Objective: Systematic-reviews of analgesics for low back pain generally include published data only. Obtaining data from unpublished trials is potentially important because they may impact effect sizes in meta-analyses. We determined whether including unpublished data from trial registries changes the effect sizes in meta-analyses of analgesics for low back pain.

Study Design and Setting: Trial registries were searched for unpublished data that conformed to the inclusion criteria of n=5 individual source systematic-reviews. We reproduced the meta-analyses using data available from the original reviews then re-ran the same analyses with the addition of new unpublished data.

Purpose:Explore the confidence of determining signs/symptoms of phlebitis by nurse practitioners.Design:Cross-Sectional Questionnaire SurveyMethods:Fifteen signs/symptoms of phlebitis identified from a systematic review were included in the questionnaire survey among the nurses from Infusion Nurse Network of Thailand. Respondents were asked for their confidence in determining each of the 15 signs/symptoms, using a 5-point Likert scale.Findings: From 344 participants (77.83%) responded to the survey, the mean age was 34.73 years, 98.8% were female, 77.62% had no experience with phlebitis management. Lung cancer leads in mortality among all types of cancer in the US and Non-small cell lung cancer (NSCLC) is the major type of lung cancer. Immuno-compromised mice bearing xenografts of human lung cancer cells represent the most common animal models for studying lung cancer biology and for evaluating potential anticancer agents. ปมประสาท ซีลีอารี่ เป็นปมประสาทในเบ้าตาที่โดนทำลายหรือทำให้บาดเจ็บได้ง่ายจากการทำหัตถการต่างๆในเบ้าตา ปมประสาทซีลีอารี่จะอยู่ทางด้านหลังในเบ้าตาที่ค่อนมาทางผนังเบ้าตาส่วนนอกเป็นผลให้หัตถการต่างๆที่เข้าทางเบ้าตาส่วนนอกมักส่งผลให้ปมประสาทโดนทำลายหรือบาดเจ็บ ซึ่งการที่ปมประสาทนี้เกิดการบาดเจ็บจะทำให้เสียหน้าที่การทำงานที่ทำให้เกิดม่านตาขยาย หรือ ภาวะรูม่านตาค้าง (tonic pupil) การศึกษารายละเอียดของปมประสาทซีลีอารี่ใน 40 ลูกตา จากอาจารย์ใหญ่ 40 ท่าน ทำโดยใช้แว่นขยายสำหรับผ่าตัดที่มีกำลังขยาย 3.5 เท่า เพื่อศึกษาลักษณะของรากที่เข้าสู่ปมประสาทซีลีอารี่ จำนวนของเส้นประสาทชอทซีลีอารี่ (short ciliary nerves) ตำแหน่งของปมประสาทซีลีอารี่ ขนาดของปมประสาทซีลีอารี่ และระยะห่างระหว่างปมประสาทซีลีอารี่กับโครงสร้างอื่นๆในเบ้าตา