Gefitinib is a medication that was originally developed as an epidermal growth factor receptor (EGFR) blocker. It has been used to treat non-small cell lung cancer with varying success. We describe a patient who presented with Pancoasts syndrome and after being deemed surgically unresectable had limited success with chemotherapy and radiation but had an excellent response to the medication Gefitinib. The patient was disease free for several years and after 11 years of follow up with CT scans, a PET CT showed an isolated area of FDG avidity which on biopsy was small cell carcinoma of the lung. A Rare Case Of Asystole During A Laparoscopic Appendectomy In A Young Patient, Rachit Gupta, George Yousef, Melissa Lester

Laparoscopic surgery has become the mainstay of treatment for uncomplicated appendicitis cases. We describe a case of a 29 year old male with a pertinent past medical history of poorly controlled hypertension who was undergoing a routine laparoscopic appendectomy for confirmed appendicitis and developed bradycardia and subsequent asystolic cardiac arrest approximately one hour into the operation. Urgent cardiopulmonary resuscitation and intravenous (IV) atropine were required to reinitiate sinus cardiac rhythm. Hemimegalencephaly is uncommon congenital malformation with unilateral enlargement of the hemicerebrum. Here we report a 1 month-old male child presented at our neurosurgery outpatient’s clinic with recurrent seizures and facial asymmetry. CT and MRI of the brain revealed diagnostic characteristic features of Hemimegalencephaly associated with ipsilateral Facial Congenital Infiltrating Lipomatosis.

Effect Of A Brief Patient Communication Teaching On Both Attendee And Non-Attendee Family Medicine Residents., Dilip Nair, Adrienne Mays, Mohit Harsh

Physician-patient communication skills are important for physicians to acquire. Teaching skills is thought to require attendance by learners but this is difficult in graduate medical education settings. We asked if an educational intervention on physician-patient communication was associated with a "spill-over" effect to non-attendees in the same family medicine residency program.

We surveyed residents regarding communicating instructions to patients before the intervention and one month later, regardless of their attendance. Residents’ assessment of their patients’ understanding increased significantly post-intervention only if non-attendees were included.

This pilot study suggests a beneficial “spill-over” effect to non-attendee residents that warrants further study.

Background: Kawasaki disease is one of the leading causes of acquired heart disease in children. It is an acute self-limited vasculitis that predominantly affects infants and children younger than 5 years of age. Abstract: Although rare, toxic leukoencephalopathy (TLE) associated with heroin inhalation have been reported. ‘Chasing the dragon’ may lead to progressive spongiform degeneration of the brain and presents with a large range of neuropsychological sequelae. A thorough social history and clinical suspicion are paramount in recognizing the diagnosis. This case is an example of TLE in a middle-aged white male with a history of polysubstance abuse. He presented with a three week history of progressive neuropsychological symptoms, including abulia, bradyphrenia, hyperreflexia, and visual hallucinations. This study describes the experience of being diagnosed with early stage breast cancer for women age 65 and older. This qualitative study used content analysis to analyze twenty interviews of older women with early stage breast cancer. A variety of positive and negative factors influence the experience of being diagnosed with breast cancer. These factors include: their initial reactions, interactions with their respective medical teams, family support, religion, education, and misinformation. Unplanned pregnancies and sexually transmitted infections (STI) are increasing at a faster rate among young, low income Latinas as compared to other low income groups of girls. It is a common misconception that individuals with schizophrenia are significantly more dangerous and violent than individuals free of mental illness. This stigmatization may lead to harsher sentences when people with schizophrenia are involved in criminal activities and sentenced by a jury. This study presented four conditions to which participants were randomly assigned, alone or in a group of three, and were asked to sentence a defendant, either with or without schizophrenia. Previous research has shown a unique effect of red light on visual processing related to both schizophrenia and positive schizotypy. The current study examines whether this effect is influenced by sex in a more broadly-defined schizotypy sample. A location backward masking (BM) task with three color background conditions (red, green, and gray) was administered to 34 undergraduate students (59% female) with a high score on the Schizotypal Personality Questionnaire (SPQ) and 39 students (51% female) with a low score. A review of literature related to community acquired Clostridium difficile -associated disease (CA-CDAD) was conducted. Nine relevant studies were identified using the Cumulative Index to Nursing and Allied Health (CINAHL) and MEDLINE-EBSCOhost databases. Clinical practice recommendations were obtained from the Centers for Disease Control and Prevention. The studies provided information about epidemiology of infection due to C. difficile in the community and interventions to reduce transmission. Multiple studies found underlying gastrointestinal disorders and use of cephalosporin antibiotics to be a risk factor. Another risk factor was administration of gastric acid suppressive drugs. Nephron progenitor number determines nephron endowment; a reduced nephron count is linked to the onset of kidney disease. Several transcriptional regulators including Six2, Wt1, Osr1, Sall1, Eya1, Pax2, and Hox11 paralogues are required for specification and/or maintenance of nephron progenitors. However, little is known about the regulatory intersection of these players. Here, we have mapped nephron progenitor-specific transcriptional networks of Six2, Hoxd11, Osr1, and Wt1. We identified 373 multi-factor associated 'regulatory hotspots' around genes closely associated with progenitor programs. To examine their functional significance, we deleted 'hotspot' enhancer elements for Six2 and Wnt4. Removal of the distal enhancer for Six2 …

Objective Since the extreme lateral lumbar interbody fusion procedure was first reported by Ozgur in 2006, a large number of clinical studies have been published. Anatomical studies which explore methods to avoid visceral structures, such as the kidney, with this approach have not been examined in detail. We dissected the retroperitoneal space to analyze how the extreme lateral transpsoas approach to the lumbar spine could damage the kidney and related structures.  The extreme lateral interbody fusion technique (XLIF) is a modification of the retroperitoneal approach to the lumbar spine. This is a minimally invasive technique allowing direct access to the disc space without peritoneal or posterior paraspinal musculature damage. Nevertheless, the retroperitoneal part of the colon can be injured in this operative technique.
