Double-Row Repair Technique For Bursal-Sided Partial-Thickness Rotator Cuff Tears, Hytham Salem, Aaron Carter, Md, Fotios Tjoumakaris, Kevin Freedman

Department of Orthopaedic Surgery Faculty Papers

Rotator cuff pathology is a common cause of shoulder pain in the athletic and general population. Partial-thickness rotator cuff tears (PTRCT) are commonly encountered and can be bursal-sided, articular-sided, or intratendinous. Various techniques exist for the repair of bursal-sided PTRCTs. The 2 main distinctions when addressing these lesions include tear completion versus preservation of the intact fibers, and single- versus double-row suture anchor fixation. We present our method for addressing bursal-sided PTRCTs using an in situ repair technique with double-row suture anchors. © 2017 Arthroscopy Association of North America

A Multidimensional Approach To Improve Cardiovascular Quality Measure Documentation In A Native American Population, Jessica A. Mastbergen

Doctoral Projects

Native Americans are more likely than white counterparts to be diagnosed with heart disease (Office of Minority Health [OMH], 2017). The scholarly project was conducted in response to the need for improvement in cardiovascular lifestyle interventions for Native Americans and their documentation to meet Government Performance Results Act quality measures. The clinical question addressed was: what is an evidence-based multidimensional approach to improve cardiovascular quality measure documentation in a Native American primary care clinic? The multidimensional interventions included educational sessions for Community Health Representatives (CHRs), a provider education session on appropriate documentation in order to satisfy quality measures, chart review …

Evidence Based Toolkit To Improve Oncology Referrals To Palliative Care, Katelyn M. Gettel Mar 2018

Evidence Based Toolkit To Improve Oncology Referrals To Palliative Care, Katelyn M. Gettel

Doctoral Projects

As cancer related mortality decreases, the number of patients living with cancer symptoms and the side effects of cancer treatment will continue to grow. In 2016, over 1.5 million Americans were newly diagnosed with cancer (American Cancer Society, 2016). Several major medical organizations including the American Society of Clinical Oncology (ASCO), the National Comprehensive Cancer Network (NCCN), and the World Health Organization (WHO) endorse the integration of palliative care into cancer care (Ramchandran, 2015). Although many cancer centers report offering palliative care, this remains largely limited to inpatient services.

The stakeholders of the hospice and palliative care division of a …

Articles, Abstracts, and Reports

Severe alcoholic hepatitis (sAH) is associated with a poor prognosis. There is no proven effective treatment for sAH, which is why early transplantation has been increasingly discussed. Hepatoblastoma-derived C3A cells express anti-inflammatory proteins and growth factors and were tested in an extracorporeal cellular therapy (ELAD) study to establish their effect on survival for subjects with sAH. Adults with sAH, bilirubin ≥8 mg/dL, Maddrey's discriminant function ≥ 32, and Model for End-Stage Liver Disease (MELD) score ≤ 35 were randomized to receive standard of care (SOC) only or 3-5 days of continuous ELAD treatment plus SOC. After a minimum follow-up of …

Architecting A System Model For Personalized Healthcare Delivery And Managed Individual Health Outcomes, Inas S. Khayal, Amro M. Farid Mar 2018

Architecting A System Model For Personalized Healthcare Delivery And Managed Individual Health Outcomes, Inas S. Khayal, Amro M. Farid

Dartmouth Scholarship

In recent years, healthcare needs have shifted from treating acute conditions to meeting an unprecedented chronic disease burden. The healthcare delivery system has structurally evolved to address two primary features of acute care: the relatively short time period, on the order of a patient encounter, and the siloed focus on organs or organ systems, thereby operationally fragmenting and providing care by organ specialty. Much more so than acute conditions, chronic disease involves multiple health factors with complex interactions between them over a prolonged period of time necessitating a healthcare delivery model that is personalized to achieve individual health outcomes. Using …