ปมประสาทซีลีอารี่ถูกพบว่าอยู่ที่ยอดหลังสุดของเบ้าตา ด้านข้างทางด้านนอกต่อเส้นประสาทออพติก ด้านข้างทางด้านในต่อกล้ามเนื้อตา lateral rectus ปมประสาทซีลีอารี่มีความกว้างประมาณ 2.24 มิลลิเมตร และความยาวประมาณ 3.50 มิลลิเมตร ระยะห่างโดยเฉลี่ยระหว่างปมประสาทซีลีอารี่และส่วนท้ายสุดของลูกตามีขนาด 16.04 มิลลิเมตร ระยะห่างโดยเฉลี่ยระหว่างปมประสาทซีลีอารี่และกล้ามเนื้อตา lateral rectus มีขนาด 2.88 มิลลิเมตร ระยะห่างโดยเฉลี่ยระหว่างปมประสาทซีลีอารี่และเส้นประสาทออพติก มีขนาด 1.47 มิลลิเมตร ระยะห่างโดยเฉลี่ยระหว่างปมประสาทซีลีอารี่และจุดเกาะที่ตาขาวของกล้ามเนื้อตา lateral rectus มีขนาด 31.53 มิลลิเมตร นอกจากนี้ความสัมพันธ์ระหว่างระยะห่างจากปมประสาทซีลีอารี่ถึงจุดที่อยู่ท้ายสุดของลูกตาเป็นแบบผกผันกัน กับ ระยะห่างจากปมประสาทซีลีอารี่ถึงกล้ามเนื้อตา lateral rectus นั่นหมายถึงยิ่งปมประสาทอยู่ห่างจากลูกตาจะยิ่งอยู่ใกล้กล้ามเนื้อ lateral rectus ส่วนจำนวนของรากประสาทมอเตอร์ (motor root) ถูกพบว่ามี 1 ราก 2 ราก และ ยังพบถึง 3 รากโดยที่ยังไม่มีรายงานมาก่อน รากประสาทรับความรู้สึกพบว่ามี 1 ราก รากประสาทซิมพาเททิก (sympathetic root) พบ 1 รากเช่นเดียวกับรากประสาทรับความรู้สึก จำนวนของเส้นประสาทชอทซีลีอารี่จะพบประมาณ 6-14 เส้น แพทย์ผู้ทำหัตถการในส่วนเบ้าตาควรตระหนักถึง ความรู้ทางด้านกายวิภาคและลักษณะต่างๆของปมประสาทซีลีอารี่เพื่อลดการเกิดผลแทรกซ้อนจากปมประสาทนี้บาดเจ็บเช่น ม่านตาขนาดผิดปกติ ในรายงานฉบับนี้ยังแสดงถึงระยะห่างระหว่างปมประสาทซีลีอารี่กับจุดเกาะของกล้ามเนื้อตา lateral rectus ที่ตาขาวซึ่งยังไม่มีรายงานมาก่อนและควรคำนึงเพื่อลดการบาดเจ็บของปมประสาทซีลีอารี่ในหัตถการฉีดยา โบทูลินัม (botulinum toxin) เข้ากล้ามเนื้อตา lateral …

ผลฉับพลันของระยะเวลาในการแช่น้ำเย็นที่มีต่อการฟื้นสมรรถภาพและปริมาณแลคเตท ในการว่ายน้ำท่าฟรอนท์ครอว์ลระยะ 100 เมตร, อภิสิทธิ์ เสลาหอม

การวิจัยครั้งนี้เป็นการศึกษารูปแบบการฟื้นฟูสมรรถภาพโดยการแช่น้ำแย็นในระยะเวลาที่แตกต่างกัน ร่วมกับการนั่งพักแบบหยุดนิ่ง ที่ส่งผลต่อสถิติเวลา ปริมาณค่าความเข้มข้นของแลคเตทในเลือด และค่าอัตราของหัวใจ โดยกลุ่มตัวอย่างที่ใช้ในการวิจัยครั้งนี้เป็นนักกีฬาว่ายน้ำตัวแทนทีมชาติไทย อายุระหว่าง 18-24 ปี จำนวน 12 คน โดยเลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจง กลุ่มตัวอย่างได้รับมอบหมายให้ทำการว่ายน้ำท่าฟรอนท์ครอว์ลด้วยความเร็วสูงสุด ระยะทาง 100 เมตร หลังจากนั้นทำการฟื้นฟูสมรรถภาพใน 3 รูปแบบ ได้แก่ การนั่งพักแบบหยุดนิ่ง 20 นาที(รูปแบบควบคุม) กับการแช่น้ำเย็นที่อุณหภูมิของน้ำเท่ากับ 14 องศาเซลเซียส โดยการแช่น้ำ 10 นาที ตามด้วยนั่งพักแบบหยุดนิ่ง 10 นาที(รูปแบบทดลองที่ 1) และการแช่น้ำ 15 นาที