. Brown, Hannah Kinder, Garrett Stang, Wendy Shumpert
Community & Environmental Health Faculty Publications
Authors used an andragogy framework to help undergraduate allied health students better understand social determinants of health (SDOH) using a photo essay assignment. The study examined students’ perceptions of SDOH in various communities, description of health outcomes associated with their chosen SDOH, and lessons learned and suggestions to improve the assignment for future cohorts. Data were extracted from photo essays from 2019–2021 and entered in Microsoft Excel and Word for data analysis after course completion. Conventional qualitative content analysis was used to analyze student evaluation data from open-ended questions. Data were extracted from 53 student essays from 2019 to 2021. …

วัตถุประสงค์: ผู้ป่วยโรคกรดไหลย้อนบางราย แม้จะได้รับการรักษาด้วยยายับยั้งโปรตอนปั๊มขนาดมาตรฐาน ก็ยังมีอาการของกรดไหลย้อนตอนกลางคืน ซึ่งงานวิจัยนี้มีวัตถุประสงค์ เพื่อศึกษาเปรียบเทียบประสิทธิภาพของการนอนยกศีรษะสูงโดยใช้หมอนรูปลิ่มร่วมไปกับการรักษาด้วยยายับยั้งโปรตอนปั๊มขนาดมาตรฐาน กับการรักษาด้วยยายับยั้งโปรตอนปั๊มขนาดสองเท่าวันละสองเวลาในการรักษาผู้ป่วยโรคกรดไหลย้อนที่มีอาการตอนกลางคืน รวมถึงเพื่อศึกษาความพึงพอใจของการใช้หมอนรูปลิ่ม วิธีการวิจัย: ผู้วิจัยคัดเลือกผู้ป่วยที่ยังมีอาการของกรดไหลย้อนตอนกลางคืนมากกว่าหรือเท่ากับ 2 ครั้งต่อสัปดาห์ แม้จะได้รับยายับยั้งโปรตอนปั๊มขนาดมาตรฐานมาแล้วอย่างน้อย 4 สัปดาห์ โดยมีเกณฑ์ในการคัดเลือกผู้ป่วยออกจากโครงการวิจัย เช่น ผู้ป่วยที่มีภาวะอุดกั้นหายใจขณะหลับอย่างรุนแรง โรคอ้วน มีความผิดปกติของโรคของการทำงานของหลอดอาหารที่ผิดปกติรุนแรง หลังจากนั้นจะสุ่มผู้เข้าวิจัยเป็น 2 กลุ่ม คือ กลุ่มที่รักษาด้วยการนอนยกศีรษะสูงโดยใช้หมอนรูปลิ่มขนาดความสูง 20 เซนติเมตรร่วมไปกับการรักษาด้วยยายับยั้งโปรตอนปั๊มขนาดมาตรฐาน (กลุ่ม wedge pillow) กับกลุ่มที่รักษาด้วยยายับยั้งโปรตอนปั๊มขนาดสองเท่าวันละสองเวลา (กลุ่ม twice-daily PPI) เป็นระยะเวลา 4 สัปดาห์ เพื่อศึกษาผลของการรักษาต่ออาการกรดไหลย้อนตอนกลางคืน โดยประเมินจากแบบสอบถาม NGSSIQ ที่สัปดาห์ 0, 2 และ 4 รวมทั้งผลของการรักษาต่อคุณภาพชีวิตและคุณภาพการนอนหลับ นอกจากนี้ยังประเมิน parameter จากการวัดค่าความเป็นกรดในหลอดอาหารและกระเพาะอาหาร 24 ชั่วโมงที่สัปดาห์เริ่มต้นและสัปดาห์ที่ 4 โดยใช้ electronic pressure sensor timekeeper ในการตรวจ compliance ของอาสาสมัคร และเพื่อศึกษาความพึงพอใจของการใช้หมอนรูปลิ่ม ซึ่งจะประเมินจาก GPE scale ผลการศึกษา: ผู้ป่วยเข้าร่วมโครงการวิจัยมีทั้งหมด 24 ราย (ชาย:หญิง 7:17, อายุเฉลี่ย 57±13 ปี) ผลการศึกษาพบว่าทั้งสองกลุ่มมีระดับคะแนน NGSSIQ ลดลงอย่างมีนัยสำคัญ ณ สัปดาห์ที่ 2 และ 4 เมื่อเทียบกับค่าเริ่มต้น (p<0.05) ) แต่ไม่พบความแตกต่างกันของคะแนน NGSSIQ ที่สัปดาห์ที่ 4 ระหว่างสองกลุ่ม เมื่อสิ้นสุดสัปดาห์ที่ 4 พบว่า ในกลุ่ม wedge pillow มีประสิทธิภาพในการเพิ่มคุณภาพการนอนหลับและช่วยลดระยะเวลาในการเคลื่อน ลงของอาหารในหลอดอาหาร (bolus contact time) ในเวลากลางคืนได้ดีกว่ากลุ่ม twice-daily PPI อย่างมีนัยสำคัญ และ โดยผู้ป่วยจำนวน 10 จาก 12 รายในกลุ่ม wedge pillow (ร้อยละ 83.3) รู้สึกพึงพอใจต่อหมอนรูปลิ่ม สรุปผล: การรักษาทั้ง 2 วิธีต่างก็มีประสิทธิภาพในการลดอาการกรดไหลย้อนตอนกลางคืนได้อย่างมีนัยสำคัญไม่แตกต่างกัน แต่อย่างไรก็ตาม วิธีการนอนยกศีรษะสูงโดยใช้หมอนรูปลิ่มร่วมไปกับการรักษาด้วยยายับยั้งโปรตอนปั๊มขนาดมาตรฐานก่อนอาหารเช้า มีประสิทธิภาพในการเพิ่มคุณภาพการ นอนหลับ และการลด bolus contact time ในเวลากลางคืนดีกว่าวิธีการรักษาด้วยยายับยั้งโปรตอนปั๊มขนาดสองเท่าวันละสองเวลา นอกจากนี้อาสาสมัครมี ความพึงพอใจต่อการนอนยกศีรษะสูงโดยใช้หมอนรูปลิ่ม และยินดีที่จะใช้หมอนนี้ในระยะยาว ดังนั้นการนอนยกศีรษะสูงโดยใช้หมอนรูปลิ่มจึงสามารถใช้เป็น ตัวเลือกในการรักษาผู้ป่วยโรคกรดไหลย้อนที่มีอาการตอนกลางคืน

