Cost-Effectiveness Of A Weight Loss Intervention: An Adaptation Of The Look Ahead Lifestyle Intervention In The Us Military, Karina Manz

Objective: To assess whether a counselor-initiated adaptation (CI) of the Look AHEAD Intensive Lifestyle Intervention (ILI) in a military setting was cost-effective relative to a self- paced adaptation (SP).
Methods: We performed cost-effectiveness analysis from a payer perspective alongside a 2014- 2017 randomized behavioral weight loss trial among 248 active-duty military personnel stationed at a US Air Force Base in Texas. We calculated incremental cost-effectiveness ratios (ICERs) for weight loss, reductions in waist circumference, and quality-adjusted life years (QALYs).
Results: After 12 months, the CI adaptation cost more per participant compared to the SP adaptation ($1,081 vs. $120), but achieved …

Human Papillomavirus (HPV) is a group of more than 150 virus that can cause genital warts and cancers of the oropharynx, cervix, vagina, vulva, penis, and anus. A preventive vaccine currently exists that protects against seven oncogenic HPV strands and two strands that cause genital warts. Current guidelines for HPV9 vaccination consist of a 2-dose series delivered at 0 and 6 months for males and females ages 9-14, and a 3-dose series delivered at 0, 2, and 6 months for those ages 15-26.

This Article is the first to address the challenges of federalism in protecting medical privacy in workers’ compensation after the promulgation of the HPR and to propose legal change. The Article argues that workers’ compensation programs must align with the federal privacy protections of the HPR and proposes actions for the U.S. Department of Health and Human Services (HHS) and states to remedy departures. Part I discusses the complex relationship between the HPR and workers’ compensation. Drug compounding is the practice of mixing, combining, or altering drug ingredients to create a product that meets the medical needs of a patient whose needs cannot otherwise be met with commercially-available products. Compounding is essential for patients that are allergic to certain ingredients or cannot tolerate the route of administration or dosage form of a commercially-available drug. Compounding may also enable pharmacists to supply medications when there is a shortage of commercially-available drugs. Many laws use family members as a regulatory tool to influence the decisions or behavior of their loved ones, i.e., they deputize family. Involuntary treatment laws for substance use disorder are a clear example; such laws empower family members to use information shared by their loved ones to petition to force their loved ones into treatment without consent. Fungal peritonitis (FP) is a fatal complication in peritoneal dialysis (PD). Detection of fungal infection using standard culture is time-consuming and has a low sensitivity. While, Polymerase Chain Reaction (PCR) are often inapplicable in remote hospital and laboratory. In this study, Loop-mediated Isothermal Amplification (LAMP) assay was developed for rapid and specific detection of fungi in peritoneal dialysis. LAMP primers were designed based on the conserved sequence of fungal 18s rRNA gene. The reaction was optimized for 60 min at 64 ◦C. The sensitivity of LAMP assay was 1 pg/reaction which is a hundred times more sensitive than that of …

วัตถุประสงค์: ประเมินผลของการใช้แบบสอบถาม PROMPT ในการให้บริบาลทางเภสัชกรรม ในด้านการสื่อสารเรื่องคุณภาพชีวิตด้านการใช้ยาระหว่างเภสัชกรและผู้ป่วย การค้นหาปัญหาจากการใช้ยา การติดตามผลการแก้ไขปัญหาจากการใช้ยา ความร่วมมือในการใช้ยา การเปลี่ยนแปลงแนวทางการรักษาผู้ป่วยของแพทย์ และคุณภาพชีวิตด้านการใช้ยา วิธีการวิจัย: การทดลองแบบสุ่มที่มีกลุ่มควบคุม ในผู้ป่วยนอกจำนวน 315 คน ที่เข้ารับการรักษาที่โรงพยาบาลสมุทรปราการระหว่างเดือนสิงหาคม 2561 ถึง กุมภาพันธ์ 2562 แบ่งผู้ป่วยเป็น 2 กลุ่มคือ กลุ่มมาตรฐาน (กลุ่มที่ไม่ใช้แบบสอบถาม PROMPT ในการบริบาลทางเภสัชกรรม) และกลุ่มศึกษา (กลุ่มที่ใช้แบบสอบถาม PROMPT ในการบริบาลทางเภสัชกรรม) โดยมีการเก็บข้อมูลจำนวน 2 ครั้ง คือ ก่อนการแทรกแซกและหลังการแทรกแซงเมื่อผู้ป่วยมาพบแพทย์ในครั้งถัดไป ผลการวิจัย: คะแนนการสื่อสารของผู้ป่วยทั้ง 2 กลุ่ม มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p < 0.001) กลุ่มศึกษาพบปัญหาจากการใช้ยามากกว่ากลุ่มมาตรฐาน (p<0.05) และมีคะแนน คุณภาพชีวิตด้านการใช้ยาดีขึ้นอย่างมีนัยส าคัญทางสถิติเมื่อเทียบกับกลุ่มมาตรฐาน (p<0.01) สรุปผลการวิจัย: การให้บริบาลทางเภสัชกรรมโดยใช้แบบสอบถาม PROMPT ช่วยเพิ่มการ สื่อสารเรื่องคุณภาพชีวิตการใช้ยาระหว่างผู้ป่วยกับเภสัชกร และช่วยให้คุณภาพชิวิตด้านการใช้ยาของ ผู้ป่วยดีขึ้นได้