ตามด้วยนั่งพักแบบหยุดนิ่ง 5 นาที(รูปแบบที่ 2) การทดสอบ 1 ครั้งจะทำการฟื้นฟูสมรรถภาพ 1 รูปแบบ กลุ่มตัวอย่างจะเว้นระยะของการทดสอบ 48 ชั่วโมง(วันเว้นวัน) โดยขณะฟื้นฟูได้ทำการเก็บข้อมูลปริมาณค่าความเข้มข้นของแลคเตทในเลือดจากบริเวณปลายนิ้ว และค่าอัตราการเต้นของหัวใจช่วง 3นาที 5นาที 10นาที 15นาที และ20นาที หลังจากนั้นว่ายน้ำท่าฟรอนท์ครอว์ล ระยะทาง 100 เมตร ด้วยความเร็วสูงสุดอีกครั้งหนึ่ง เพื่อดูสถิติเวลาของการว่ายน้ำหลังจากการฟื้นฟูสมรรถภาพ และนำข้อมูลที่ได้มาวิเคราะห์โดยใช้สถิติการวัดความแปรปรวน 2 ทางชนิดวัดซ้ำ โดยกำหนดนัยสำคัญทางสถิติที่ระดับที่.05 ผลการวิจัยพบว่า เมื่อเปรียบเทียบค่าสถิติเวลาเฉลี่ยภายหลังการฟื้นฟูสมรรถภาพของทั้ง 3 รูปแบบ พบว่าค่าเฉลี่ยสถิติเวลาการว่ายน้ำ ไม่แตกต่างอย่างกันอย่างมีนัยสำคัญทางสถิติ ในขณะที่ค่าเฉลี่ยปริมาณกรดแลคเตทและค่าเฉลี่ยอัตราการเต้นของหัวใจหลังจากฟื้นฟูสมรรถภาพ ของรูปแบบทดลองที่ 1 และรูปแบบทดลองที่ 2 ภายหลังของการฟื้นฟูสมรรถภาพ มีค่าเฉลี่ยที่ต่ำกว่ารูปแบบควบคุม อย่างมีนัยสำคัญทางสถิติ(p<.50 ) สรุปได้ว่าการใช้การแช่น้ำเย็นอุณหภูมิ 14 องศาเซลเซียสโดยการแช่น้ำ 10 นาที นั่งพักแบบหยุดนิ่ง 10 นาที และการแช่น้ำ 15 นาที นั่งพักแบบหยุดนิ่ง 5 นาที ส่งผลให้ค่าปริมาณค่าความเข้มข้นของแลคเตทในเลือดและอัตตราการเต้นของหัวใจลงลง ทำให้การฟื้นฟูสมรรถภาพได้อย่างเร็ว

คุณภาพการนอนหลับในผู้ป่วยโรคหลอดเลือดสมองในโรงพยาบาลพระมงกุฎเกล้า, จิรัฏฐ์ โลทะกะ

Background : โรคหลอดเลือดสมองเป็นปัญหาของระบบสาธารณสุขที่สำคัญปัญหาหนึ่งของประชากรทั่วโลกผลกระทบจากโรคหลอดเลือดสมองทำให้เกิดการพิการและยังเป็นปัญหาที่ส่งผลต่อคุณภาพการนอนหลับ วัตถุประสงค์ เพื่อศึกษาคุณภาพการนอนหลับและการใช้เครื่องมือ Wearable tracker ในการเก็บข้อมูลการนอนหลับของผู้ป่วยโรคหลอดเลือดสมองในโรงพยาบาลพระมงกุฎเกล้า วิธีการศึกษา เป็นการศึกษาเชิงพรรณนา (Descriptive Studies) ณ จุดเวลาใดเวลาหนึ่ง (Cross sectional descriptive design) กลุ่มตัวอย่างคือผู้ป่วยโรคหลอดเลือดสมองในโรงพยาบาลพระมงกุฎเกล้า ตั้งแต่มกราคม ถึงเดือนเมษายน พ.ศ.2563 จำนวน 66 คน โดยผู้วิจัยบันทึกข้อมูลแบบสอบถามจากผู้ป่วยในส่วนของข้อมูลทั่วไป ได้แก่ ข้อมูลส่วนบุคคล ข้อมูลด้านการเจ็บป่วย หลังจากนั้นนําอุปกรณ์วัดระดับกิจกรรม Wearable tracker ใส่ให้ผู้ป่วยและทำแบบประเมินคุณภาพการนอนหลับ (PSQI) ให้ครบถ้วนโดยใส่ตลอดเวลา นำเสนอข้อมูลคุณภาพการนอนหลับและการใช้เครื่องมือ Wearable tracker เป็นค่าคะแนน วิเคราะห์หาระดับความพึงพอใจของเครื่องมือ Wearable tracker และวิเคราะห์ความสัมพันธ์กับปัจจัยต่าง ๆ ผลการศึกษา พบว่าผู้ป่วยโรคหลอดเลือดสมองในโรงพยาบาลพระมงกุฎเกล้า ร้อยละ 93.9 มีคุณภาพการนอนหลับที่ไม่ดี และร้อยละ 