Research regarding community-based nurse voluntary and involuntary termination rates are limited and outdated. This study explores faith community nursing termination using a survey. Survey questions pertaining to why the termination occurred, supportive resources used during termination, and post termination behaviors are addressed. The first three reasons for termination were due to site leadership change, not a strategic priority of the hospital or faith community leadership, and organization restructuring or elimination of the program. The fourth through seventh reasons for termination were financial in nature. They were: not a financial priority to hospital or health care organization, not a financial priority …

The principal intent for colorectal cancer screening is early detection and primary prevention of colorectal cancer. In spite of these intentions, there is a reluctance of the general population to participate in screening. Colorectal cancer is preventable. However, screening non-adherence contributes to both cancer mortality and incidence rates and results in expensive, unnecessary health care costs. A faith-based community nurse-driven educational session, using an American Cancer Society PowerPoint, was developed to address barriers of screening non-adherence. The educational session was offered to adults who participate in multiple faith-based organizations. The session provided education regarding colorectal cancer and colorectal cancer screening …

There are many benefits when a specialty is able to translate nursing interventions into a standardized language. One of the benefits is that nurses from different specialties, geographic areas, or countries are able to understand terminologies and concepts. The aim of this paper is to examine three standardized nursing languages and their use in describing faith community nursing through an integrative literature review.
General descriptions, recognitions, populations, translations, reliability/validation/utility, and components of the Omaha System, the Nursing Intervention Classification, and the International Classification for Nursing Practice were presented. Context: Janda’s Upper-Crossed Syndrome (UCS) is characterized by alternating patterns of tightness and weakness, which is indicative of muscle imbalances and movement dysfunction usually seen in unilateral athletes. These muscle imbalances can cause abnormal movement patterns and sometimes manifest as pain. Anecdotal evidence seen in the athletic training clinic also supported the use of non-dominant side movement patterns to improve dominant side functioning and decrease pain. Objective: Based on the UCS, application of non-dominant shoulder exercises may decrease muscle imbalances and movement dysfunction. A literature review was conducted between the months of September and December 2017 to determine whether using heparinized central lines to draw coagulation studies is best evidence based practice. The purpose was to research the efficacy of using a heparinized central line to draw coagulation studies. This research is for the Abben Cancer Center affiliated with Spencer Hospital in Spencer, Iowa. Ten articles focusing on patients with heparinized lines or discard volume were reviewed. Results showed that heparin primarily has an effect on Activated Partial Thromboplastin Time (aPTT). It was found that using heparinized central lines to draw blood studies …

Medical decision-making in the paediatric population is complicated by the wide variation in physical and psychological development that occurs as children progress from infancy to adolescence. Parents and legal guardians are the de facto decision-makers in early infancy, but thereafter, the roles of parents/ legal guardians and paediatric patients become ever more complex. Health care providers (HCPs), while not decision-makers per se, have a significant role in medical decision-making throughout childhood. This statement outlines the ethical principles of medical decision-making for HCPs involved in caring for paediatric patients. AIMS: Carotid intima media thickness (CIMT) predicts cardiovascular (CVD) events, but the predictive value of CIMT change is debated. We assessed the relation between CIMT change and events in individuals at high cardiovascular risk.
METHODS AND RESULTS: From 31 cohorts with two CIMT scans (total n = 89070) on average 3.6 years apart and clinical follow-up, subcohorts were drawn: (A) individuals with at least 3 cardiovascular risk factors without previous CVD events, (B) individuals with carotid plaques without previous CVD events, and (C) individuals with previous CVD events. Bilateral carotid artery stenosis (BCAS) is one experimental model of vascular dementia thought to preferentially impact brain white matter. Indeed, few studies report hippocampal and cortical pathology prior to 30 days post-stenosis; though it is unclear whether those studies examined regions outside the white matter. Since changes in the blood-brain barrier (BBB) permeability precede more overt brain pathology in various diseases, we hypothesized that changes within the BBB and/or BBB-associated extracellular matrix (ECM) could occur earlier after BCAS in the hippocampus, cortex and striatum and be a precursor of longer term pathology. L’objectif principal de ce projet était de combiner différents senseurs et instruments pour expérimenter, en laboratoire et sur le terrain, un système de mesures permettant d’estimer quantitativement l’exposition physique des manutentionnaires. This experimental investigation studies the effectiveness of N95 respirators at blocking particles between 11.5 and 154 nm in diameter during 4 different construction activities. It is well known that large amounts of dust and debris are generated during construction activities. New research has found that significantly more nanoparticles are generated during construction activities than previously thought. Consequently, the N95 respirators commonly worn to protect against these particles are only rated to block 95% particles larger than 0.3 microns in size. Heart failure is a prevalent, high-risk condition associated with high mortality and frequent hospitalizations. Heart failure is characterized by acute exacerbations of signs and symptoms, and affects almost six million Americans leading to one of the most costly illnesses in the United States. Management of heart failure is complex and individualized relying on appropriate education, self-care management, medications, and adequate follow-up to succeed in treatment. Many organizations have initiated evidence-based heart failure management or educational programs that provide easy-to-learn education on diagnosis, medication, and self-care behaviors. Radioembolization of liver tumors using yttrium-90 (Y-90) labeled microspheres is a proven method of treatment. The primary decay mode of Y-90 is beta-minus decay. The electrons produced during decay deliver a cytotoxic radiation dose in a localized volume. The radiation dose is sufficient to kill tumor cells yet spare the normal liver. The Y-90 microspheres are delivered via a catheter that has been placed in an artery that supplies blood to the tumor. Microspheres are injected and the blood distributes the microspheres. The law ordinarily recognizes the woman who gives birth as the mother of a child, but in certain jurisdictions, it will recognize the commissioning couple as the legal parents of a child born to a commercial surrogate. Some commissioning parents have, however, effectively abandoned the children they commission, and in such cases commercial surrogates may find themselves facing unexpected maternal responsibility for children they had fully intended to give up. Any assumption that commercial surrogates ought to assume maternal responsibility for abandoned children runs contrary to the moral suppositions that typically govern contract surrogacy, in particular assumptions that gestational carriers …