Telotristat ethyl, a tryptophan hydroxylase inhibitor, was efficacious and well tolerated in the phase 3 TELESTAR study in patients with carcinoid syndrome (CS) experiencing ≥ 4 bowel movements per day (BMs/day) while on somatostatin analogs (SSAs). TELECAST, a phase 3 companion study, assessed the safety and efficacy of telotristat ethyl in patients with CS (diarrhea, flushing, abdominal pain, nausea or elevated urinary 5-hydroxyindoleacetic acid (u5-HIAA)) with < 4 BMs/day on SSAs (or ≥ 1 symptom or ≥ 4 BMs/day if not on SSAs) during a 12-week double-blind treatment period followed by a 36-week open-label extension (OLE). The primary safety and efficacy endpoints were incidence of treatment-emergent adverse events (TEAEs) and percent change from baseline in 24-h u5-HIAA at week 12. Patients (N = 76) were randomly assigned (1:1:1) to receive placebo or telotristat ethyl 250 mg or 500 mg 3 times per day (tid); 67 continued receiving telotristat ethyl 500 mg tid during the OLE. …

BACKGROUND: This study examines whether racial disparities in hospitalization outcomes persist between African-American and White women with ovarian cancer after matching on demographic, presentation, and treatment factors.

METHODS: Using data from the Nationwide Inpatient Sample database, 5,164 African-American ovarian cancer patients were sequentially matched with White patients on demographic (e.g., age, income), presentation (e.g., stage, comorbidities), and treatment (e.g., surgery, radiation) factors. Racial differences in-hospital length of stay, post-operative complications, and in-hospital mortality were evaluated using conditional logistic regression models.

RESULTS: White ovarian cancer patients had relatively higher odds of post-operative complications when matched on demographics (OR 1.35, 95% CI …

Therapeutic Effects Of Instrument-Assisted Soft Tissue Mobilization And The Use In Athletic Populations: A Literature Review, Jenna Treloar Mar 2018

Therapeutic Effects Of Instrument-Assisted Soft Tissue Mobilization And The Use In Athletic Populations: A Literature Review, Jenna Treloar

Honors Program: Senior Projects (Public)

This literature review examines the mechanism and application of instrument-assisted soft tissue mobilization (IASTM) in athletic populations. IASTM is a treatment technique used for soft tissue pathologies. Experimental studies have been performed to determine the effects of IASTM on range of motion, pain, and inflammation in upper and lower extremities. Conflicting evidence exists for the effect of IASTM on the inflammatory reparative process of the body while more evidence exists in support of the technique’s benefits on pain reduction and improvement in range of motion. Understanding the effects of IASTM on range of motion, pain, and inflammation may be useful …

Schwannomais a benign tumor derived from Schwann cell which is the myelinated nerve sheath. This tumor usually occurs in the head and neck region but intraosseous schwannoma in this region is extremely rare, i.e. less than 1% of the benign tumors of jaws. This article reports a case of unusual intraosseous schwannoma in the mandible. A 22-year-old male patient revealed facial swelling at the right side of the chin with occasional sharp shooting pain and numbness on pressing. The radiographic examination showed a well-defined round shaped unilocular heterogeneous radiolucency with sclerotic border and buccal bone expansion at the right side …

We examined the relative importance of 23 community issues among elected officials and health directors in Connecticut in 2016. For this cross-sectional study, 74 elected officials (40.7% response rate) and 47 health directors (62.7% response rate), who were purposively sampled, completed a questionnaire to rate their perceived importance of 23 community issues. Eight of these issues were related to active living, healthy eating, or obesity. We used χ2 tests to evaluate differences in responses. Compared with elected officials, health directors significantly more often perceived obesity, access to healthy groceries, poor nutrition, lack of pedestrian walkways, and pedestrian safety as important. …

A Continuum Model Of Mucosa With Glycan-Ion Pairing, James D. Sterling, Shenda M. Baker Mar 2018

A Continuum Model Of Mucosa With Glycan-Ion Pairing, James D. Sterling, Shenda M. Baker

Business and Information Technology Faculty Research & Creative Works

Advances in the study of glycosoaminoglycan biohydrogels, label-free electrokinetic analysis of soft-diffuse layers in contact with saline solutions, and elucidation of ion-specific behavior in many biochemical systems offer the opportunity to marry these principal features in a new mathematical model of the mucosal glycocalyx. The model is based on the electroquasistatic subset of Maxwell's equations in the form of the steady-state continuum Poisson–Boltzmann equation for electrostatics with explicit incorporation of pairwise binding of ions to fixed charged-groups in the hydrogel. The pairwise association is modeled using reversible bimolecular reactions via stoichiometric dissociation constants that represent the rule of matching water …

The Loose End: Neurology Without Rehabilitation, Ahmad Zaheer Qureshi, Farooq Azam Rathore