6.1 มีคุณภาพการนอนหลับที่ดี ตามเกณฑ์การประเมินคุณภาพการนอนหลับ (PSQI) ปัจจัยที่มีผลต่อคุณภาพการนอนหลับประกอบด้วย ปัจจัยส่วนบุคคล ได้แก่ ระดับปริญญาตรีขึ้นไป (p = 0.003) ซึ่งมีค่านัยสำคัญทางสถิติ P < 0.01 เวลาที่ผู้ป่วยได้เข้านอนทั้งหมด (p = 0.021) เวลาที่เข้านอน หลังจาก 00:00 น. (p = 0.014) เวลาที่ตื่นนอน 04:00 – 06:00 น. (p = 0.012) และเวลาที่หลับลึก (p = 0.039) มีนัยสำคัญทางสถิติ < 0.05 และปัจจัยด้านการเจ็บป่วย ได้แก่ ระยะเวลาที่ป่วยมากกว่า 1 สัปดาห์ (p = 0.032) และการใช้ยา Antidiabetic (p = 0.024) อย่างมีนัยสำคัญทางสถิติ < 0.05 และจากศึกษาการใช้เครื่องมือ Wearable tracker ในการเก็บข้อมูลพบว่า ปัจจัยที่วัดได้จากอุปกรณ์วัดระดับกิจกรรม Wearable tracker ได้แก่ เวลานอนทั้งหมด (p = 0.032) เวลาที่เข้านอน มากกว่า 00.00 น. (p = 0.032) เวลาที่ตื่นนอน 04.00 - 06.00 น. P = 0.032) เวลาที่ตื่นนอน มากกว่า 06.00 น. (p = 0.032) และเวลาที่หลับลึก (p = 0.032) มีผลอย่างมีนัยสำคัญทางสถิติ < 0.05 สรุป ผู้ป่วยโรคหลอดเลือดสมองในโรงพยาบาลพระมงกุฎเกล้า ร้อยละ 93.9 มีคุณภาพการนอนหลับที่ไม่ดี ซึ่งปัจจัยที่มีผลต่อคุณภาพการนอนหลับ ประกอบด้วย ปัจจัยส่วนบุคคล และปัจจัยด้านการเจ็บป่วย นอกจากนั้นจากศึกษาการใช้เครื่องมือ Wearable tracker ในการเก็บข้อมูลพบว่า ปัจจัยที่วัดได้จากอุปกรณ์วัดระดับกิจกรรม Wearable tracker ก็มีผลที่มีความสัมพันธ์กัน ดังนั้นจึงแสดงให้เห็นว่าอุปกรณ์วัดระดับกิจกรรม Wearable tracker สามารถใช้เป็นเครื่องมือในการหาคุณภาพการนอนหลับได้

โรคใหลตายเป็นโรคหัวใจเต้นผิดจังหวะที่พบได้ยาก โดยโรคนี้เพิ่มความเสี่ยงของการเสียชีวิตจากภาวะหัวใจหยุดเต้นอย่างกะทันหันได้ ซึ่งผู้ป่วยไม่เคยมีความผิดปกติของโครงสร้างหัวใจ ลักษณะอาการทางคลินิกที่พบ คือ ไม่เคยมีอาการแสดงของโรคมาก่อน, เคยมีประวัติเป็นลมหมดสติ, เคยมีประวัติหายใจเฮือก ๆ ขณะนอนหลับและภาวะหัวใจหยุดเต้นอย่างเฉียบพลันหรือเสียชีวิตอย่างกะทันหัน โรคใหลตายมีผลต่อคุณภาพชีวิตของผู้ป่วยเนื่องจากผู้ป่วยโรคใหลตายต้องเข้ารับการตรวจติดตามอาการอย่างต่อเนื่อง วัตถุประสงค์ : การศึกษาคุณภาพชีวิตและปัจจัยที่เกี่ยวข้องกับคุณภาพชีวิตของผู้ป่วยโรคใหลตายที่ยังมีชีวิตอยู่ วิธีการวิจัย : ผู้ป่วยโรคใหลตายจำนวน 29 คนที่เข้ารับบริการในเดือน สิงหาคม พ.ศ.2562 ถึงเดือนเมษายน พ.