Given the widespread popularity of mobile health technology nowadays, conducting health education and behaviour change intervention through Facebook groups is a simple and efficient way to promote exclusive breastfeeding. However, limited studies have been investigating its effectiveness. This study aimed to assess the effectiveness of Facebook Breastfeeding Support Group on exclusive breastfeeding among Myanmar migrant mothers in Samut Sakhon Province, Thailand. A 2-arm, parallel-group, randomized controlled trial was conducted with 198 pregnant mothers between 28th and 34th weeks of gestation at the Antenatal Care clinic of Samut Sakhon Hospital. This in vitro study aimed to investigate the effect of different chromium metal alloys on enamel wear through longitudinal observation across various simulated chewing cycles. Four groups of alloys were studied (n=10 per group): casting Ni-Cr alloys (NC), casting Co-Cr alloys (CC), SLM Co-Cr alloys (CS), and heat-treated SLM Co-Cr alloys (CS-H). The test simulated chewing of enamel cusps against the alloys in distilled water under specific conditions (49 N load, 0.7 mm sliding movement, and 1.6 Hz frequency). Pituitary neuroendocrine tumors (PitNETs) are tumors of the pituitary gland. Although most are benign, they can cause severe morbidity if compression of surrounding tissue and/or endocrinopathies occur.
Aggressive PitNETs are notably detrimental and difficult to predict, and their effects are further exacerbated by challenges in treatment. Although histological studies can detect certain markers of tumor aggressiveness, they are insufficient at wholly predicting PitNET aggressiveness, making the clinical behavior of PitNETs challenging to determine. Since treatment of aggressive tumors also remains suboptimal, this further results in negative impacts on health and quality of life.

