โรคหลั่งเร็ว (premature ejaculation, PE) หรือที่เรียกกันว่า “ล่มปากอ่าว”, “นกกระจอกไม่ทันกินนํ้า” เป็นโรคที่พบได้มากที่สุด ในปัญหาสุขภาพทางเพศของชายทั่วโลก พบได้ถึงร้อยละ 20 - 30 ในชายทุกกลุ่มอายุ นอกจากจะทำให้เกิดความไม่พึงพอใจในการมีเพศสัมพันธ์ทั้งฝ่ายชายและฝ่ายหญิงแล้ว ยังส่งผลกับผู้ป่วยด้านความมั่นใจ ความเครียด เสียบุคลิกภาพ รวมไปถึงเกิดปัญหาชีวิตคู่ผู้ป่วยส่วนใหญ่เมื่อมีอาการหลั่งเร็วมักอายที่จะเข้าไปรับคำปรึกษาจากแพทย์ บางส่วนไม่ทราบว่าโรคนี้สามารถรักษาได้ อย่างไรก็ตาม ถึงแม้ความรู้ความเข้าใจเกี่ยวกับ premature ejaculation จะได้รับการพัฒนาอย่างต่อเนื่อง ในช่วงกว่าร้อยปีที่ผ่านมา แต่ในปัจจุบันคำจำกัดความที่เป็นสากลของโรคกลไกการเกิดโรค และสาเหตุนั้นยังไม่ชัดเจนแต่เกี่ยวข้องกับเวลา ความสามารถในการควบคุมการหลั่งและความกังวลในการใช้ชีวิตคู่.

Objectives: The objectives of the study are to evaluate the effectiveness of Homoeopathic Constitutional remedy (HC) and Homoeopathic Constitutional + Organ remedy (HCOM) in comparison to Placebo (PL) in cases of benign prostatic hyperplasia (BPH) using International Prostate Symptom Score (IPSS), ultrasonographic changes in prostate volume, post-void residual urine, uroflowmetry and in WHO Quality of Life (QOL)-BREF. Materials and Methods: A multicentric, three-armed, randomised clinical trial was conducted at five centres. Objective: To evaluate the effectiveness of homoeopathic remedies in improving quality of life (QoL) of chronic urticaria (CU) patients. Methods: Setting: The study population included patients attending the Outpatient Department of State Homoeopathic Dispensary, Ahmadpur, Aligarh, Uttar Pradesh, India. CU-QoL questionnaire (CU-Q2oL) and average Urticaria Activity Score for 7 days (UAS7) questionnaires were filled questionnaires were filled at baseline and 3rd, 6th, 9th and 12th months. EUS guided core biopsy was once rarely performed but is now entering mainstream practice. Neuroendocrine tumors often warrant core biopsy as sufficient tissue must be obtained to allow for special staining to ensure a correct diagnosis. Traditionally these lesions were sampled with FNA needles. We performed a retrospective pilot study to evaluate the clinical value and efficacy of the a new EUS core needle biopsy needle as compared to a standard EUS FNA needle in the evaluation of patients with known or suspected neuroendocrine tumors.

The present study investigated the articulatory and acoustic characteristics of vowel production in individuals with Down syndrome (DS). Speech production deficits and reduced intelligibility are consistently noted in this population, attributed to any combination of phonological, structural, and/or motor control deficits. Speakers with DS have demonstrated impaired vowel production, as indicated by perceptual, acoustic, and articulatory data, with emerging evidence of vowel centralization. Participants in the study included eight young adults with DS, as well as eight age- and gender-matched controls. Background: Mitochondrial Encephalomyopathy, Lactic Acidosis, and Stroke-like Episodes (MELAS) is a maternally inherited progressive multisystemic disorder. Occurrence of seizure and/or stroke-like episode, as well as classic biomarkers (i.e., cerebral lactic acidosis, depleted N-acetylaspartate (NAA)), have been linked to neuropsychological deficit and trigger the developmental cascade of neurodegeneration affecting posterior prior to anterior brain regions. While a pattern of global deterioration has been reported, systematic examination in a large MELAS cohort has not been conducted.