วัตถุประสงค์ เพื่อศึกษาและเปรียบเทียบตัวแปรทางคิเนมาติกส์ของไม้ช่วงบอลกระทบหน้าไม้ในท่าตีก้าวขาไปข้างหน้าในนักกีฬาคริกเก็ตหญิงทีมชาติไทยและนักกีฬาคริกเก็ตหญิงทีมเยาวชนรุ่นอายุไม่เกิน 19 ปี และในท่าตีก้าวขาไปข้างหน้าที่สามารตีไปถึงเส้นขอบสนามนอกและไปไปถึงเส้นขอบสนามนอก วิธีการดำเนินงานวิจัย ผู้เข้าร่วมงานวิจัยเป็นนักกีฬาคริกเก็ตหญิง จำนวน 14 คน แบ่งเป็นกลุ่มนักกีฬาคริกเก็ตหญิงทีมชาติไทยจำนวน 9 คน และกลุ่มนักกีฬาคริกเก็ตหญิงเยาวชนอายุไม่เกิน 19 ปีจำนวน 5 คน บันทึกวิดีโอท่าตีก้าวขาไปข้างหน้าในกีฬาคริกเก็ตเพื่อวิเคราะห์ท่าทางที่สามารถตีบอลไปได้ไกลสุด ทำการศึกษาตัวแปรเชิงคิเนมาติกส์ของไม้ช่วงบอลกระทบหน้าไม้ ผลการวิจัย กลุ่มนักกีฬาคริกเก็ตหญิงทีมชาติไทยมีระยะทางบอล มุมสะท้อนของบอล มุมของไม้ ความสูงระหว่างของจุดศูนย์กลางกับพื้น และระยะห่างระหว่างของจุดศูนย์กลางไม้กับสะโพกมากกว่ากลุ่มนักกีฬาคริกเก็ตหญิงเยาวชนอายุไม่เกิน 19 ปี อย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05 สรุปผลการวิจัย ค่าคิเนเมติกส์ของไม้ในกลุ่มนักกีฬาคริกเก็ตหญิงทั้งสองทีมมีท่าตีก้าวขาไปข้างหน้าแตกต่างกันและท่าตีก้าวขาไปข้างหน้าที่สามารถตีบอลไปถึงเส้นขอบสนามนอกกับท่าตีที่ไม่สามารถตีไปถึงขอบสนามมีความแตกต่างเช่นกัน

