Objective: The purpose of this quality improvement project is to implement a best practice obstetric rapid response team (OB-RRT) and conduct in-situ drills of this team. The expected outcome is improved response times, self-confidence of OB-RRT team members, communication, and outcomes for obstetrical emergencies.
Design: The methodology for this project is a post-test descriptive design. There will be an educational session followed by drills to evaluate the effectiveness of the education.
Setting/Local Problem: Rural Level III Labor and Delivery Unit is part of a Level III Trauma Center campus hospital system in Southeastern North Carolina.

Patient Aligned Care Teams (PACTs) within the Veteran's Affairs system are required to manage national performance measures, specifically related to patients with Ambulatory Care Sensitive Conditions (ACSCs). This project focuses on the prevention and progression of renal complications for patients with diabetes by performing urine microalbumin assessments in alignment with the National Kidney Foundation's (NKF) evidence-based practice algorithm. Anticipated findings are increased microalbumin patient orders, timely completion of specimen collection, and follow up collaboration with PACTs lead medical provider, resulting in improved quality of life for patients with diabetes.

Abstract
It is well documented in the literature that breastfeeding provides a myriad of health benefits for mothers and infants across the lifespan. However, rates of exclusive breastfeeding and breastfeeding duration have remained relatively stagnant nationwide. Breastfeeding rates are particularly low for women enrolled in the Women, Infants, and Children (WIC) nutrition program. Breastfeeding intent and breastfeeding self-efficacy are maternal factors that have been shown to increase breastfeeding rates. The objective of this quality improvement initiative was to increase intent to breastfeed and enhance breastfeeding self-efficacy by implementing a text-messaging intervention for women in the WIC breastfeeding peer-counseling program to …

Acute pancreatitis results in approximately 300,000 hospital admissions in the United States each year, at a cost of approximately $2.2 billion. Necrotizing pancreatitis, the more severe form, is defined as necrosis of the pancreatic parenchyma with or without necrosis of the peripancreatic tissues. There has been an increase in readmission rates for this patient population, especially with necrotizing pancreatitis. It occurs as a complication in 20%–30% of patients with acute pancreatitis and historically has been associated with high rates of morbidity (34%–95%) and mortality. A crisis pregnancy center (CPC) is an anti-abortion organization that “counsels” pregnant individuals while pretending to be pro-choice, often giving out false or misleading medical information and discouraging sex outside of marriage. These centers are usually affiliated with evangelical Christian groups and outnumber actual abortion clinics: it is estimated by pro-life groups that over 2,500 CPCs currently operate across the United States.

Children learning General American English (GAE) show a relationship between their case marking of subject pronouns and their marking of finite verbs, and this relationship has been found for children with specific language impairment (SLI) and typically developing controls (TD). The relationship between children’s case marking of subject pronouns and finite verb marking has not been examined in children who speak dialects of English that differ from GAE. The current study was designed to assess the feasibility of creating videos depicting Enhanced Milieu Teaching (EMT) support strategies and presenting them to graduate students in a communication sciences and disorders program, and whether differences in accuracy existed between first- and second-year graduate students. EMT support strategies were chosen due to the previous successes found within the literature. Twelve videos were presented to ten graduate students. Each participant response was analyzed for accuracy and overall responses were compiled to create overall video clarity.

The study of anatomy contends that “form follows function”; a disciplinary theme purporting that anatomical structures (i.e., cells, tissues, and organs) have a shape that serves its proper function. With this in mind, it is unclear why human arterial distribution off the aortic arch is asymmetrical, while the corresponding venous anatomy is symmetrical. We investigated the evolutionary, developmental, and physiological bases for the asymmetry of aortic arch branches in humans. GABA type-A (GABA-A) receptors containing the α2 subunit (GABRA2) are expressed in most brain regions and are critical in modulating inhibitory synaptic function. Genetic variation at the GABRA2 locus has been implicated in epilepsy, affective and psychiatric disorders, alcoholism and drug abuse. Gabra2 expression varies as a function of genotype and is modulated by sequence variants in several brain structures and populations, including F2 crosses originating from C57BL/6J (B6J) and the BXD recombinant inbred family derived from B6J and DBA/2J. Among college students, flu vaccination rates are abysmal. The unique barriers students face are numerous. This effort explores the value of a multidisciplinary approach to increasing flu vaccination. To improve vaccine rates, several departments have teamed up for a multidisciplinary approach to flu education and prevention. Plans for the entire semester are in place to highlight the importance flu prevention. Events include vaccination clinics, a library exhibit, talks, movie screenings, student work, themed menu items at the student café, and a competition. DNA damages that cause double-strand breaks (DSBs) to the chromosome are most harmful. Subsequent choices have critical consequences for cell fate. Without repair, cells will face certain death. Low-fidelity repair will introduce mutations that could transform the cells, leading to carcinogenesis.
How cells make the decision is not well-understood. A single DSB can lead to apoptosis for some cells, whereas others can repair up to 25 DSBs and survive. It has been postulated that decision to repair DSBs is a stochastic process.

