Background: Older Thai Buddhist monks are usually accustomed to a unique lifestyle that potentially relates to knee osteoarthritis (OA) such as long-standing kneeling on the hard surface. Understanding the association among pain catastrophizing, depressive symptoms, and activity of daily living (ADL) in this population with knee OA may help reduce the psychosocial problems and promote their sacred activities. The re-emergence of Zika virus (ZIKV), an arbovirus, poses a major global human health concern because of its ability to cause congenital abnormalities and neurological diseases. While many candidate vaccines and antiviral drugs are in the developmental pipeline, none have been approved for use against ZIKV infection. This dissertation describes the characterization of one vaccine and two antiviral drug candidates against ZIKV infection. A bacterial ferritin-based nanoparticle vaccine, termed zDIII-F, is designed to display multiple copies of ZIKV E protein domain III on its surface. These stable nanoparticles are shown to induce robust antibody-mediated protection against lethal ZIKV infection in …

Background: Prediabetes and Type 2 diabetes (T2DM) are major health challenges among the patient population at a family medical clinic in northwest Alabama. Poorly managed T2DM can cause complications which could decrease individuals’ quality of life and lead to mortality.
Purpose: The purpose of this project was to assess the feasibility and efficacy of a self-management intervention aimed at empowering prediabetic and T2DM patients to achieve better diabetes management and glycemic control (hbA1c < 5.7 percent).

Schizophrenia is a debilitating condition that affects 1% of the population, causes significant hardship and though there are treatments available they are characterised by several limitations. It is a complex mental disorder where some individuals show mild subclinical cognitive symptoms before psychosis onset in adolescence. The treatments available only target a portion of the symptoms and although extensive research has been conducted, a comprehensive understanding of the nature of schizophrenia remains elusive. Objective: The COVID-19 pandemic has placed a new emphasis on defining aerosol generating procedures and evaluating ways to reduce those aerosols. Both biocontaminants and particulate matter can potentially be transmitted with dental aerosols with negative health effects associated with each, especially aerosols with particulate matter of 2.5 micrometers or less (PM2.5). Objective: Conventional nickel-titanium (NiTi) archwires have widespread application in orthodontics due to their unique properties and ability to move teeth with light, continuous forces, but a shortcoming is that they apply near-uniform force along anterior and posterior teeth. Since the force needed to effectively move teeth is a function of the root surface area and bony support, these archwires cause single-rooted teeth to move more than the multi-rooted molars. Introduction: Low-magnitude, high-frequency (LMHF) mechanical vibration is proven to be anabolic and can increase alveolar bone mass. The objectives of this study were to generate an orthodontic retention cell culture model and investigate the effects of LMHF mechanical vibration on human periodontal ligament fibroblast (HPLF) cells after orthodontic loading (retention) in vitro. Materials and Methods: HPLF cells were seeded at a density of 4x105/well in 6-well Flexcell culture plates (Day 0). Purpose: To determine the number of loosening/tightening preload cycles prosthetic retaining screws can withstand before fracture, and to record the reverse torque values on each of the 10 applications of screw tightening and loosening. Materials and methods: The testing assemblies consisted of 30 temporary cylinder/multiunit abutment/internal connection implants, 10 each from Biohorizons, Zimmer Biomet Dental, and Dentsply Sirona Astra; the specimens were inserted into resin blocks. Temporary cylinders were connected to the abutments with new prosthetic screws and torqued to the appropriate preloads. A Caucasian man in early 80s was seen in Gastroenterology Clinic, following, referral from the Endocrinology Clinic for concerns for CT Abdomen requested for tiredness and weight loss of three kilograms. The patient also had microcytic picture with low MCV and Ferritin and hypomagnesemia. The CT suggested gross circumferential thickening of the wall of stomach with advice for invasive investigations to further characterise the CT findings. The Endoscopy suggested grossly enlarged rugae in the stomach, and enlarged gastric polyps. Patient was assured no new sinister abnormality. Stroke results in the death of around 6.5 million people annually with a majority of these occurring in developing countries. Serum calcium has been hypothesised to play a significant role in causing ischaemic stroke. This retrospective observational study was conducted to determine the correlation, if any, between serum calcium and the severity of acute ischaemic stroke in our population. Two hundred and seventy-nine patients admitted with acute ischaemic stroke were enrolled in the study. Of the 279 patients 162 (58%) were male and mean age was 62.4 ± 3.8 years. Cancers of the oral cavity (OC) and oropharynx (OP) account for 3% of cancers diagnosed in the United States each year. A primary cause of death among the Hispanic population in the United States is cancer, accounting for 20% of annual mortality. The Rio Grande Valley (RGV) is a medically-underserved area of South Texas with a large Hispanic population facing health disparities. Diabetic retinopathy is an ocular condition caused by a multitude of factors as a result of elevated blood glucose. One of the primary mechanisms of damage is a result of oxidative stress. It has been found that levels of IRBP in the vitreous are decreased in the setting of diabetic retinopathy, which could be associated with damage from resulting oxidative stress.