ศ.2563 ที่โรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย โดยทำการเก็บข้อมูลทั่วไป, แบบสอบถามเกี่ยวกับคุณภาพชีวิต โดยใช้แบบสอบถาม MacNew และ SF-36, แบบสอบถาม HADS ฉบับภาษาไทยและแบบทดสอบภาวะการรู้คิด คือ Grooved Pegboard, Trail A-B และ CERAD การวิเคราะห์ข้อมูล ใช้การวิเคราะห์แบบพรรณณาและการวิเคราะห์สถิติถดถอยเชิงเส้น ผลการวิจัย : ผู้ป่วยโรคใหลตายเป็นเพศชายร้อยละ 96.60 อายุเฉลี่ย 45.17±13.83 ปี, คะแนนเฉลี่ยโดยรวมแบบสอบถาม MacNew 5.62±0.75 คะแนน, แบบสอบถาม SF-36 ด้านร่างกาย คะแนนเฉลี่ย 49.84±6.42 คะแนนและด้านจิตใจคะแนนเฉลี่ย 52.90±6.31 คะแนน, คะแนนเฉลี่ยความวิตกกังวลและความซึมเศร้า คือ 4.41±2.92 และ 3.34±3.30 คะแนนตามลำดับ, การทดสอบภาวะการรู้คิดมี 3 ส่วน ส่วนที่หนึ่ง การทดสอบ Grooved Pegboard มือข้างที่ถนัดและมือข้างที่ไม่ถนัด ใช้เวลาเฉลี่ย 78.41±27.87 วินาทีและ 84.52±25.97 วินาที ส่วนที่สอง การทดสอบ Trail A และ B ใช้เวลาเฉลี่ย 53.69 ± 20.65 และ 181.00 ± 80.91 วินาที ในส่วนสุดท้าย การทดสอบ the Word …

การใช้แบบประเมินสโตรค ริสโกมิเตอร์ ในการพยากรณ์การหนาตัวผิดปกติของผนังหลอดเลือดแดงอิน เทอร์นอลคาโรติดจากหลอดเลือดแดงแข็ง ซึ่งตรวจพบโดยการตรวจด้วยคลื่นเสียงความถี่สูงในกลุ่มประชากรไทยที่มีอายุมากกว่า 45 ปี ที่ไม่เคยมีอาการและไม่เคยได้รับการวินิจฉัยว่าเป็นโรคหลอดเลือดสมอง ที่มาของการศึกษา การประยุกต์ใช้แบบประเมินความเสี่ยงโดยใช้แบบประเมินสโตรกริสโกมิเตอร์ (stroke riskometer) แทนการตรวจการหนาตัวผิดปกติของผนังหลอดเลือดแดงอินเทอร์นอลคาโรติดจากหลอดเลือดแดงแข็งจากการตรวจด้วยการตรวจคลื่นความถี่สูง วัตถุประสงค์ เพื่อศึกษาความสัมพันธ์ของการหนาตัวผิดปกติของผนังหลอดเลือดแดงอินเทอร์นอลคาโรติดจากหลอดเลือดแดงแข็งจากการตรวจด้วยการตรวจคลื่นความถี่สูง กับความเสี่ยงที่ประเมินได้จากแบบประเมินสโตรกริสโกมิเตอร์ (stroke riskometer) วิธีการวิจัย การศึกษาเชิงวิเคราะห์แบบตัดขวางในอาสาสมัครไทย อายุมากกว่า 45 ปี ที่ไม่เคยมีอาการและไม่เคยได้รับการวินิจฉัยโรคหลอดเลือดสมองมาก่อน โดยแบ่งเป็นกลุ่มที่มีความเสี่ยงสูงมีระดับความเสี่ยงในการเกิดโรคหลอดเลือดสมองขาดเลือดใน 10 ปี มากกว่าหรือเท่ากับร้อยละ 10 และกลุ่มที่มีความเสี่ยงต่ำ ความเสี่ยงน้อยกว่าร้อยละ 10 ทั้งสองกลุ่มจะได้ตรวจคลื่นความถี่สูงบริเวณหลอดเลือดแดงคาโรติดเพื่อประเมินการหนาตัวผิดปกติของผนังหลอดเลือดแดงอินเทอร์นอลคาโรติดจากหลอดเลือดแดงแข็ง ผลการศึกษา อาสาสมัครทั้งหมด 169 ราย แบ่งเป็นกลุ่มความเสี่ยงสูง 87 ราย กลุ่มความเสี่ยงต่ำ 82 ราย พบการหนาตัวผิดปกติของผนังหลอดเลือดแดงอินเทอร์นอลคาโรติดจากหลอดเลือดแดงแข็งร้อยละ 52.1 กลุ่มความเสี่ยงสูงพบร้อยละ 86.4 ต่างจากกลุ่มความเสี่ยงต่ำซึ่งพบร้อยละ 14.8 อย่างมีนัยสำคัญทางสถิติ โดยกลุ่มความเสี่ยงสูงมีปัจจัย อายุ สูบบุหรี่ ความดันโลหิตสูงและได้รับยาลดความดันโลหิต โรคเบาหวาน มีประวัติกล้ามเนื้อหัวใจขาดเลือด หัวใจเต้นผิดจังหวะ กล้ามเนื้อหัวใจโต และมีปัญหาเรื่องความจำ หลังใช้สถิติการวิเคราะห์พหุตัวแปรพบว่า โรคเบาหวาน ภาวะความดันโลหิตสูงและได้รับยาลดความดันโลหิต หัวใจห้องล่างซ้ายโต การสูบบุหรี่ และความจำบกพร่อง มีความสัมพันธ์กับการตรวจพบการหนาตัวผิดปกติของผนังหลอดเลือดแดงอินเทอร์นอลคาโรติดจากหลอดเลือดแดงแข็ง โดยการใช้ค่าความเสี่ยงที่มากกว่าหรือเท่ากับร้อยละ 10 ใน 10 ปี มีความไว (sensitivity) ร้อยละ 86.36 และ ความจำเพาะ (specificity) อยู่ที่ร้อยละ 85.1 ในการทำนายการตรวจพบการหนาตัวผิดปกติของผนังหลอดเลือดอินเทอร์นอลคาโรติดจากหลอดเลือดแดงแข็ง สรุป การใช้แบบประเมินสโตรค ริสโกมิเตอร์ (stroke riskometer) สามารถนำมาใช้ในการทำนายการตรวจพบการหนาตัวผิดปกติของผนังหลอดเลือดแดงอินเทอร์นอลคาโรติดจากหลอดเลือดแดงแข็งได้

We present the first demonstration of a fully-flexible, self-powered glucose indicator system that synergizes two flexible electronic technologies: a flexible self-powering unit in the form of a biofuel cell, with a flexible electronic device - a circuit-board decal fabricated with biocompatible microbial nanocellulose. Our proof-of-concept device, comprising an enzymatic glucose fuel cell, glucose sensor and a LED indicator, does not require additional electronic equipment for detection or verification; and the entire structure collapses into a microns-thin, self-adhering, single-centimeter-square decal, weighing less than 40 mg. Acute myeloid leukemia (AML) is a typically fatal malignancy and new drug and treatment need to be developed for a better survival outcome. Background: Despite its efficacy, medication-assisted treatment (MAT) is rarely available in the criminal justice system in the United States, including in problem-solving courts or diversionary settings. Previous studies have demonstrated criminal justice administrators' hostility towards MAT, especially in prisons and jails. Yet, few studies have examined attitudes among court personnel or compared beliefs among different types of personnel. Also, few studies have explored the relationship between MAT education/training and attitudes. Sierra Leone had the highest number of cases of Ebola virus disease in history during the 2014 Ebola epidemic. The purpose of this quantitative, cross-sectional study was to examine the relationship between sociocultural and behavioral risk factors and Ebola status among women and men ages 15 to 49 years in Sierra Leone. Medicinal plants are important elements of indigenous medical system that have persisted in developing countries. Many of the botanical chemo-preventions currently used as potent anticancer agents.
