Tanzania Journal of Science

This study aimed to compare the dose distributions of three-dimensional conformal treatment planning using two and six radiation beams among paediatric patients receiving radiotherapy for Wilms tumour at Ocean Road Cancer Institute. CT scans of 53 patients were used to generate 106 treatment plans, 53 plans for two beams, and 53 plans for six beams. Planning target volumes and organs at risk parameters were compared between both plans using paired samples t-test.  A p-value < 0.05 was considered statistically significant. The findings showed that the D95% and Dmean of the target are better with six beams than with the two-beam plans (p ˂ 0.001). The conformity index and monitor unit are significantly better with two-beam plans than with six-beam plans (p ˂ 0.001). No volume of the contralateral kidney received more than 12 Gy for both two and six plans. There were no differences in Dmax for the spinal cord (p = 0.208), while Dmean was observed to be lower in two compared to six beams (p ˂ 0.001) for the liver. Six beam plans have good target coverage, while two beam plans have good conformity index and monitor unit. The observed doses to the organs at risk in both plans were lower than their normal tolerance. Leiomyosarcoma (LMS) is a rare, aggressive mesenchymal tumor with smooth muscle differentiation. LMS is one of the most common histologic subtypes of soft tissue sarcoma; it most frequently occurs in the extremities, retroperitoneum, or uterus. LMS often demonstrates aggressive tumor biology, with a higher risk of developing distant metastatic disease than most sarcoma histologic types. The prognosis is poor, particularly in patients with uterine disease, and there is a need for the development of more effective therapies. Genetically, LMS is karyotypically complex and characterized by a low tumor mutational burden, with frequent alterations in TP53, RB1, PTEN, …

Etoposide (ETO) is an anticancer drug that targets topoisomerase II (TOP2). It stabilizes a normally transient TOP2-DNA covalent complex (TOP2cc), thus leading to DNA double-strand breaks (DSBs). Tyrosyl-DNA phosphodiesterases two (TDP2) is directly involved in the repair of TOP2cc by removing phosphotyrosyl peptides from 5'-termini of DSBs. Recent studies suggest that additional factors are required for TOP2cc repair, which include the proteasome and the zinc finger protein associated with TDP2 and TOP2, named ZATT. ZATT may alter the conformation of TOP2cc in a way that renders the accessibility of TDP2 for TOP2cc removal. In this study, our genome-wide clustered regularly …

INTRODUCTION: Standard-of-care systemic chemotherapies for pancreatic ductal adenocarcinoma (PDAC) currently have limited clinical benefits, in addition to causing adverse side effects in many patients. One factor known to contribute to the poor chemotherapy response is the poor drug diffusion into PDAC tumors. Novel treatment methods are therefore drastically needed to improve targeted delivery of treatments. Here, we evaluated the efficacy of the 3DNA® Nanocarrier (3DNA) platform to direct delivery of therapeutics to PDAC tumors in vivo.

MATERIALS AND METHODS: A panel of PDAC cell lines and a patient tissue microarray were screened for established tumor-specific proteins to identify targeting moieties …

Sepsis is a life-threatening organ dysfunction triggered by a dysregulated host immune response to eliminate an infection. After the host immune response is activated, a complex, dynamic, and time-dependent process is triggered. This process promotes the production of inflammatory mediators, including acute-phase proteins, complement system proteins, cytokines, chemokines, and antimicrobial peptides, which are required to initiate an inflammatory environment for eliminating the invading pathogen. The physiological response of this sepsis-induced systemic inflammation can affect blood-brain barrier (BBB) function; subsequently, endothelial cells produce inflammatory mediators, including cytokines, chemokines, and matrix metalloproteinases (MMPs) that degrade tight junction (TJ) proteins and decrease BBB …

Exploring Communication In Healthcare Through Decision-Making Interventions And A Novel Measure Of Clinician Spoken Plain Language, Renata W. Yen

Dartmouth College Ph.D Dissertations

Statement of the Problem

Good communication in clinical encounters is a key component of quality healthcare. Improving clinician communication would benefit all populations, however patients with lower health literacy or who are socially disadvantaged stand to benefit the most due to the complex content in clinical encounters. Shared decision-making can improve outcomes, but shared decision-making cannot occur without good clinician communication. In this dissertation, I reviewed interventions for facilitating shared decision-making for socially disadvantaged patients. Then, I examined the use of spoken plain language, or clinician use of familiar, clear verbal language when communicating with patients, and developed a prototype …

Hyperglycemia, which is a chronic metabolic condition caused by either a defect in insulin secretion or insulin resistance, is a hallmark of diabetes mellitus (DM). Sustained hyperglycemia leads to the onset and development of many health complications. Despite the number of available antidiabetic medications on the market, there is still a need for novel treatment agents with increased efficacy and fewer adverse effects. Many medicinal plants offer a rich supply of bioactive compounds that have remarkable pharmacological effects with less toxicity and side effects. According to published evidence, natural antidiabetic substances influence pancreatic β-cell development and proliferation, inhibit pancreatic β-cell …

The leukocyte NADPH oxidase 2 (NOX2) regulates inflammation independent of its antimicrobial activity. Inherited defects in NOX2 lead to chronic granulomatous disease (CGD), associated with recurrent bacterial and fungal infections, often with excessive neutrophilic inflammation that results in significant inflammatory burden and tissue damage. We previously showed that excessive leukotriene B4 (LTB4) production by NOX2-deficient mouse neutrophils was a key driver of elevated lung neutrophil infiltration in the initial response to pulmonary challenge with the model fungal particle zymosan. We now identify interleukin-1β (IL-1β) and downstream granulocyte colony-stimulating factor (G-CSF) as critical amplifying signals that augment and sustain neutrophil accrual …

The Benefits Of Using Patient Transfer Devices In The Emergency Department, Abby Lawhorn, Mikaylen Dearmond

Scholars Week

The Emergency Department (ED) is known as one of the most fast-paced environments in which nurses work. Because of the nature of their responsibilities and daily tasks, ED nurses are at high risk of injury. One of those tasks that occur multiple times throughout the day is patient transfers. Three main studies that implemented a 24 hour or later bathing program were chosen and reviewed to assess for trends in hypothermia, hypoglycemia, and the exclusive breastfeeding rates of newborns.

Results: In each study reviewed, the rates of hypoglycemia and hypothermia decreased while the rates of exclusive breastfeeding increased.

Implications: By delaying newborn …