Background: Delay in diagnosis and access to specialist care is a major problem for many children and young people with rheumatic disease in sub-Saharan Africa. Most children with symptoms of rheumatic disease present to nonspecialists for care. There is an urgent need to understand and scale-up paediatric rheumatology knowledge and skills amongst non-specialist healthcare workers to promote early diagnosis, prompt referral, and management.

Knee osteoarthropathy is a common degenerative joint disorder that can cause pain and functional impairment in affected individuals. Various intra-articular injection therapies have been explored as potential treatments for knee osteoarthropathy, including corticosteroid, hyaluronic acid, dextrose and platelet-rich plasma. Intra-articular corticosteroid injection is commonly used for short-term pain relief. Patients with higher baseline pain level and milder changes of degeneration on imaging may benefit more from corticosteroid injection. However, excessive use may lead to detrimental effects on joint tissues. Shoulder pain has many etiologies, with the most common causes being the shoulder joint itself and the surrounding soft tissues, such as tendons, bursae, or ligaments. However, rare pathologies can occur. This report presents a case of chronic right shoulder pain eventually diagnosed as a rare case of extra-skeletal osteosarcoma. The referred right shoulder pain was likely caused by tumor invasion of the pericardium, whose sensory innervation is provided by the phrenic nerve. Although calcaneus fractures may be the most common pediatric tarsal bone fracture, most occult calcaneus fractures are not visible on plain radiography, resulting in delayed diagnosis. However, ultrasonography can aid early diagnosis of this fracture with an unspecified mechanism of injury and unremarkable radiographic findings. In a pediatric patient with progressive ankle swelling and pain, initial plain radiography was unremarkable, and antibiotics were administered for suspected infective arthritis. After limping developed, ultrasonography showed symmetric growth plates of bilateral distal tibia, fibula, and tarsal bone; a step-off calcaneus cortex with periosteal reaction; increased power Doppler flow; and probe tenderness, indicating a …

Lipomas are benign tumors that are common in subcutaneous tissue and are usually asymptomatic. However, intramuscular lipomas, which are less common, can cause nerve compression in rare cases. Herein, we report the case of a 61-year-old man with an intramuscular lipoma on his right forearm. The lipoma entrapped the deep branch of the radial nerve and caused wrist drop and finger drop. A nerve conduction examination and electromyography were conducted to quantify the degree of and localize the nerve injury. Despite technological advances in medicine and public health innovations, maternal mortality in the United States remains significantly higher than in other developed countries. Specifically, Georgia ranks as the second worst place to give birth in the country. Maternal mental health conditions are common, manageable complications that can happen at any point during pregnancy, childbirth, or in the first year following delivery, with 1 in 5 women affected. Unfortunately, 75% of women who experience MMH symptoms are left untreated.

