Sickle cell anemia (SCA) is an inherited autosomal recessive disorder of red blood cell caused by an abnormality of hemoglobin called sickle hemoglobin (HbS) (Centers for Disease Control and Prevention (CDC), 2017). African countries carry the highest incident of individuals with the SCA(CDC, 2017). In the United State, approximately 1 in 12 African-Americans carry the trait for SCA, and 1 of every 350 African-American infants born have the disorder (CDC, 2017). Individuals with sickle cell disease exhibit significant morbidity and mortality. Mycobacterium tuberculosis (Mtb) is an intracellular pathogen which can remain dormant within the human body for years (latent tuberculosis), and once active requires extensive multidrug therapies for treatment. Long-term antibiotic(s) exposure have led to the emergence of multidrug-resistant (MDR) and extensively drug-resistant (XDR) strains which require novel treatment approaches to combat Mtb resistance. Mtb may also utilize efflux pumps to expel antibiotics and promote bacterial survival, potentially mediating drug tolerance, which has been hypothesized to contribute to antibiotic resistance. Background
With support of the national tuberculosis (TB) program, KHANA (a local non-governmental organization in Cambodia) has implemented an innovative approach using a seed-and-recruit model to actively find TB cases in the community. The model engaged community members including TB survivors as seed and newly diagnosed people with TB as recruiters to recruit presumptive TB cases in their social network in a snowball approach for screening and linkage to treatment. This study aimed to explore the acceptability of the active case finding with the seed-and-recruit model in detecting new TB cases and determine the characteristics of successful seeds.

Sleep related infant deaths continue to be an issue in Kentucky with 95% of deaths having at least one sleep related risk factor identified. Evidence from the literature suggests safe sleep practice in the neonatal intensive care unit is inconsistent and nurses may face many barriers when implementing safe sleep. The purpose of this project was to determine current nursing knowledge and practice of safe sleep, as well as how parent education occurs in a local NICU through the use of a survey. Unplanned extubations represent a serious adverse event that occurs within neonatal intensive care units. The current literature on unplanned extubations suggests that the incidence of these events can be reduced in the clinical setting. UEs represent the primary respiratory-related adverse event in the neonatal intensive care unit (NICU) and can result in cardiopulmonary collapse requiring emergent reintubation (Carvalho, Mezzacappa, Calil, & Machado, 2010). Staff members of the University of Louisville’s neonatal intensive care unit collected data surrounding unplanned neonatal extubations from November 2017 through December 2018. Objective: This study updates Haaland’s 1999 dental hygiene mapping study. By identifying core journals and estimating database coverage, it characterizes changes in dental hygiene research and aids librarians in collection development and user education.
Method: Cited references from a three-year (2015–2017) sample of core dental hygiene journals were collected, categorized into five formats, and analyzed by format and publication year according to Bradford’s Law of Scattering. CINAHL Complete, MEDLINE, and EMBASE were surveyed to determine the indexing coverage of cited journals.
Results: The number of cited journal titles increased from 389 in 1999 to 1,675 in 2018. Core Zone 1 …