Mongooses, a nonnative species, are a known reservoir of rabies virus in the Caribbean region. A cross-sectional study of mongooses at 41 field sites on the US Virgin Islands of St. Croix, St. John, and St. Thomas captured 312 mongooses (32% capture rate). We determined the absence of rabies virus by antigen testing and rabies virus exposure by antibody testing in mongoose populations on all three islands. USVI is the first Caribbean state to determine freedom-from-rabies for its mongoose populations with a scientifically-led robust cross-sectional study. Ongoing surveillance activities will determine if other domestic and wildlife populations in USVI are …

Discard mortality can make fishing unsustainable, even in catch-and-release or highly regulated fisheries. However, fishing practices and gears that minimize hook injury, handling, and air exposure can considerably improve fitness and survival in released fish. This study tested whether modified hooks could allow anglers to successfully land and then release fish in the water and without handling. Standard, barbless, and bite-shortened hooks were used to catch Spotted Seatrout Cynoscion nebulosus (n > 75 per hook type), and logistic models were used to assess the differences in landing success and hook self-release success by hook type and categorical fish size. Clinical reasoning (CR) is defined as a complex multi-factorial metacognitive process for diagnosis formulation. Clinical reasoning begins as a student and develops over a career. Students are typically taught an analytical approach defined as hypothetico-deductive reasoning (HDR). Expert clinicians use a non-analytical approach defined as the Knowledge Based Model (KBM) of CR. It is accepted that clinicians use the KBM with cases that they have more experience to streamline the evaluation process. Unfortunately, because of the nuance of CR there have been limited investigations within athletic training to evaluate CR outside of the student population.

Clinical experiences are an essential aspect of athletic training education that offer students opportunities to practice their skills and engage in a variety of settings. The Commission on Accreditation of Athletic Training Education (CAATE) sets accreditation standards for athletic training programs to meet, including clinical education standards. In fall 2020, new standards took effect that include a few additions to the standards surrounding clinical education; these additions require programs to afford students opportunities to engage in immersive clinical experiences (ICEs), implement behaviors associated with core competencies, and identify strategies that account for patients’ social determinants of health. The practice of medicine has become the prescribing of medicine. Reflecting a construct of health defined by Rationalism, individualism, and biomedical science, medicines (pharmaceuticals) are politically constructed to be the first – and sometimes only prescribed – line of defense against illness and disease. Pharmaceuticals also represent a highly desirable, ‘recession-proof’ component of many Nation-states’ (states’) export strategies, helping advanced economies, in particular, to maintain favorable trade balances and economic growth amidst the headwinds of deindustrialization.