Purpose: One in 7 children ages 2-8 have a behavioral, mental or developmental diagnosis. The prevalence of mental health disorders increases in adolescence to over 20%. Only 36% of youth who have a mental health disorder in their lifetime receive treatment. School-based services can address barriers to care. The purpose of this study was to determine the frequency and indication of referrals for students to 19 hospital-employed clinical social workers placed within a school district during one academic year (2017- 2018) and how the indication varies with level of schooling.

Pandemic and seasonal influenza viruses can be transmitted through aerosols and droplets, in which viruses must remain stable and infectious across a wide range of environmental conditions. Using humidity-controlled chambers, we studied the impact of relative humidity on the stability of 2009 pandemic influenza A(H1N1) virus in suspended aerosols and stationary droplets. Contrary to the prevailing paradigm that humidity modulates the stability of respiratory viruses in aerosols, we found that viruses supplemented with material from the apical surface of differentiated primary human airway epithelial cells remained equally infectious for 1 hour at all relative humidities tested. Background—Fragmentation of the tunica media is a hallmark of intracranial aneurysm formation, often leading to aneurysmal progression and subsequent rupture. The objective of this study is to determine the plasma level of elastin fragments in the lumen of ruptured versus unruptured human intracranial aneurysms. Methods and Results—One hundred consecutive patients with/without ruptured saccular intracranial aneurysms undergoing endovascular coiling or stent-assisted coiling were recruited. Blood samples were collected from the lumen of intracranial aneurysm using a microcatheter. Cyclical curriculum review and continuous course improvement is an unarguable measure of quality nursing education. Higher education and professional regulators across the Middle East and North Africa (MENA) and African (Afro) regions are increasingly mandating cyclical review within accreditation and programme approval standards. Many nursing education providers in the region operate multi-site and multi-country campuses. A key challenge is to ensure that programmes keep pace with international standards in nursing education, and, that inter-campus drift does not occur in programme content and quality. วัตถุประสงค์: เพื่อศึกษาประสบการณ์ของบุคลากรทางด้านสุขภาพ ที่ประยุกต์ใช้การบูรณาการมิติเพศภาวะในการปฏิบัติงานดูแลผู้ป่วยวัณโรค\n\nแบบแผนการวิจัย: การวิจัยใช้ระเบียบวิธีวิจัยเชิงปฏิบัติการแบบมีส่วนร่วม (Participatory Action Research: PAR) เพื่อศึกษาการบูรณาการมิติเพศภาวะในการดูแลรักษาผู้ป่วยวัณโรคจากประสบการณ์ของบุคลากรด้านสุขภาพ\n\nวิธีดำเนินการวิจัย: ผู้ให้ข้อมูล คือ บุคลากรด้านสุขภาพที่ดูแล รักษาผู้ป่วยวัณโรค คัดเลือกแบบเจาะจงจากโรงพยาบาลรัฐใน 5 จังหวัด จำนวน 78 ราย เก็บข้อมูลด้วยการสังเกต สัมภาษณ์เชิงลึก และสนทนากลุ่ม การวิเคราะห์ข้อมูลและตีความผ่านการให้รหัสเปิด (Open coding) เพื่อตอบวัตถุประสงค์การวิจัย\n\nผลการวิจัย: ประสบการณ์ของบุคลากรด้านสุขภาพในการดูแลผู้ป่วยวัณโรคโดยใช้การบูรณาการมิติเพศภาวะ แบ่งออกได้ 4 ประเด็น ดังนี้ 1) ประสบการณ์การวิเคราะห์เพศภาวะในการเยี่ยมบ้านผู้ป่วยวัณโรค 2) ประสบการณ์การบูรณาการมิติเพศภาวะในความสัมพันธ์ระหว่างบุคลากรด้านสุขภาพกับผู้ป่วย เพื่อลดการใช้อำนาจกับผู้ป่วย และเสริมพลังอำนาจให้ผู้ป่วย 3) ประสบการณ์การบูรณาการมิติเพศภาวะในการทำงานระดับชุมชน เพื่อลดการตีตราผู้ป่วยในชุมชน และ 4) ประสบการณ์การบูรณาการมิติเพศภาวะในการจัดระบบบริการสุขภาพ\n\nสรุป: การบูรณาการมิติเพศภาวะในการดูแลผู้ป่วยวัณโรคนั้นมีความสำคัญต่อการทำงานของบุคลากรด้านสุขภาพอย่างยิ่งทั้งช่วยให้เกิดความเข้าใจต่อผู้ป่วยวัณโรคที่มีความต้องการ และข้อจำกัดที่แตกต่างกันจนสามารถนำไปสู่การให้บริการ การวางแผนการรักษาที่เหมาะสมจนเกิดให้เกิดผลสำเร็จในการรักษาได้ ซึ่งบุคลากรด้านสุขภาพสามารถนำไปประยุกต์ใช้ และวางแผนด้านการพยาบาลในการดูแลผู้ป่วยวัณโรค และผู้ป่วยที่มีปัญหาด้านสุขภาพอื่น ๆ ต่อไปได้