Background: High-flow nasal cannula (HFNC) is a type of non-invasive respiratory support that has decreased the rate of intubation in infants and children. It is utilized in the PICU and the wards. The innate immune system includes a menagerie of different cell types, each with a different role in the process of monitoring the body for invaders and presenting gathered debris (antigen) to the adaptive immune system. Somatic cells have intracellular receptors for the same purpose. Cancer cells, however, have avoided these methods of detection despite, in many cases, the tumor’s immunogenic traits. Immuno-oncology is a field dedicated to the immunological traits of tumors, more recently finding ways of instigating an immune response against tumors. The cost of health care within the United States has continued to increase, whereas the quality of patient care has generally decreased in some areas. With the continued use of Medicare's former physician reimbursement algorithm, termed sustainable growth rate, national expenditures within the United States have been expected to increase 5.6% annually. To modernize the delivery and financing of care, Congress has introduced the Medicare Access and CHIP Reauthorization Act of 2015 (MACRA), which has permanently eliminated and replaced the sustainable growth rate. Tumor necrosis factor alpha (TNF-α) is a potent cytokine involved in systemic inflammation and immune modulation. Signaling responses that involve TNF-α are context dependent and capable of stimulating pathways promoting both cell death and survival. TNF-α treatment has been investigated as part of a combined therapy for acute myeloid leukemia due to its modifying effects on all-trans retinoic acid (ATRA) mediated differentiation into granulocytes. To investigate the interaction between cellular differentiation and TNF-α, we performed RNA-sequencing on two forms of the human HL-60/S4 promyelocytic leukemia cell line treated with TNF-α. Necrotizing Enterocolitis (NEC) is the most common gastrointestinal emergency seen in the Neonatal Intensive Care Unit (NICU) (Cleveland Clinic, 2015). NEC occurs most frequently in premature infants, accounting for approximately 60 to 80 percent of all cases (Singh et al., 2008). Mortality rates associated with NEC are estimated to be between 20 and 30 percent reaching upwards of 50 percent for patients requiring surgical intervention (Cleveland Clinic, 2015). Work-related musculoskeletal disorders (WRMDs) are a common occurrence among operating room (OR) personnel due to certain job requirements including standing in a static position for long periods of time. The American Association of Operating Room Nurses (AORN) ergonomic guidelines support interventions (anti-fatigue mats, shoe in-soles, etc.) aimed at decreasing the rates of WRMDs among OR personnel. A 3-month pre- and post-intervention trial was established at one Southern Indiana hospital to assess the efficacy of anti-fatigue mats in the OR theatre. Two anti-fatigue mats were placed in each OR, cardiovascular operating room (CVOR), and endoscopy room for use with each case. …

Deoxyuridine nucleotidyl transferase (dUTPase) is an enzyme found in all organisms that have thymine as a component of DNA. It catalyzes the hydrolysis of dUTP to dUMP and pyrophosphate thus precluding the buildup of dUTP pools as well as providing the substrate, dUMP, for the de novo synthesis of thymidylate. In Homo sapiens, there are four isoforms: mitochondrial (mDut), nuclear (nDut), variant 3 and variant 4. This work is largely focused on nDut. Using structural and MS analyses of recombinant dUTPase constructs, an intermolecular disulfide bridge between cysteine-3 of each nDut monomer was discovered. It was found that these two …

This study investigates the needs of incarcerated individuals upon reintegration into the community. In this article, we use thematic analysis to dissect two prison documentaries in order to best assess the needs of these individuals to inform occupational therapy intervention. Occupational therapy seeks to empower individuals while analyzing needs and how the individuals can be best supported. Our analysis shows that there are many needs that go unaddressed prior to community reintegration leading to a higher chance of recidivism recurring. Importance Using opioids for acute pain can lead to long-term use and associated morbidity and mortality. Injury has been documented as a gateway to long-term opioid use in some populations, but data are limited for injured workers.
Objective To evaluate the prevalence and risk factors of long-term opioid use after injury among workers in Tennessee who were opioid free at the time of injury.

Some trace elements are altered with chronic kidney disease. Selenium, zinc, and manganese tend to be wasted, and there is growing evidence that selenium deficiency is associated with mortality on dialysis. Other trace elements accumulate, such as chromium, cobalt, lead, molybdenum, and vanadium. The highest chromium levels are found in dialysis patients. The dialysis modality may further affect these levels, especially in hemodialysis patients, where even small contaminations in the dialysis feed water may lead to a concentration gradient that increases the concentration of certain trace elements. © 2019 The Authors Purpose: Postoperative rehabilitation is important for maximizing patient outcomes after surgical pulley reconstruction. The purpose of this study was to identify the optimal wrist position in which rehabilitation should be undertaken to decrease the load on surgically reconstructed pulleys. Methods: We tested 14 digits composed of the index, middle, and ring fingers from 5 cadaveric specimens in a novel in vitro finger motion simulator designed to achieve full finger flexion and extension actively.