This review provides an update on the neurocognitive phenotype of pediatric obstructive sleep apnea (OSA). Pediatric OSA is associated with neurocognitive deficits involving memory, learning, and executive functioning. Adenotonsillectomy (AT) is presently accepted as the first-line surgical treatment for pediatric OSA, but the executive function deficits do not resolve postsurgery, and the timeline for recovery remains unknown. This finding suggests that pediatric OSA potentially causes irreversible damage to multiple areas of the brain. Objective
The objective of this theoretical clinical reasoning model is to provide a framework to help clinicians integrate linear and non-linear clinical reasoning approaches to minimize clinical reasoning errors related to logically fallacious thinking and cognitive biases.
Methods
This masterclass proposes a hypothesis-driven and probabilistic approach that uses clinical reasoning for managing LBP that seeks to eliminate the challenges related to using any single diagnostic paradigm.

Background: Evidence suggests that combining tools that gather short- and long-term dietary data may be the optimal approach for the assessment of diet–disease associations in epidemiologic studies. Online technology can reduce the associated burdens for researchers and participants, but feasibility must be demonstrated in real-world settings before wide-scale implementation. Don’t blame the technology. COVID-19 control technology depends on the cooperation of people to be fully effective, says SMU Professor Mark Findlay. Singapore has one of the best subscribed COVID-19 tracing, tracking and safe entry apps worldwide. Then how is it that we are back to Phase 2 (Heightened Alert)? It would be tempting but wrong to blame the technology as being insufficiently intrusive or robust. A look at the recent outbreak suggests we should avoid an over-reliance on tech solutions when incompatible human behaviours are at work.

Hypoxemia of the acute respiratory distress syndrome can be reduced by turning patients prone. Prone positioning (PP) is labor intensive, risks unplanned tracheal extubation, and can result in facial tissue injury. We retrospectively examined prolonged, repeated, and early versus later PP for 20 patients with COVID-19 respiratory failure. Blood gases and ventilator settings were collected before PP, at 1, 7, 12, 24, 32, and 39 h after PP, and 7 h after completion of PP. Analysis of variance was used for comparisons with baseline values at supine positions before turning prone. PP for >39 h maintained PaO2/FiO2 (P/F) ratios when …

Introduction: Tobacco use, which is directly responsible for 10% of total deaths per year globally, remains consistently high, with approximately 20% of the population reporting regular consumption globally. Moreover, health disparities regarding tobacco consumption and smoking cessation are growing between rural and urban populations worldwide. Social media interventions for tobacco cessation may effectively reach both groups. The objective of this study was to evaluate the efficacy of a WeChat-delivered smoking cessation intervention among rural and urban Chinese smokers, and to assess moderating variables that may contribute to differential intervention efficacy. Midportion Achilles tendinopathy (AT) is a chronic, painful condition of the long tendon that attaches the triceps surae muscle group (the soleus, medial and lateral gastrocnemius muscles), which are the primary plantar flexor muscles, to the calcaneus bone. There is incomplete understanding of the underlying pathophysiology, pain, and role of plantar flexor function (strength and fatigability) in functional impairment in AT. Deficits in plantar flexor muscle function are assumed, but evidence is sparce and inconclusive. The gustatory cortex (GC) has long been studied as the main cortical area encoding taste stimuli and likely integrates sensory, visceral, and emotional information to guide taste-related behaviors. However, our understanding of cortical taste coding on a single-cell level has only become clear in recent years. The anatomical location of GC on the lateral and ventral surface of the brain makes it difficult to target with traditional imaging methods. Purpose: Reducing opioid prescriptions after third molar extraction may decrease the risk of opioid dependence. This study compared prescribed morphine milligram equivalents (MMEs) in patients undergoing mandibular third molar removal with and without use of liposomal bupivacaine (LB). Methods: This retrospective cross-sectional study included deidentified data from electronic medical records of patients who underwent extraction of ≥1 partial bony—or full bony—impacted mandibular third molar at 2 oral surgery centers in the United States in 2012 or 2018.