Chondrosarcomas are the second most common cause of bone cancer and are removed surgically with wide margins. On recurrence, they are resistant to chemo and radiation therapy and new treatment options are critically required. This tumor type produces hyaline cartilage, a cartilage normally formed under hypoxic and acidic environment due to lack of vasculature in cartilage. Paradoxically, chondrosarcomas arise in the well vascularized, oxygen rich environment of the bone. Hypoxia and acidosis are two stressors where the cellular effects are typically reported separately even though cells experience combined effects of hypoxia and acidosis. Acute respiratory distress syndrome (ARDS) is a severe respiratory condition characterized by increased lung permeability, hyper-inflammatory state, and fluid leak into the alveolar spaces. ARDS is a heterogeneous disease, with multiple direct and indirect causes that result in a mortality of up to 40%. Due to the ongoing Covid-19 pandemic, its incidence has increased up to ten-fold. Extracellular vesicles (EVs) are small liposome-like particles that mediate intercellular communication and play a major role in ARDS pathophysiology. Nanodiamonds (ND) have attracted significant interest for their use in several biomedical applications. These applications can be very useful if the safety and compatibility of ND are proven. We assessed the effects of ND (100 nm, Carboxylated) on primary macrophages and a macrophage-like cell line and found that these particles are not toxic to these cells at lower concentrations but may interfere with cell functions and differentiation. Internalization of ND by these cells in a time- and dose-dependent manner was mostly via phagocytosis and clathrin-dependent endocytosis and localized to the cytoplasm but not into the nucleus. Cancer ablation therapies aim to be efficient while minimizing damage to healthy tissues. Nanosecond pulsed electric field (nsPEF) is a promising ablation modality because of its selectivity against certain cell types and reduced neuromuscular effects. We compared cell killing efficiency by PEF (100 pulses, 200 ns–10 µs duration, 10 Hz) in a panel of human esophageal cells (normal and pre-malignant epithelial and smooth muscle). Normal epithelial cells were less sensitive than the pre-malignant ones to unipolar PEF (15–20% higher LD50, p < 0.05). Smooth muscle cells (SMC) oriented randomly in the electric field were more sensitive, with 30–40% lower LD50 (p < 0.01). Trains of ten, 300-ns pulses at 10 kV/cm caused twofold weaker electroporative uptake of YO-PRO-1 dye in normal epithelial cells than in either pre-malignant cells or in SMC oriented perpendicularly to the field. Aligning SMC with the field reduced the dye uptake fourfold, along with a twofold reduction in Ca2+ transients. Obstructive sleep apnea (OSA) is an increasingly recognized condition affecting patient health and public welfare. While positive airway pressure (PAP) therapy has remained the preferred treatment for OSA since 1981, PAP therapy adherence rates have also remained stubbornly suboptimal. Measures for improving PAP therapy adherence have been under constant study, and current literature demonstrates telemonitoring interventions to be promising. Furthermore, the American Academy of Sleep Medicine (AASM) suggests that telemonitoring interventions be integrated into standard practice to support patients as they initiate and acclimate to PAP therapy. The current standard of care for patients undergoing endoscopic sinus surgery is verbal preoperative education at the office visit supplemented with printed material. Introduction/Hypothesis Young children with scald burns experience severe complications compared to older children. A paucity of data exists to support these observations. This study compares clinical features and complications of younger versus older pediatric patients with scald burns at four children’s hospitals participating in the Pediatric Injury Quality Improvement Collaborative (PIQIC). This collaborative includes five pediatric burn centers that utilize research to improve quality of burn care. 485 Methods This is a retrospective cohort study of scald burn patients admitted from 2010 to 2020 to four pediatric hospitals participating in PIQIC. Patients with total body surface area (TBSA) < 15% were excluded. Patients were categorized by age into three groups: toddlers (0–3 years), children (3-10 years), and adolescents (>10 years). …

The innate immune system is the first line of defense against external threats through the initiation and regulation of inflammation. Macrophage differentiation into functional pheno- types influences the fate of nanomaterials taken up by these immune cells. High-resolution electron microscopy was used to investigate the uptake, distribution, and biotransformation of nanoceria in human and murine M1 and M2 macrophages in unprecedented detail. We found that M1 and M2 macrophages internalize nanoceria differently. M1-type macrophages predominantly sequester nanoceria near the plasma membrane, whereas nanoceria are more uniformly distributed throughout M2 macrophage cytoplasm. Growing up in an urban food desert can significantly affect children's development, health, and well-being (Jencks et al., 1990; Leventhal et al., 2000). Compared to their more affluent peers, youth living in low-socioeconomic urban neighborhoods are at greater risk of experiencing poor-quality diets, food insecurity, unhealthy body weights, and mental health problems (Duncan et al., 1997; Sampson et al., 1997). Framed by the social identity theory of leadership, one question that is beginning to receive attention is how intergroup leaders can lead across distinct subgroups and improve inter-subgroup relations without provoking social identity-related concerns (e.g., subgroup identity distinctiveness threat). Past studies have found that leaders can use their rhetoric and boundary spanning behavior to meet their members’ identity needs and garner support. In addition, the self-uncertainty literature has suggested that leaders can strategically elevate and resolve members’ self-uncertainty through their rhetoric. Whether this is due to the community’s geographical (remote) location, lack of education, or other factors, the members of these communities experience disproportionate levels of care. My focus with this project will be the idea that everyone deserves adequate healthcare, regardless of where they live.