วัตถุประสงค์: เพื่อศึกษาปัจจัยที่มีความสัมพันธ์กับความวิตกกังวลจากการพรากจากของเด็กวัยก่อนเรียนที่เข้ารับบริการในศูนย์พัฒนาเด็กเล็ก\n\nรูปแบบการวิจัย: เป็นการวิจัยแบบพรรณาเชิงหาความสัมพันธ์\n\nวิธีดำเนินการวิจัย: เก็บรวบรวบข้อมูลจากครอบครัวผู้ดูแลหลักของเด็กที่เข้ารับบริการในศูนย์พัฒนาเด็กเล็ก จังหวัดจันทบุรี จำนวน 74 คน เลือกกลุ่มตัวอย่างด้วยวิธีสุ่มจำนวนศูนย์พัฒนาเด็กเล็กแบบ Cluster sampling ได้จำนวน 5 ศูนย์ หลังจากนั้นเลือกผู้ดูแลหลักของเด็กจากศูนย์พัฒนาเด็กเล็กตามคุณสมบัติที่กำหนด เก็บข้อมูลระหว่างเดือนพฤษภาคม - มิถุนายน 2558 เครื่องมือวิจัยประกอบด้วยแบบบันทึกพฤติกรรมความวิตกกังวลจากการพรากจาก มีค่าความเชื่อมั่น (Kuder Richardson 20: KR20) เท่ากับ .93 แบบสอบถามพื้นฐานอารมณ์ของเด็กวัยก่อนเรียน แบบสอบถามสัมพันธภาพระหว่างเด็กกับผู้ดูแล และแบบสอบถามรูปแบบการอบรมเลี้ยงดูเด็กของมารดา มีค่าสัมประสิทธิ์แอลฟาของครอนบาค เท่ากับ .80, .93 และ .93 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติพรรณนา ค่าสัมประสิทธิ์สหสัมพันธ์ของเพียร์สัน\n\nผลการวิจัย: สัมพันธภาพระหว่างเด็กกับผู้ดูแลหลัก มีความสัมพันธ์ทางลบกับความวิตกกังวลจากการพรากจากอย่างมีนัยสำคัญทางสถิติ (r = -.23, p < .05) และมีความสัมพันธ์ทางลบกับความวิตกกังวลจากการพรากจากระยะประท้วงอย่างมีนัยสำคัญทางสถิติ (r = -.28, p = .01) พื้นฐานอารมณ์ของเด็กและรูปแบบการอบรมเลี้ยงดูแบบตามใจมีความสัมพันธ์ทางลบกับความวิตกกังวลจากการพรากจากในระยะปฏิเสธอย่างมีนัยสำคัญทางสถิติ (r = -.28, p = .01) และ r = -.23 p < .05 ตามลำดับ) แต่ไม่มีความสัมพันธ์กับความวิตกกังวลจากการพรากจากในภาพรวม (p < .05) \n\nสรุป: พยาบาลและผู้เกี่ยวข้องในการดูแลเด็กควรส่งเสริมสัมพันธภาพระหว่างเด็กกับผู้ดูแลหลักและพื้นฐานอารมณ์ที่เหมาะสมสำหรับเด็กวัยก่อนเรียน และส่งเสริมการเลี้ยงดูเด็กแบบให้ความรักมาก การควบคุมอยู่ในระดับปานกลาง มีการใช้เหตุผล ไม่ปล่อยตามใจ เพื่อลดความวิตกกังวลจากการพรากจากของเด็กวัยก่อนเรียนที่เข้ารับบริการในศูนย์พัฒนาเด็กเล็ก