The ability to track animals accurately is critical for behavioral experiments. For video-based assays, this is often accomplished by manipulating environmental conditions to increase contrast between the animal and the background in order to achieve proper foreground/background detection (segmentation). Modifying environmental conditions for experimental scalability opposes ethological relevance. The biobehavioral research community needs methods to monitor behaviors over long periods of time, under dynamic environmental conditions, and in animals that are genetically and behaviorally heterogeneous. Kentucky experiences some of the nation’s worst health outcomes related to obesity, diabetes, high blood pressure, and other age-related chronic diseases linked with oxidative stress and inflammation, which in turn are associated with poor diet, lack of physical activity, and exposure to certain environmental pollutants. In the Commonwealth, deteriorating infrastructure, inappropriate waste disposal, and potential occupational injury related to mining, agriculture, and other regionally important industries exacerbate the need for residents to have basic knowledge of potential environmental health threats. Tobacco smoking was examined as a risk for dementia and neuropathological burden in 531 initially cognitively normal older adults followed longitudinally at the University of Kentucky’s Alzheimer’s Disease Center. The cohort was followed for an average of 11.5 years; 111 (20.9%) participants were diagnosed with dementia, while 242 (45.6%) died without dementia. At baseline, 49 (9.2%) participants reported current smoking (median pack-years = 47.3) and 231 (43.5%) former smoking (median pack-years = 24.5). The hazard ratio (HR) for dementia for former smokers versus never smokers based on the Cox model was 1.64 (95% CI: 1.09, 2.46), while the HR for …

Predictors for Exclusive Breastfeeding Failure: Study at Buaran Community Health Center Pekalongan Regency, Central Java, Indonesia. The failure of exclusive breastfeeding (EBF) is still a phenomenon in the community. In 2015 Pekalongan Regency had exclusive breastfeeding coverage of 30.3%. Maternal deaths in Timor Island, East Nusa Tenggara Province, are one of the contributors to the overall maternal mortality rate (MMR) of Indonesia. The MMR of Western Timor Island was150/100,000 live births in 2015. The aim of this qualitative study was to explore the perceived causes of maternal death due to postpartum hemorrhage. Data were obtained from family members, traditional birth attendants, and Posyandu cadres, as well as health providers. Preschool-aged children who are hospitalized for infusions will often exhibit a strong fearful response because their concept of body integrity has not fully developed. HMGB1 is a ubiquitously expressed intracellular protein that binds DNA and transcription factors and regulates chromosomal structure and function. Under conditions of cell death or stress, it is actively or passively released by cells into the extracellular environment, where it functions as damage-associated molecular pattern (DAMP) that orchestrates pro-inflammatory cytokine release and inflammation. Our results demonstrate that HMGB1 is secreted in the tumor microenvironment after oncolytic HSV (oHSV) infection in vitro and in vivo. Neonatal abstinence syndrome (NAS) refers to a constellation of signs occurring in newborn infants who were exposed to opioids or opiates in utero. These manifestations include poor feeding, gastrointestinal disorders, abnormal sleep patterns, and neurological signs such as jitteriness, tremors, and seizures (1, 2). Myoclonus, jitteriness, and tremors often may be interpreted as seizures and therefore treated as epileptic seizures.
Objective: To determine whether seizure like activity observed in infants with NAS correlate with electroencephalogram (EEG) findings.

BACKGROUND: Vulvovaginal candidiasis (VVC) is a common infection affecting women worldwide. Reports of patterns/risk factors/trends for episodic/recurrent VVC (RVVC) are largely outdated. The purpose of this study was to obtain current patient perspectives of several aspects of VVC/RVVC.
METHODS: Business cards containing on-line survey information were distributed to healthy volunteers and patients seeking standard, elective, or referral gynecologic care in university-affiliated Obstetrics/Gynecology clinics. The internet-based questionnaire was completed by 284 non-pregnant women (78% Caucasian, 14% African American, 8% Asian).

Background. Full practice authority for NPs is optimal for high-quality, cost-effective healthcare. However, a complete picture of utilization after states have adopted full practice authority needs to be determined. Objective. To review the evidence regarding practice-level utilization (PLU) of Nurse Practitioners (NP) PLU in comparison to state-level regulations (SLR). Data Sources. Studies published in English and based on US populations were identified through PubMed, CINAHL, and SCOPUS (January 1, 1989 - December 31, 2018), and bibliographies of retrieved articles.