Pakistan Journal of Neurological Sciences (PJNS)

In recent years, successive therapies for previously untreatable neurological conditions have been introduced in clinical practice and these advance interventions have remarkably changed the landscape of neurosciences1,2 .But, rehabilitation of the functional impairments caused by neurological diseases is often overlooked or sometimes even neglected. In other words, the role of a comprehensive and early multidisciplinary rehabilitation in neurological disease is unrecognized and poorly understood even among neurologists and neurosurgeons. Usually, lifesaving neurosurgical management or solving a diagnostic riddle in neurology is attributed to successful patient management; however in reality, this is the first step in establishing a care plan for …

The Rare And The Unexpected; Miller Fisher Syndrome, Saad Waheed, Jawad Hussian, Aqsa Shehzadi

Pakistan Journal of Neurological Sciences (PJNS)

Miller Fisher Syndrome (MFS) is a rare variant of Guillain-Barré syndrome (GBS) which is an idiopathic neuromuscular paralysis. It is considered as autoimmune disease because there are antibodies which attack the nerve cell fibers resulting in paralysis of muscles. This disease exhibits a classic clinical triad of ataxia, areflexia, and ophthalmoplegiawhereas it could have more neurological symptoms.Basically it is diagnosed clinically but have a serological test for its confirmation i.e. anti-GQ1b antibody levels. Here, we report a stereotypical case of MFS with all of its cardinal symptoms with addition of some other symptoms like diplopia, ptosis of an eye and unilateral facial …

Biopsy Proven Case Of Creutzfeldt- Jacob Disease From Pakistan, Rizwana Shahid, Saad Shafqat

Pakistan Journal of Neurological Sciences (PJNS)

Creutzfeldt-Jakob disease (CJD) is a rare type of prion disease leading to a rapidly progressive dementia (RPD) and other associated neurological features specially startle myoclonus. The annual incidence of CJD has been reported to be 1 per million or 271 people in USA2, but from Asia especially from Pakistan, only few case reports have been published. The probable cause behind the rarity of this disease in our society is still unclear. The diagnosis of CJD requires a high level of suspicion and lack of the availability of specific markers like CSF 14-3-3 in our setup makes it more difficult. Similarly brain …

Bilateral Wrist Drop In A Middle Age Man, Ghulam Shabbir, Zakir Jan, Haris Majid Rajput

Pakistan Journal of Neurological Sciences (PJNS)

Peripheral neuropathy is a common disorder which often causes disability.Among metals lead intoxication leads to bilateral radial neuropathy. Patients with lead neuropathy present with weakness that primarily involves the wrist and finger extensors. Lead intoxication in humans causes axonal degeneration. We report a case of middle age male who presented with bilateral wristsdrop, workup revealed that he had bilateral radial nerve neuropathy which was secondary to his occupational exposure to lead causing lead intoxication. Chronic lead exposure as a risk factor for neuropathy should be identified and treated promptly as its avoidance and treatment can save major disabilities.

A Rare Case: Rupture Of Internal Pudendal And Uterine Artery In A Vaginal Delivery, Novera G. Chughtai, Raheela Mohsin Rizvi

Department of Obstetrics & Gynaecology

The management of puerperal hematomas after normal delivery has always been challenging for obstetricians. Vulvar, vulvovaginal, or paravaginal hematomas are common. On the other hand, retroperitoneal hematomas are uncommon and can be life-threatening. The diagnosis of vascular injury is rarely made preoperatively as atonic or traumatic postpartum hemorrhage (PPH), uterine rupture and amniotic fluid embolism are more common differential diagnoses. Injury to internal pudendal and uterine vessels is extremely rare in cases of vaginal delivery and, therefore, the literature on this topic is very scarce. We present a rare case of both internal pudendal and uterine artery rupture in a …

INTRODUCTION: Patient-reported outcome measures (PROs) exist for psoriasis but not genital psoriasis (GenPs). METHODS: This cross-sectional, qualitative study in patients with moderate-to-severe GenPs was conducted to support development of a PRO for measuring the impact of GenPs on sexual activity and to establish content validity. The impacts of GenPs were identified in a literature review. Findings from the literature review were discussed with clinicians, and then patients with GenPs were interviewed. RESULTS: From the literature review, 52 articles, 44 abstracts, and 41 clinical trials met predefined search criteria. Of these, 11 concepts emerged as having theoretical support for use as …