วัตถุประสงค์: เพื่อศึกษาเปรียบเทียบภาระของผู้ดูแลที่เป็นคู่สมรส ผู้สูงอายุที่มีภาวะสมองเสื่อมก่อน ระหว่าง และหลังได้รับโปรแกรมการทบทวนชีวิตคู่\n\nรูปแบบการวิจัย: การวิจัยกึ่งทดลอง\n\nวิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้สูงอายุชายและหญิงที่มีอายุ 60 ปีบริบูรณ์ขึ้นไป ซึ่งมีผู้ดูแลที่เป็น คู่สมรส ได้รับการวินิจฉัยว่ามีภาวะสมองเสื่อมระดับเล็กน้อยถึงปานกลาง และเข้ารับการรักษาแบบผู้ป่วยนอกของคลินิกความจำ โรงพยาบาลมหาวิทยาลัยแห่งหนึ่ง จำนวน 16 คน ทำการศึกษากลุ่มเดียววัดแบบอนุกรมเวลา โดยผู้สูงอายุที่มีภาวะสมองเสื่อมและผู้ดูแลที่เป็นคู่สมรสได้รับโปรแกรมการทบทวนชีวิตคู่ เครื่องมือที่ใช้ในการทดลอง คือ โปรแกรมการทบทวนชีวิตคู่ตามแนวคิดของ Ingersoll-Dayton และคณะ เก็บรวบรวมข้อมูลโดยใช้แบบสอบถามภาระของผู้ดูแล มีค่าสัมประสิทธิ์แอลฟาครอนบาคเท่ากับ .90 วิเคราะห์ข้อมูลโดยใช้สถิติการวิเคราะห์ความแปรปรวนแบบวัดซ้ำ\n\nผลการวิจัย: ภาระของผู้ดูแลที่เป็นคู่สมรสผู้สูงอายุที่มีภาวะสมองเสื่อม ระหว่างได้รับโปรแกรมการทบทวนชีวิตคู่ และหลังได้รับโปรแกรมการทบทวนชีวิตคู่ต่ำกว่าก่อนได้รับโปรแกรมการทบทวนชีวิตคู่อย่างมีนัยสำคัญทางสถิติที่ระดับ .01 โดยภาระของผู้ดูแลที่เป็นคู่สมรสผู้สูงอายุที่มีภาวะสมองเสื่อมลดลงต่อเนื่องอย่างมีนัยสำคัญทางสถิติที่ระดับ .01 ตั้งแต่สัปดาห์ที่ 3 เป็นต้นไป\n\nสรุป: โปรแกรมการทบทวนชีวิตคู่สามารถลดภาระของผู้ดูแลที่เป็นคู่สมรสผู้สูงอายุที่มีภาวะสมองเสื่อมได้