Fluoxetine เป็นยากลุ่ม SSRI (selective serotonin reuptake inhibitor) ที่ใช้ในการรักษาโรคซึมเศร้า ยาหลายตัวในกลุ่ม SSRI สามารถจับแบบจำเพาะ sigma-1 receptor แต่ในกรณี fluoxetine จะมีกลไกออกฤทธิ์จำเพาะต่อ serotonin transporter (Ki = 0.81 nM) มากกว่า sigma-1 receptor (Ki = 214–240 nM) หลายงานวิจัยรายงานว่า sigma-1 receptor แสดงออกบนเซลล์มะเร็งหลายชนิด และมีรายงานว่า antagonist สามารถลดการเพิ่มจำนวนและเหนี่ยวนำให้เกิด apoptosis แต่การทำงานร่วมกันระหว่าง fluoxetine กับ sigma-1 receptor ต่อเซลล์มะเร็ง ยังไม่ทราบกลไกการออกฤทธิ์ที่ชัดเจน งานวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาฤทธิ์ของ fluoxetine ในการยับยั้งการแบ่งตัวและเหนี่ยวนำให้เกิด apoptosis ผ่าน sigma-1 receptor ในเซลล์มะเร็งเม็ดเลือดขาว ผลการทดสอบพบว่า fluoxetine สามารถลดเปอร์เซ็นต์การรอดชีวิตของเซลล์ Jurkat และ U937 ได้ โดยมีค่า IC50 เท่ากับ 28 ± 1.4 และ 36 ± 1.6 uM ตามลำดับ เมื่อเซลล์ Jurkat ถูกทดสอบด้วย fluoxetine ตามระดับความเข้มข้นและระยะเวลาที่เพิ่มขึ้น พบว่าfluoxetine สามารถยับยั้งการแบ่งตัวของเซลล์ Jurkat ได้อย่างมีนัยสำคัญทางสถิติ โดยมีกลไกหยุดยั้งวัฎจักรเซลล์ที่ระยะ G2/M และลดการหลั่งไซโตไคน์ IL-2 รวมทั้งเหนี่ยวนำให้เกิด apoptosis โดยการลดการแสดงออกของโปรตีน pro caspase 9 ผ่านการยับยั้งการเกิด phosphorylation ของ AKT ในวิถี PI3K/Akt และ การถูกกระตุ้นของ sigma-1 receptor …

Tumors of the oral cavity account for 8% of all canine neoplasia. Malignant oral mass such as melanoma, oral squamous cell carcinoma (OSCC) and fibrosarcoma covers approximately 59% of canine oral neoplasia. Among these, canine oral melanoma (COM) is the most common oral tumors in dogs, with approximately 30–40% of all oral tumors. With high recurrence and/or distant metastasis rate, biomarkers are required for early detection, diagnosis, prognosis, and monitoring treatment response. Near-Infrared Spectroscopy (NIRs) technique was used to quantitatively analyzed 5 mg amlodipine tablets. The aim of this study was to develop calibration model by NIRs technique to accurately determine content uniformity of amlodipine tablets during commercial scale production. Amlodipine tablets were initially manufactured in laboratory scale to generate calibration model with concentrations ranging from 80% to 120% labeled amount using NIRs analysis with spectral range of 4000 –10000 cm-1. Results were compared with reference High-Performance Liquid Chromatographic (HPLC) method. Advancements in medicine, policy, and societal attitudes have improved life expectancy, socialization, and participation for individuals with intellectual and developmental disabilities (IDD). However, inconsistent or a lack of services may drive healthcare utilization toward expensive emergency care services. Urinary incontinence is a health condition that is associated with involuntary leakage of urine. Stress urinary incontinence (SUI) describes involuntary leakage of urine on effort or exertion and can impact one’s ability to participate in activities and affect quality of life. Furthermore, clinical management of this health condition is challenging as individuals who experience urinary incontinence often do not report this concern to a health care provider.

Low back pain in collegiate athletes has been reported at a rate of 37% from a wide array of sports including soccer, volleyball, football, swimming, and baseball. Whereas, in a military population the prevalence of low back pain is 70% higher than the general population. Compensatory movement strategies are often used as an attempt to reduce pain. Though compensatory movement strategies may effectively reduce pain, they are often associated with altered lower extremity loading patterns. Approximately 200,000 anterior cruciate ligament (ACL) injuries occur each year with about 100,000 of these injuries undergoing reconstruction (ACLR). The impetus of ACLR is to allow previously high functioning, physically active individuals to return to desired levels of sports participation and to engage in recommended levels of physical activity. However, 1 out of 3 patients after ACLR fail to return to competitive levels of sport and meet recommended levels of physical activity. This dissertation focuses on the aging voice - specifically the aging elite vocal athlete. It is comprised of four components; a series of research studies and a viewpoint piece designed to explore the awareness, struggles, and vocal compensations of aging singers dealing with age related vocal and performance problems. The overarching goal of these studies is to inform the development of a voice care protocol for the aging rock star to guide customized intervention for these elite vocal athletes that is focused on optimizing both vocal output and performance.

Students report that these experiences are often observation-based. Despite there being a strong push toward family-centered care, there remains a disconnect in outpatient pediatric settings when working with children ages 3-18.