Background: Family doctors (FDs) focus on biopsychosocial components of health during consultations. However, much of the evidence employed by these doctors is produced by researchers who are not routinely involved in family practice. Family doctors competent in both clinical practice and research are essential to addressing this gap. Pain management is the culmination of physician, nurse, and patient expectations. Successful pain management is impacted by the patient’s perception and expectation of the provided interventions, as well as the perception of the care provider of the patient’s pain. Pain assessment tools have been utilized for decades and still pain control is rated as inadequate. Providing scripted education to the patient on the purpose and utilization of these tools impacts the patient’s perception of pain management. The primary purpose of this research project was to identify physical limitations or asymmetries in regular body movement using the Functional Movement Screening (FMS) system on the Bemidji State University female track athletes. A secondary purpose was to examine if fear of injury influences FMS overall scores. The FMS has been frequently used in the exercise and medical communities. The primary intent of this project was to identify how the FMS can be used to recognize deficiencies, in prior physical training methods. In infants intolerant of enteral feeding because of intestinal disease, parenteral nutrition may be associated with cholestasis, which can progress to end-stage liver disease. Here we show the function of hepatic macrophages and phytosterols in parenteral nutrition-associated cholestasis (PNAC) pathogenesis using a mouse model that recapitulates the human pathophysiology and combines intestinal injury with parenteral nutrition. We combine genetic, molecular, and pharmacological approaches to identify an essential function of hepatic macrophages and IL-1β in PNAC. Pharmacological antagonism of IL-1 signaling or genetic deficiency in CCR2, caspase-1 and caspase-11, or IL-1 receptor (which binds both IL-1α and IL-1β) prevents PNAC in …

INTRODUCTION: Studies that report outcomes after staged bilateral shoulder arthroplasty (BSA) are limited. This study compared the overall improvement between the first and second operated shoulders after BSA.
METHODS: Preoperative and postoperative motion and patient-reported outcomes for function, pain, and general health were assessed for patients who underwent BSA with a 2-year follow-up. Ultimate outcomes and the efficacy of treatment were compared between the first and second operated shoulders.
RESULTS: Seventy-three patients met the inclusion criteria (mean follow-up, 51.4 months). There were no notable differences between the first and second operated shoulders for all preoperative and postoperative variables except for …

Presynaptic voltage-gated K+ (Kv) channels in dorsal root ganglion (DRG) neurons are thought to regulate nociceptive synaptic transmission in the spinal dorsal horn. However, the Kv channel subtypes responsible for this critical role have not been identified. The Kv3.4 channel is particularly important because it is robustly expressed in DRG nociceptors, where it regulates action potential (AP) duration. Furthermore, Kv3.4 dysfunction is implicated in the pathophysiology of neuropathic pain in multiple pain models. We hypothesized that, through their ability to modulate AP repolarization, Kv3.4 channels in DRG nociceptors help to regulate nociceptive synaptic transmission. To test this hypothesis, we …

Inter-professional education is a topic that is widely gaining popularity throughout the healthcare field. Because inter-professional practice in the hospital and beyond has proven its value time and time again, universities must take it upon themselves to train their healthcare students to work efficiently and communicate effectively with other disciplines in the healthcare field. Through a thorough literature review, it was demonstrated that inter-professional education studies and simulations that were performed in a range of settings have had positive effects on a variety of factors, including communication, quality of care given, and perceptions of professional roles.