วัตถุประสงค์: 1) เพื่อพัฒนาแบบประเมินสมรรถนะหัวหน้าหอผู้ป่วย โรงพยาบาลสุรินทร์ 2) เพื่อตรวจสอบคุณภาพของแบบประเมินสมรรถนะหัวหน้าหอผู้ป่วย โรงพยาบาลสุรินทร์\nแบบแผนการวิจัย: การวิจัยเชิงพรรณนา\n\nวิธีดำเนินการวิจัย: การศึกษาแบ่งเป็น 2 ขั้นตอน คือ 1) การพัฒนาแบบประเมิน โดยทบทวนวรรณกรรมและสนทนากลุ่ม ผู้เชี่ยวชาญคัดเลือกแบบเฉพาะเจาะจง จำนวน 10 คน เพื่อกำหนดรายการสมรรถนะ ตรวจสอบความตรงตามเนื้อหาโดยผู้ทรงคุณวุฒิ จำนวน 5 คน ตรวจสอบความเที่ยงโดยใช้สูตรสัมประสิทธิ์แอลฟาของครอนบาค ตรวจสอบความตรงเชิงโครงสร้างโดยการวิเคราะห์องค์ประกอบด้วยวิธีสกัดตัวประกอบหลัก หมุนแกนแบบออโธโกนอลด้วยวิธีแวริแมกซ์กับกลุ่มตัวอย่างพยาบาลวิชาชีพ จำนวน 330 คน โดยการเลือกแบบเจาะจงและสุ่มแบบมีระบบ นำผลที่ได้ไปสร้างแบบประเมินสมรรถนะหัวหน้าหอผู้ป่วย โรงพยาบาลสุรินทร์ 2) การตรวจสอบคุณภาพของแบบประเมิน โดยนำแบบประเมินที่พัฒนาขึ้นไปทดลองใช้กับกลุ่มตัวอย่าง 3 กลุ่ม คือ ผู้บังคับบัญชา หัวหน้าหอผู้ป่วย และผู้ใต้บังคับบัญชา จำนวน 350 คน โดยการเลือกแบบเจาะจงและสุ่มแบบมีระบบ ประกอบด้วย การตรวจสอบหาความสอดคล้องของการประเมิน ความเที่ยงของแบบประเมิน และความเที่ยงของการใช้แบบประเมิน\n\nผลการวิจัย: 1) แบบประเมินสมรรถนะหัวหน้าหอผู้ป่วย โรงพยาบาลสุรินทร์ ประกอบด้วย 8 ด้าน มีจำนวนข้อรายการสมรรถนะย่อย 62 ข้อ สามารถอธิบายความแปรปรวนได้ร้อยละ 80.41 ดังนี้ ด้านการพัฒนาคุณภาพบริการพยาบาลและการบริหารความปลอดภัย 11 ข้อ ด้านเทคโนโลยีสารสนเทศ 9 ข้อ ด้านภาวะผู้นำ 9 ข้อ ด้านคุณธรรม จริยธรรม 7 ข้อ ด้านการจัดการทรัพยากรมนุษย์ 9 ข้อ ด้านการพยาบาลข้ามวัฒนธรรม 6 ข้อ ด้านการบริหารจัดการเชิงกลยุทธ์ 6 ข้อ และด้านวิชาการ วิจัยและนวัตกรรม 5 ข้อ\n\n2) การตรวจสอบคุณภาพของแบบประเมินสมรรถนะหัวหน้าหอผู้ป่วย โรงพยาบาลสุรินทร์ พบว่า การประเมินของผู้ประเมิน 3 กลุ่ม (ผู้บังคับบัญชา หัวหน้าหอผู้ป่วย และผู้ใต้บังคับบัญชา) มีความสอดคล้องกัน โดยมีค่าเฉลี่ยโดยรวมอยู่ในระดับมาก คือ …

Objective: Implementing evidence-based practices requires a multi-faceted approach. Electronic clinical decision support (ECDS) tools may encourage evidence-based practice adoption. However, data regarding the role of mobile ECDS tools in pediatrics is scant. Our objective is to describe the development, distribution, and usage patterns of a smartphone-based ECDS tool within a national practice standardization project.

Children need to develop a variety of skill sets to optimize their development and manage toxic stress. Research demonstrates that developmentally appropriate play with parents and peers is a singular opportunity to promote the social-emotional, cognitive, language, and self-regulation skills that build executive function and a prosocial brain. Furthermore, play supports the formation of the safe, stable, and nurturing relationships with all caregivers that children need to thrive.

Egypt has a substantive and rich history in nursing education with continuous evolution over many years. A government commitment to further enhance the capacity of the nursing workforce recently required all nursing curricula to be upgraded and presented in a competency-based format. Given that we have had historic increases in inequality, as well as stagnant poverty levels for several decades, I will tell the story of poverty and inequality in the U.S. from the perspective of the metrics of happiness and well-being. My research—and the book that resulted—found that inequality is part of this story, but not the only part. The story was much more complicated than I anticipated. The dynorphin/kappa opioid receptor (DYN/KOR) system appears to be a key mediator of the behavioral effects of chronic exposure to alcohol. Although KOR opioid receptor antagonists have been shown to decrease stress-related behaviors in animal models during acute ethanol withdrawal, the role of the DYN/KOR system in regulating long-term behavioral changes following protracted abstinence from ethanol is not well understood. Malignant aerodigestive fistula (ADF) is an uncommon condition complicating thoracic malignancies. It results in increased morbidity and mortality and warrants therapeutic intervention. The management approach depends on symptoms, configuration, location, and extent of the fistula. This article will discuss the therapeutic considerations in the management of ADF.
