The need for social distancing due to the COVID-19 pandemic has compromised traditional medical education. Educators and students in low-resource settings face diverse challenges in clinical teaching, learning, and assessment. The pandemic has necessitated a rapid transition from traditional pedagogical strategies to virtual platforms. Challenges in low resource settings include knowledge and training in technology, limited resource allocation for faculty development in virtual teaching methods, and financial constraints limiting the use of available technologies. There is also a lack of constructive alignment between outcomes, instructional strategies, and assessment of curriculum delivery in virtual platforms as the earlier curriculum design was …

In the course of a routine dissection class for medical students, it was observed that there was no oblique fissure, cardiac notch, and lingula in an embalmed cadaver. Due to their absence, the superior and inferior lobes of this lung could not be distinguished clearly. The lobes were attached together, however, there were no structural variations in its hilum. No variation was found in the fissures and hilum of the right lung in this cadaver. We opine that this variation can be helpful to cardiothoracic surgeons, mainly during the resection of the lobe of the left lung.

Solitary Plasmacytoma Of Manubrium Sterni - A Case Report, Shyamasunder N. Bhat, Sp Mohanty
Manipal Journal of Medical Sciences
Solitary plasmacytoma is a tumour consisting of neoplastic monoclonal plasma cells without systemic involvement. It constitutes about 5% of all plasma cell dyscrasias and is mainly detected in the spine, pelvis and ribs and rarely in the sternum.
Here we report a case of a 45-year-old man presenting with pain and swelling in the midline of the chest over manubrium sterni. Plain radiograph and CT scan revealed an expansile, osteolytic lesion in the manubrium sterni. There was no M Band in protein electrophoresis, and the iliac bone marrow aspirate did not show plasmacytoma. However, histopathology of the lesion was suggestive …

Genome-wide association studies (GWAS) have identified immune-related genes as risk factors for Alzheimer’s disease (AD), including TREM2 and CD33, frequently passing a stringent false-discovery rate. These genes either encode or signal through immunomodulatory tyrosine-phosphorylated inhibitory motifs (ITIMs) or activation motifs (ITAMs) and govern processes critical to AD pathology, such as inflammation and amyloid phagocytosis. To investigate whether additional ITIM and ITAM-containing family members may contribute to AD risk and be overlooked due to the stringent multiple testing in GWAS, we combined protein quantitative trait loci (pQTL) data from a recent plasma proteomics study with AD associations in a recent …

The status of fungi in the phylogenetic setup still continues to be an intriguing question in biological sciences. Having long been considered as close relatives of plants, over the years, their resemblance towards animals has been brought into the greater limelight. While the vast diversity of the group demands a separate kingdom, evolution biology coaxes us to trace back their ancestry and establish similarities between different kingdoms and as newer pieces of evidence emerge up, the question as to whether fungi are plants or animals, remains contentious.

Incidental durotomies, or dural tears, can be very difficult and time consuming to repair properly when they are encountered in confined spaces. A novel dural repair device was developed to address these situations. In this paper, the novel device was assessed against the use of traditional tools and techniques for dural repairs in two independent studies using an intricate clinical simulation model. The aim was to examine the results of the two assessments and link the outcomes to the clinical use of the novel device in the operating room. The novel device outperformed conventional techniques as measured by dural repair …

Autophagy, apoptosis, and the unfolded protein response (UPR) are fundamental biological processes essential for manifold cellular functions in health and disease. Idiopathic pulmonary fibrosis (IPF) is a progressive and lethal pulmonary disorder associated with aging that has limited therapies, reflecting our incomplete understanding. We conducted an observational study linking molecular markers of cell stress response pathways (UPR: BiP, XBP1; apoptosis: cleaved caspase-3; autophagy: LC3β) in lung tissues from IPF patients and correlated the expression of these protein markers to each subject's lung function measures. We hypothesized that changes in lung tissue expression of apoptosis, autophagy, and UPR markers correlate with …

Development of resistance to therapy in ovarian cancer is a major hinderance for therapeutic efficacy; however, new mechanisms of the resistance remain to be elucidated. NADPH oxidase 4 (NOX4) is responsible for higher NADPH activity to increase reactive oxygen species (ROS) production. In this study, we showed that higher levels of NOX4 were detected in a large portion of human ovarian cancer samples. To understand the molecular mechanism of the NOX4 upregulation, we showed that NOX4 expression was induced by HIF-1α and growth factor such as IGF-1. Furthermore, our results indicated that NOX4 played a pivotal role in chemotherapy and …

BACKGROUND: The oral glucose tolerance test (OGTT)-glucose response curves (GRCs; incessant increase, monophasic, and biphasic) reflect insulin sensitivity and β-cell function, being worse in the former and superior in the latter. Here, we examined if the OGTT-GRC pattern is worse in obese antibody (glutamic acid decarboxylase 65-kDa [GAD65] and insulinoma-associated protein-2 [IA-2])-positive (Ab
METHODS: Forty-seven obese youth, 15 Ab
RESULTS: Incessant increase OGTT-GRC is the most frequent curve type and is three-fold higher in Ab
CONCLUSION: Severe insulin deficiency, a characteristic of type 1 diabetes, seems to be related to higher prevalence of incessant increase in Ab

Obesity is a serious global health problem because of its increasing prevalence and comorbidities, but its treatments are limited. The serotonin 2C receptor (5-HT2CR), a G-protein-coupled receptor, activates proopiomelanocortin (POMC) neurons in the arcuate nucleus of hypothalamus (ARH) to reduce appetite and weight gain. However, several 5-HT analogs targeting this receptor, e.g., lorcaserin (Lor), suffer from diminished efficacy to reduce weight after prolonged administration. Here, we show that barbadin (Bar), a novel β-arrestin/β2-adaptin inhibitor, can prevent 5-HT2CR internalization in cells and potentiate long-term effects of Lor to reduce appetite and body weight in male mice. Mechanistically, …

Alternative Modality In The Treatment Of Acne Vulgaris: Low Level Laser Therapy, Rudi Chandra, Nelva K. Jusuf
Journal of General - Procedural Dermatology and Venereology Indonesia
Acne vulgaris is a chronic inflammatory disease of the pilosebaceous unit with polymorphic manifestations. The four key elements leading to the formation of acne lesions are alteration of follicular keratinization that leads to comedones, increased and altered sebum production under androgen control, follicular colonization by Propionibacterium acnes, and complex inflammatory mechanisms that involve both innate and acquired immunity. Phototherapy (light, lasers, and photodynamic therapy) has been proposed as an alternative therapeutic modality to treat acne vulgaris and is proposed to have less side effects compared to other treatment options. Recently, low-level laser (light) therapy (LLLT) which refers to the use …
